Utilisation of district health information system and its associated factors among health professionals working at public health facilities of the southwest of Ethiopia: cross-sectional survey.

ABSTRACT:

Objectives

To assess utilisation of district health information system and its associated factors among health professionals in the southwest of Ethiopia, 2020.

Setting

Public health facilities in the southwest of Ethiopia.

Participants

A facility-based cross-sectional study was conducted among a sample of 260 participants.

Main outcome measures

The main outcome measure was utilisation of the district health information system.

Results

Overall, 149 (57.3%) of study participants had good utilisation of district health information systems (95% CI 50 to 64.2). Sufficient skills (Adjusted Odds Ratio (AOR) 3.83, 95% CI 1.92 to 7.64), being trained (AOR 3.90, 95% CI 1.95 to 7.79), high motivation (AOR 3.93, 95% CI 1.99 to 7.76), feedback provided (AOR 2.93, 95% CI 1.53 to 5.77) and regular supervision (AOR 3.06, 95% CI 1.56 to 6.01) were associated with utilisation of district health information systems.

Conclusions

In general, more than half of the respondents had good utilisation of district health information systems. Providing regular supportive supervision and feedback, having good skills on district health information system use, high motivation and being trained on district health information system will help to bring good utilisation of district health information system for decision making.

Project description:BackgroundEssential newborn care (ENC) is the most vital care, particularly during the first hour of the newborn's life. This study aimed to assess health professionals' knowledge and practice of ENC at public health facilities in the Bench-Sheko Zone, southwest Ethiopia.MethodsA cross-sectional study was conducted among health professionals at selected public health facilities in the Bench-Sheko Zone of southwest Ethiopia. The data were collected using a self-administered, tested, and structured questionnaire. The data collected were entered in Epi info version 7 and analyzed using SPSS version 22. Bivariate and multivariable analyses were performed to determine the association between the dependent and independent variables. Categorical variables were presented in tabulations by frequencies and percentages. Continuous data were summarized by the mean score and standard deviation. Statistical significance was set at a p-value < 0.05.ResultsAmong the 157 respondents, 60 (38.2%) and 97 (61.8%) had good knowledge and good practice of ENC, respectively. The factors associated with good knowledge of ENC were being female (AOR = 0.72, 95% CI [0.48, 0.86]), the availability of on-the-job training (AOR = 2.00, 95% CI [1.01, 3.97]), and interest in working in the delivery room (AOR = 2.5, 95% CI [1.27, 4.94]). Better educational qualification (AOR = 4.12, 95% CI [1.67, 10.18]) and the availability of on-the-job training (AOR = 3.60, 95% CI [1.58, 8.18]) were the factors associated with good practice of ENC.ConclusionKnowledge of essential newborn care among health professionals was very low; however, the practice of essential newborn care was somewhat average compared to other studies in Ethiopia. Being female, the availability of on-the-job training and interest in working in the delivery room were the factors associated with a good knowledge of ENC; while educational qualification and the availability of on-the-job training were the factors associated with a good practice of ENC. Therefore, concerned bodies should consider the provision of refreshment on-the-job training, upgrading the qualification of health professionals, and providing incentives and motivators to improve interest in working in the delivery room.

Project description:BackgroundEthiopia has the potential to use the district health information system, which is a building block of the health system. Thus, it needs to assess the performance level of the system by identifying the satisfaction of end users. There is little evidence about users' satisfaction with using this system. As a result, this study was conducted to fill this gap by evaluating user satisfaction and associated factors of district health information system among healthcare providers in Ethiopia, using the information system success model.MethodsAn institutional-based cross-sectional study was conducted from November to December 2022 in the Oromia region of southwest Ethiopia. A total of 391 health professionals participated in the study. The study participants were selected using a census. Using a self-administered questionnaire, data were collected. Measurement and structural equation modeling analyses were used to evaluate reliability, the validity of model fit, and to test the relationship between the constructs, respectively, using analysis of moment structure (AMOS) V 26.ResultsSystem quality had a positive direct effect on the respondent's system use (β = 0.18, P-value < 0.001), and satisfaction (β = 0.44, P-value < 0.001). Service quality had also a direct effect on the respondent's system use (β = 0.37, P-value < 0.01), and satisfaction with using the district health information system (β = 0.36, P-value < 0.01). Similarly, system use had also a direct effect on the respondent's satisfaction (β = 0.53, P-value < 0.05). Moreover, computer literacy had a direct effect on the respondent's system use (β = 0.63, P-value < 0.05), and satisfaction (β = 0.51, P-value < 0.01).ConcussionsThe overall user satisfaction with using the district health information system in Ethiopia was low. System quality, service quality, and computer literacy had a direct positive effect on system use and user satisfaction. In addition, system use and information quality had a direct positive effect on healthcare professionals' satisfaction with using the district health information system. The most important factor for enhancing system use and user satisfaction was computer literacy. Accordingly, for the specific user training required for the success of the district health information system in Ethiopia, the manager should offer additional basic computer courses for better use of the system.

