Semiparametric regression analysis of length-biased interval-censored data.

ABSTRACT: In prevalent cohort design, subjects who have experienced an initial event but not the failure event are preferentially enrolled and the observed failure times are often length-biased. Moreover, the prospective follow-up may not be continuously monitored and failure times are subject to interval censoring. We study the nonparametric maximum likelihood estimation for the proportional hazards model with length-biased interval-censored data. Direct maximization of likelihood function is intractable, thus we develop a computationally simple and stable expectation-maximization algorithm through introducing two layers of data augmentation. We establish the strong consistency, asymptotic normality and efficiency of the proposed estimator and provide an inferential procedure through profile likelihood. We assess the performance of the proposed methods through extensive simulations and apply the proposed methods to the Massachusetts Health Care Panel Study.

Project description:Interval-censored competing risks data arise when each study subject may experience an event or failure from one of several causes and the failure time is not observed directly but rather is known to lie in an interval between two examinations. We formulate the effects of possibly time-varying (external) covariates on the cumulative incidence or sub-distribution function of competing risks (i.e., the marginal probability of failure from a specific cause) through a broad class of semiparametric regression models that captures both proportional and non-proportional hazards structures for the sub-distribution. We allow each subject to have an arbitrary number of examinations and accommodate missing information on the cause of failure. We consider nonparametric maximum likelihood estimation and devise a fast and stable EM-type algorithm for its computation. We then establish the consistency, asymptotic normality, and semiparametric efficiency of the resulting estimators for the regression parameters by appealing to modern empirical process theory. In addition, we show through extensive simulation studies that the proposed methods perform well in realistic situations. Finally, we provide an application to a study on HIV-1 infection with different viral subtypes.

Semiparametric regression analysis of interval-censored data with informative dropout.

Project description:Interval-censored data arise when the event time of interest can only be ascertained through periodic examinations. In medical studies, subjects may not complete the examination schedule for reasons related to the event of interest. In this article, we develop a semiparametric approach to adjust for such informative dropout in regression analysis of interval-censored data. Specifically, we propose a broad class of joint models, under which the event time of interest follows a transformation model with a random effect and the dropout time follows a different transformation model but with the same random effect. We consider nonparametric maximum likelihood estimation and develop an EM algorithm that involves simple and stable calculations. We prove that the resulting estimators of the regression parameters are consistent, asymptotically normal, and asymptotically efficient with a covariance matrix that can be consistently estimated through profile likelihood. In addition, we show how to consistently estimate the survival function when dropout represents voluntary withdrawal and the cumulative incidence function when dropout is an unavoidable terminal event. Furthermore, we assess the performance of the proposed numerical and inferential procedures through extensive simulation studies. Finally, we provide an application to data on the incidence of diabetes from a major epidemiological cohort study.

A Semiparametric Regression Cure Model for Interval-Censored Data.

Project description:Motivated by medical studies in which patients could be cured of disease but the disease event time may be subject to interval censoring, we presents a semiparametric non-mixture cure model for the regression analysis of interval-censored time-to-event datxa. We develop semiparametric maximum likelihood estimation for the model using the expectation-maximization method for interval-censored data. The maximization step for the baseline function is nonparametric and numerically challenging. We develop an efficient and numerically stable algorithm via modern convex optimization techniques, yielding a self-consistency algorithm for the maximization step. We prove the strong consistency of the maximum likelihood estimators under the Hellinger distance, which is an appropriate metric for the asymptotic property of the estimators for interval-censored data. We assess the performance of the estimators in a simulation study with small to moderate sample sizes. To illustrate the method, we also analyze a real data set from a medical study for the biochemical recurrence of prostate cancer among patients who have undergone radical prostatectomy. Semiparametric Regression Analysis of Multiple Right- and Interval-Censored Events.

Project description:Health sciences research often involves both right- and interval-censored events because the occurrence of a symptomatic disease can only be observed up to the end of follow-up, while the occurrence of an asymptomatic disease can only be detected through periodic examinations. We formulate the effects of potentially time-dependent covariates on the joint distribution of multiple right- and interval-censored events through semiparametric proportional hazards models with random effects that capture the dependence both within and between the two types of events. We consider nonparametric maximum likelihood estimation and develop a simple and stable EM algorithm for computation. We show that the resulting estimators are consistent and the parametric components are asymptotically normal and efficient with a covariance matrix that can be consistently estimated by profile likelihood or nonparametric bootstrap. In addition, we leverage the joint modelling to provide dynamic prediction of disease incidence based on the evolving event history. Furthermore, we assess the performance of the proposed methods through extensive simulation studies. Finally, we provide an application to a major epidemiological cohort study. Maximum likelihood estimation for semiparametric regression models with multivariate interval-censored data.

Project description:Interval-censored multivariate failure time data arise when there are multiple types of failure or there is clustering of study subjects and each failure time is known only to lie in a certain interval. We investigate the effects of possibly time-dependent covariates on multivariate failure times by considering a broad class of semiparametric transformation models with random effects, and we study nonparametric maximum likelihood estimation under general interval-censoring schemes. We show that the proposed estimators for the finite-dimensional parameters are consistent and asymptotically normal, with a limiting covariance matrix that attains the semiparametric efficiency bound and can be consistently estimated through profile likelihood. In addition, we develop an EM algorithm that converges stably for arbitrary datasets. Finally, we assess the performance of the proposed methods in extensive simulation studies and illustrate their application using data derived from the Atherosclerosis Risk in Communities Study.

