Project description:To describe the development and psychometric testing of the Seniors' Outdoor Survey (SOS), an instrument for evaluating how well the outdoor space in a long-term care setting supports the preferences and outdoor usage of residents.Content validity of the main SOS items initially was based on relevant literature and preliminary studies in diverse long-term care settings. After conducting a multiregional pilot study with 152 outdoor spaces at 68 assisted living facilities, the instrument was substantially revised and tested for interrater and test-retest reliability with 22 outdoor spaces at 12 long-term care settings, using 2 raters. Validity was examined using content analysis of resident survey responses (N = 1,128) from the multiregional study and specific item validation by subject matter experts (N = 53).The final instrument contains 60 ratable items organized in 5 domains: access to nature (14 items), outdoor comfort and safety (15 items), walking and outdoor activities (14 items), indoor-outdoor connection (11 items), and connection to the world (6 items). Intraclass correlation coefficient (ICC) estimates of interrater reliability were .91 for the overall instrument, ranging from .83 to .98 for the 5 domains. Interrater reliability (ICC) was above .70 for more than 79% of individual items. Test-retest reliability (ICC) was .92, ranging from .81 to .98 for domains.The SOS tool fills a gap in the available environmental assessment instruments, providing a reliable way for researchers, providers, and designers to evaluate and compare the supportive potential of outdoor spaces for long-term care residents.

Project description:Large-scale campus resembles a small "semi-open community," harboring disturbances from the exchanges of people and vehicles, wherein stressors such as temperature and population density differ among the ground surfaces of functional partitions. Therefore, it represents a special ecological niche for the study on microbial ecology in the process of urbanization. In this study, we investigated outdoor microbial communities in four campuses in Wuhan, China. We obtained 284 samples from 55 sampling sites over six seasons, as well as their matching climatic and environmental records. The structure of campus outdoor microbial communities which influenced by multiple climatic factors featured seasonality. The dispersal influence of human activities on microbial communities also contributed to this seasonal pattern non-negligibly. However, despite the microbial composition alteration in response to multiple stressors, the overall predicted function of campus outdoor microbial communities remained stable across campuses. The spatial-temporal dynamic patterns on campus outdoor microbial communities and its predicted functions have bridged the gap between microbial and macro-level ecosystems, and provided hints toward a better understanding of the effects of climatic factors and human activities on campus micro-environments.

Project description:Background: In different parts of the world new models of senior housing have rapidly appeared, which indicates that existing housing and care models are not fulfilling the hopes and needs of current and new generations of older people. Material and Methods: This qualitative study focuses on one type of communal senior housing complex located in a mid-sized town in Central Finland. The complex was designed to have accessible low-maintenance apartments and common spaces, and to be near easily accessible green spaces, amenities, services, and public transport. The complex has a part-time community coordinator. The minimum age limit is set at 55 years. The data consists of 36 qualitative interviews with residents (21 women, 15 men) aged 66-93, conducted between November 2018 and February 2019. The semi-structured interviews were recorded and transcribed. The data analysis focused on how different aspects of the manmade, natural, and social environment were portrayed in residents' descriptions of day-to-day life. Theoretical framework adopted for the study draws from the ideas of environmental and geographic gerontology. The data was analyzed using positioning analysis which is one form of discourse analysis. Results: The senior housing in this study fulfilled its promise of providing accessible a physical and social environment which encourages and enables residents to be physically active and independent, yet which also provides social activities and feeling safe. In this respect, the senior housing complex offered an environment which supports well-being and healthy aging. However, the residents' interpretations of what the senior housing complex represented varied. For some of the residents it was first and foremost a social place, which provided opportunities for social contacts and social activities. For some of the residents the most important were maintenance-free apartments and outdoor areas. The question remains as to how social practices, in the form of government policies and market systems can support the development of different kinds of senior housing which are affordable and accessible for all.

