Is Hemoglobin Concentration a Linear Predictor of Mortality in Older Adults From Chinese Longevity Regions?

ABSTRACT: Introduction: The association patterns of hemoglobin (HB) concentrations with mortality among the longevity older adults are unclear. We aimed to evaluate the relationship among older adults form Chinese longevity regions. Methods: We included 1,785 older adults aged ≥65 years (mean age, 86.7 years; 1,002 women, 783 men) from the community-based Chinese Longitudinal Healthy Longevity Survey. We estimated the hazard ratios (HRs) and 95% confidence intervals (CIs) for all-cause mortality using multivariable Cox proportional hazards models and Cox models with restricted cubic spline. Results: In total, 999 deaths occurred during a median follow-up of 5.4 years from 2011 to 2017. Restricted cubic spline analysis found no non-linear association between HB concentrations and all-cause mortality after a full adjustment for covariates among the older adults form longevity regions (p > 0.05 for non-linearity). The risk for all-cause mortality was significantly higher in the groups with HB concentration of <11.0 g/dL (HR: 1.37, 95% CI: 1.10-1.70) and 11.0-12.0 g/dL (HR: 1.25, 95% CI: 1.01-1.54); the risk of all-cause mortality was significantly lower in the groups with HB concentration ≥14.0 g/dL (HR: 0.76, 95% CI: 0.60-0.97) compared with the reference group (13.0-13.9 g/dL). Conclusions: Among older adults form Chinese longevity regions, HB concentrations were found to be inversely and linearly associated with all-cause mortality. Further prospective intervention trials are needed to confirm whether higher HB concentrations had a lower risk of mortality in these older adults.

Project description:ObjectivesWhile both vitamin D deficiency and cognitive impairment have individually been linked to a greater risk of all-cause mortality, the combined effects of these two different conditions have not previously been explored in this context. We aimed to investigate the combined impact of vitamin D concentration and cognitive impairment on all-cause mortality in older adults.MethodsThe analyzed data were collected from community-dwelling adults ≥65 years of age that were enrolled in the Chinese Longitudinal Healthy Longevity Survey (n = 1,673). The Mini-Mental Status Examination (MMSE) was used to assess cognitive function, while the plasma 25-hydroxyvitamin D [25(OH)D] test was used to assess vitamin D status. The associations between vitamin D concentration, cognitive function, and all-cause mortality were assessed with Cox proportional hazards models. We used restricted cubic splines to examine the dose-response relationship between vitamin D and the risk of all-cause mortality and used joint effect testing to explore interactions between vitamin D concentration and cognitive function.ResultsDuring a mean (SD) follow-up of 3.8 (1.9) years, 899 (53.7%) deaths occurred. A negative dose-response relationship was observed between 25(OH)D concentration and cognition impairment at baseline, as well as the odds of all-cause mortality during follow-up. Similarly, cognitive impairment was significantly related to all-cause mortality risk (HR 1.81, 95% CI: 1.54 to 2.12). The combined analyses showed positive associations, with the highest mortality risk observed in older adults with both low vitamin D and cognitive impairment (HR 3.04, 95% CI: 2.40 to 3.86). Moreover, the interaction between 25(OH)D concentration and cognitive function was found to be significant in relation to the risk of mortality (p for interaction <0.001).ConclusionLower plasma 25(OH)D and cognitive impairment were, respectively, associated with increased all-cause mortality risks. The 25(OH)D concentration and cognitive impairment exhibited a combined additive effect on all-cause mortality among older Chinese adults.

Project description:ObjectiveWith a rapidly aging population, identifying effective prognostic predictors has always been a research hotspot in older patients with hip fracture. Recently, the score combining hemoglobin, albumin, lymphocyte and platelet (HALP) has been proposed as a novel biomarker for reflecting systemic inflammation and nutritional status. However, it is unclear whether the HALP score could be a potential prognostic indicator for mortality following hip fracture. Thus, the objective of this study was to explore the relationship between the HALP score and mortality risk in older patients with hip fracture, and further evaluate its predictive value.MethodsConsecutive patients were identified from our institutional hip fracture database over the period from January 1, 2013 to December 31, 2022. Outcomes were 90-day all-cause mortality and overall mortality at the longest follow-up. Multivariate Cox proportional hazard models and restricted cubic spline (RCS) models were employed to assess this relationship. The incremental predictive performance of the HALP score was assessed using the c statistic, net reclassification improvement (NRI) and integrated discrimination improvement (IDI).ResultsIn total, 1707 patients were included with a median follow-up duration of 44.4 months, of whom 174 cases (10.2%) died within 90 days and 667 cases (39.1%) died at the longest follow-up. After multivariate adjustment, compared to tertile 1 group, tertile 2 and tertile 3 groups exhibited 0.676-fold (95% confidence interval [CI]: 0.471-0.972, p = 0.035) and 0.598-fold (95% CI: 0.390-0.918, p = 0.019) lower risks of 90-day mortality, as well as 0.681-fold (95% CI: 0.566-0.820, p < 0.001) and 0.618-fold (95% CI: 0.504-0.759, p < 0.001) decreased risks of overall mortality, respectively. For each unit increase in the HALP score, there was a significant decrease in 90-day mortality by 1.1% and in overall mortality by 1.0% (all p < 0.05). RCS analyses indicated nonlinear relationships between the HALP score and 90-day and overall mortality (all P for nonlinear <0.01). Moreover, adding the HALP score significantly improved the predictive ability for 90-day and overall mortality (all p < 0.05).ConclusionThe HALP score was independently associated with mortality risk, highlighting its potential as a useful prognostic indicator for predicting mortality in older patients with hip fracture.
