Utilisation of district health information system and its associated factors among health professionals working at public health facilities of the southwest of Ethiopia: cross-sectional survey.

ABSTRACT:

Objectives

To assess utilisation of district health information system and its associated factors among health professionals in the southwest of Ethiopia, 2020.

Setting

Public health facilities in the southwest of Ethiopia.

Participants

A facility-based cross-sectional study was conducted among a sample of 260 participants.

Main outcome measures

The main outcome measure was utilisation of the district health information system.

Results

Overall, 149 (57.3%) of study participants had good utilisation of district health information systems (95% CI 50 to 64.2). Sufficient skills (Adjusted Odds Ratio (AOR) 3.83, 95% CI 1.92 to 7.64), being trained (AOR 3.90, 95% CI 1.95 to 7.79), high motivation (AOR 3.93, 95% CI 1.99 to 7.76), feedback provided (AOR 2.93, 95% CI 1.53 to 5.77) and regular supervision (AOR 3.06, 95% CI 1.56 to 6.01) were associated with utilisation of district health information systems.

Conclusions

In general, more than half of the respondents had good utilisation of district health information systems. Providing regular supportive supervision and feedback, having good skills on district health information system use, high motivation and being trained on district health information system will help to bring good utilisation of district health information system for decision making.

Project description:ObjectiveThe study aimed to assess health management information utilisation and associated factors among health professionals working at public health facilities in North Wollo Zone, Northeast Ethiopia.SettingThe study was conducted at public health facilities in the North Wollo Zone, Northeast Ethiopia.ParticipantsA total of 664 (56.3% male and 43.7% female) health professionals participated in the study. All health professionals permanently working in North Wollo Zone were included in this study. However, health professionals who were not present during the data collection period by any means and who had less than 6 months of experience were not included in this study.Primary and secondary outcome measuresThe main outcome measure was health management information utilisation.ResultAbout 58.4% (n=388) (95% CI: 54.4% to 62.0%) of the study participants use health management information. The multivariable logistic regression model indicated that participants who had managerial positions are more likely to use health management information with an adjusted OR (AOR) of 3.11 and 95% CI 1.84 to 5.24. Similarly, having a good motivation level (AOR=4.42 (95% CI: 2.82 to 6.93)), perceived good culture of health information (AOR=6.17 (95% CI: 3.35 to 11.36)), a standard set of indicators (AOR=4.11 (95% CI: 2.65 to 6.38)), having good governance of health information system (AOR=1.75 (95% CI:1.13 to 2.72)) and health management information system (HMIS) training (AOR=3.10 (95% CI: 1.89 to 5.07)) were the predictors positively associated with higher utilisation of health management information.ConclusionThis study revealed that utilisation of health management information was still inadequate. Enhancing motivation, building a culture of information use, having standardised indicators, strengthening the governance of health information systems and comprehensive HMIS training were measures to be taken to improve utilisation of health management information in this study setting.

Project description:BackgroundThe level of health professional work engagement affects retention, burnout, job satisfaction, patient satisfaction, and outcomes; however, there is a paucity of evidence that benefit health professional work engagement. Therefore, this study aimed to assess work engagement and associated factors among health professionals at public health facilities in the Bench-Sheko zone in southwest Ethiopia.MethodsFacility-based cross-sectional study was conducted among 605 health professionals from 29 March to 29 April 2021. A simple random sampling technique was used to select the participants. Data were collected using a self-administered questionnaire. Linear regression was fitted and those variables with p-value < 0.2 in simple linear regression were entered into multiple linear regression analysis. Unstandardized β-coefficient with 95% CI and p-value < 0.05 were used as the cut of points to determine the factors associated with work engagement.ResultsMean score percentage of work engagement was 71.8%. Health center staff (β = 0.31; 95% CI: 0.22, 0.40), married professionals (β = 0.10; 95% CI: 0.005, 0.17), co-worker support (β = 0.06; 95% CI: 0.004, 0.11), role clarity (β = 0.14; 95% CI: 0.07, 0.21), reward (β = 0.10; 95% CI: 0.05, 0.15), resilience (β = 0.14 95%; CI: 0.07, 0.21), self-efficacy (β = 0.24; 95% CI: 0.16, 0.31) and optimism (β = 0.20; 95% CI: 0.15, 0.26) were positively associated with work engagement. On the contrary, cognitive demand (β= -0.06; 95% CI: -0.11, -0.01) was negatively associated with work engagement.ConclusionIn this study, health professionals had a moderate level of work engagement. Health facilities shall improve their culture of co-worker support, role clarity, reward, resilience, self-efficacy, and optimism to enhance work engagement. Future researchers shall be done further studies to evaluate the relationship between cognitive demand and work engagement among health professionals.

