A study of the relationship between nurses' experience, structural empowerment, and attitudes toward computer use.

ABSTRACT: Objective
This study examined the relationship between structural empowerment and nurses' experience and attitudes toward computer use.Methods
This study was conducted using a cross-sectional quantitative design. A total of 184 registered nurses from four hospitals in Jordan participated in the current study. Data were collected using a demographics questionnaire, the Conditions for Work Effectiveness Questionnaire-II (CWEQ-II), and the Pretest for Attitudes toward Computers in Healthcare (PATCH).Results
The median of experience in years among nurses was 5.0, ranging from one to 26 years. The mean score for the attitudes toward computer use was 61.90 ± 11.38. Almost half of the participants, 45.11%, were in the category of "feel comfortable using user-friendly computers." The participants' mean average of the total structural empowerment was 12.40 ± 2.43, and the values for its four subscales were: opportunity 3.57 ± 0.87, resources 2.83 ± 0.85, information 3.06 ± 0.79, and support 2.95 ± 0.86. The frequencies analysis revealed that most participants had a moderate level of empowerment (n = 127, 69.02%). The bivariate correlation between nurses' experience and attitudes toward computer use was significant (r = -0.17, P < 0.05). The relationship between the total structural empowerment score and attitudes toward computer use was positive but weak (r = 0.20, P < 0.01).Conclusion
The results indicated that more experienced nurses are more reluctant toward computer use. However, creating an empowering work environment can facilitate nurses' attitudes toward computer use.

Project description:To understand factors affecting nurses' attitudes toward the Therapeutic Hypothermia After Pediatric Cardiac Arrest trials and association with approach/consent rates.Cross-sectional survey of pediatric/cardiac intensive care nurses' perceptions of the trials.Study was conducted at 16 of 38 self-selected study sites.Pediatric and cardiac intensive care nurses.The primary outcome was the proportion of nurses with positive perceptions, as defined by agree or strongly agree with the statement "I am happy to take care of a Therapeutic Hypothermia after Pediatric Cardiac Arrest patient". Associations between perceptions and study approach/consent rates were also explored. Of 2,241 nurses invited, 1,387 (62%) completed the survey and 77% reported positive perceptions of the trials. Nurses, who felt positively about the scientific question, the study team, and training received, were more likely to have positive perceptions of the trials (p < 0.001). Nurses who had previously cared for a research patient had significantly more positive perceptions of Therapeutic Hypothermia After Pediatric Cardiac Arrest compared with those who had not (79% vs 54%; p < 0.001). Of the 754 nurses who cared for a Therapeutic Hypothermia After Pediatric Cardiac Arrest patient, 82% had positive perceptions, despite 86% reporting it required more work. Sixty-nine percent believed that hypothermia reduces brain injury and mortality; sites had lower consent rates when their nurses believed that hypothermia was beneficial. Institution-specific approach rates were positively correlated with nurses' perceptions of institutional support for the trial (r = 0.54; p = 0.04), ICU support (r = 0.61; p = 0.02), and the importance of conducting the trial in children (r = 0.61; p = 0.01).The majority of nurses had positive perceptions of the Therapeutic Hypothermia After Pediatric Cardiac Arrest trials. Institutional, colleague, and study team support and training were contributing factors. Despite increased work, nurses remained enthusiastic demonstrating that studies with intensive bedside nursing procedures are feasible. Institutions whose nurses believed hypothermia was beneficial had lower consent rates, suggesting that educating nurses on study rationale and equipoise may enhance study participation.

