Human brucellosis is caused by Brucella species and remains a major burden in both human and domesticated animal populations, especially in Inner Mongolia, China. Project description:ObjectiveIn recent years, the increasing incidence of brucellosis in children has become more serious. However, relatively few studies have been conducted to characterize the spatialtemporal distribution of brucellosis in children. This study aimed to analyze the spatiotemporal distribution characteristics and ecological influencing factors of brucellosis incidence among children in Inner Mongolia.MethodsThis study used data on brucellosis incidence in children aged 0-14 years reported in Inner Mongolia from 2016 to 2020. A Bayesian model was used to analyze the spatial and temporal distribution of brucellosis in children from 2016 to 2020 in Inner Mongolia. Geographical weighted regression model was used to analyze the ecological factors related to the incidence of brucellosis in children.ResultBayesian spatiotemporal analysis indicated that the highest brucellosis risk and increased disease incidence were observed in Hinggan, Inner Mongolia, in children aged 0-14 years. Alxa had the lowest risk but the incidence rate increased rapidly. The incidence of childhood brucellosis was positively associated with the number of sheep at the year-end (β: 2.5909 ~ 2.5926, P < 0.01), average temperature (β: 2.8978 ~ 2.9030, P < 0.05), and precipitation level (β: 3.3261 ~ 3.3268, P < 0.01).ConclusionFrom 2016 to 2020, the overall incidence of brucellosis in children in Inner Mongolia showed an upward trend, with cases exhibiting spatial aggregation. We should focus on areas where the incidence of brucellosis in children is rising rapidly. The incidence of childhood brucellosis was associated with the number of sheep at the year-end, average temperature and precipitation level.Implications and contributionThe findings suggest that brucellosis in children is not to be taken lightly. For children should also focus on protection, take corresponding protective measures. While we focus on high-risk areas, we must also monitor areas where the risk of disease is low, but the incidence is rising fast, to prevent outbreaks in low-risk areas from becoming high-risk areas.

Project description:Revealing the patterns and influencing mechanisms of spatial and temporal distribution of rural settlements is crucial for rural revitalization and sustainable development. However, our limited understanding of the rural settlements in China's ethnic minority border areas has hindered the process of their agricultural and rural modernization. Based on data on rural settlements in Inner Mongolia, China in four periods between 1990 and 2020, this study reveals its spatial and temporal distribution characteristics and describes the dynamic transformation process of settlement. Using a geographical detector approach, 17 factors are explored to identify the influencing mechanisms of each factor on the distribution of rural settlements in different regions. The results show obvious regional differences in the spatial distribution of rural settlements in Inner Mongolia, with the largest kernel density values in the west (Hetao irrigation area) and higher kernel densities in the central (Hohhot) and the east (Chifeng and Tongliao). While rural population decreases, rural settlements expand into cultivated land, grassland, and unused land resources. Its spatial distribution is significantly influenced by the factors of distance to cultivated land, distance to towns, and population density. The east of the study area is mainly controlled by temperature, while vegetation type and vegetation coverage have a greater impact in the west. The interactions between two influencing factors possess bilinear or nonlinear enhancement relationships. This study enriches the understanding of the rural settlements in ethnic minority border areas, which provide reference for the improvement of rural human settlement environment in Inner Mongolia.

Project description:BackgroundBrucellosis poses a significant public health challenge in China. Inner Mongolia, characterized by its developed livestock industry, is the most severe endemic area for human brucellosis. This study aims to describe the epidemiology, explore the spatial-temporal distribution patterns, and clustering characteristics of human brucellosis in Inner Mongolia.MethodsData on human brucellosis cases from 2010 to 2021 were obtained from the Centers for Disease Control and Prevention in Inner Mongolia. Spatial autocorrelation analysis was used to identify high-risk areas, while spatial-temporal scan statistics were employed to detect changes in clusters over time.ResultsA total of 153,792 brucellosis cases were reported in Inner Mongolia from 2010 to 2021, with an average annual incidence rate of 52.59 per 100,000 persons. The incidence showed a decreasing trend from 2010 to 2016, followed by a significant increase from 2016 to 2021. The disease exhibited distinct seasonality, peaking in spring and summer (March to August). Middle-aged individuals, males, and farmers/herdsmen had higher incidence rates. Spatially, incidence rates decreased from north to south and from the central and eastern regions to the west. Clear spatial clusters were observed during 2010-2013 and 2016-2021 in the global Moran's I test. Local spatial autocorrelation analysis revealed that high-high clusters expanded from the central and eastern regions towards the west over time. Spatio-temporal scan analysis further indicated that high-risk clusters were primarily concentrated in the central and eastern regions, with a continuous expansion towards the west and south, leading to an increasingly broad geographical spread.ConclusionHuman brucellosis cases in Inner Mongolia exhibit spatio-temporal clustering, with spatial concentration in the central and eastern regions, but also observed expansion towards the western and southern regions. The most of cases occur between March and August each year. For high-risk areas and populations, more timely and effective prevention and control measures should be implemented to mitigate the spread of brucellosis and protect public health.

