What motivates individual public service motivation and cooperation at the initial stage of the COVID-19 outbreak: A cross-sectional survey.

ABSTRACT:

Objectives

This study assesses the status of public service motivation and explains it's the positive association with public cooperation during the initial stage of the COVID-19 crisis. Moreover, potential causes of Chinese citizens' public service motivation have been explored.

Methods

A cross-sectional survey of 30 provinces in China was conducted using an online questionnaire. The study was conducted in February 2020 during the initial stage of the COVID-19 pandemic. Socio-demographic factors, public service motivation, public satisfaction, public confidence, and public cooperation were assessed using questionnaires. Hierarchical multiple regression was used to identify clusters of interrelationships among public service motivation, public satisfaction, public confidence, and public cooperation among Chinese citizens.

Results

We found that participants' public satisfaction with COVID-19-related public services had a positive association with public confidence (B = 0.456, p < 0.001) and public service motivation (B = 0.177, p < 0.001). Moreover, public confidence regarding anti-COVID-19 measures had a positive influence on public service motivation (B = 0.308, p < 0.001) while mediating the relationship between public satisfaction and public service motivation. Public service motivation were positively associated with public cooperation with anti-COVID-19 measures during the initial stage of the COVID-19 pandemic.

Conclusion

A considerably high level of public service motivation among Chinese citizens appeared during the initial stage of the COVID-19 pandemic, resulting in an increase in public cooperation. Moreover, the findings prove that a high degree of satisfaction with the government's initiatives regarding anti-COVID-19 measures encouraged greater personal public service motivation in respondents by increasing confidence in governmental responses and performance.

Project description:ObjectivesWe aimed at exploring the underlying mechanisms and contextual conditions by which leadership may influence 'public service motivation' of health providers in Moroccan hospitals.DesignWe used the realist evaluation (RE) approach in the following steps: eliciting the initial programme theory, designing the study, carrying out the data collection, doing the data analysis and synthesis. In practice, we adopted a multiple embedded case study design.SettingsWe used purposive sampling to select hospitals representing extreme cases displaying contrasting leadership practices and organisational performance scores using data from the Ministry of Health quality assurance programmes from 2011 to 2016.ParticipantsWe carried out, on average, 17 individual in-depth interviews in 4 hospitals as well as 7 focus group discussions and 8 group discussions with different cadres (administrators, nurses and doctors). We collected relevant documents (eg, performance audit, human resource availability) and carried out observations.ResultsComparing the Intervention-Context-Actor-Mechanism-Outcome configurations across the hospitals allowed us to confirm and refine our following programme theory: "Complex leaders, applying an appropriate mix of transactional, transformational and distributed leadership styles that fit organisational and individuals characteristics [I] can increase public service motivation, organisational commitment and extra role behaviours [O] by increasing perceived supervisor support and perceived organizational support and satisfying staff basic psychological needs [M], if the organisational culture is conducive and in the absence of perceived organisational politics [C]".ConclusionsIn hospitals, the archetype of complex professional bureaucracies, leaders need to be able to balance between different leadership styles according to the staff's profile, the nature of tasks and the organisational culture if they want to enhance public service motivation, intrinsic motivation and organisational commitment.

Project description:BackgroundCOVID-19 broke out in China and spread rapidly in January and February 2020. Following the prevention and control measures of the Chinese government, the outbreak was gradually brought under control after March. The changes in people's attention to the epidemic, individual prevention practice and psychological effect from the early outbreak stage to the under controlled stage need to be evaluated.MethodsTwo cross-sectional, population-based online surveys were conducted from January 28 to February 1, 2020 and from February1 to March 18, 2020. Socio-demographic information and individual protective practice were collected and the State-Trait Anxiety Inventory (STAI) was used for measuring anxiety. The range of STAI score was 5-25, and the higher the score, the more anxious it was. The respondents of the two surveys were matched on a one-to-one basis according to their province, gender, age, education, and marriage. Wilcoxon signed ranks test and Mann-Whitney U test were used to compare STAI score changes in two stages and in different demographic characteristics.ResultsWe included 9,764 individuals in the first survey and 1,669 in the second survey, covering 30 provincial administrative regions in Mainland China. COVID-19 has affected almost every aspect of people's normal life, especially lifestyle. The proportion of people who paid attention to it every day had dropped from 97.6 to 88.9%. We identified that vast majority people wore masks when they went out. The proportion has declined from 96.5 to 92.4% for hand hygiene and from 98.4 to 95.3% for not attending parties. People's anxiety (STAI score) across the country has decreased from a median of 19 in the early outbreak stage to a median of 12, including people with all demographic characteristics, but some have increased in 16 provinces.ConclusionPeople's attention to information about the epidemic has declined slightly, but a high proportion of people maintained good practices such as wearing masks, hand hygiene, and not attending parties. People's anxiety had generally declined from the early outbreak stage to the under controlled stage, but it was still at a high level.

Project description:BackgroundThe national tuberculosis strain typing service (TB-STS) was introduced in England in 2010. The TB-STS involves MIRU-VNTR typing of isolates from all TB patients for the prospective identification, reporting and investigation of TB strain typing clusters. As part of a mixed-method evaluation, we report on a repeated cross-sectional survey to illustrate the challenges surrounding the evaluation of a complex national public health intervention.MethodsAn online initial and follow-up questionnaire survey assessed the knowledge, attitudes and practices of public health staff, physicians and nurses working in TB control in November 2010 and March 2012. It included questions on the implementation, experience and uptake of the TB-STS. Participants that responded to both surveys were included in the analysis.Results248 participants responded to the initial survey and 137 of these responded to the follow-up survey (56% retention). Knowledge: A significant increase in knowledge was observed, including a rise in the proportion of respondents who had received training (28.6% to 67.9%, p = 0.003), and the self-rated knowledge of how to use strain typing had improved ('no knowledge' decreased from 43.2% to 27.4%). Attitudes: The majority of respondents found strain typing useful; the proportion that reported strain typing to be useful was similar across the two surveys (95.7% to 94.7%, p = 0.67). Practices: There were significant increases between the initial and follow-up surveys in the number of respondents who reported using strain typing (57.0% to 80.5%, p < 0.001) and the proportion of time health protection staff spent on investigating TB (2.74% to 7.08%, p = 0.04).ConclusionsEvaluation of a complex public health intervention is challenging. In this example, the immediate national roll-out of the TB-STS meant that a controlled survey design was not possible. This study informs the future development of the TB-STS by identifying the need for training to reach wider professional groups, and argues for its continuation based on service users' perception that it is useful. By highlighting the importance of a well-defined sampling frame, collecting baseline information, and including all stakeholders, it provides lessons for the implementation of similar services in other countries and future evaluations of public health interventions.