Project description:BackgroundEssential newborn care (ENC) is the most vital care, particularly during the first hour of the newborn's life. This study aimed to assess health professionals' knowledge and practice of ENC at public health facilities in the Bench-Sheko Zone, southwest Ethiopia.MethodsA cross-sectional study was conducted among health professionals at selected public health facilities in the Bench-Sheko Zone of southwest Ethiopia. The data were collected using a self-administered, tested, and structured questionnaire. The data collected were entered in Epi info version 7 and analyzed using SPSS version 22. Bivariate and multivariable analyses were performed to determine the association between the dependent and independent variables. Categorical variables were presented in tabulations by frequencies and percentages. Continuous data were summarized by the mean score and standard deviation. Statistical significance was set at a p-value < 0.05.ResultsAmong the 157 respondents, 60 (38.2%) and 97 (61.8%) had good knowledge and good practice of ENC, respectively. The factors associated with good knowledge of ENC were being female (AOR = 0.72, 95% CI [0.48, 0.86]), the availability of on-the-job training (AOR = 2.00, 95% CI [1.01, 3.97]), and interest in working in the delivery room (AOR = 2.5, 95% CI [1.27, 4.94]). Better educational qualification (AOR = 4.12, 95% CI [1.67, 10.18]) and the availability of on-the-job training (AOR = 3.60, 95% CI [1.58, 8.18]) were the factors associated with good practice of ENC.ConclusionKnowledge of essential newborn care among health professionals was very low; however, the practice of essential newborn care was somewhat average compared to other studies in Ethiopia. Being female, the availability of on-the-job training and interest in working in the delivery room were the factors associated with a good knowledge of ENC; while educational qualification and the availability of on-the-job training were the factors associated with a good practice of ENC. Therefore, concerned bodies should consider the provision of refreshment on-the-job training, upgrading the qualification of health professionals, and providing incentives and motivators to improve interest in working in the delivery room.

Project description:BackgroundEthiopia has the potential to use the district health information system, which is a building block of the health system. Thus, it needs to assess the performance level of the system by identifying the satisfaction of end users. There is little evidence about users' satisfaction with using this system. As a result, this study was conducted to fill this gap by evaluating user satisfaction and associated factors of district health information system among healthcare providers in Ethiopia, using the information system success model.MethodsAn institutional-based cross-sectional study was conducted from November to December 2022 in the Oromia region of southwest Ethiopia. A total of 391 health professionals participated in the study. The study participants were selected using a census. Using a self-administered questionnaire, data were collected. Measurement and structural equation modeling analyses were used to evaluate reliability, the validity of model fit, and to test the relationship between the constructs, respectively, using analysis of moment structure (AMOS) V 26.ResultsSystem quality had a positive direct effect on the respondent's system use (β = 0.18, P-value < 0.001), and satisfaction (β = 0.44, P-value < 0.001). Service quality had also a direct effect on the respondent's system use (β = 0.37, P-value < 0.01), and satisfaction with using the district health information system (β = 0.36, P-value < 0.01). Similarly, system use had also a direct effect on the respondent's satisfaction (β = 0.53, P-value < 0.05). Moreover, computer literacy had a direct effect on the respondent's system use (β = 0.63, P-value < 0.05), and satisfaction (β = 0.51, P-value < 0.01).ConcussionsThe overall user satisfaction with using the district health information system in Ethiopia was low. System quality, service quality, and computer literacy had a direct positive effect on system use and user satisfaction. In addition, system use and information quality had a direct positive effect on healthcare professionals' satisfaction with using the district health information system. The most important factor for enhancing system use and user satisfaction was computer literacy. Accordingly, for the specific user training required for the success of the district health information system in Ethiopia, the manager should offer additional basic computer courses for better use of the system.