Project description:BackgroundDespite the abundance of research on computerized dementia screening tests, the attitudes of hospital personnel toward this screening method have not been investigated.Objective1) To conduct a confirmatory factor analysis of the first part of a two-part questionnaire about computerized dementia screening. 2) To assess the attitudes of Greek nurses toward computerized dementia screening. 3) To assess barriers to future implementation of computerized dementia screening in the Greek healthcare system, as reported by nurses.Methods161 Greek nurses from two urban public general hospitals who participated in a dementia training program were recruited. They were asked to complete a two-part questionnaire about computerized dementia screening. The first part of the questionnaire assesses attitudes toward dementia screening while the second part of the questionnaire assesses barriers to its implementation.ResultsConfirmatory factor analysis on the first part of the questionnaire suggested a two-factor structure (feasibility/acceptability). The total score of all items loading on each factor was calculated. For feasibility, scores ranged between 10 and 25 (M = 19.38, SD = 3.80). For acceptability, scores ranged between 6 and 20 (M = 15.27, SD = 2.76). The main barriers to implementation were cost of equipment, insufficient training, lack of a plan for the integration of computerized screening tests in the daily routine of the hospital and time needed for staff training.ConclusionThe positive attitude of nurses supports the implementation of computerized dementia screening in public hospitals as long as identified barriers are addressed.

Project description:The aim of the study reported in this article was to investigate staff nurses' perceptions and experiences about structural empowerment and perceptions regarding the extent to which structural empowerment supports safe quality patient care. To address the complex needs of patients, staff nurse involvement in clinical and organizational decision-making processes within interdisciplinary care settings is crucial. A qualitative study was conducted using individual semi-structured interviews of 11 staff nurses assigned to medical or surgical units in a 600-bed university hospital in Belgium. During the study period, the hospital was going through an organizational transformation process to move from a classic hierarchical and departmental organizational structure to one that was flat and interdisciplinary. Staff nurses reported experiencing structural empowerment and they were willing to be involved in decision-making processes primarily about patient care within the context of their practice unit. However, participants were not always fully aware of the challenges and the effect of empowerment on their daily practice, the quality of care and patient safety. Ongoing hospital change initiatives supported staff nurses' involvement in decision-making processes for certain matters but for some decisions, a classic hierarchical and departmental process still remained. Nurses perceived relatively high work demands and at times viewed empowerment as presenting additional. Staff nurses recognized the opportunities structural empowerment provided within their daily practice. Nurse managers and unit climate were seen as crucial for success while lack of time and perceived work demands were viewed as barriers to empowerment.

Project description:ObjectivesThis study aims to investigate if experience in smoking intervention training influences attitudes toward smoking, discuss the role of health management programs of small- and medium-sized enterprises, and analyze the current attitude of occupational health nurses regarding the hazards of smoking and responsibility to smokers to effectively facilitate smoking cessation support programs.MethodsWe conducted an anonymous self-administered cross-sectional survey of 108 nurses employed in occupational health services outsourcing specialized agency in Korea. We assessed the difference in attitude about smoking according to training experience in smoking interventions and perceived competence in counseling smokers using chi-square test and Fisher's exact test.ResultsOccupational health nurses with the training experience of smoking interventions tend to perceive the harmful effects of smoking more seriously, compared to occupational health nurses without the training experience (P = .024, Fisher's exact test) and the OHSO nurses with the training experience tend to have professional ethics as health care professionals (P = .017, Fisher's exact test). Occupational health nurses having expertise in smoking cessation counseling tended to have professional ethics (P = .047, Fisher's exact test) and social responsibility as health care professionals (P = .022, Fisher's exact test).ConclusionThe occupational health nurses with training experience and expertise in smoking cessation counseling perceive the harmful effects of smoking more strongly and can enhance their professional ethics and social responsibility as health care professionals.