Project description:BackgroundRoutine health information systems (RHIS) are vital for the acquisition of data for health sector planning, monitoring, and evaluation. However, in developing countries the insufficient quality of the data produced by RHIS limits their usefulness in decision-making. As routine health information utilization is still low in Ethiopia, this study aimed to assess the magnitude of routine health data utilization and associated factors among health care professionals in some public health institutions in North Gondar, northwest Ethiopia.MethodsAn institution based cross-sectional study was conducted from March to April2017, at public health institutions of North Gondar Zone, northwest Ethiopia. A total of 720 health care professionals were selected from public health institutions using the multi-stage sampling technique. Data were collected using a structured self-administered questionnaire and an observational checklist, cleaned, coded, and entered into Epi-info version 3.5.3 and transferred into SPSS version 20 for further statistical analysis. In the multiple logistic regression analysis, a less than 0.05 P-vale was considered statistically significant.ResultIn this study, the level of good routine health information utilization among health professionals was 78.5% (95% CI: 73.2%, 84.3%). According to the multivariable logistic regression analysis, sex (AOR?=?2.19, 95% CI: 1.47, 3.27), type of institution (AOR?=?3.57, 95% CI: 2.39, 5.32), standard indicators (AOR?=?3.28, 95% CI: 1.90, 5.65), data analysis skills (AOR?=?1.90, 95% CI: 1.12, 3.23), and good governance (AOR?=?1.97, 95% CI: 1.31, 2.95), were found significantly associated with a good level of health information utilization.ConclusionOver three-fourths of the health care professionals working at public health institutions of North Gondar utilized health information better than the respondents in previous studies. Sex, type of institution, standard indicators, data analysis skills, and governance were factors related to routine health information utilization. Therefore, standard indicators, data analysis skills and good governance are highly recommended for improving routine health data utilization of health care professionals working at public health institutions.

Project description:BACKGROUND:World Health Organization (WHO) predicted that there will be a shortfall of skilled healthcare by 2035 with the greatest shortfall in Africa and Southeast Asia due to satisfaction with payment and incentives. Low job satisfaction of health workers can result in increased staff turnover and absenteeism, which affects the efficiency of health services. Ethiopia has been affected by a shortage of health professionals due to a brain drain of health professionals. Our study, therefore, aimed at assessing the impact of the working environment and benefits packages on the level of satisfaction among health professionals working in selected public Health facilities in Eastern Ethiopia. METHODS:Institutional based Cross-sectional study design was conducted among 422 selected health professionals in Bale Zone Public Health Facilities. After selecting 2 hospitals and 32 health centers by lottery method, proportional allocation of the sample was done for selected Hospitals and Health Centers. Then, to select individual health professional from each health center and hospital, a systematic sampling method was employed using the worker's registration log book. Then, data were collected, cleaned and entered into EpiData software version 3.1 and then exported to IBM SPSS version 21 for analysis. Both descriptive and inferential statistics were done. The principal component analysis was employed for all Likert scale instruments to extract factor(s) representing each of the scales and have factor scores, which facilitate treatment of the variables as continuous during further analysis. Using this regression factor score, multiple linear regression analysis was performed and the effect of independent variables on the regression factor score of the outcome variable was quantified. A significance level of less than 0.05 was used in all cases to judge statistical significance. RESULT:This study showed that the prevalence of job satisfaction of health professionals was 38.5% (95%CI: 33.82-43.2%). Age of health professionals ((??=?0.252, (95% CI: 0.067, 0.437))), type of health facility (??=?-?0.280, (95% CI; -?0.519, -?0.041), service year (??=?0.487, (95%CI: 0.025, 0.998)), supply they need to do their job (??=?0.10, (95% CI: 0.009 to 0.19)), perception of health professional on allowances (??=?-?0.216, (95% CI: -?0.306, -?0.125)) and perception of health professionals on employment benefits (??=?0.225, (95% CI: 0.135 to 0.315)) were statistically significant that affect job satisfaction factor score. CONCLUSION:level of job satisfaction of health professionals was found to be low. Level of job satisfaction was influenced by the age of the health professionals, type of health facility in which they were working, years of service they had in the health sectors, their working environment, professional allowance and benefits like financial rewards and benefits of being employed. Hence, policy makers and health managers need to pay special attention to increase the satisfaction of the health workforce at all levels in the health system. Moreover, special emphasis should be given for the benefits packages of health workers at different levels.

