Project description:BackgroundLinguistic accessibility has an important impact on the reception and utilization of translated health resources among multicultural and multilingual populations. Linguistic understandability of health translation has been understudied.ObjectiveOur study aimed to develop novel machine learning models for the study of the linguistic accessibility of health translations comparing Chinese translations of the World Health Organization health materials with original Chinese health resources developed by the Chinese health authorities.MethodsUsing natural language processing tools for the assessment of the readability of Chinese materials, we explored and compared the readability of Chinese health translations from the World Health Organization with original Chinese materials from the China Center for Disease Control and Prevention.ResultsA pairwise adjusted t test showed that the following 3 new machine learning models achieved statistically significant improvement over the baseline logistic regression in terms of area under the curve: C5.0 decision tree (95% CI -0.249 to -0.152; P<0.001), random forest (95% CI 0.139-0.239; P<0.001) and extreme gradient boosting tree (95% CI 0.099-0.193; P<0.001). There was, however, no significant difference between C5.0 decision tree and random forest (P=0.513). The extreme gradient boosting tree was the best model, achieving statistically significant improvement over the C5.0 model (P=0.003) and the random forest model (P=0.006) at an adjusted Bonferroni P value at 0.008.ConclusionsThe development of machine learning algorithms significantly improved the accuracy and reliability of current approaches to the evaluation of the linguistic accessibility of Chinese health information, especially Chinese health translations in relation to original health resources. Although the new algorithms developed were based on Chinese health resources, they can be adapted for other languages to advance current research in accessible health translation, communication, and promotion.

Project description:IntroductionPredictive models have been used to aid early diagnosis of PCOS, though existing models are based on small sample sizes and limited to fertility clinic populations. We built a predictive model using machine learning algorithms based on an outpatient population at risk for PCOS to predict risk and facilitate earlier diagnosis, particularly among those who meet diagnostic criteria but have not received a diagnosis.MethodsThis is a retrospective cohort study from a SafetyNet hospital's electronic health records (EHR) from 2003-2016. The study population included 30,601 women aged 18-45 years without concurrent endocrinopathy who had any visit to Boston Medical Center for primary care, obstetrics and gynecology, endocrinology, family medicine, or general internal medicine. Four prediction outcomes were assessed for PCOS. The first outcome was PCOS ICD-9 diagnosis with additional model outcomes of algorithm-defined PCOS. The latter was based on Rotterdam criteria and merging laboratory values, radiographic imaging, and ICD data from the EHR to define irregular menstruation, hyperandrogenism, and polycystic ovarian morphology on ultrasound.ResultsWe developed predictive models using four machine learning methods: logistic regression, supported vector machine, gradient boosted trees, and random forests. Hormone values (follicle-stimulating hormone, luteinizing hormone, estradiol, and sex hormone binding globulin) were combined to create a multilayer perceptron score using a neural network classifier. Prediction of PCOS prior to clinical diagnosis in an out-of-sample test set of patients achieved an average AUC of 85%, 81%, 80%, and 82%, respectively in Models I, II, III and IV. Significant positive predictors of PCOS diagnosis across models included hormone levels and obesity; negative predictors included gravidity and positive bHCG.ConclusionMachine learning algorithms were used to predict PCOS based on a large at-risk population. This approach may guide early detection of PCOS within EHR-interfaced populations to facilitate counseling and interventions that may reduce long-term health consequences. Our model illustrates the potential benefits of an artificial intelligence-enabled provider assistance tool that can be integrated into the EHR to reduce delays in diagnosis. However, model validation in other hospital-based populations is necessary.

