The Dental Aesthetic Index and Its Association with Dental Caries, Dental Plaque and Socio-Demographic Variables in Schoolchildren Aged 12 and 15 Years.

ABSTRACT: The Dental Aesthetic Index (DAI) was determined in 12- and 15-year-old schoolchildren to ascertain the prevalence of malocclusion and to assess its association with dental caries experience, dental plaque accumulation, and socio-demographic variables. We performed a cross-sectional study with a stratified two-stage sampling design. An oral health survey and oral examination were conducted, and socio-demographic data were recorded. The sample comprised 1453 schoolchildren aged 12 (868) and 15 (585). These two samples were analyzed separately because statistically significant differences were found: the 12-year-old age group displayed a higher frequency of schoolchildren who attended state-run public schools (p = 0.004) and belonged to a lower social class (p = 0.001); the 15-year-old age group registered higher levels of caries (p = 0.001) and lower levels of dental plaque (p < 0.001). The malocclusion was 9.5% higher (p = 0.001), and the global mean DAI score was likewise higher among the 12-year-olds (p < 0.001). The multivariate regression analysis not only showed that caries and dental plaque were the variables that were the most strongly associated with malocclusion, but that caries (OR = 1.5) and dental plaque (OR > 2) were also risk factors for malocclusion in both groups. In conclusion, this study revealed a higher prevalence of malocclusion and dental plaque at age 12. A higher risk of caries and dental plaque was found to be related to the presence of malocclusion in both age groups.

Project description:BackgroundTo assess the prevalence and severity of caries in 12- and 15-year-old schoolchildren, and to analyse the related risk factors.MethodsWe conducted a cross-sectional study on a random sample of 1843 schoolchildren aged 12 and 15 from Galicia (northwest of Spain). Self-administered questionnaire and dental clinical examination were performed to obtain information about oral health habits, dental caries and oral hygiene. A logistic regression model including dental-caries-related variables was generated for each age group.ResultsThe respective findings for 12- and 15-years-old were as follows: decayed, missing, filled teeth index both for permanent and temporary dentition (DMFT/dmft) of 0.89 (95% CI, 0.87-0.91) and 1.38 (95% CI, 1.33-1.43), respectively; caries prevalence 39.6% (95% CI, 36.3-42.9) and 51.7% (95% CI, 48.0-55.4), respectively. In the 12-year-old group, individuals who occasionally, never or hardly ever brushed their teeth had higher values of caries (OR = 1.83, 95% CI 1.07-3.15, and OR = 9.14, 95% CI1.63-51.17, respectively). Also, the presence of plaque on more than 1/3 gingival was statistically associated with an increase of caries (OR = 2.03; 95% CI, 1.11-3.70), and living in a rural environment was a risk factor (OR = 1.3; 95% CI,1.02-1.80). In the 15-year-old group, higher caries risk was found when brushing was performed once a day (OR = 1.61; 95% CI,1.03-2.50), and among individuals who visited private clinics (OR = 1.77; 95% CI, 1.17-2.66), while electric toothbrush was associated with a lower caries risk (OR = 0.50; 95% CI, 0.29-0.86).ConclusionsThis study revealed that risk factors of dental caries showed differences in schoolchildren of 12- and 15-year-old. Strongest evidence related to caries in 12-year-old group were found in frequency of toothbrushing and dental plaque. In 15-year old group, electric toothbrush, time since the last visit to the dentist and type of dental care (public/private) had a stronger association with dental caries. Caries prevalence and mean DMFT/dmft increased from 12- to 15-year-old, in spite of improvement in oral hygiene at the age of 15.