Project description:Globally, the COVID-19 pandemic has significantly increased morbidity and mortality. Health professionals are at the frontline of COVID-19 pandemic exposure and are identified as a priority target group that needs to receive COVID-19 vaccines. Data on Ethiopian healthcare workers' use of the COVID-19 vaccine and associated factors, however, are scarce. Using a simple random sampling method, 398 health professionals were recruited through an institutional-based cross-sectional study design. Health professionals working in Debre Markos town public health facilities filled out a self-administered questionnaire that had been pretested to obtain the data. Then, the data were entered into Epi data version 4.2; and analyzed by SPSS 25. Descriptive statistics and multivariable logistic regression analysis were computed after model assumptions were checked. The adjusted odds ratio with 95% CI was calculated and statistical significance was declared at P-value < 0.05 after model adequacy was checked using the Hosmer-Lemeshow test. The uptake of the COVID-19 vaccine among health professionals was 61.56% (95% CI: 56.67%, 66.23%). Factors associated with the uptake of the COVID-19 vaccine were age > = 35 years (AOR: 4.39, 95% CI: 1.89, 10.19), having a higher income (>9056 Birr) (AOR: 1.79, 95% CI: 1.03, 3.10), who practiced COVID-19 Prevention methods (AOR: 2.39, 95% CI: 1.51, 3.77), Adulthood previous other immunization histories (AOR: 1.63, 95% CI: 1.15, 2.56) and having a chronic disease (AOR:1.90,95% CI: 1.07, 3.74). This study revealed that the uptake of the COVID-19 vaccine was low. Age > = 35 years, having a higher income, who practiced COVID-19 prevention methods, having adulthood previous immunization histories, and having chronic disease were statistically significantly identified factors for COVID-19 vaccine uptake. Therefore, policymakers and health managers should think about the requirement of immunization of healthcare workers and develop plans for administering the COVID-19 vaccine.

Project description:IntroductionAdolescents and youths who need exceptional healthcare are the shapers and leaders of our global future. However, many of them are died prematurely, while others suffer from diseases partly because of the poor quality of health services. Thus, this study aimed to assess the quality of Adolescent and Youth-friendly Health Services (AYFHS) and associated factors in the public health facilities of Dehana district, Northeast Ethiopia.MethodsA facility-based quantitative cross-sectional study supported with a qualitative component was conducted from February 24 to March 30, 2020. The quality of AYFHS was measured using the Donbidean framework (structure, process and output component). Accordingly, a total of 431 adolescents and youths, five health facilities, twenty-five client-provider interaction observations, and nine key informant interviews were conducted. Binary logistic regression analysis was done, and variables with a p-value of < 0.05 and Adjusted Odds Ratio (AOR) with its 95% Confidence Interval (CI) were used to measure the association. The qualitative data were audio-recorded and transcribed verbatim. Then, Open Code 4.03 software was used to manage and analyze the data.ResultsThe quality of adolescent and youth-friendly health services was 58.8, 46.4, and 47.2% for structural, process, and output quality dimensions, respectively. The predictor variables for output quality were, being a student (AOR: 2.07, 95%CI: 1.07-3.40), farmers (AOR: 2.59, 95%CI: 1.25-5.39), own income sources (AOR: 1.99, 95%CI: 1.03-3.85), exempted services (AOR: 2.30, 95%CI: 1.43-3.71) and long waiting time (AOR: 3.8495%CI: 1.80-8.23).ConclusionsThe overall quality of adolescent and youth-friendly health services was still lower than the WHO good quality standards. The structural quality dimension was affected by the unavailability of adequate and trained health service providers, poor engagement of adolescents and youths in the facility governance structure, unavailability of guidelines, protocols and procedures. In contrast, the process quality dimension was also compromised due to the provider's poor compliance with the national AYFHS guidelines. Therefore, health facilities need to engage adolescents and youths in the health facility governance structure, and providers should comply with the national guideline.

Project description:BackgroundRecent evidence suggests that mistreatment of women during childbirth is a global challenge facing health care systems. This study seeks to explore the prevalence of mistreatment of women in public health facilities of Ethiopia, and identify associated factors.MethodsA two-stage cross sectional sampling design was used to select institutions and women. The study was conducted in hospitals and health centers across four Ethiopian regions. Quantitative data were collected from postpartum women. Mistreatment was measured using four domains: (1) physical abuse, (2) verbal abuse, (3) failure to meet professional standards of care, and (4) poor rapport between women and providers. Percentages of mistreatment and odds ratios for the association between its presence and institutional and socio demographic characteristics of women were calculated using bivariate and multivariable logistic regression modeling.ResultsA total of 379 women were interviewed, of whom 281 (74%) reported any mistreatment. Physical and verbal abuse were reported by 7 (2%) and 31 (8%) women interviewed respectively. Failure to meet professional standards of care and poor rapport between women and providers were reported by 111 (29%) and 274 (72%) women interviewed respectively. Multivariable logistic regression analysis revealed that the odds of reporting mistreatment were higher among women with four or more previous births (aOR?=?3.36 95%CI 1.22,9.23, p?=?0.019) compared to women with no previous childbirth, Muslim women (aOR?=?3.30 95%CI 1.4,7.77, p?=?0.006) and women interviewed in facilities with less than 17 births per MNH staff in a month (aOR?=?3.63 95%CI 1.9,6.93, p?<?0.001). However, the odds of reporting mistreatment were lower among women aged 35 and older (aOR?=?0.22 95%CI 0.06, 0.73, p?=?0.014) and among women interviewed between 8 and 42?days after childbirth (aOR?=?0.37 95%CI 0.15, 0.9, p?=?0.028).ConclusionMistreatment during childbirth in Ethiopia is commonly reported. Health workers need to consider provision of individualized care for women and monitor their experiences in order to adjust quality of their services.