Project description:Brucellosis, a zoonotic infectious disease caused by the Brucella genus, remains a significant global public health concern, with Inner Mongolia, China, being a notable hotspot affecting both human health and livestock. In response to this, we developed a dynamic SEIVWShIahIch model to simulate the transmission of brucellosis in Inner Mongolia. We calculated the basic reproduction number (R0 = 2.86), indicating a severe epidemic trend with cases expected to increase over the coming decades. The model was fitted to human brucellosis data, and key transmission parameters were estimated to predict future trends. We also evaluated the impact of various control measures, including vaccination, disinfection, culling, and health education. While these measures positively influence disease control, they cannot completely eliminate brucellosis, suggesting that a single control strategy is insufficient. Combining vaccination and culling proved more effective, with identified threshold values for R0 and to keep R0 < 1. Our findings indicate that a comprehensive strategy integrating vaccination, culling, disinfection, and health education is essential for effectively curbing brucellosis in Inner Mongolia, providing a strong foundation for optimizing future prevention and control strategies.

Project description:BackgroundEsophageal cancer is among the leading cancer types in Inner Mongolia. This study aimed to investigate the incidence and mortality rates of esophageal cancer in 2015 and the trends in these rates in the 2010-2015 period in this region.MethodsNational Colorectal Cancer Roundtable (NCCR) screening methods and criteria were used to extract data from 10 cancer registries stratified by area (urban/rural), sex, and age group. The Chinese standard population in 2000 and Segi's world population were used to calculate age-standardized rates. The annual percentage change (APC) in these rates was calculated using the Joinpoint Regression Program.ResultsIn 2015, Inner Mongolia had 4324 new cases (4027 male vs. 297 female patients) and 3559 deaths (3300 male vs. 259 female patients) from esophageal cancer. The crude incidence, age-standardized incidence by Chinese population, age-standardized incidence by world population, and cumulative incidence were 13.45/100 000, 9.92/100 000, 10.18/100 000, and 1.30%, respectively. The corresponding figures for mortality were 11.32/100 000, 8.35/100 000, 8.53/100 000, and 1.04%. The incidence and mortality increased with age between 40 and 80 years. The rates in rural dwellers, especially men, showed negative APC (-13.25% vs. -11.08%; P < 0.05).ConclusionsThe incidence and mortality rates of esophageal cancer in Inner Mongolia increased between 2010 and 2015. The rates were higher in men in rural areas, middle-aged and elderly individuals. Prevention and control programs focused on these groups, in addition to early diagnosis and treatment of esophageal cancer, are needed to reduce these rates.Key pointsSignificant findings of the study: In Inner Mongolia, incidence and mortality rates of esophageal cancer were higher in urban areas than in rural areas and in men than in women between 2010 and 2015, and in middle-aged and elderly than in younger people.What this study addsPrevention and control programs, in addition to early diagnosis and treatment of esophageal cancer, should be tailored to specifically target men in rural areas and middle-aged and elderly individuals in order to reduce the incidence and mortality rates of this pathology.

Project description:Climate change significantly affects the distribution of plant species, particularly that of relict plants. Tetraena mongolica Maxim. is a first-class endangered relict plant in China, primarily found in Inner Mongolia. This study explored the impact of multiple factors on its potential distribution under climate change. Considering a comprehensive set of 42 potential influencing variables, including climate, soil, net primary productivity (NPP), human activities, and topography, 29 variables were selected. The maximum entropy (MaxEnt) model was used to construct separate climate and soil niche models, and an "overlay function" was employed to construct a dual-suitability model. By establishing five different scenarios, we analyzed the effects of climate, human activities, and NPP on T. mongolica distribution. The results showed that climate is the most significant factor, soil constraints limit its distribution, and human activities reduce its suitable habitats. Although the direct influence of NPP is limited, it may indirectly affect T. mongolica distribution by improving habitat conditions. Future climate change is expected to sharply reduce suitable habitat areas, with the center of distribution migrating eastward. The study's findings imply that climate change, human activities, and soil conditions significantly impact the distribution and survival of the endangered plant T. mongolica, necessitating comprehensive conservation measures to mitigate habitat loss and ensure its preservation.