Project description:BackgroundRoutine health information systems (RHIS) are vital for the acquisition of data for health sector planning, monitoring, and evaluation. However, in developing countries the insufficient quality of the data produced by RHIS limits their usefulness in decision-making. As routine health information utilization is still low in Ethiopia, this study aimed to assess the magnitude of routine health data utilization and associated factors among health care professionals in some public health institutions in North Gondar, northwest Ethiopia.MethodsAn institution based cross-sectional study was conducted from March to April2017, at public health institutions of North Gondar Zone, northwest Ethiopia. A total of 720 health care professionals were selected from public health institutions using the multi-stage sampling technique. Data were collected using a structured self-administered questionnaire and an observational checklist, cleaned, coded, and entered into Epi-info version 3.5.3 and transferred into SPSS version 20 for further statistical analysis. In the multiple logistic regression analysis, a less than 0.05 P-vale was considered statistically significant.ResultIn this study, the level of good routine health information utilization among health professionals was 78.5% (95% CI: 73.2%, 84.3%). According to the multivariable logistic regression analysis, sex (AOR = 2.19, 95% CI: 1.47, 3.27), type of institution (AOR = 3.57, 95% CI: 2.39, 5.32), standard indicators (AOR = 3.28, 95% CI: 1.90, 5.65), data analysis skills (AOR = 1.90, 95% CI: 1.12, 3.23), and good governance (AOR = 1.97, 95% CI: 1.31, 2.95), were found significantly associated with a good level of health information utilization.ConclusionOver three-fourths of the health care professionals working at public health institutions of North Gondar utilized health information better than the respondents in previous studies. Sex, type of institution, standard indicators, data analysis skills, and governance were factors related to routine health information utilization. Therefore, standard indicators, data analysis skills and good governance are highly recommended for improving routine health data utilization of health care professionals working at public health institutions.

Project description:ObjectivesThis study investigated information security behaviors of professionals working in the public health sector to guide policymakers toward focusing their investments in infrastructure and training on the most vulnerable segments. We sought to answer the following questions: (1) Are certain professional demographics more vulnerable to cybersecurity threats? (2) Do professionals in different institution types (i.e., hospitals vs. primary care clinics) exhibit different cybersecurity behaviors? (3) Can Internet usage behaviors by professionals be indicative of their cybersecurity awareness and the risk they introduce?MethodsA cross-sectional, anonymous, paper-based survey was distributed among professionals working in public health care organizations in Kuwait. Data were collected about each professional's role, experience, work environment, cybersecurity practices, and understanding to calculate a cybersecurity score which indicates their level of compliance to good cybersecurity practices. We also asked about respondents' internet usage and used K-means cluster analysis to segment respondents into three groups based on their internet activities at work. Ordinary least squares regression assessed the association between the collected independent variables in question on the overall cybersecurity behavior.ResultsA total of 453/700 (64%) were responded to the survey. The results indicated that professionals with more work experience demonstrated higher compliance with good cybersecurity practices. Interestingly, nurses demonstrate higher cybersecurity aptitude relative to physicians. Professionals that were less inclined to use the internet for personal use during their work demonstrated higher cybersecurity aptitude.ConclusionOur findings provide some guidance regarding how to target health care professional training to mitigate cybersecurity risks. There is a need for ensuring that physicians receive adequate cybersecurity training, despite the opportunity costs and other issues competing for their attention. Additionally, classifying professionals based on their internet browsing patterns may identify individuals vulnerable to cybersecurity incidents better than more discrete indicators such as age or gender.

