Project description:BackgroundWe aimed to assess the efficacy and safety of erenumab, a fully human monoclonal antibody inhibiting the calcitonin gene-related peptide receptor (CGRPr), for the prevention of migraine in a real-life setting.Main bodyWe included in our observational study all patients with episodic or chronic migraine treated with erenumab during the year 2019 in the Abruzzo region, central Italy, and with a 6-month follow-up. We included 89 patients; 76 (85.4%) received 6 doses of erenumab, 11 (12.4%) autonomously withdrew the drug due to perceived inefficacy, and 2 (2.2%) due to adverse events. Seventy-eight patients (87.6%) were female, with a mean age of 46.8 ± 11.2 years; 84 (94.4%) had chronic migraine, and 64 (71.9%) medication overuse. All patients had ≥2 prior preventive treatment failures. Fifty-three patients (69.7%) had a 50% decrease in monthly migraine days (MMDs) within the first three doses; 46 (71.9%) of 64 patients withdrew medication overuse. In the 76 patients who completed a 6-dose treatment, erenumab decreased median MMDs from 19 (interquartile range [IQR] 12-27.5) to 4 (IQR 2-9.5; P < 0.001), median monthly days of analgesic use from 10 (IQR 4.5-20) to 2 IQR 0-5; P < 0.001), and median monthly days of triptan use from 5 (IQR 0-15.5) to 1 (IQR 0-4; P < 0.001). We recorded 27 adverse events in 20 (22.5%) patients, the most common being constipation (13.5%). One adverse event, i.e. allergic reaction, led to treatment discontinuation in one patient.ConclusionsOur real-life data confirm the efficacy and tolerability of erenumab for the prevention of migraine in a difficult-to-treat population of patients with a high prevalence of chronic migraine and medication overuse.

Project description:The purpose of this retrospective study was to describe the causes of morbidity and mortality in free-ranging raptors admitted to a wildlife rehabilitation center (WRC) in Abruzzo Italy from 2005 to 2016 and the associated risk factors. A total of 2496 free-ranging raptors were included in the study. We analyzed the raptors' medical records, epidemiological information, bird characteristics, cause of admission, final diagnosis, and outcome. The prevalence rates of nocturnal and diurnal raptors were 49% and 51%, respectively. Nocturnal raptors showed trauma as the primary cause of admission (45.8%, 558/1219), followed by nestling (including birds on their first flight attempt or presumed abandoned by their parents) (39.2%, 478/1219), and starvation (5.6%, 68/1219). Diurnal raptors showed trauma (73.1%, 934/1277), starvation (12.1%, 155/1277), and nestling (5.8%, 74/1277) accordingly. A description of the dangers for wild birds of prey in the Abruzzo region was provided to assist in the planning of rescue and rehabilitation activities in the WRC. Finally, the cause of admission, GAP, and BCS can be used as prognostic factors during the bird entry process.

Project description:The complete genomic sequence of Usutu virus (USUV, genus Flavivirus, family Flaviviridae) strain MB119/06, detected in a pool of Culex pipiens mosquitoes in northeastern Spain (Viladecans, Catalonia) in 2006, was determined and analyzed. The phylogenetic relationship with all other available complete USUV genome sequences was established. The Spanish sequence investigated showed the closest relationship to the USUV prototype strain SA AR 1776 isolated in South Africa in 1959 (96.9% nucleotide and 98.8% amino acid identities). Conserved structural elements and enzyme motifs of the putative polyprotein precursor were identified. Unique amino acid substitutions were recognized; however, their potential roles as virulence markers could not be verified. Comparisons of the polyprotein precursor sequences of USUV strains detected in mosquitoes, birds, and humans could not confirm the predicted role of unique amino acid substitutions in relation to virulence in humans. Phylogenetic analysis of a partial coding section of the NS5 protein gene region indicated that USUV strains circulating in Europe form three different genetic clusters. Broad and targeted surveys for USUV in mosquitoes could reveal further details of the geographic distribution and genetic diversity of the virus in Europe and in Africa.

Project description:Listeria monocytogenes (Lm) is a foodborne pathogen causing listeriosis. Invasive forms of the disease mainly manifest as septicaemia, meningitis and maternal-neonatal infections. Lm-associated respiratory infections are very rare and little known. We reported two Lm respiratory infection cases occurred in Central Italy during the summer of 2020, in the midst of the SARS-CoV2 pandemic. In addition to collect the epidemiological and clinical characteristics of the patients, we used Whole Genome Sequencing to study the genomes of the Lm isolates investigating their virulence and antimicrobial profiles and the presence of genetic mobile elements. Both the strains belonged to hypervirulent MLST clonal complexes (CC). In addition to the Listeria Pathogenicity Island 1 (LIPI-1), the CC1 strain also carried LIPI-3 and the CC4 both LIPI-3 and LIPI-4. Genetic determinants for antimicrobial and disinfectants resistance were found. The CC1 genome presented prophage sequences but they did not interrupt the comK gene, involved in the phagosomal escape of Lm. None of the strains carried plasmids. Lm is an important, although rare, opportunistic pathogen for respiratory tract and lung infections. To avoid dangerous diagnostic delays of these severe clinical forms, it is important to sensitize hospital laboratories to this rare manifestation of listeriosis considering Lm in the differential diagnosis of respiratory infections.

Project description:The evaluation of leaf detritus processing (decomposition and breakdown) is one of the most simple and cost-effective method to assess the functional characteristics of freshwater ecosystems. However, in comparison with other freshwater habitats, information on leaf litter breakdown in spring ecosystems is still scarce and fragmentary. In this paper, we present results of the first application of a variant of the leaf-bags method to assess structure of macroinvertebrate assemblages and leaf-litter breakdown in a Central Apennines (Italy) cold spring which was investigated from July 2016 to October 2016. Notwithstanding the stable conditions of almost all hydrological and physico-chemical parameters, we found significant temporal differences in (i) % of mass loss of poplar leaves (ii) number of Ephemeroptera, Plecoptera and Trichoptera taxa, (iii) shredder and predator densities. We demonstrate that detritus processing in cold springs may be faster than or as fast as in warmer streams/rivers. Shredders activity and biocoenotic interactions, rather than temperature and nutrients load, were the main drivers of the process. A routine application of the modified leaf-bags may contribute to expand our knowledge on detritus processing in cold springs and may help to predict impacts of climate warming on freshwater ecosystem functioning.

Project description:Two different West Nile virus (WNV) strains caused lethal encephalitis in a flock of geese and a goshawk in southeastern Hungary in 2003 and 2004, respectively. During the outbreak in geese, 14 confirmed human cases of WNV encephalitis and meningitis were reported in the same area. Sequencing of complete genomes of both WNV strains and phylogenetic analyses showed that the goose-derived strain exhibits closest genetic relationship to strains isolated in 1998 in Israel and to the strain that emerged in 1999 in the United States. WNV derived from the goshawk showed the highest identity to WNV strains of lineage 2 isolated in central Africa. The same strain reemerged in 2005 in the same location, which suggests that the virus may have overwintered in Europe. The emergence of an exotic WNV strain in Hungary emphasizes the role of migrating birds in introducing new viruses to Europe.
