Measuring Interoception: The CARdiac Elevation Detection Task.

ABSTRACT: Interoception has increasingly been the focus of psychiatric research, due to its hypothesized role in mental health. Existing interoceptive tasks either suffer from important methodological limitations, impacting their validity, or are burdensome and require specialized equipment, which limits their usage in vulnerable populations. We report on the development of the CARdiac Elevation Detection (CARED) task. Participants' heart rate is recorded by a wearable device connected to a mobile application. Notifications are sent to participants' mobile throughout the day over a period of 4 weeks. Participants are asked to state whether their heart rate is higher than usual, rate their confidence and describe the activity they were involved in when the notification occurred. Data (N = 30) revealed that 1/3 of the sample was classified as interoceptive and that participants presented overall good insight into their interoceptive abilities. Given its ease of administration and accessibility, the CARED task has the potential to be a significant asset for psychiatric and developmental research.

Project description:The concept of mentalizing has been widely studied, but almost exclusively through tasks with explicit instructions. Recent studies suggest that people also mentalize on a more implicit level. However, to our knowledge, no study to date has directly contrasted the effects of implicit and explicit mentalizing processes on an implicit dependent measure within-subjects. We implemented this by using two versions of an object detection task, differing only on secondary catch questions. We hypothesized that if explicit mentalizing relies on complementary processes beyond those underlying implicit mentalizing, this would be reflected in enhanced belief effects in the explicit version. Twenty-eight healthy adults watched movies in which, during the first phase, both they themselves and another agent formed a belief about the location of a ball, and although irrelevant, these beliefs could influence their ball detection reaction times in the second phase. After this response phase, there were occasional catch questions that were different for the explicit and implicit task version. Finally, self-report measures of autism spectrum disorder (ASD) symptomatology were included, as the literature suggests that ASD is related to a specific deficit in implicit mentalizing. Both in the explicit and implicit version, belief conditions had a significant effect on reaction times, with responses being slower when neither the participant nor the other agent expected the ball to be present compared to all other conditions. Importantly, after the implicit version, participants reported no explicit mentalizing awareness. In our neurotypical sample, ASD symptoms were not found to correlate with either explicit or implicit mentalizing. In conclusion, the reaction time patterns in the explicit and implicit version of the task show strikingly similar effects of mentalizing, indicating that participants processed beliefs to the same extent regardless of whether they mentalized explicitly or implicitly, with no additional effects for explicit processing.

Project description:BackgroundIntramyocardial hemorrhage (IMH) following ST-elevation myocardial infarction (STEMI) is associated with poor prognosis. In cardiac magnetic resonance (MR), T2* mapping is the reference standard for detecting IMH while cardiac diffusion tensor imaging (cDTI) can characterize myocardial architecture via fractional anisotropy (FA) and mean diffusivity (MD) of water molecules. The value of cDTI in the detection of IMH is not currently known.HypothesiscDTI can detect IMH post-STEMI.Study typeProspective.SubjectsA total of 50 patients (20% female) scanned at 1-week (V1) and 3-month (V2) post-STEMI.Field strength/sequenceA 3.0 T; inversion-recovery T1-weighted-imaging, multigradient-echo T2* mapping, spin-echo cDTI.AssessmentT2* maps were analyzed to detect IMH (defined as areas with T2* < 20 msec within areas of infarction). cDTI images were co-registered to produce averaged diffusion-weighted-images (DWIs), MD, and FA maps; hypointense areas were manually planimetered for IMH quantification.StatisticsOn averaged DWI, the presence of hypointense signal in areas matching IMH on T2* maps constituted to true-positive detection of iron. Independent samples t-tests were used to compare regional cDTI values. Results were considered statistically significant at P ≤ 0.05.ResultsAt V1, 24 patients had IMH on T2*. On averaged DWI, all 24 patients had hypointense signal in matching areas. IMH size derived using averaged-DWI was nonsignificantly greater than from T2* (2.0 ± 1.0 cm2 vs 1.89 ± 0.96 cm2 , P = 0.69). Compared to surrounding infarcted myocardium, MD was significantly reduced (1.29 ± 0.20 × 10-3 mm2 /sec vs 1.75 ± 0.16 × 10-3 mm2 /sec) and FA was significantly increased (0.40 ± 0.07 vs 0.23 ± 0.03) within areas of IMH. By V2, all 24 patients with acute IMH continued to have hypointense signals on averaged-DWI in the affected area. T2* detected IMH in 96% of these patients. Overall, averaged-DWI had 100% sensitivity and 96% specificity for the detection of IMH.Data conclusionThis study demonstrates that the parameters MD and FA are susceptible to the paramagnetic properties of iron, enabling cDTI to detect IMH.Evidence level1 TECHNICAL EFFICACY: Stage 2.