Project description:BACKGROUND:Structural empowerment is an ever-evolving concept interpreted and applied in many different ways as it focuses on the structures in a healthcare organisation to allow competent nurses to manage empowering opportunities in a professional manner. At a public hospital in the Western Cape, nurses complained about a lack of access to structural empowerment in a hospital, including structures of power, such as clear information, to partake in important decisions. OBJECTIVES:The purpose of this study was to describe how nurse managers could support nurses in accessing structural empowerment through power resources. METHOD:A quantitative design was followed with a survey. The accessible population in this study was different categories of nurses of professional, enrolled and assistant nurses (N = 200), which were on duty at the time of data gathering. The sample was selected by means of probability sampling (n = 110). An existing instrument based on a five-point Likert scale was distributed that took 45 minutes to complete. Descriptive and inferential statistics were calculated, and the chi-square was used to indicate statistical significance differences among the nursing categories on the items (p 0.05). RESULTS:The general results indicated that the majority of nurses had challenges to access structural empowerment through power sources (information, support and resources). Significant differences were found between nurse categories for having the necessary supplies for the job (p = 0.043) and rewards for unusual job performance (p = 0.023). Those aspects on which no significant differences were found are of utmost importance, as they indicate the urgency of addressing limitations in power sources for all categories of nurses. CONCLUSION:Empowerment can be achieved by enabling access to structural empowerment through power sources (i.e. opportunities, information, resources and support) at different levels for all categories of nurses.

Project description:This study aimed to examine the mediating effects of self-stigma and depression on the public stigma and nurses' attitudes toward psychiatric help. A cross-sectional study with 184 nurses at one general hospital in South Korea was conducted employing a self-administered survey, using the Attitudes toward Seeking Psychological Help Scale, the Beck Depression Inventory-II Scale, and the modified Depression Stigma Scale. A multiple-mediation analysis procedure was applied to analyze the data. Each indirect effect of self-stigma (B = -0.0974, bootLLCI, bootULCI: -0.1742, -0.0436) and depression (B = -0.0471, bootLLCI, bootULCI: -0.1014, -0.0060) is statistically significant in the relationship between public stigma and attitudes toward psychiatric help. The individualized intervention for enhancing positive attitude or motivation for seeking help at the personal level of the nurse and depression tests-including regular physical health check-ups-is necessary.

Project description:BackgroundCulture influences nurses' attitudes towards caring for older adults. Additionally, nursing students' perceptions and attitudes towards older adults affect their behavior, possibly their career choices and/or the quality of care provided to older adults after graduation. In the context of lower-middle-income countries with a faster growing older adults population compared to upper income countries, the improvement of the quality care, inclusive of nurses' attitudes towards older adults, is one of the strategies for strengthening nursing and midwifery in Africa. Furthermore, examining nurses and nursing students' attitudes towards older adults will answer the United Nations' call for more data to understand the needs and the status of older adults in Africa.MethodsThis scoping review will be guided by Arksey and O'Malley's framework. The search will be performed using Scopus, PubMed databases, Academic search complete, CINAHL with full text, Education source, Health source: Nursing/Academic Edition, with words related to the topic. The reviewers will also use Google Scholar and the reference lists of the relevant articles. Primary studies and grey literature addressing the research question will be included. The search process will include a first stage where two reviewers will perform the title screening and the removal of duplicates, followed by a parallel abstract screening according to eligibility criteria. The second stage will involve the reading of full articles and the exclusion of articles, in accordance with the eligibility criteria. Data will be collated by two reviewers independently and parallel, using a predetermined data extraction form. Discrepancies will involve a third reviewer. The Mixed Methods Appraisal Tool, version 2018 will be used to assess the quality of the data of eligible articles. A narrative approach containing summary tables and graphs will facilitate synthesis.DiscussionThe review will provide insight into nurses' and nursing students' attitudes towards older adults in African countries. The outcomes will guide future research, practice, and education in nursing.