Project description:BackgroundWorld Health Organization (WHO) predicted that there will be a shortfall of skilled healthcare by 2035 with the greatest shortfall in Africa and Southeast Asia due to satisfaction with payment and incentives. Low job satisfaction of health workers can result in increased staff turnover and absenteeism, which affects the efficiency of health services. Ethiopia has been affected by a shortage of health professionals due to a brain drain of health professionals. Our study, therefore, aimed at assessing the impact of the working environment and benefits packages on the level of satisfaction among health professionals working in selected public Health facilities in Eastern Ethiopia.MethodsInstitutional based Cross-sectional study design was conducted among 422 selected health professionals in Bale Zone Public Health Facilities. After selecting 2 hospitals and 32 health centers by lottery method, proportional allocation of the sample was done for selected Hospitals and Health Centers. Then, to select individual health professional from each health center and hospital, a systematic sampling method was employed using the worker's registration log book. Then, data were collected, cleaned and entered into EpiData software version 3.1 and then exported to IBM SPSS version 21 for analysis. Both descriptive and inferential statistics were done. The principal component analysis was employed for all Likert scale instruments to extract factor(s) representing each of the scales and have factor scores, which facilitate treatment of the variables as continuous during further analysis. Using this regression factor score, multiple linear regression analysis was performed and the effect of independent variables on the regression factor score of the outcome variable was quantified. A significance level of less than 0.05 was used in all cases to judge statistical significance.ResultThis study showed that the prevalence of job satisfaction of health professionals was 38.5% (95%CI: 33.82-43.2%). Age of health professionals ((β = 0.252, (95% CI: 0.067, 0.437))), type of health facility (β = - 0.280, (95% CI; - 0.519, - 0.041), service year (β = 0.487, (95%CI: 0.025, 0.998)), supply they need to do their job (β = 0.10, (95% CI: 0.009 to 0.19)), perception of health professional on allowances (β = - 0.216, (95% CI: - 0.306, - 0.125)) and perception of health professionals on employment benefits (β = 0.225, (95% CI: 0.135 to 0.315)) were statistically significant that affect job satisfaction factor score.Conclusionlevel of job satisfaction of health professionals was found to be low. Level of job satisfaction was influenced by the age of the health professionals, type of health facility in which they were working, years of service they had in the health sectors, their working environment, professional allowance and benefits like financial rewards and benefits of being employed. Hence, policy makers and health managers need to pay special attention to increase the satisfaction of the health workforce at all levels in the health system. Moreover, special emphasis should be given for the benefits packages of health workers at different levels.

Project description:ObjectivesTo assess work-related stress (WRS) and associated factors among health professionals working in Ambo town governmental health facilities, in 2021.DesignInstitution-based analytical cross-sectional study.SettingInstitution-based analytical cross-sectional study was conducted in Ambo town from 15 July 2021 to 15 August 2021. A simple random sampling technique was used to select 420 participants. Data were collected by using structured self-administered questionnaire. Descriptive statistics was used to show the magnitude of WRS. Multivariate logistic regression was employed to identify variables that are significantly associated with WRS at 95% CI and p value <0.05.ParticipantsA total of 407 participants were involved in the study making the response rate 96.9%. The age of the respondents ranged from 20 to 52 with the mean age of 29 years (SD=4.8). Among all participants, 261 (64.1%) were male health professionals.ResultsThis study found that the overall prevalence of WRS was 52.33% (47.5, 57.2). Being female (adjusted OR (AOR)=1.73, 95% CI 1.06, 2.81), home-work interface (AOR=1.93, 95% CI 1.19, 3.14), job insecurity (tendency not knowing continue current job (AOR=3.22, 95% CI 1.87, 5.56) and major life events (serious injury to close relatives (AOR=3.13, 95% CI 1.68, 5.84), death of close relative (AOR=2.09, 95% CI 1.16, 3.77), being violated by other (AOR=3.10, 95% CI 1.65, 5.83) and anything else seriously upset (AOR=2.63, 95% CI 1.60, 4.32) were factors significantly associated with WRS.ConclusionThe prevalence of WRS among health professionals working in Ambo town public health facilities was high as compared with other studies. Sex, job insecurity, home-work interface and occurrence of major life events were factors that positively related to WRS and make the work of health professionals stressful. Health facilities, programme managers and policy makers should consider those identified factors while designing public health interventions to reduce WRS among health professionals.
