Project description:Mechanically fibrillated cellulose nanofibrils (CNFs) have attracted special attention as building blocks for the development of advanced materials and composites. A correlation exists between CNF morphology and the properties of the materials they form. However, this correlation is often evaluated indirectly by process-centered approaches or by accessing a single dimensionality of CNFs adsorbed on solid supports. High-resolution imaging is currently the best approach to describe the morphological features of nanocelluloses; nevertheless, adsorption effects need to be accounted for. For instance, possible deformations of the CNFs arising from capillary forces and interactions with the substrate need to be considered in the determination of their cross-sectional dimensions. By considering soft matter imaging and adsorption effects, we provide evidence of the deformation of CNFs upon casting and drying. We determine a substantial flattening associated with the affinity of CNFs with the substrate corresponding to a highly anisotropic cross-sectional geometry (ellipsoidal) in the dried state. Negative-contrast scanning electron microscopy is also introduced as a new method to assess the dimensions of the CNFs. The images obtained by the latter, a faster imaging method, were correlated with those from atomic force microscopy. The cross-sectional area of the CNF is reconstructed by cross-correlating the widths and heights obtained by the two techniques.

Project description:BackgroundThis study aimed to assess the quality of antenatal care (ANC) women received in Migori county, Kenya-including both service provision and experience dimensions-and to examine factors associated with each dimension.MethodsWe used survey data collected in 2016 in Migori county from 1031 women aged 15-49 who attended ANC at least once in their most recent pregnancy. ANC quality service provision was measured by nine questions on receipt of recommended ANC services, and experience of care by 18 questions on information, communication, dignity, and facility environment. We summed the responses to the individual items to generate ANC service provision and experience of care scores. We used both linear and logistic regression to examine predictors.ResultsThe average service provision score was 10.9 (SD = 2.4) out of a total of 16. Most women received some recommended services once, but not at the frequency recommended by the Kenyan Ministry of Health. About 90% had their blood pressure measured, and 78% had a urine test, but only 58 and 14% reported blood pressure monitoring and urine test, respectively, at every visit. Only 16% received an ultrasound at any time during ANC. The average experience score is 27.3 (SD = 8.2) out of a total score of 42, with key gaps demonstrated in communication. About half of women were not educated on pregnancy complications. Also, about one-third did not often understand the purposes of tests and medicines received and did not feel able to ask questions to the health care provider. In multivariate analysis, women who were literate, employed, and who received all their ANC in a health center had higher experiences scores than women who were illiterate (coefficient = 1.52, CI:0.26,2.79), unemployed (coefficient = 2.73, CI:1.46,4.00), and received some ANC from a hospital (coefficient = 1.99, CI: 0.84, 3.14) respectively. The wealthiest women had two times higher odds of receiving an ultrasound than the poorest women (OR = 2.00, CI:1.20,3.33).ConclusionQuality of ANC is suboptimal in both service provision and experience domains, with disparities by demographic and socioeconomic factors and facility type. More efforts are needed to improve quality of ANC and to eliminate the disparities.

Project description:While sleep positively impacts well-being, health, and productivity, the effects of societal factors on sleep remain underexplored. Here we analyze the sleep of 30,082 individuals across 11 countries using 52 million activity records from wearable devices. Our data are consistent with past studies of gender and age-associated sleep characteristics. However, our analysis of wearable device data uncovers differences in recorded vs. self-reported bedtime and sleep duration. The dataset allowed us to study how country-specific metrics such as GDP and cultural indices relate to sleep in groups and individuals. Our analysis indicates that diverse sleep metrics can be represented by two dimensions: sleep quantity and quality. We find that 55% of the variation in sleep quality, and 63% in sleep quantity, are explained by societal factors. Within a societal boundary, individual sleep experience was modified by factors like exercise. Increased exercise or daily steps were associated with better sleep quality (for example, faster sleep onset and less time awake in bed), especially in countries like the U.S. and Finland. Understanding how social norms relate to sleep will help create strategies and policies that enhance the positive impacts of sleep on health, such as productivity and well-being.