Project description:BackgroundIn clinical practice, the physician's treatment decision making is influenced by many factors besides the patient's clinical conditions and is the fundamental cause of healthcare inequity and discrimination in healthcare settings. Type 2 diabetes mellitus (T2DM) is a chronic disease with high prevalence, long average length of stay and high hospitalization rate. Although the treatment of T2DM is well guideline driven, there is a large body of evidence showing the existence of treatment disparities. More empirical studies from the provider side are needed to determine if non-clinical factors influence physician's treatment choices.ObjectiveTo determine the hospital and patient influencing factors of treatment schemes given to T2DM inpatients in Inner Mongolia, China.MethodsA cross-sectional, hospital-based survey using a cluster sampling technique was conducted in three tertiary hospitals and three county hospitals in Inner Mongolia, China. Treatment schemes were categorized as lifestyle management, oral therapy or insulin therapy according to the national guideline. Socio-demographic characteristics and variables related to severity of disease at the individual level and hospital level were collected. Weighted multinomial logistic regression models were used to determine influencing factors of treatment schemes.ResultsRegardless of patients' clinical conditions and health insurance types, both hospital and patient level variables were associated with treatment schemes. Males were more likely to be given oral therapy (RRR=1.72, 95% CI=1.06-2.81) and insulin therapy (RRR=1.94, 95% CI=1.29-2.91) compared to females who were given lifestyle management more frequently. Compared to the western region, hospitals in the central regions of Inner Mongolia were less likely to prescribe T2DM patients oral therapy (RRR = 0.18, 95% CI=0.05-0.61) and insulin therapy (RRR = 0.20, 95% CI=0.06-0.67) than lifestyle management. Compared with non-reformed tertiary hospitals, reformed tertiary hospitals and county hospitals were less likely to give T2DM patients oral therapy (RRR = 0.07 and 0.1 respectively) and insulin therapy (RRR = 0.11 and 0.17 respectively).ConclusionGender was the only socio-demographic factors associated with treatment scheme for T2DM patients. Hospitals from different regions have different T2DM treatment patterns. Implementation of reform was shown to be associated with controlling medication use for T2DM inpatients. Further studies are needed to investigate the causes of unreasonable treatment disparities so that policies can be generated accordingly.

Project description:BACKGROUND:Despite the considerable efforts made to address the issue of brucellosis worldwide, its prevalence in dairy products continues to be difficult to estimate and represents a key public health issue around the world today. The aim of the present study was to better understand the epidemiology of this disease in mainland China. We set out to investigate the yearly spatial distribution and possible hotspots of the disease. METHODS:Human brucellosis data from mainland China between 2007 and 2016 were collected from the China Information System for Disease Control and Prevention. A geographic information system ArcGIS10.3 (ESRI, Redlands) was used to identify potential changes in the spatial and temporal distribution of human brucellosis in mainland China during the study period. These distributions were evaluated using three-dimensional trend analysis and spatial autocorrelation analyse. A gravity-center was used to analyse the migration track of human brucellosis. RESULTS:A total of 399,578 cases of human brucellosis were reported during the 10-year study period. The monthly incidence of brucellosis in China demonstrates clear seasonality. Spring and summer are the peak seasons, while May is the peak month for brucellosis. Three-dimensional trend analysis suggests that brucellosis is on the rise from south to north, and that the epidemic situation in northern China is more severe. Between 2007 and 2016, the overall migration distance of the brucellosis incidence gravity-center was 906.43?km, and the direction was southwest. However, the overall gravity center of brucellosis was still in the northern part of China. In the global autocorrelation analysis, brucellosis in China demonstrated a non-random distribution between 2013 and 2014, with spatial autocorrelation (Z?>?1.96, P?<?0.05) and a clustering trend, while no clustering trend was found from 2007 to 2012 or from 2015 to 2016. In the local autocorrelation analysis, a Low-Low cluster phenomenon was found in the south of China in 2013 and 2014. CONCLUSION:Human brucellosis remains a widespread challenge, particularly in northern China. The hotspots highlight potential high-risk areas which may require special plans and resources for monitoring and controlling the disease.

Project description:ObjectivesA major goal of our study was to identify the associations between lifestyle factors and obesity in adolescents and young adults at risk by surveying students in Inner Mongolia Medical University. A second goal was to determine these factors differed by gender.DesignCross-sectional study.SettingStudents grade 1-3 in Inner Mongolia Medical University.Participants5471 grade 1-3 medical students, composed of 3891 female and 1580 male students. Students with body mass index (BMI) ≥25 were defined as overweight.ResultsBMI for male students was 22.1±2.9 and 21.2±2.2 for female students. The prevalence of overweight was 7.6%, with the prevalence being higher for male students compared with females, urban higher than rural and being an only child higher than having sibling children. For male students, urban residence was a risk factor, while for female students being an only child and staying up at night were risk factors, with physical activity a protective factor. A dose-dependency relationship was found between physical fitness and overweight prevalence.ConclusionsThis study shows that being an only child and resident in an urban area are risk factors; staying up late and lack of physical activities increased the risk of being overweight. BMI was associated with declines in physical fitness. Our study provides more insight into adolescent obesity problems.
