The World Health Organization has recommended the use of the Sum of O<sub>3</sub> Means Over 35 ppb (SOMO35) to perform Health Impact Assessments (HIA) for long-term exposure to O<sub>3</sub>. Project description:China is in a critical air quality management stage. Rapid industrial development and urbanization has resulted in non-ignorable air pollution, which seriously endangers human health. Assessment of the health impacts and economic losses of air pollution is essential for the prevention and control policy formulation. Based on ozone (O3) and fine particulate matter concentration (PM2.5) monitoring data in 331 Chinese cities from 2015 to 2020, this study evaluated the health effects and the corresponding economic losses of O3 and PM2.5 pollution on three health endpoints. The ratio of population exposed to O3 levels that exceeded the Chinese Ambient Air Quality Standards (CAAQS) increased from 13.35% in 2015 to 14.15% in 2020, which resulted in 133,415 (2015) - 156,173 (2020) all-cause deaths, 88,941 (2015) - 104,051 (2020) cardiovascular deaths, and 28,614 (2015) - 33,456 (2020) respiratory deaths. The ratio of population exposed to PM2.5 levels that exceeded the CAAQS decreased, but in many regions, especially in North China and the Yangtze River Delta, the PM2.5 concentration remained high. By 2020, nearly half of the population in China was still exposed to PM2.5 levels that exceeded the CAAQS, and the corresponding economic losses reached CNY 3.46 and 3.05 billion, respectively. These results improved the understanding of the spatial-temporal variation trends of major air pollutants at city scale in China, and emphasize the continued coordination urgently needed for controlling O3 and PM2.5 following the implementation of the 2013 policy to mitigate air pollution to protect human health.

Project description:Background: Bicycling has been associated with health benefits. Local and national authorities have been promoting bicycling as a tool to improve public health and the environment. Mexico is one of the largest Latin American countries, with high levels of sedentarism and non-communicable diseases. No previous studies have estimated the health impacts of Mexico's national bicycling scenarios. Aim: Quantify the health impacts of Mexico urban bicycling scenarios. Methodology: Quantitative Health Impact Assessment, estimating health risks and benefits of bicycling scenarios in 51,718,756 adult urban inhabitants in Mexico (between 20 and 64 years old). Five bike scenarios were created based on current bike trends in Mexico. The number of premature deaths (increased or reduced) was estimated in relation to physical activity, road traffic fatalities, and air pollution. Input data were collected from national publicly available data sources from transport, environment, health and population reports, and surveys, in addition to scientific literature. Results: We estimated that nine premature deaths are prevented each year among urban populations in Mexico on the current car-bike substitution and trip levels (1% of bike trips), with an annual health economic benefit of US $1,897,920. If Mexico achieves similar trip levels to those reported in The Netherlands (27% of bike trips), 217 premature deaths could be saved annually, with an economic impact of US $45,760,960. In all bicycling scenarios assessed in Mexico, physical activity's health benefits outweighed the health risks related to traffic fatalities and air pollution exposure. Conclusion: The study found that bicycling promotion in Mexico would provide important health benefits. The benefits of physical activity outweigh the risk from traffic fatalities and air pollution exposure in bicyclists. At the national level, Mexico could consider using sustainable transport policies as a tool to promote public health. Specifically, the support of active transportation through bicycling and urban design improvements could encourage physical activity and its health co-benefits.

Project description:UNLABELLED:Geostatistical interpolation methods to estimate individual exposure to outdoor air pollutants can be used in pregnancy cohorts where personal exposure data are not collected. Our objectives were to a) develop four assessment methods (citywide average (CWA); nearest monitor (NM); inverse distance weighting (IDW); and ordinary Kriging (OK)), and b) compare daily metrics and cross-validations of interpolation models. We obtained 2008 hourly data from Mexico City's outdoor air monitoring network for PM10, PM2.5, O3, CO, NO2, and SO2 and constructed daily exposure metrics for 1,000 simulated individual locations across five populated geographic zones. Descriptive statistics from all methods were calculated for dry and wet seasons, and by zone. We also evaluated IDW and OK methods' ability to predict measured concentrations at monitors using cross validation and a coefficient of variation (COV). All methods were performed using SAS 9.3, except ordinary Kriging which was modeled using R's gstat package. Overall, mean concentrations and standard deviations were similar among the different methods for each pollutant. Correlations between methods were generally high (r=0.77 to 0.99). However, ranges of estimated concentrations determined by NM, IDW, and OK were wider than the ranges for CWA. Root mean square errors for OK were consistently equal to or lower than for the IDW method. OK standard errors varied considerably between pollutants and the computed COVs ranged from 0.46 (least error) for SO2 and PM10 to 3.91 (most error) for PM2.5. OK predicted concentrations measured at the monitors better than IDW and NM. Given the similarity in results for the exposure methods, OK is preferred because this method alone provides predicted standard errors which can be incorporated in statistical models. The daily estimated exposures calculated using these different exposure methods provide flexibility to evaluate multiple windows of exposure during pregnancy, not just trimester or pregnancy-long exposures. IMPLICATIONS:Many studies evaluating associations between outdoor air pollution and adverse pregnancy outcomes rely on outdoor air pollution monitoring data linked to information gathered from large birth registries, and often lack residence location information needed to estimate individual exposure. This study simulated 1,000 residential locations to evaluate four air pollution exposure assessment methods, and describes possible exposure misclassification from using spatial averaging versus geostatistical interpolation models. An implication of this work is that policies to reduce air pollution and exposure among pregnant women based on epidemiologic literature should take into account possible error in estimates of effect when spatial averages alone are evaluated.