Project description:ImportanceWith rapid urbanization, understanding the role of residential environments in the development of depression has gained importance. However, the potential associations of residential space and density with depression have been understudied.ObjectiveTo investigate longitudinal associations of residential livable space and density with depression.Design, setting, and participantsThis longitudinal cohort study used data from 2 waves of the FAMILY Cohort, a population-based cohort in the Hong Kong Special Administrative Region, China, recruited between February 28, 2009, and March 28, 2011, at baseline and followed up between August 3, 2011, and June 19, 2013, at wave 2. Data were analyzed from September 1, 2020, through August 10, 2021. A total of 16 968 participants aged 16 years or older underwent assessment using the Patient Health Questionnaire-9 (PHQ-9) and had complete data across all variables.ExposuresResidential density was objectively assessed at 3 scales (within apartment, building block, and at neighborhood level) at baseline and follow-up.Main outcomes and measuresDepressive symptoms were defined in terms of a PHQ-9 threshold greater than or equal to 5 and probable major depression with a cutoff value greater than or equal to 10. A continuous PHQ-9 score was used as a secondary outcome. Multilevel logistic regression models were used to examine associations of the residential density with probable major depression and depressive symptoms adjusted for time-varying sociodemographic and lifestyle factors, comorbidities, and other environment variables.ResultsOf the 16 968 participants with complete data, the mean (SD) age was 45.5 (16.7) years, and 9328 participants (55.0%) were women at baseline. The mean follow-up was 2.2 years (range, 1.3-3.6 years). At the household level, after full adjustments, each IQR increment in residential livable area was associated with lower odds of probable major depression (adjusted odds ratio [aOR], 0.84; 95% CI, 0.71-0.98; P = .03) and depressive symptoms (aOR, 0.93; 95% CI, 0.86-1.00; P = .04). At the building-block level, each IQR increment in block density was associated with higher odds of depressive symptoms (aOR, 1.11; 95% CI, 1.01-1.22; P = .03), but only in single-housing environment models. The results remained consistent in models with continuous PHQ-9 evaluation.Conclusions and relevanceThis study's findings suggest that residential livable space and block density may play a role in the development of depression. Integrated mental health and housing policies promoting mental capital in cities, such as health-optimized density at the household and building-block levels, might serve as a population approach to reduce the burden of depression.

Project description:The human oral cavity has an indigenous microbiota known to include a robust community of viruses. Very little is known about how oral viruses are spread throughout the environment or to which viruses individuals are exposed. We sought to determine whether shared living environment is associated with the composition of human oral viral communities by examining the saliva of 21 human subjects; 11 subjects from different households and 10 unrelated subjects comprising 4 separate households. Although there were many viral homologues shared among all subjects studied, there were significant patterns of shared homologues in three of the four households that suggest shared living environment affects viral community composition. We also examined CRISPR (clustered regularly interspaced short palindromic repeat) loci, which are involved in acquired bacterial and archaeal resistance against invading viruses by acquiring short viral sequences. We analyzed 2?065?246 CRISPR spacers from 5 separate repeat motifs found in oral bacterial species of Gemella, Veillonella, Leptotrichia and Streptococcus to determine whether individuals from shared living environments may have been exposed to similar viruses. A significant proportion of CRISPR spacers were shared within subjects from the same households, suggesting either shared ancestry of their oral microbiota or similar viral exposures. Many CRISPR spacers matched virome sequences from different subjects, but no pattern specific to any household was found. Our data on viromes and CRISPR content indicate that shared living environment may have a significant role in determining the ecology of human oral viruses.

Project description:PurposeWe report the distribution of visual acuity impairment (VAI), contrast sensitivity impairment (CSI) and their associations with physical health in an aging population.MethodsIn this cross-sectional analysis, VAI was categorized as mild (20/40-20/60) and moderate or greater (<20/60) in the better eye for distance and near vision. CSI was categorized as moderate (1.04-1.50 logCS) and severe or profound (<1.04 logCS). Physical outcomes included the short physical performance battery (SPPB) scores, self-reported quality of life (QoL) scores, physical limitations, difficulty with activity of daily living (ADL) and instrumental ADL (IADL). The associations between VAI and CSI with physical outcomes were explored overall and by community.ResultsThere were 494 Black Jackson and 558 White Washington County participants. The mean age was 80 years, 63% were female, and 15% had VAIdistance presenting. Moderate or greater VAInear presenting was associated with higher prevalence of greater physical limitations (prevalence ratio, 1.25; 95% confidence interval, 1.09-1.44) and IADL difficulties (prevalence ratio, 1.77; 95% confidence interval, 1.32-2.38), but not ADL difficulties. Associations of VAIdistance presenting with physical limitations and lower SPPB scores, and CSI with physical limitations, IADL difficulties, lower QoL, and lower SPPB scores were found. A stratified analysis showed stronger associations in Jackson.ConclusionsVAI and CSI were associated with poor physical health. These associations should be understood in the context of community differences.Translational relevanceCommunity-based factors may mitigate the impact of vision loss on physical outcomes. Public health endeavors are needed to address VAI and CSI to optimize physical health in the older adults with poor vision.

Project description:ObjectivesThis study explores the gender difference in unmet need for assistance with activities of daily living among older adults with disabilities in China.DesignLogistic regression analysis was employed to examine the gender difference and identify influential factors among disabled male and female seniors.Setting23 provinces throughout China.ParticipantsA total of 1700 disabled seniors were included in the analysis.ResultsOf 1700 respondents, 619 (36.4%) were disabled male seniors. Overall, the possibility of unmet need for activity of daily living assistance among disabled female seniors was significantly lower than that among male group (OR 0.728; 95% CI 0.559 to 0.948) than males. Family care resources, economic status and loneliness were influential factors among disabled seniors regardless of genders. Furthermore, disabled female seniors from rural area (p=0.011), whose primary caregiver was willing to take care of them (p=0.022), whose community could provide daily life service (p=0.002) were more likely to have unmet need. Meanwhile, disabled female seniors whose community could provide medical service(p=0.001) were less likely to report unmet need.ConclusionsThe study showed that disabled male seniors were more likely to experience unmet need compared with female ones. Reducing unmet need for assistance with activities of daily living among disabled seniors and existing gender disparities therefore requires not only universal strategy, but also targeted policies which should be made or modified for disabled seniors of different genders.