Project description:OBJECTIVES:Predictive models for hepatocellular carcinoma (HCC) have been limited by modest accuracy and lack of validation. Machine-learning algorithms offer a novel methodology, which may improve HCC risk prognostication among patients with cirrhosis. Our study's aim was to develop and compare predictive models for HCC development among cirrhotic patients, using conventional regression analysis and machine-learning algorithms. METHODS:We enrolled 442 patients with Child A or B cirrhosis at the University of Michigan between January 2004 and September 2006 (UM cohort) and prospectively followed them until HCC development, liver transplantation, death, or study termination. Regression analysis and machine-learning algorithms were used to construct predictive models for HCC development, which were tested on an independent validation cohort from the Hepatitis C Antiviral Long-term Treatment against Cirrhosis (HALT-C) Trial. Both models were also compared with the previously published HALT-C model. Discrimination was assessed using receiver operating characteristic curve analysis, and diagnostic accuracy was assessed with net reclassification improvement and integrated discrimination improvement statistics. RESULTS:After a median follow-up of 3.5 years, 41 patients developed HCC. The UM regression model had a c-statistic of 0.61 (95% confidence interval (CI) 0.56-0.67), whereas the machine-learning algorithm had a c-statistic of 0.64 (95% CI 0.60-0.69) in the validation cohort. The HALT-C model had a c-statistic of 0.60 (95% CI 0.50-0.70) in the validation cohort and was outperformed by the machine-learning algorithm. The machine-learning algorithm had significantly better diagnostic accuracy as assessed by net reclassification improvement (P<0.001) and integrated discrimination improvement (P=0.04). CONCLUSIONS:Machine-learning algorithms improve the accuracy of risk stratifying patients with cirrhosis and can be used to accurately identify patients at high-risk for developing HCC.

Project description:BackgroundPulmonary embolisms (PE) are life-threatening medical events, and early identification of patients experiencing a PE is essential to optimizing patient outcomes. Current tools for risk stratification of PE patients are limited and unable to predict PE events before their occurrence.ObjectiveWe developed a machine learning algorithm (MLA) designed to identify patients at risk of PE before the clinical detection of onset in an inpatient population.Materials and methodsThree machine learning (ML) models were developed on electronic health record data from 63,798 medical and surgical inpatients in a large US medical center. These models included logistic regression, neural network, and gradient boosted tree (XGBoost) models. All models used only routinely collected demographic, clinical, and laboratory information as inputs. All were evaluated for their ability to predict PE at the first time patient vital signs and lab measures required for the MLA to run were available. Performance was assessed with regard to the area under the receiver operating characteristic (AUROC), sensitivity, and specificity.ResultsThe model trained using XGBoost demonstrated the strongest performance for predicting PEs. The XGBoost model obtained an AUROC of 0.85, a sensitivity of 81%, and a specificity of 70%. The neural network and logistic regression models obtained AUROCs of 0.74 and 0.67, sensitivity of 81% and 81%, and specificity of 44% and 35%, respectively.ConclusionsThis algorithm may improve patient outcomes through earlier recognition and prediction of PE, enabling earlier diagnosis and treatment of PE.

Project description:BackgroundBirth weight is a significant determinant of the likelihood of survival of an infant. Babies born at low birth weight are 25 times more likely to die than at normal birth weight. Low birth weight (LBW) affects one out of every seven newborns, accounting for about 14.6 percent of the babies born worldwide. Moreover, the prevalence of LBW varies substantially by region, with 7.2 per cent in the developed regions and 13.7 per cent in Africa, respectively. Ethiopia has a large burden of LBW, around half of Africa. These newborns were more likely to die within the first month of birth or to have long-term implications. These are stunted growth, low IQ, overweight or obesity, developing heart disease, diabetes, and early death. Therefore, the ability to predict the LBW is the better preventive measure and indicator of infant health risks.MethodThis study implemented predictive LBW models based on the data obtained from the Ethiopia Demographic and Health Survey 2016. This study was employed to compare and identify the best-suited classifier for predictive classification among Logistic Regression, Decision Tree, Naive Bayes, K-Nearest Neighbor, Random Forest (RF), Support Vector Machine, Gradient Boosting, and Extreme Gradient Boosting.ResultsData preprocessing is conducted, including data cleaning. The Normal and LBW are the binary target category in this study. The study reveals that RF was the best classifier and predicts LBW with 91.60 percent accuracy, 91.60 percent Recall, 96.80 percent ROC-AUC, 91.60 percent F1 Score, 1.05 percent Hamming loss, and 81.86 percent Jaccard score.ConclusionThe RF predicted the occurrence of LBW more accurately and effectively than other classifiers in Ethiopia Demographic Health Survey. Gender of the child, marriage to birth interval, mother's occupation and mother's age were Ethiopia's top four critical predictors of low birth weight in Ethiopia.