Project description:Purpose The purpose of this paper is to describe a quality improvement (QI) intervention in primary health facilities providing childbirth care in rural Southern Tanzania. Design/methodology/approach A QI collaborative model involving district managers and health facility staff was piloted for 6 months in 4 health facilities in Mtwara Rural district and implemented for 18 months in 23 primary health facilities in Ruangwa district. The model brings together healthcare providers from different health facilities in interactive workshops by: applying QI methods to generate and test change ideas in their own facilities; using local data to monitor improvement and decision making; and health facility supervision visits by project and district mentors. The topics for improving childbirth were deliveries and partographs. Findings Median monthly deliveries increased in 4 months from 38 (IQR 37-40) to 65 (IQR 53-71) in Mtwara Rural district, and in 17 months in Ruangwa district from 110 (IQR 103-125) to 161 (IQR 148-174). In Ruangwa health facilities, the women for whom partographs were used to monitor labour progress increased from 10 to 57 per cent in 17 months. Research limitations/implications The time for QI innovation, testing and implementation phases was limited, and the study only looked at trends. The outcomes were limited to process rather than health outcome measures. Originality/value Healthcare providers became confident in the QI method through engagement, generating and testing their own change ideas, and observing improvements. The findings suggest that implementing a QI initiative is feasible in rural, low-income settings.

Project description:This study employs the impact and mechanism of internal audit quality on the comparability of corporate accounting information. Using manually collected internal audit data, the study focuses on non-financial listed companies of A-share market spanning from 2007 to 2022 in China. The findings are as follows: (1) Internal audit significantly enhances the comparability of accounting information, the higher the quality of internal audit, the more pronounced its contribution. This conclusion remains robust after conducting endogeneity tests. (2) Mechanism testing reveals that internal audit enhances accounting information comparability through two channels: mitigating agency costs and improving financial information transparency. (3) Moderation effect test proves that the quality of internal control and the high-tech industry will strengthen the positive relationship between internal audit and comparability of accounting information, while the degree of industry competition and the level of capital occupation by major shareholders will weaken the positive relationship.

Project description:ObjectiveTo investigate whether accounting for past patient composition in evaluations of the association between public quality reports and patient selectivity changes findings and conclusions.Data sourcesSecondary data analysis of public reports of Assisted Reproductive Technology Clinic success rates between 2011 and 2018.Study designTwo sets of fixed effects models, (1) a standard fixed-effects model (FE) and (2) a dynamic panel model using structural equation modeling estimated with maximum-likelihood (ML-SEM) with one- and two-year lagged patient characteristics, are compared. The outcome variables are patient composition features associated with success rates, including two age categories and eight diagnoses of infertility. Two-year lagged success rates for any live birth and a singleton live birth are central predictor variables.Data collection/extraction methodsClinics with complete records for the 2011-2018 period were included (N = 303).Principal findingsFor live birth success rates, the two models show increases in the two-year lagged success rate is associated with a reduction in (1) the transformed percentage of patients with endometriosis (FE: β = -0.006, SE = 0.002, p < 0.01; ML-SEM: β = -0.005, SE = 0.002, p < 0.01) and (2) the percentage of patients with tubal diagnoses (FE: β = -0.090, SE = 0.017, p < 0.001; ML-SEM: β = -0.064, SE = 0.027, p < 0.05). For singleton birth success rates, the models show positive associations between the two-year lagged success rate and the percent of patients over 35 years old (FE: β = 0.219, SE = 0.033, p < 0.001; ML-SEM: β = 0.095, SE = 0.047, p < 0.05). Overall, the FE models show numerous significant associations with the two-year lagged success rate not observed in the ML-SEM models. Thus, the preferred and theoretically appropriate model (ML-SEM) and the more commonly used model (FE) yield different results.ConclusionsResearchers and policymakers should use models that account for past patient composition when evaluating the associations between quality reports and patient selectivity.