Project description:Monitoring social inequalities in dental caries is crucial for establishing priorities in oral health systems. This study aimed to assess economic inequalities in dental caries and its contributing factors among Iranian schoolchildren. Data were obtained from the first phase of the Shahroud Schoolchildren Eye Cohort Study in 2015. A total of 4992 children aged 6-12 years old were included in the analysis. Dental examinations were conducted following the diagnostic methods and standards of the Oral Health Examination Survey, as recommended by the World Health Organization. The concentration index (C) was utilized to assess economic inequalities in dental caries. Additionally, the decomposition of C was employed to explain the determinants of the measured inequalities. In total, 71.4% of the schoolchildren had dental caries in primary dentition (dft≥1), and 41.6% of the schoolchildren had dental caries in permanent dentition (DMFT≥1). The Cs of dft≥1, primary decayed teeth (pdt≥1), and permanent missing teeth (PMT≥1) were -0.136 (95% CI: -0.167, -0.104), -0.164 (95% CI: -0.194, -0.134), and -0.208 (95% CI: -0.262, -0.153), respectively, which indicates their more concentration among low-economic children. Conversely, pft≥1 and PFT≥1 had Cs of 0.327 (95% CI: 0.292, 0.361) and 0.218 (95% CI: 0.179, 0.256), showing more concentration among high-economic children. Basic health insurance coverage and age were the main contributors that explained 28.6% and 19.2% of the economic inequality in dft≥1, and 25.7% and 16.6% of the pdt≥1 inequality, respectively. Economic status, residence in rural areas, mother education, father education, and age were the main contributors to the measured inequality in pft≥1 by 80.5% and 26.5%, 21.9%, 22%, and -18.3%, respectively. Economic status, having a housekeeper mother, residence in rural areas, having basic health insurance coverage, mother education, and father education positively contributed to the measured inequality in PMT≥1 by 45.4%, 42.4%, 37.8%, 35.1%, 21.3%, and 15.2%, respectively, while age had a negative contribution of -19.3%. For PFT≥1, economic status, age, and father education accounted for 76%, 25.4%, and 20.3% of the measured inequality, respectively. Pro-rich economic inequalities were observed in children's primary and permanent teeth caries. Thus, government interventions to reduce these inequalities should aim to expand the coverage of basic and supplementary health insurance in line with increasing the coverage of dental health costs in these plans, training and providing access to required dental health services for low-socioeconomic children, including the poor, rural, and those who have low-educated parents and a housekeeper mother, especially at younger ages.

Project description:Due to the complex microecology and microenvironment of dental plaque, novel caries prevention strategies require modulating the microbial communities ecologically and reducing the cariogenic properties effectively. Antimicrobial peptide GH12 reduced the lactic acid production and exopolysaccharide (EPS) synthesis of a Streptococcus mutans biofilm and a three-species biofilm in vitro in previous studies. However, the anticaries effects and microecological effects of GH12 remained to be investigated in a complex biofilm model in vitro and an animal caries model in vivo In the present study, GH12 at 64 mg/liter showed the most effective inhibition of lactic acid production, EPS synthesis, pH decline, and biofilm integrity of human dental plaque-derived multispecies biofilms in vitro, and GH12 at 64 mg/liter was therefore chosen for use in subsequent in vitro and in vivo assays. When treated with 64-mg/liter GH12, the dental plaque-derived multispecies biofilms sampled from healthy volunteers maintained its microbial diversity and showed a microbial community structure similar to that of the control group. In the rat caries model with a caries-promoting diet, 64-mg/liter GH12 regulated the microbiota of dental plaque, in which the abundance of caries-associated bacteria was decreased and the abundance of commensal bacteria was increased. In addition, 64-mg/liter GH12 significantly reduced the caries scores of sulcal and smooth surface caries in all locations. In conclusion, GH12 inhibited the cariogenic properties of dental plaque without perturbing the dental plaque microbiota of healthy individuals and GH12 regulated the dysbiotic microbial ecology and arrested caries development under cariogenic conditions.IMPORTANCE The anticaries effects and microecological regulation effects of the antimicrobial peptide GH12 were evaluated systematically in vitro and in vivo GH12 inhibited the cariogenic virulence of dental plaque without overintervening in the microbial ecology of healthy individuals in vitro GH12 regulated the microbial ecology of dental plaque to a certain extent in vivo under cariogenic conditions, increased the proportion of commensal bacteria, and decreased the abundance of caries-associated bacteria. GH12 significantly suppressed the incidence and severity of dental caries in vivo This study thus describes an alternative antimicrobial therapy for dental caries.