Project description:Background: Particulate air pollution, especially PM2.5, is highly correlated with various adverse health impacts and, ultimately, economic losses for society, however, few studies have undertaken a spatiotemporal assessment of PM2.5-related economic losses from health impacts covering all of the main cities in China. Methods: PM2.5 concentration data were retrieved for 190 Chinese cities for the period 2014⁻2016. We used a log-linear exposure⁻response model and monetary valuation methods, such as value of a statistical life (VSL), amended human capital (AHC), and cost of illness to evaluate PM2.5-related economic losses from health impacts at the city level. In addition, Monte Carlo simulation was used to analyze uncertainty. Results: The average economic loss was 0.3% (AHC) to 1% (VSL) of the total gross domestic product (GDP) of 190 Chinese cities from 2014 to 2016. Overall, China experienced a downward trend in total economic losses over the three-year period, but the Beijing⁻Tianjin⁻Hebei, Shandong Peninsula, Yangtze River Delta, and Chengdu-Chongqing regions experienced greater annual economic losses. Conclusions: Exploration of spatiotemporal variations in PM2.5-related economic losses from long-term health impacts could provide new information for policymakers regarding priority areas for PM2.5 pollution prevention and control in China.

Project description:Health technology assessment (HTA) provides evidence-based information on healthcare technology to support decision making in many countries. Environmental impact is a relevant dimension of a health technology's value, but it has been poorly addressed in HTA processes in spite of the commitment that the health sector must have to contribute to mitigating the effects of climate change. This study aims to identify the state of the art and challenges for quantifying environmental impacts that could be incorporated into the economic evaluation (EE) of HTA. We performed a scoping review that included 22 articles grouped into four types of contribution: (1) concepts to draw up a theoretical framework, (2) HTA reports, (3) parameter designs or suitable indicators, and (4) economic or budgetary impact assessments. This review shows that evaluation of the environmental impact of HTAs is still very incipient. Small steps are being taken in EE, such as carbon footprint estimations from a life-cycle approach of technologies and the entire care pathway.

Project description:CO2-enhanced oil recovery (EOR) can have less GHG emissions compared to conventional oil production methods. The economy of CO2-EOR can significantly benefit from the recent rise of carbon prices in carbon markets due to its greenhouse gas (GHG) emission savings. This study conducted a life cycle assessment (LCA) of CO2-EOR in major hydrocarbon provinces of the world. Estimated net GHG emissions of CO2-EOR were compared with GHG emissions of average produced oil in the given country. When sourcing CO2 from coal-fired power plants, Kazakhstan and China have net GHG emissions of CO2-EOR of 276 and 380 kg CO2 eq/bbl, respectively, which are lower than the GHG emission factor of average oil produced in each of them. Significantly lower GHG emissions of CO2-EOR are observed in other hydrocarbon provinces (Iraq, Saudi Arabia, Kuwait, etc.), where CO2 could be delivered from Natural Gas Combined Cycle (NGCC) power plants. However, the cost of CO2 capture is higher at NGCC power plants than at coal-fired power plants. Further, we developed a techno-economic assessment (TEA) model of the CO2-EOR and integrated it with LCA to thoroughly consider carbon credits in its economy. The model was built based upon previous investigations and used statistics from a large industrial data set of CO2-EOR to produce accurate estimates of the CO2-EOR economy. The technical model iteratively estimated the balance of three fluids (crude oil, CO2, and water) in the CO2-EOR system with a 25 year operational lifespan and obtained actual data for the LCA and TEA models. The model was simulated for the Kazakhstan case with its oil market conditions for a demonstration purpose. TEA results showed that, with the available low-cost CO2 capture source or high CO2 cost in carbon trading, CO2-EOR can compete with current upstream projects in Kazakhstan by simultaneously increasing oil production and reducing GHG emissions.