Project description:The Multidimensional Poverty Index is used increasingly to measure poverty in developing countries. The index is constructed using selected indicators that cover health, education, and living standards dimensions. The accuracy of this tool, however, depends on how each indicator is measured. This study explores the effect of accounting for water quality in multidimensional poverty measurement. Access to drinking water is traditionally measured by water source types. The study uses a more comprehensive measure, access to safely managed drinking water services, which are free from E. coli contamination, available when needed and accessible on premises in line with Sustainable Development Goal target 6.1. The study finds that the new measure increases national multidimensional headcount poverty by 5-13 percentage points, which would mean that 5-13 million more people are multidimensionally poor. It also increases the poverty level in urban areas to a greater extent than in rural areas. The finding is robust to changes in water contamination risk levels and Multidimensional Poverty Index aggregation approaches and weighting structures.

Project description:Mutations affect individual health, population persistence, adaptation, diversification, and genome evolution. There is evidence that the mutation rate varies among genotypes, but the causes of this variation are poorly understood. Here, we link differences in genetic quality with variation in spontaneous mutation in a Drosophila mutation accumulation experiment. We find that chromosomes maintained in low-quality genetic backgrounds experience a higher rate of indel mutation and a lower rate of gene conversion in a manner consistent with condition-based differences in the mechanisms used to repair DNA double strand breaks. These aspects of the mutational spectrum were also associated with body mass, suggesting that the effect of genetic quality on DNA repair was mediated by overall condition, and providing a mechanistic explanation for the differences in mutational fitness decline among these genotypes. The rate and spectrum of substitutions was unaffected by genetic quality, but we find variation in the probability of substitutions and indels with respect to several aspects of local sequence context, particularly GC content, with implications for models of molecular evolution and genome scans for signs of selection. Our finding that the chances of mutation depend on genetic context and overall condition has important implications for how sequences evolve, the risk of extinction, and human health.

Project description:The Adult Strabismus Quality of Life Questionnaire (AS-20) and the Amblyopia & Strabismus Questionnaire (A&SQ) both measure health-related quality of life in strabismus patients. We evaluated to what extent these instruments cover similar domains by identifying the underlying quality-of-life factors of the combined questionnaires.Participants were adults from a historic cohort with available orthoptic childhood data documenting strabismus and/or amblyopia. They had previously completed the A&SQ and were now asked to complete the AS-20. Factor analysis was performed on the correlation-matrix of the combined AS-20 and A&SQ data to identify common underlying factors. The identified factors were correlated with the clinical variables of angle of strabismus, degree of binocular vision, and visual acuity of the worse eye.One hundred ten patients completed both questionnaires (mean age, 44 years; range, 38-51 years). Six factors were found that together explained 78% of the total variance. The factor structure was dominated by the first four factors. One factor contained psychosocial and social-contact items, and another factor depth-perception items from both questionnaires. A third factor contained seven items-only from the AS-20-on eye strain, stress, and difficulties with reading and with concentrating. A fourth factor contained seven items-only from the A&SQ-on fear of losing the better eye and visual disorientation, specific for amblyopia. Current visual acuity of the worse eye correlated with depth-perception items and vision-related items, whereas current binocular vision correlated with psychosocial and social-contact items, in 93 patients.Factor analysis suggests that the AS-20 and A&SQ measure a similar psychosocial quality-of-life domain. However, functional problems like avoidance of reading, difficulty in concentrating, eye stress, reading problems, inability to enjoy hobbies, and need for frequent breaks when reading are represented only in the AS-20. During the development of the A&SQ, asthenopia items were considered insufficiently specific for strabismus and were excluded a priori. The patients who generated the items for the AS-20 had, in majority, adulthood-onset strabismus and diplopia and were, hence, more likely to develop such complaints than our adult patients with childhood-onset strabismus and/or amblyopia.