Project description:Outdoor play is important for children's health and development, yet many preschool-aged children in childcare settings do not receive the recommended 60 minutes/day of outdoor play. Childcare providers have previously described parent-related barriers to increasing outdoor playtime, including parents not providing appropriate clothing for their children and parents' preference for academics over active play. This study explored parent perceptions and knowledge of outdoor playtime in childcare environments. On average, parents reported wanting their child to spend significantly more time playing outside during a full day of childcare than the recommended minimum. However, over one-half of parents reported that they did not know how much time their child actually spent playing outside and 43% reported that they did not know their childcare center's outdoor play policies.Childcare providers may over-perceive parent-related barriers to outdoor play. Parents generally support outdoor play for their preschooler during center-based childcare but are not well informed about outdoor playtime and policies. Encouraging communication between parents and early childhood educators about these topics could lead to more universal support and strategies for promoting outdoor and active play opportunities for children which are important for their health and development.

Project description:We examined the effect of proximity to specific mobile, area, and point sources on the residential outdoor concentrations of fine particulate matter PM (PM(2.5)) and several of its particle components. Integrated (48-h) PM(2.5) samples were collected outside non-smoking residences in Elizabeth, NJ, between summer 1999 and spring 2001. Samples were analyzed for PM(2.5) mass, organic and elemental carbon (OC and EC, respectively), trace elements, particle-phase polycyclic aromatic hydrocarbons (p-PAHs), and other important particle species. Information about the proximity of the study homes to potential mobile and area sources of OC, EC, p-PAHs, sulfur (S), and selenium (Se) (including urban interstate highways, local roadways, the Newark International Airport, the Elizabeth seaport, and a nearby refinery in Linden, NJ) were retrieved from a database that included detailed emissions, meteorological, and geographical data for the study area. The dependence of residential outdoor concentrations on source proximity and on various meteorological parameters was then examined for each species by multiple linear regression analysis. As expected, the predicted ambient air concentrations of all particle species (except S, Se) decreased with increasing distance from the sources. Although the enhancement in PM(2.5) and OC levels outside the study homes closest to primary PM sources was modest (e.g., 1.6 and 2.5 times the background levels 37 m from interstate highways), the elevation of EC and p-PAH concentrations was substantial outside the closest study homes (i.e., about 20 times for p-PAHs 37 m from interstate highways and about 14 times for EC 192 m from the refinery in Linden, NJ). The predicted EC concentrations 192 and 500 m from the oil refinery were 22.8 and 3.0 microgC/m(3), compared with an urban background of 1 microgC/m(3). Thus, emissions from this source might dramatically affect EC exposure for residents living in its close proximity.

Project description:BackgroundThe availability of outdoor recreational facilities is associated with increased leisure-time physical activity (PA). We investigated how much of this association is attributable to selection effects, and explored whether usage of recreational facilities was an explanatory mechanism.MethodsWe analysed data from 5199 participants in the SPOTLIGHT survey residing in five European urban regions. Adults completed a survey and a Google Street View-based virtual audit was conducted to objectively measure the availability of outdoor recreational facilities in the residential neighbourhood. We used negative binomial GEE models to examine the association between objective and subjective availability of outdoor recreational facilities and leisure-time PA, and explored whether this association was attenuated after adjustment for socioeconomic status and preference for neighbourhoods with recreational facilities (as indicators of self-selection). We examined whether reported use of recreational facilities was associated with leisure-time PA (as explanatory mechanism), and summarized the most important motivations for (not) using recreational facilities.ResultsSubjective - but not objective - availability of outdoor recreational facilities was associated with higher levels of total leisure-time PA. After adjustment for self-selection (which attenuated the association by 25%), we found a 25% difference in weekly minutes of total leisure-time PA between individuals with and without self-reported availability of outdoor recreational facilities. For our study population, this translates to about 28 min per week. Participants who reported outdoor recreational facilities to be present but indicated not to use them (RR?=?1.19, 95% CI?=?1.03;1.22), and those reporting outdoor recreational facilities to be present and to use them (RR?=?1.33, 95% CI?=?1.22, 1.45) had higher levels of total leisure-time PA than those who reported outdoor recreational facilities not to be present. Proximity to outdoor recreational facilities was the most important motivation for use.ConclusionThe modest attenuation in the association between availability of outdoor recreational facilities and self-reported leisure-time PA suggests that individuals' higher activity levels may be due more to the perceived availability of outdoor recreational facilities than to self-selection. The use of these facilities seemed to be an important underlying mechanism, and proximity was the main motivator for using recreational facilities.