Project description:In recent years, wildland fires have occurred more frequently and with increased intensity in many fire-prone areas. In addition to the direct life and economic losses attributable to wildfires, the emitted smoke is a major contributor to ambient air pollution, leading to significant public health impacts. Wildfire smoke is a complex mixture of particulate matter (PM), gases such as carbon monoxide, nitrogen oxide, and volatile and semi-volatile organic compounds. PM from wildfire smoke has a high content of elemental carbon and organic carbon, with lesser amounts of metal compounds. Epidemiological studies have consistently found an association between exposure to wildfire smoke (typically monitored as the PM concentration) and increased respiratory morbidity and mortality. However, previous reviews of the health effects of wildfire smoke exposure have not established a conclusive link between wildfire smoke exposure and adverse cardiovascular effects. In this review, we systematically evaluate published epidemiological observations, controlled clinical exposure studies, and toxicological studies focusing on evidence of wildfire smoke exposure and cardiovascular effects, and identify knowledge gaps. Improving exposure assessment and identifying sensitive cardiovascular endpoints will serve to better understand the association between exposure to wildfire smoke and cardiovascular effects and the mechanisms involved. Similarly, filling the knowledge gaps identified in this review will better define adverse cardiovascular health effects of exposure to wildfire smoke, thus informing risk assessments and potentially leading to the development of targeted interventional strategies to mitigate the health impacts of wildfire smoke.

Project description:Exposure to air pollution is a leading risk factor for premature death globally; however, the complexity of its formation and the diversity of its sources can make it difficult to address. The Group of Twenty (G20) countries are a collection of the world's largest and most influential economies and are uniquely poised to take action to reduce the global health burden associated with air pollution. We present a framework capable of simultaneously identifying regional and sectoral sources of the health impacts associated with two air pollutants, fine particulate matter (PM2.5) and ozone (O3) in G20 countries; this framework is also used to assess the health impacts associated with emission reductions. This approach combines GEOS-Chem adjoint sensitivities, satellite-derived data, and a new framework designed to better characterize the non-linear relationship between O3 exposures and nitrogen oxides emissions. From this approach, we estimate that a 50% reduction of land transportation emissions by 2040 would result in 251 thousand premature deaths avoided in G20 countries. These premature deaths would be attributable equally to reductions in PM2.5 and O3 exposure which make up 51% and 49% of the potential benefits, respectively. In our second application, we estimate that the energy generation related co-benefits associated with G20 countries staying on pace with their net-zero carbon dioxide targets would be 290 thousand premature deaths avoided in 2040; action by India (47%) would result in the most benefits of any country and a majority of these avoided deaths would be attributable to reductions in PM2.5 exposure (68%).

Quantitative analysis of the prospects of this technology is hindered by the lack of flexible techno-economic assessment (TEA) models that enable evaluation of the processing costs under different deployment scenarios. In this protocol, we explain how to convert literature data into metrics useful for evaluation of the emerging electrolysis technologies, derive TEA models, and illustrate their use with a CO2-to-ethylene example. Project description:BackgroundChildhood exposure to air pollution has been linked with maladaptive cognitive development; however, less is known about the association between prenatal fine particulate matter (PM2.5) exposure and childhood behavior.ObjectivesOur aim was to assess the association between prenatal PM2.5 exposure and behavioral development in 4-6 year old children residing in Mexico City.MethodsWe used data from 539 mother-child pairs enrolled in a prospective birth cohort in Mexico City. We estimated daily PM2.5 exposure using a 1 km2 satellite-based exposure model and averaged over each trimester of pregnancy. We assessed childhood behavior at 4-6 years of age using the parent-completed Behavioral Assessment Scale for Children (BASC-2) composite scores and subscales. We used linear regression models to estimate change in BASC-2 T-scores with trimester specific 5-μg/m3 increases in PM2.5. All models were mutually adjusted for PM2.5 exposures during the other trimesters, maternal factors including age, education, socioeconomic status, depression, and IQ, child's age at study visit, and season. We additionally assessed sex-specific effects by including an interaction term between PM2.5 and sex.ResultsHigher first trimester PM2.5 exposure was associated with reduced Adaptive Skills scores (β: -1.45, 95% CI: -2.60, -0.30). Lower scores on the Adaptive Skills composite score and subscales indicate poorer functioning. For PM2.5 exposure during the first trimester, decrements were consistent across adaptive subscale scores including Adaptability (β: -1.51, 95% CI: -2.72, -0.30), Social Skills (β: -1.63, 95% CI: -2.90, -0.36), and Functional Communication (β: -1.21, 95% CI: -2.21, -0.21). The association between 1st trimester PM2.5 and depression was stronger in males than females (β for males: 1.52, 95% CI: -0.41, 3.45; β for females: -0.13, 95% CI: -1.99, 1.72; p-int: 0.07).ConclusionsExposure to PM2.5 during early pregnancy may be associated with impaired behavioral development in children, particularly for measures of adaptive skills. These results suggest that air pollution impacts behavioral domains as well as cognition, and that the timing of exposure may be critical.
