The Walker & Avant (2005) concept analysis method was followed.Methods
A search of PubMed, Academic Search Complete, CINAHL, ERIC, Google and Google Scholar was performed.Results
Through this process, uses of the concept were assessed including definitions and categories of tremors. Defining attributes were found to include "movement disorder," "shaking motions," "involuntary," "oscillatory," "rhythmic," "not painful or life threatening," "always present but variable" and "can sometimes be repressed." We identified two model cases and a borderline case, antecedents, consequences and empirical referents (including measurement tools) of tremors.Conclusion
The concept analysis process has clarified and illuminated an operational definition of tremors: that tremors are a movement disorder characterized by shaking motions that are involuntary, oscillatory, rhythmic, non-painful, always present although vary in severity, and can be repressed by changing posture or going into a rest position.

Project description:PurposeOphthalmic surgery involves the manipulation of micron-level sized structures such as the internal limiting membrane where tactile sensation is practically absent. All humans have physiologic tremors that are of low amplitude and not discernible to the naked eye; they do not adversely affect the majority of the population's daily functioning. However, during microsurgery, such tremors can be problematic. In this review, we focus on the impact of physiological tremors on ophthalmic microsurgery and offer a comparative discussion on the impact of such tremors on other surgical specialties.MethodsA single investigator used the MEDLINE database (via PubMed) to search for and identify articles for inclusion in this systematic review. Ten key factors were identified as potentially having an impact on tremor amplitude: beta-blockers, muscle fatigue, robotic systems, handheld tools/micromanipulators, armrests/wrist supports, caffeine, diet, sleep deprivation, consuming alcohol, and workouts (exercise). These key terms were then searched using the advanced Boolean search tool and operators (i.e., AND, OR) available on PubMed: (*keyword*) AND (surgeon tremor OR microsurgery tremor OR hand steadiness OR simulator score).ResultsTen studies attempted to quantify the baseline severity of operator physiologic tremor. Approximately 89% of studies accessing the impact of tremors on performance in regards to surgical metrics reported an improvement in performance compared to 57% of studies concluding that tremor elimination was of benefit when considering procedural outcomes.ConclusionsRobotic technology, new instruments, exoskeletons, technique modifications, and lifestyle factors have all demonstrated the potential to assist in overcoming tremors in ophthalmology.

Project description:Tremors are reported as the most frequent neurological manifestation of Wilson's disease (WD) in some series. Postural tremors, rest tremors, action tremors and wing-beating (rubral) tremors are the different types of tremors seen in WD. We report a patient of WD with unilateral rubral tremors refractory to 1-year therapy with Penicillamine and anti-tremor medications. The tremors decreased considerably after adding chelation therapy with dimercaprol. Combination of Penicillamine and dimercaprol is an effective decoppering measure in rubral tremors of WD.

Project description:Background and objectiveEssential tremor (ET) lacks effective treatments because its underlying mechanism is largely unknown, but may involve gut microbiota via the microbiome-gut-brain axis. We explored the effects of gut microbiota on ET in mice.MethodsSpecific pathogen-free C57BL/6J mice were gavaged with stools from ET patients or matched healthy individuals. After 3 weeks of gavaging, behavioral tests were performed on all mice. Next, each mouse was injected with harmaline to induce tremors. The tremor duration was recorded; the tremor score was estimated every 30 min. Behavioral tests were repeated after modeling. Intestinal tissues and fecal samples of the mice were examined using histology and 16Sr DNA sequencing, respectively.ResultsCompared with mice receiving microbiota from healthy controls, mice receiving fecal suspensions from ET patients showed worse performance in the pre-modeling behavioral tests. After modeling, ET-group mice showed significantly greater tremor scores, longer tremor duration, and worse motor performance. They also had significantly lower body weight and lower fecal pellet count. Pathological scoring revealed more severe intestinal lesions in ET-group mice. The 16S rDNA sequencing data revealed significant differences in microbiota indices, and a correlation between these indices and tremors in mice. Functional predictions indicated that the abundance of GABA-related enzymes was altered in ET-group mice.ConclusionMice transplanted with gut microbiota from ET patients showed worse performance in behavioral tests. After modeling, ET-group mice presented longer tremor duration, higher tremor score, and worse motor performance. This study provides evidence for gut microbiota dysbiosis that may affect the pathogenesis of ET.