Project description:BackgroundThe American Society of Clinical Oncology views patient-physician discussion of costs as a component of high-quality care. Few data exist on patients' views regarding how cost should be addressed in the clinic.MethodsWe distributed a self-administered, anonymous, paper survey to consecutive patients with breast cancer presenting for a routine visit within 5 years of diagnosis at an academic cancer center. Survey questions addressed experience and preferences concerning discussions of cost and views on cost control. Results are primarily descriptive, with comparison among participants on the basis of disease stage, using chi-square and Fisher's exact tests. All p values are two-sided.ResultsWe surveyed 134 participants (response rate 86%). Median age was 61 years, and 28% had stage IV disease. Although 44% of participants reported at least a moderate level of financial distress, only 14% discussed costs with their doctor; 94% agreed doctors should talk to patients about costs of care. Regarding the impact of costs on decision making, 53% felt doctors should consider direct costs to the patient, but only 38% felt doctors should consider costs to society. Moreover, 88% reported concern about costs of care, but there was no consensus on how to control costs.ConclusionMost breast cancer patients want to discuss costs of care, but there is little consensus on the desired content or goal of these discussions. Further research is needed to define the role of cost discussions at the bedside and how they will contribute to the goal of high-quality and sustainable cancer care.

Project description:BACKGROUND:Recent studies suggest that autonomous and controlled motivation may mediate the relationship between perfectionism and attitudes toward doping. The purpose of this study was to investigate the association among perfectionistic strivings, perfectionistic concerns, autonomous motivation, controlled motivation, and attitudes toward doping in young athletes. The potential mediating role of autonomous and controlled motivation was examined. METHODS:Two hundred and forty-three Chinese athletes were surveyed using questionnaires. Structural equation modeling (SEM) was conducted to evaluate the potential mediating role of autonomous and controlled motivation between perfectionism and attitudes toward doping. RESULTS:The results showed that perfectionistic strivings was negatively associated with attitudes toward doping, whereas perfectionistic concerns positively predicted attitudes toward doping. Autonomous motivation was negatively associated with attitudes toward doping, whereas controlled motivation positively predicted attitudes toward doping. The mediation analyses showed that the relationship between perfectionistic strivings and attitudes toward doping was partially mediated by autonomous motivation. The relationship between perfectionistic concerns and attitudes toward doping was partially mediated by controlled motivation. CONCLUSIONS:The findings suggested that fostering perfectionistic strivings and autonomous motivation and reducing perfectionistic concerns and controlled motivation may be considered for inclusion in athletes' anti-doping education.

Project description:In Korea, there is a need for research on human-animal relationships because of an increase in the number of companion animals and the positive changes in public perception toward them. Few studies have examined these changes. This epidemiological study investigates the characteristics of Korean dog owners and their pet dogs and identifies the owner- and dog-dependent factors that influence the owners' attitudes toward pets. We conducted a cross-sectional study of dog owners by asking them to complete a Pet Attitude Scale-based questionnaire about their dogs and themselves. The participants included 654 young adults between 19 and 39 years of age who lived in Seoul and owned dogs. We found that most dogs were owned by single, educated, high-income men who preferred small purebred dogs. Most were also likely to underestimate their dog's body condition score (BCS). The multivariable logistic regression (odds ratio, OR) and the multiple linear regression (unstandardized coefficients, B) models suggested that positive pet attitudes were associated with nine factors: overweight (OR = 2.68, B = 5.28) or a normal BCS (OR = 2.09, B = 5.58), having a medical history of related diseases (OR = 2.36, B = 6.38) and vaccination (OR = 2.10, B = 6.22), buying the pet dog (OR = 0.60, B = -3.85), having a small dog (?10 kg) (OR = 1.66), visiting the veterinarian frequently (OR = 1.08, B = 0.39), spending more time with the dog (OR = 1.23, B =1.32), and keeping other species in the house (B = -4.27). This study is the first to identify the relationships between owner- and dog-dependent factors and pet owner attitude toward pets, all within a Korean cultural context. This study highlights the factors associated with the development of relationships between pet dogs and their owners. The exploratory study is novel because it examines pet ownership in the context of the Korean culture; previous pet ownership studies were set in the West and are analyzed with Western cultural values in mind.
