Efficacy of Cognitive Behavioral Therapy on Mood Disorders, Sleep, Fatigue, and Quality of Life in Parkinson's Disease: A Systematic Review and Meta-Analysis.

ABSTRACT: Objective: The aim of this study was to perform a quantitative analysis to evaluate the efficacy of cognitive behavioral therapy (CBT) on mood disorders, sleep, fatigue, and its impact on quality of life (QOL) in Parkinson's Disease (PD). Methods: We searched for randomized controlled trials in three electronic databases. Fourteen studies, including 507 patients with PD, met the inclusion criteria. We determined the pooled efficacy by standard mean differences and 95% confidence intervals, using I ² to reveal heterogeneity. Results: The result showed CBT had a significant effect on depression [-0.93 (95%CI, -1.19 to -0.67, P < 0.001)] and anxiety [-0.76 (95%CI, -0.97 to -0.55, P < 0.001)]. Moderate effect sizes were noted with sleep disorders [-0.45 (95% CI, -0.70 to -0.20, P = 0.0004)]. There was no evident impact of CBT on fatigue or QOL. We found an intervention period >8 weeks was advantageous compared with <8 weeks, and CBT implemented in non-group was more effective than in group. Between the delivery methods, no significant difference was found. Conclusion: We found that CBT in patients with PD was an efficacious therapy for some non-motor symptoms in PD, but not efficacious for fatigue and QOL. These results suggest that CBT results in significant improvement in PD and should be used as a conventional clinical intervention.

Project description:Background: The clinical and etiological heterogeneity of mood disorders impede identification of effective treatments for the individual patient. This highlights a need for early neuronal and behavioral biomarkers for treatment efficacy, which can provide a basis for more personalized treatments. The present systematic review aimed to identify the most consistent neuronal and behavioral predictors of treatment efficacy on mood symptoms and cognitive impairment in mood disorders. Methods: We identified and included 60 original peer-reviewed studies investigating neuroimaging and behavioral predictors of treatment efficacy within the domains of emotional and non-emotional cognition, structural neuroimaging, and resting state functional connectivity in patients with unipolar or bipolar disorder. Results: Lower baseline responsivity in limbic regions coupled with heightened medial and dorsal prefrontal responses to emotional stimuli were the most consistent predictors of response to pharmacotherapy for depression. In contrast, heightened limbic and ventral prefrontal reactivity to emotional stimuli seemed to predict efficacy of psychological interventions. Early modulation of fronto-limbic activity and reduction in negative bias were also associated with treatment response. Better performance on non-emotional tests at baseline was relatively consistently associated with efficacy on mood symptoms, whereas the association between neural activity during non-emotional tests and treatment response was less clear. Other baseline factors associated with treatment response were greater white matter integrity, resting state functional connectivity, more prefrontal gray matter volume as well as an early increase following short administered treatment. Finally, emerging evidence indicates that baseline cognitive deficits are associated with greater chances of achieving treatment efficacy on cognition. Conclusions: Patients' profile of emotional and non-emotional cognition and neural activity-and the early treatment-associated changes in neural and cognitive function-may be useful for guiding treatments for depression. While cognitive deficits at baseline seem to improve chances of treatment efficacy on cognition, more studies of this association are urgently needed.

Project description:Background and Objectives: Environmental pollution in regions like the Sarno River Basin in southern Italy significantly affects physical and psychological health. This study aimed to validate three novel psychometric tools-REST, HEAL-BDLC, and PEACE-for assessing fatigue, mood disturbances, and sleep quality in environmentally exposed populations. While correlations with heavy metal exposure will be addressed in a separate manuscript, this study focuses solely on psychometric validation. Materials and Methods: The PREVES-STOP Initiative recruited 88 participants aged 30-65 years from the Sarno River Basin. Participants completed psychometric questionnaires tailored to measure fatigue (REST), symptoms of depression and anxiety (HEAL-BDLC), and sleep quality (PEACE). Internal consistency, construct validity, and reliability were analyzed using Cronbach's alpha, correlation analyses, and principal component analysis (PCA). A subgroup received a nutraceutical intervention for us to explore their responsiveness to change over a two-week period. Results: REST (α = 0.969), HEAL-BDLC (α = 0.962), and PEACE (α = 0.736) demonstrated strong reliability. PCA confirmed the unidimensional structure of REST and the two-component structure of HEAL-BDLC (depression and anxiety dimensions) and PEACE (insomnia and sleep quality). Correlations with established measures, such as the WHO Well-Being Index, supported construct validity. Among the intervention participants, significant improvements were observed in fatigue (-12.5 REST median score), mood (-13.0 HEAL-BDLC median score), sleep (+1.5 PEACE median score), and overall well-being (+4.0 WHO-5 median score). Conclusions: REST, HEAL-BDLC, and PEACE are reliable and valid instruments for assessing nuanced health outcomes in environmentally exposed populations. They hold potential for guiding public health interventions and evaluating environmental remediation impacts. These findings lay the groundwork for future studies linking psychometric outcomes with heavy metal exposure.