Project description:The relative motion of tectonic plates is accommodated at boundary faults through slow and fast ruptures that encompass a wide range of source properties. Near the Parkfield segment of the San Andreas fault, low-frequency earthquakes and slow-slip events take place deeper than most seismicity, at temperature conditions typically associated with stable sliding. However, laboratory experiments indicate that the strength of granitic gouge decreases with increasing temperature above 350°C, providing a possible mechanism for weakening if temperature is to vary dynamically. Here, we argue that recurring low-frequency earthquakes and slow-slip transients at these depths may arise because of shear heating and the temperature dependence of frictional resistance. Recurring thermal instabilities can explain the recurrence pattern of the mid-crustal low-frequency earthquakes and their correlative slip distribution. Shear heating associated with slow slip is sufficient to generate pseudotachylyte veins in host rocks even when fault slip is dominantly aseismic.

Project description:The Arunachal Himalaya has been hosting some notable events in the recent past. The tectonic history of Arunachal Himalaya is complex and has been influenced by several major tectonic events, including the 1950 Mw8.6 Assam-Tibet earthquake. In this study, we explored the effect of dynamic stresses generated by teleseismic events on the triggering of seismicity in the region. We analyzed 34 large teleseismic events since 2010 and found triggering during six events. The change in seismicity was also confirmed by analysis with the STA/LTA method. The triggering in the region occurred in the form of earthquakes and tremors. The dynamic stress as low as 1 kPa was found capable of triggering. The back-azimuth angle does not play an important role in the triggering. The angle direction of incoming waves with respect to the fault ~ 60° and ~ 120° is the possible reason for triggering in the region. The triggering occurred in the Mishmi and Main Central Thrust regions. The largest triggered event, ML2.3, was triggered 7.5 h after the 2012 Indian Ocean earthquake of Mw8.6. The region is tectonically very sensitive and tiny stresses are capable of triggering seismicity in Arunachal Pradesh.

Project description:Objective:Caregiver burden is used frequently within the nursing literature. It has not yet been clearly defined as there are different opinions regarding this concept. The purpose of this paper is to provide clarity surrounding the concept caregiver burden. Methods:An electronic search of MEDLINE, CINAHL, Health Source Nursing/Academic Edition and Academic Search Complete (ASC) of EBSCO, China National Knowledge Infrastructure (CNKI) and Google Scholar were searched with a limit of 10 years and published in the English or Chinese language. The paper adopted the framework by Walker and Avant. The attributes, antecedents, consequences and uses of the concept were identified. Results:At total of 33 articles were included. The three attributes of caregiver burden were identified as self-perception, multifaceted strain, and over time. The antecedents included insufficient financial resources, multiple responsibility conflict, lack of social activities. The consequences of caregiver burden resulted in negative change which included decreased care provision, decrease in quality of life, physical and psychological health deterioration. Conclusion:A definition of caregiver burden was developed. Tools to measure caregiver burden were identified. The findings from this analysis can be used in nursing practice, nursing education, research and administration.

Project description:Objective Suicide resilience is gaining increasing attention from researchers because of its potential role in preventing suicide. However, it has not been clearly analyzed, and there are various meanings and terms regarding this issue. The purpose of this analysis, therefore, was to conceptualize the concept of suicide resilience. Methods Walker and Avant's method of concept analysis was used to identify the antecedents, attributes, and consequences of suicide resilience. The literature was searched using PubMed, PsychINFO, Embase, Web of Science, CINAHL, CNKI, and WanFang databases with no limitation on publication date. The search included peer-reviewed journal articles and dissertations related to suicide resilience published in English or Chinese. Results 52 articles were identified to provide information for this concept analysis. Five defining attributes of suicide resilience were identified: social support, coping strategies, psychological capital, meaning in life, and sense of responsibility. Antecedents of suicide resilience were high suicide risk events, and consequences of suicide resilience were keeping vulnerable individuals stay away from or overcoming suicidality. Conclusions The result of the analysis provided a more clear definition of suicide resilience. The identified defining attributes, antecedents, and consequences can be further tested and used to develop potential interventions. Future research is needed and will help to advance our understanding of the scope.

Project description:ObjectivesThere is extensive literature from various disciplines on self-care, an important aspect of nursing intervention via evaluation and education, but its meaning remains unprecise due to the difficulty integrating the diverse definitions developed over time across disciplines. Therefore, it is vital to clarify the meaning of self-care and formulate the defining attributes, antecedents, and consequences to self-care.MethodsWalker and Avant's concept analysis approach was used to analyze the concept of self-care. A search of the literature was completed using the databases CINAHL, PubMed, and EBSCOhost for years 1975-2020; literature search included peer-review articles, full-text publications, and available in English. A total of 31 articles were reviewed, and saturation was reached.ResultsAn extensive review of the literature revealed salient characteristics that reflected the most frequently used terms associated with the concept. Guided by Walker and Avant's method, three defining attributes emerged as common themes: awareness, self-control, and self-reliance.ConclusionsA clarified definition was identified: the ability to care for oneself through awareness, self-control, and self-reliance in order to achieve, maintain, or promote optimal health and well-being.
