Adjusting the family's life: A grounded theory of caring for children with special healthcare needs in rural areas, Thailand.

ABSTRACT: This study aims to understand the experiences of families of children with special healthcare needs in rural areas in Thailand. Grounded theory (GT) was employed to understand families' experiences when caring for children with special healthcare needs (CSHCN) in rural areas. Forty-three family members from thirty-four families with CSHCN participated in in-depth interviews. Interviews were recorded and transcribed. The constant comparative method was used for data analysis and coding analysis. Adjusting family's life was the emergent theory which included experiencing negative effects, managing in home environment, integrating care into a community health system, and maintaining family normalization. This study describes the process that families undergo in trying to care for CSHCN while managing their lives to maintain a sense of normalcy. This theory provides some intervention opportunities for health care professionals when dealing with the complexities in their homes, communities and other ambulatory settings throughout the disease trajectory, and also indicates the importance of taking into consideration the family's cultural background.

Project description:Software development is an iterative process from designing to implementation, and to testing, in which product development staff should be closely integrated with users. Satisfying user needs effectively is often the pain point for developers. In order to alleviate this, this paper manages to establish the quantitative connection between users' online reviews and APP (Application Program) downloads. By analyzing user online comments, companies can dig out user needs and preferences. This could benefit them by making accurate market positioning of their APP products, and therefore iteratively innovating products based on user needs, which hopefully will increase the volume of APP downloads. This paper regards WeChat APP during 47 updates periods as the research object. Based on Grounded Theory, user needs are extracted after data cleaning. Next, by using semantic analysis and word frequency analysis, we are able to obtain the implicit feedbacks such as emotion tendency, satisfaction and requirements lie under online reviews. Then, we construct a quantile regression model to study the impact of users' online reviews on downloads based on the influencing factors we extracted so as to provide a decision basis for enterprises to iteratively update their products. Results show that: (1) Generally speaking, needs of WeChat users mainly focus on performance, reliability, usability, functional deficiency, functional insufficiency, and system adaptability; (2) For those APP versions with relatively fewer downloads, user needs are mostly about functional deficiency, followed by functional insufficiency, performance, usability, and system adaptability. At this stage, it is found out that users' emotion tendency and user satisfaction significantly affect the volume of downloads; (3) When the volume of APP downloads is moderate, the user needs are functional deficiency, functional insufficiency, and system adaptability. While under this circumstances, users' star ratings have a significant impact on downloads; (4) In addition, when the volume of App downloads is high, user needs are performance, usability, and system adaptability. Our methods effectively extract users' requirements from online reviews and then successfully build up the quantitative connection between the implicit feedbacks from those requirements and APP downloads.

Project description:BackgroundTriage is a dynamic process prioritising the patient coming to the emergency department. Caring behaviour and patient safety during the triage process are essential for ensuring a good care experience and treatment outcome.ObjectiveTo describe triage nurses' perceptions on caring behaviors and patient safety in the triage area.DesignStrauss and Corbin's Grounded theory method was used to develop the model.MethodsThe study was conducted in the emergency department in northeastern Slovenia. Semi-structured interviews were used for data collection, and 19 triage nurses were selected by theoretical sampling, guided by emerging categories between November 2021 and July 2022. The data analysis was conducted according to Strauss and Corbin's coding framework.ResultsThe analysis of the interviews generated one category: The process of creating a caring and safe triage encounter for the patient, together with two categories that explain the key phenomenon: (1) Triage caring and (2) Safety in the triage process. Within the category "Triage caring", four subcategories were developed: (1) Assurance of triage nurses' presence, (2) Connectedness, (3) Respectful attitude, and (4) Knowledge and skills. The category Safety in the triage process consists of three identified subcategories: (1) Conception and perception of safety, (2) Factors influencing patient safety, and (3) Improving the triage safety.ConclusionsThe triage nurses' perceptions about caring for the patient and his safety in the triage area show that caring and safety are inseparably linked and coincide when triaging a patient. Namely, caring for the patient means ensuring the patient's safety at the same time.Implications for the nursing fieldA better understanding of the importance of triage nurses' caring behavior and patient safety emerges from the findings, highlighting the challenges faced in a busy emergency department where nurses must balance providing care and responding to patients' needs while ensuring safety. Findings in the study show that patient care and safety are inseparably linked and coincide when triaging a patient. Moreover, applying caring behaviour during triage encounter results in greater patient safety.No patient or public contributionThe study's design, evaluation of the findings, and execution did not need the involvement of patients or the general public. Participants were triage nurses working in the emergency department. Triage nurses were interviewed about their perceptions of triage nurses on caring behaviors and patient safety during triage encounter.

Project description:BackgroundChild undernutrition remains a serious public health concern particularly in poor households where limited financial resources hinder proper child nutrition. Improving women's access to productive resources and employment is crucial for addressing undernutrition. However, maternal employment may reduce the amount of time available for childcare, leading to poor child nutrition and health. This study explored rural community perceptions and experiences regarding maternal employment in relation to aspects of child nutrition and health in northeastern Uganda to inform the design of interventions that can effectively support maternal employment without compromising child nutrition and health.MethodsTwelve gender-separated focus group discussions (FGDs) were conducted with pastoral and agro-pastoral communities in northeastern Uganda. Data from the FGDs were analysed using a constructivist grounded theory approach.ResultsAnalysis of the FGD data revealed that cultural norms and economic pressure were crucial factors in maternal employment decisions. We constructed a model that represents maternal employment as a delicate balance to maximize the benefits for child nutrition and health while minimizing the negative consequences. The model comprises the core category, mothers' solitary struggle for better child nutrition and health, and four interrelated categories: (1) mothers conforming to societal expectations, (2) mediating the effect of maternal employment on child nutrition and health, (3) suffering consequences of maternal employment, and (4) mothers striving for better child nutrition and health.ConclusionAccess to income emerged as the main pathway through which maternal employment leads to better child nutrition and health. However, this benefit often comes at the cost of reduced time for childcare, which is perceived to be detrimental to child nutrition and health. Based on these results, enhancing access to productive resources and employment, particularly for women is crucial for addressing undernutrition. To ensure that maternal employment does not compromise childcare, policies must support the development and access to childcare options that are affordable and appropriate for women in rural areas and in informal employment to effectively balance employment and childcare.