Project description:Drug-induced liver injury (DILI) is one the most unpredictable adverse reactions to xenobiotics in humans and the leading cause of postmarketing withdrawals of approved drugs. To date, these drugs have been collated by the FDA to form the DILIRank database, which classifies DILI severity and potential. These classifications have been used by various research groups in generating computational predictions for this type of liver injury. Recently, groups from Pfizer and AstraZeneca have collated DILI in vitro data and physicochemical properties for compounds that can be used along with data from the FDA to build machine learning models for DILI. In this study, we have used these data sets, as well as the Biopharmaceutics Drug Disposition Classification System data set, to generate Bayesian machine learning models with our in-house software, Assay Central. The performance of all machine learning models was assessed through both the internal 5-fold cross-validation metrics and prediction accuracy of an external test set of compounds with known hepatotoxicity. The best-performing Bayesian model was based on the DILI-concern category from the DILIRank database with an ROC of 0.814, a sensitivity of 0.741, a specificity of 0.755, and an accuracy of 0.746. A comparison of alternative machine learning algorithms, such as k-nearest neighbors, support vector classification, AdaBoosted decision trees, and deep learning methods, produced similar statistics to those generated with the Bayesian algorithm in Assay Central. This study demonstrates machine learning models grouped in a tool called MegaTox that can be used to predict early-stage clinical compounds, as well as recent FDA-approved drugs, to identify potential DILI.

Project description:ML algorithms are used to develop prognostic and diagnostic models and so to support clinical decision-making. This study uses eight supervised ML algorithms to predict the need for intensive care, intubation, and mortality risk for COVID-19 patients. The study uses two datasets: (1) patient demographics and clinical data (n = 11,712), and (2) patient demographics, clinical data, and blood test results (n = 602) for developing the prediction models, understanding the most significant features, and comparing the performances of eight different ML algorithms. Experimental findings showed that all prognostic prediction models reported an AUROC value of over 0.92, in which extra tree and CatBoost classifiers were often outperformed (AUROC over 0.94). The findings revealed that the features of C-reactive protein, the ratio of lymphocytes, lactic acid, and serum calcium have a substantial impact on COVID-19 prognostic predictions. This study provides evidence of the value of tree-based supervised ML algorithms for predicting prognosis in health care.

Project description:BackgroundThere is an increasing body of research on the development of machine learning algorithms in the evaluation of online health educational resources for specific readerships. Machine learning algorithms are known for their lack of interpretability compared with statistics. Given their high predictive precision, improving the interpretability of these algorithms can help increase their applicability and replicability in health educational research and applied linguistics, as well as in the development and review of new health education resources for effective and accessible health education.ObjectiveOur study aimed to develop a linguistically enriched machine learning model to predict binary outcomes of online English health educational resources in terms of their easiness and complexity for international tertiary students.MethodsLogistic regression emerged as the best performing algorithm compared with support vector machine (SVM) (linear), SVM (radial basis function), random forest, and extreme gradient boosting on the transformed data set using L2 normalization. We applied recursive feature elimination with SVM to perform automatic feature selection. The automatically selected features (n=67) were then further streamlined through expert review. The finalized feature set of 22 semantic features achieved a similar area under the curve, sensitivity, specificity, and accuracy compared with the initial (n=115) and automatically selected feature sets (n=67). Logistic regression with the linguistically enhanced feature set (n=22) exhibited important stability and robustness on the training data of different sizes (20%, 40%, 60%, and 80%), and showed consistently high performance when compared with the other 4 algorithms (SVM [linear], SVM [radial basis function], random forest, and extreme gradient boosting).ResultsWe identified semantic features (with positive regression coefficients) contributing to the prediction of easy-to-understand online health texts and semantic features (with negative regression coefficients) contributing to the prediction of hard-to-understand health materials for readers with nonnative English backgrounds. Language complexity was explained by lexical difficulty (rarity and medical terminology), verbs typical of medical discourse, and syntactic complexity. Language easiness of online health materials was associated with features such as common speech act verbs, personal pronouns, and familiar reasoning verbs. Successive permutation of features illustrated the interaction between these features and their impact on key performance indicators of the machine learning algorithms.ConclusionsThe new logistic regression model developed exhibited consistency, scalability, and, more importantly, interpretability based on existing health and linguistic research. It was found that low and high linguistic accessibilities of online health materials were explained by 2 sets of distinct semantic features. This revealed the inherent complexity of effective health communication beyond current readability analyses, which were limited to syntactic complexity and lexical difficulty.