Project description:OBJECTIVE:Gingivitis and poor oral hygiene status are the most prevalent oral diseases among primary school students. Poor oral hygiene status, gingivitis and socio-demographic determinants have been shown to be associated with periodontal diseases. There is limited information on the gingivitis and oral hygiene status among Iranian children. In the present study, the status of gingivitis, oral hygiene status, and their association with socio-demographic determinants among schoolchildren aged 7-12 years old in Hamadan were investigated. METHODS:In this cross-sectional study, 988 primary school students aged 7-12 years old were selected. The oral hygiene status was measured through Simplified Oral Hygiene Index (OHI-S) and Community Periodontal Index (CPI) was used to evaluate gingival bleeding and calculus. CPI was measured using a standardized protocol to investigate gum bleeding and calculus. The oral hygiene was classified as good, fair or poor based on calculus and debris measurements. Age, gender, educational level, occupation and residence district of parents, dental pain experience in the last year and whether parents supervised their children while brushing were assessed by the questionnaires. The collected data were analyzed using descriptive statistics and logistic regression analysis. RESULTS:The oral hygiene status was good in 644 students (65.20%), fair in 341 (34.50%) and poor in three (0.30%). Moreover, the results of CPI depicted that 639 students (64.07%) had healthy periodontium, 320 (32.40%) had periodontal bleeding and 29 (2.9%) were with calculus. Higher percentage of the boys in the age group of 12 years old had periodontal bleeding and fewer good oral hygiene. The results of CPI and OHI-S scores depicted that more than half of the primary school students had healthy gums and periodontium (64.1%) and good oral hygiene status (65.2%). There were significant statistical associations between age and residence district with calculus measured by the CPI, also between gender, age, residence district, and mother's occupation with the gingival bleeding measured by the CPI. Furthermore, age and mother's occupation were significantly associated with OHI-S index. CONCLUSIONS:In general, the periodontal health status is poorer in students attending suburban schools compared to those in urban schools in Hamadan. Since there are significant associations between gender, school districts and mother's occupation with oral hygiene index among schoolchildren in primary schools, considering them in schools' oral health program design might be useful.

Project description:BackgroundA number of studies on oral microbial diversity of early childhood caries (ECC) have tended to focus on mid- or late-stage of ECC, with a lack of research into early stage of tooth eruption and maternal influence. The aims of this study are to compare the supragingival plaque biofilm microbiota diversity between mothers with or without dental caries and their 12-month-old infants, and to explore the relationship of microbial diversity between infants and their mothers, using sequencing analysis.MethodsSupragingival plaque biofilm samples were collected from 20 pairs of mothers and their infants aged 12?months (10 mothers with dental caries and their 10 infants vs. 10 caries-free mothers and their 10 infants). The basic information of the mothers and infants had been collected through self-completed questionnaire. Pooled plaque biofilm DNA was extracted and DNA amplicons of the V4-V5 hypervariable region of the bacterial 16S rRNA gene were generated. Ilumina Miseq PE300 was used for 16S rRNA sequencing.ResultsThe results showed that high bacterial diversity was noted in the plaque biofilm of infants and their mothers with or without dental caries (dental caries mothers vs. caries-free mothers: 774 operational taxonomical units (OTUs) vs. 761 OTUs at a 3% divergence; infants whose mothers have dental caries vs. infants whose mothers are caries-free: 815 OTUs vs. 684 OTUs at 3% divergence). The Shannon microbial diversity index showed no statistically significant differences both on infants and their mothers between two groups (p?>?0.05). Mother's microbial diversity was higher than infants' based on Shannon index (p?<?0.05). Significant positive correlations were found between mothers' and their infants' Shannon index (r?=?0.656, p?=?0.002).ConclusionOral microbial diversity is significantly different between mothers and infants regardless of dental caries status, but no significant difference was found between mothers with and without dental caries or between their infants. Mother's oral microbial diversity has an overall impact on the infants aged 12?months.
