Arthroscopic Segmental Medial Meniscus Allograft Transplant Using Three Fixation Techniques.

ABSTRACT: Partial meniscectomy or failed meniscus repair can lead to pain, dysfunction, and cartilage degradation due to increased contact forces. Meniscus transplantation can lead to favorable outcomes and cartilage preservation with careful patient selection. Limited data exist on segmental meniscus allograft transplantation, with promising results using synthetic grafts and early animal and biomechanical studies on segmental allograft transplantation, showing similar results to full meniscus allograft transplantation. This article presents a technique for arthroscopic segmental medial meniscus allograft transplant and a brief review of the literature.

Arthroscopic Medial Meniscus Posterior Root Fixation Using a Modified Mason-Allen Stitch.

Project description:A complete radial tear of the meniscus posterior root, which can effectively cause a state of total meniscectomy via loss of hoop tension, requires that the torn root be repaired. Several methods have been used to repair medial meniscus posterior root tears, most of which are based on a simple stitch technique that is known to have stitch-holding strength. We applied a modified version of the Mason-Allen stitch technique, which is recognized as a method for rotator cuff repair surgery because its locking effect overcomes the potential weakness of simple stitches. This article introduces the medial meniscus posterior root tears repair procedure based on a modified Mason-Allen stitch technique in which 2 strands (i.e., 1 simple horizontal and 1 simple vertical stitch) are used.

Segmental Meniscus Allograft Transplantation.

Project description:Meniscal tears treated with partial meniscectomies have been shown to significantly increase contract pressures within the tibiofemoral joint, and a complete focal meniscal deficiency may render the entirety of the meniscus functionally incompetent. Although various techniques of meniscal transplantation have been described, these techniques may require the excision of a considerable amount of healthy meniscal tissue. Furthermore, failures continue to frequently occur. Therefore, attempts to restoring normal knee kinematics and biomechanical forces are essential. Segmental meniscus allograft transplantations may offer the advantage of a robust repair by both maintaining knee biomechanics and biology while maximizing preservation of native meniscal tissue. Also, most meniscal deficiency involves only a portion of the meniscus, and thus we developed this technique to segmentally transplant only the deficient portion. The purpose of this Technical Note is to describe a technique of segmental medial meniscus allograft transplantation in a patient with focal medial meniscus deficiency.

Arthroscopic Centralization Using Knotless Anchors for Extruded Medial Meniscus.

Project description:The load-distributing function is most critical in meniscal function, and meniscal extrusion suggests failure of this function, leading to the progression of osteoarthritis. The arthroscopic centralization technique has been developed to reduce meniscal extrusion; however, existing arthroscopic techniques sometimes fail to reduce the most extruded region, especially in cases with a medial meniscus (MM) posterior root tear, in which the most extruded region is on the posterior border of the medial collateral ligament, which is very difficult to approach. This Technical Note describes an arthroscopic technique for extrusion of the MM in which a centralization technique using knotless anchors efficiently reduces the MM extrusion at the posteromedial part and consequently restores the MM function. This technique efficiently reduces MM extrusion and restores its function, thus preventing the progression of osteoarthritis.

Project description:There have been several techniques to repair the medial meniscus posterior root tears (MMPRTs) with the goal of restoring the anatomic and firm fixation of the meniscal root to bone. Many anatomic studies about the menisci also have been developed, so a better understanding of the anatomy could help surgeons perform correct fixation of the MMPRTs. The meniscal roots have ligament-like structures that firmly attach the menisci to the tibial plateau, and this structural concept is important to restore normal biomechanics after anatomic root repair. We present arthroscopic transtibial medial meniscus posterior root reconstruction using auto-gracilis tendon.

Arthroscopic All-Inside Medial Meniscus Extrusion Reduction.

Project description:Medial meniscus extrusion is commonly seen in patients who have medial meniscus posterior root tear. Extruded meniscus results in faster progression of knee arthrosis. Thus, it is important to reduce the extrusion as well as treat the cause of extrusion. This Technical Note describes an all-inside arthroscopic technique to reduce the meniscus extrusion. An additional medial portal has to be made along with the standard anteromedial and anterolateral portals. A double-loaded suture anchor is used to secure the extrusion of the meniscus in its native position. Thus, making a transosseous tibial tunnel is not required. It is easy to perform and is an efficient technique.

Arthroscopic Centralization of the Extruded Medial Meniscus.

Project description:Tears of the posterior medial meniscus root commonly result in extrusion of the meniscus and disruption of tibiofemoral contact mechanics. Transtibial pull-through repair of the root often results in healing of the tear, but postoperative extrusion may persist. In this scenario, the meniscus is unlikely to be chondroprotective. Therefore, an additional centralization procedure is necessary to improve the extrusion. Biomechanical studies have demonstrated that centralization can improve meniscus mechanics and potentially reduce the risk of osteoarthritis. This Technical Note describes an arthroscopic technique for medial meniscus posterior root repair that combines transtibial pullout and centralization sutures.

Arthroscopic Linear Chain Fixation for the Treatment of Medial Meniscus Posterior Root Tear: A Case Study.

Project description:BackgroundThe repair and reconstruction of medial meniscus posterior root tears (MMPRTs) is an important issue in the field of orthopedic sports medicine. This study reports the first application of arthroscopic linear chain fixation for the treatment of MMPRTs.Case presentationA 78-year-old female patient presented with a 1.5-month history of right knee pain accompanied by a locked facet joint. The patient underwent surgery with the new linear chain fixation method. In this method, the suture and the loop part of the buckle-strap titanium plate were combined into a linear chain mechanical complex, and the tension of the posterior root stump was gradually increased by pulling on the two attachment lines at the external mouth of the tibial tunnel. The postoperative Lysholm score was 89, and the visual analogue scale score was 0.9, indicating a significant improvement in knee joint function. At the 7-month and 1-year post-surgery follow-up, physical and MRI examinations confirmed satisfactory healing of the MMPRTs.ConclusionThis surgical approach offers several benefits, including a simplified instrumentation setup, preservation of natural anatomical structures, and reliable residual stump fixation. It has the potential for clinical implementation.