Project description:ObjectiveTo screen for predictive obesity factors in overweight populations using an optimal and interpretable machine learning algorithm.MethodsThis cross-sectional study was conducted between June 2011 and January 2012. The participants were randomly selected using a simple random sampling technique. Seven commonly used machine learning methods were employed to construct obesity risk prediction models. A total of 5,236 Chinese participants from Ningde City, Fujian Province, Southeast China, participated in this study. The best model was selected through appropriate verification and validation and suitably explained. Subsequently, a minimal set of significant predictors was identified. The Shapley additive explanation force plot was used to illustrate the model at the individual level.ResultsMachine learning models for predicting obesity have demonstrated strong performance, with CatBoost emerging as the most effective in both model validity and net clinical benefit. Specifically, the CatBoost algorithm yielded the highest scores, registering 0.91 in the training set and an impressive 0.83 in the test set. This was further corroborated by the area under the curve (AUC) metrics, where CatBoost achieved 0.95 for the training set and 0.87 for the test set. In a rigorous five-fold cross-validation, the AUC for the CatBoost model ranged between 0.84 and 0.91, with an average AUC of ROC at 0.87 ± 0.022. Key predictors identified within these models included waist circumference, hip circumference, female gender, and systolic blood pressure.ConclusionCatBoost may be the best machine learning method for prediction. Combining Shapley's additive explanation and machine learning methods can be effective in identifying disease risk factors for prevention and control.

Project description:We aimed to develop, train, and validate machine learning models for predicting preterm birth (<37 weeks' gestation) in singleton pregnancies at different gestational intervals. Models were developed based on complete data from 22,603 singleton pregnancies from a prospective population-based cohort study that was conducted in 51 midwifery clinics and hospitals in Wenzhou City of China between 2014 and 2016. We applied Catboost, Random Forest, Stacked Model, Deep Neural Networks (DNN), and Support Vector Machine (SVM) algorithms, as well as logistic regression, to conduct feature selection and predictive modeling. Feature selection was implemented based on permutation-based feature importance lists derived from the machine learning models including all features, using a balanced training data set. To develop prediction models, the top 10%, 25%, and 50% most important predictive features were selected. Prediction models were developed with the training data set with 5-fold cross-validation for internal validation. Model performance was assessed using area under the receiver operating curve (AUC) values. The CatBoost-based prediction model after 26 weeks' gestation performed best with an AUC value of 0.70 (0.67, 0.73), accuracy of 0.81, sensitivity of 0.47, and specificity of 0.83. Number of antenatal care visits before 24 weeks' gestation, aspartate aminotransferase level at registration, symphysis fundal height, maternal weight, abdominal circumference, and blood pressure emerged as strong predictors after 26 completed weeks. The application of machine learning on pregnancy surveillance data is a promising approach to predict preterm birth and we identified several modifiable antenatal predictors.