Semiparametric analysis of correlated and interval-censored event-history data.

Project description:We propose a semiparametric multi-state frailty model to analyze clustered event-history data subject to interval censoring. The proposed model is motivated by an attempt to study the life course of dental caries at the tooth level, taking into account the multiplicity of caries states and the intra-oral clustering of observations made at periodic time points. Of particular interest is the study of the intra-oral distribution of processes leading to carious lesions, and whether this distribution varies with gender. The model assumes, in view of the covariate profile, a proportionality of the transition intensities conditional on subject-level frailties, coupled with a linear spline approximation of the log baseline intensities. The model estimation is conducted using a penalized likelihood where the smoothing parameters are estimated as reciprocal variance components under a mixed-model representation. A Bayesian method is proposed to predict tooth-level caries transition probabilities, which can be used for tailoring tooth-level caries treatment and prevention plans. Intensive simulation studies indicate that the model fitting and prediction perform reasonably well under realistic sample sizes. The practical utility of the methods is illustrated using data from a longitudinal study on oral health among children from low-income families residing in the city of Detroit, Michigan.

Semiparametric regression analysis of partly interval-censored failure time data with application to an AIDS clinical trial.

Project description:Failure time data subject to various types of censoring commonly arise in epidemiological and biomedical studies. Motivated by an AIDS clinical trial, we consider regression analysis of failure time data that include exact and left-, interval-, and/or right-censored observations, which are often referred to as partly interval-censored failure time data. We study the effects of potentially time-dependent covariates on partly interval-censored failure time via a class of semiparametric transformation models that includes the widely used proportional hazards model and the proportional odds model as special cases. We propose an EM algorithm for the nonparametric maximum likelihood estimation and show that it unifies some existing approaches developed for traditional right-censored data or purely interval-censored data. In particular, the proposed method reduces to the partial likelihood approach in the case of right-censored data under the proportional hazards model. We establish that the resulting estimator is consistent and asymptotically normal. In addition, we investigate the proposed method via simulation studies and apply it to the motivating AIDS clinical trial.

"Smooth" semiparametric regression analysis for arbitrarily censored time-to-event data.

Project description:A general framework for regression analysis of time-to-event data subject to arbitrary patterns of censoring is proposed. The approach is relevant when the analyst is willing to assume that distributions governing model components that are ordinarily left unspecified in popular semiparametric regression models, such as the baseline hazard function in the proportional hazards model, have densities satisfying mild "smoothness" conditions. Densities are approximated by a truncated series expansion that, for fixed degree of truncation, results in a "parametric" representation, which makes likelihood-based inference coupled with adaptive choice of the degree of truncation, and hence flexibility of the model, computationally and conceptually straightforward with data subject to any pattern of censoring. The formulation allows popular models, such as the proportional hazards, proportional odds, and accelerated failure time models, to be placed in a common framework; provides a principled basis for choosing among them; and renders useful extensions of the models straightforward. The utility and performance of the methods are demonstrated via simulations and by application to data from time-to-event studies.

Semiparametric regression on cumulative incidence function with interval-censored competing risks data and missing event types.

Project description:Competing risk data are frequently interval-censored, that is, the exact event time is not observed but only known to lie between two examination time points such as clinic visits. In addition to interval censoring, another common complication is that the event type is missing for some study participants. In this article, we propose an augmented inverse probability weighted sieve maximum likelihood estimator for the analysis of interval-censored competing risk data in the presence of missing event types. The estimator imposes weaker than usual missing at random assumptions by allowing for the inclusion of auxiliary variables that are potentially associated with the probability of missingness. The proposed estimator is shown to be doubly robust, in the sense that it is consistent even if either the model for the probability of missingness or the model for the probability of the event type is misspecified. Extensive Monte Carlo simulation studies show good performance of the proposed method even under a large amount of missing event types. The method is illustrated using data from an HIV cohort study in sub-Saharan Africa, where a significant portion of events types is missing. The proposed method can be readily implemented using the new function ciregic_aipw in the R package intccr.

Marginal Bayesian Semiparametric Modeling of Mismeasured Multivariate Interval-Censored Data.

Project description:Motivated by data gathered in an oral health study, we propose a Bayesian nonparametric approach for population-averaged modeling of correlated time-to-event data, when the responses can only be determined to lie in an interval obtained from a sequence of examination times and the determination of the occurrence of the event is subject to misclassification. The joint model for the true, unobserved time-to-event data is defined semiparametrically; proportional hazards, proportional odds, and accelerated failure time (proportional quantiles) are all fit and compared. The baseline distribution is modeled as a flexible tailfree prior. The joint model is completed by considering a parametric copula function. A general misclassification model is discussed in detail, considering the possibility that different examiners were involved in the assessment of the occurrence of the events for a given subject across time. We provide empirical evidence that the model can be used to estimate the underlying time-to-event distribution and the misclassification parameters without any external information about the latter parameters. We also illustrate the effect on the statistical inferences of neglecting the presence of misclassification.
