Project description:When treating a popliteal cyst in the knee joint, it is often necessary to simultaneously treat the intra-articular lesions so as to reduce the recurrence of the cyst after the operation. Herein, we propose an arthroscopic resection technique for popliteal cyst, in which the position of the popliteal cyst is indirectly located by finding out the location of the semimembranosus tendon during the operation, thereby completing the arthroscopic resection of the popliteal cyst. This method does not involve the posteromedial compartment of the knee joint. Arthroscopic treatment can be performed on the combined intra-articular lesions through anteromedial and anterolateral approaches before or after popliteal cyst resection. Technique Video Video 1 This is a patient with a right popliteal cyst. After the operative approach was established according to step 2, the arthroscope was implanted through approach 1, and a motorized shaver was implanted through approach 2. We clean the surrounding soft tissue to look for the semimembranosus tendon, and the popliteal cyst will be found on the lateral side of the semimembranosus tendon. Finally, we cleaned the cyst wall tissue completely.

Project description:Valve interstitial cells populate aortic valve cusps and have been implicated in aortic valve calcification. Here we investigate a common in vitro model for aortic valve calcification by characterizing nodule formation in porcine aortic valve interstitial cells (PAVICs) cultured in osteogenic (OST) medium supplemented with transforming growth factor beta 1 (TGF-?1). Using a combination of materials science and biological techniques, we investigate the relevance of PAVICs nodules in modeling the mineralised material produced in calcified aortic valve disease. PAVICs were grown in OST medium supplemented with TGF-?1 (OST+TGF-?1) or basal (CTL) medium for up to 21 days. Murine calvarial osteoblasts (MOBs) were grown in OST medium for 28 days as a known mineralizing model for comparison. PAVICs grown in OST+TGF-?1 produced nodular structures staining positive for calcium content; however, micro-Raman spectroscopy allowed live, noninvasive imaging that showed an absence of mineralized material, which was readily identified in nodules formed by MOBs and has been identified in human valves. Gene expression analysis, immunostaining, and transmission electron microscopy imaging revealed that PAVICs grown in OST+TGF-?1 medium produced abundant extracellular matrix via the upregulation of the gene for Type I Collagen. PAVICs, nevertheless, did not appear to further transdifferentiate to osteoblasts. Our results demonstrate that 'calcified' nodules formed from PAVICs grown in OST+TGF-?1 medium do not mineralize after 21 days in culture, but rather they express a myofibroblast-like phenotype and produce a collagen-rich extracellular matrix. This study clarifies further the role of PAVICs as a model of calcification of the human aortic valve.

Project description:Diagnosis of calcified nodules (CNs) is critical in the proper management of coronary artery disease, but CNs can be detected only using intracoronary imaging modalities. This study aimed to investigate the ability of coronary computed tomography angiography (CCTA) in predicting CNs detected using optical coherence tomography (OCT). From 138 patients who underwent OCT-guided percutaneous coronary intervention (PCI) after CCTA evaluation, 141 PCI target vessels were retrospectively enrolled and classified into CN (12 vessels/11 patients; CNs in the PCI culprit lesion) and non-CN (129 vessels/127 patients; without CNs) groups based on the OCT analysis. Retrospective CCTA analysis revealed significantly higher coronary artery calcification score (CACS), calcified plaque volume (CPV), and maximum calcified plaque area (MCPA) of the target vessel in the CN group than in the non-CN group. Receiver operating characteristic curve indicated that CACS ≥ 162 (area under the ROC curve (AUC 0.76, sensitivity 83.3%, specificity 54.2%), CPV ≥ 20.1 mm3 (AUC 0.83, sensitivity 100%, specificity 57.3%), and MCPA ≥ 4.51 mm2 (AUC 0.87, sensitivity 91.7%, specificity 78.3%) were the best cutoff values for predicting CNs. MCPA showed the highest AUC among all the CCTA parameters. In conclusion, CCTA is useful for predicting OCT-detected CNs in PCI target vessels.

Project description:BackgroundVascular endothelial growth factor is well known for its angiogenesis potential. The study was performed to determine the possible pro-angiogenic role of magnetic nanoparticles coupled to VEGF in vitro and their capacity to cross an endothelial monolayer. This novel treatment technique for angiogenesis could be potentially useful for therapeutic purposes using magnetic nanoparticles.MethodsMagnetic nanoparticles (MN) were synthesized and were conjugated with the vascular endothelial growth factor. The particles were tested in vitro in a 2D to 3D culture system. MN was seeded in different positions in relation to an HUVEC spheroid to assess a preferential migration. To evaluate the MN capacity to cross the endothelial barrier, a confluent monolayer of HUVEC cells was seeded on top of a collagen gel. MN was placed in dissolution on the cell culture media, and the MN position was determined by confocal microscopy for 24 h.ResultsHUVEC spheroids were able to generate a preferential sprouting depending on the MN position. Meanwhile, there was random migration when the MN's were placed all over the collagen gel and no sprouting when no MN was added. The trans-endothelial migration capacity of the MN was observed after 20 h in culture in the absence of external stimuli.ConclusionHere we show in vitro angiogenesis following the distribution of the MN conjugated with growth factors. These nanoparticles could be controlled with a magnet to place them in the ischemic area of interest and speed up vascular recovery. Also, MN has potentials to cross endothelium, opening the doors to a possible intravascular and extravascular treatment.

Project description:We herein report a case of percutaneous coronary intervention to a heavily calcified chronic total occlusion in the left anterior descending artery. Although we successfully performed retrograde wire crossing and wire externalization, we were unable to deliver small-sized balloon catheters in the lesion antegradely, even with strong back-up of wire externalization because of the heavy calcium mass. However, a balloon catheter was easily crossed retrogradely, and the lesion was successfully treated. Thus, retrograde balloon crossing might be a way to overcome device delivery failure in calcified lesions. <Learning objective: Dense calcium often prevents devices from passing through a chronic coronary total occlusion. In the present case, retrograde delivery through an epicardial collateral channel succeeded in balloon crossing, because changing the direction reversed the distribution of calcium for the passage.>.

Angiography revealed an exophytic calcified nodules in the distal superficial femoral artery. Angioscopy also revealed abundant exophytic atherosclerotic calcification.
