Impaction Bone Grafting for Treatment of Unstable Osteochondritis Dissecans (OCD) Lesions.

ABSTRACT: Although treatment options for unstable and unsalvageable large osteochondral lesions have largely been limited to autologous chondrocyte implantation (ACI) and osteochondral allografts, isolated impaction bone grafting represents a cost-friendly alternative, with predictable outcomes comparable to other options. Furthermore, the procedure can be completed in a single stage on an elective basis. We present our technique for impaction bone grafting of unstable osteochondritis dissecans (OCD) using either cancellous autograft or allograft.

Project description:Osteochondritis dissecans is a common osteochondral abnormality affecting the knee. In unstable lesions, the underlying bone can be significantly abnormal and necessitate treatment. Although many techniques exist, we favor an open surgical approach to ensure that the bone is properly managed. Autologous bone graft can easily be obtained locally and used to restore the bony architecture. The subsequent use of bioabsorbable implants provides a robust means of fixation that allows for single-stage surgery. This Technical Note describes a straightforward but reliable approach to a challenging pathology.

Technique for Arthroscopic Debridement and Microfracture of Unstable, Contained Osteochondritis Dissecans Lesions of the Capitellum.

Project description:Osteochondritis dissecans of the capitellum is an overuse injury that occurs primarily in adolescent athletes that causes pain and motion loss and can lead to chronic dysfunction. When diagnosed in its early stages, most lesions can be treated conservatively and without long-term consequences. Many osteochondritis dissecans lesions, however, continue to cause progressive symptoms despite nonoperative measures or are too advanced at the time of initial clinical presentation to be effectively treated without surgical intervention. The authors describe a safe and reproducible technique for the arthroscopic treatment of unstable, contained capitellar lesions.

All-Arthroscopic Bone Grafting and Primary Fixation of a Medial Femoral Condyle Osteochondritis Dissecans Lesion.

Project description:Treatment of osteochondritis dissecans (OCD) lesions poses a significant challenge for orthopaedic surgeons and can cause debilitating limitations on the activity of patients. Timing of intervention, surgical technique, and selection of graft when needed are all key elements of treatment that need to be considered carefully and discussed with patients. Primary fixation of an OCD fragment with intact subchondral bone has been shown to be beneficial in some cases. There is limited literature, however, on how to approach large chondral lesions in young patients without a large subchondral base attached to the fragment. Treatment of large OCD lesions of the knee with an all-arthroscopic approach provides several benefits, including limited dissection for exposure, improved ability to assess the stability of the OCD lesion during articulation after fixation, and an expedited recovery compared to an open approach. The purpose of this technical note is to detail a technique of performing an all-arthroscopic bone grafting and primary fixation of a medial femoral condyle OCD lesion.

Arthroscopic Microfracture for Osteochondritis Dissecans Lesions of the Capitellum.

Project description:Capitellar osteochondritis dissecans (OCD) is one of the most common causes of elbow pain and dysfunction in adolescent athletes. It typically occurs in gymnasts and overhead throwers and presents along a wide spectrum of severity. Stable lesions can typically be treated with conservative therapy; however, those presenting with instability, fragmentation, or loose bodies generally require surgical intervention. Although there are a number of described surgical options used to treat capitellar OCD lesions, microfracture is one of the most commonly performed and well studied. Patients who are candidates for microfracture generally have favorable outcomes with high rates of return to athletic activity after postoperative rehabilitation. In this work, we present our preferred arthroscopic technique for microfracture of OCD lesions of the capitellum. This technique is most suitable for patients with unstable or fragmented OCD lesions that are less than 1 cm in diameter and do not violate the lateral-most articular margin of the capitellum.

Arthroscopic preparation and internal fixation of an unstable osteochondritis dissecans lesion of the knee.

Project description:We present our arthroscopic technique for fixation of an unstable osteochondritis dissecans (OCD) lesion. This technique includes arthroscopic evaluation of cartilage and bone quality of the OCD fragment, hinging open the lesion, debridement of fibrous nonunion tissue, reducing the fragment, and obtaining multi-point compression screw fixation. This technique avoids the morbidity of an open arthrotomy and should be considered when treating an unstable OCD lesion with adequate bone for fixation.

Ultrasound Arthroscopy of Hip in Treatment of Osteochondritis Dissecans.

Project description:An ultrasound arthroscopy (UA) technique is a promising tool for the evaluation of the articular cartilage during arthroscopic examination. However, the applicability of UA for the evaluation of the hip joint is unknown. We describe a UA assessment of a patient with osteochondritis dissecans at the femoral head. An ultrasound catheter designed for intravascular imaging was inserted into the hip joint by use of conventional arthroscopic portals, and the cartilage surfaces of the femoral head and acetabulum were investigated with ultrasound. UA provided essential quantitative information on the integrity of the articular cartilage and the condition of the subchondral plate not assessable with conventional arthroscopy. Furthermore, the UA technique provided the possibility to monitor arthroscopy-assisted retrograde drilling and bone transplantation in the hip joint.

Arthroscopic Debridement and Fixation of Osteochondritis Dissecans Lesions of the Medial Femoral Condyle.

Project description:Unstable osteochondritis dissecans lesions of the medial femoral condyle have classically been treated with open reduction and fixation under direct visualization through an open arthrotomy. Given the value of avoiding open arthrotomies, we present an arthroscopic approach for lesion elevation, debridement, and fixation. The lesion is first elevated using an arthroscopic elevator, leaving a laterally based osseous hinge. Once elevated, fibrous debris is debrided from the base of the lesion. Subsequently, the fragment is reduced, and percutaneous transpatellar instrumentation is used for fixation. The use of this technique allows for excellent mobilization, debridement, and fixation of the osteochondritis dissecans lesion while minimizing violation of periarticular soft tissues.

Role of impaction bone grafting of allografts in the management of benign lesions of the proximal femur.

Project description:Purpose of studyThe use of allografts to reconstruct benign lesions of the proximal femur after curettage has seldom been reported. We report our experience of impaction bone grafting of only allografts combined with osteosynthesis to manage benign lesions involving the proximal femur.Materials and methodsBetween 2013 and 2019, 35 patients of a mean age of 23.8 years (14-41) who had a biopsy proven benign pathology and a median pre-operative Mirels' score of 9 (8-11) were managed using extended curettage, impaction bone grafting of allografts combined with osteosynthesis through a lateral approach. Radiographs were assessed to see for any recurrence at follow-ups, and functional outcomes were assessed using Musculoskeletal Tumour Society (MSTS) score and Harris hip score (HHS).ResultsAt a mean follow-up was 41.5 months (23-80), patients demonstrated favorable functional outcomes with a mean MSTS of 28.3 (18-30) and a mean HHS of 94.3 (66-100) at the last follow-up. Two cases (GCT = 1; fibrous dysplasia = 1) had a recurrence of disease. Allografts demonstrated a particular integration pattern on radiographs that involved an intermediate period of lucency followed by consolidation and integration with the parent bone.ConclusionImpaction grafting of allografts in benign lesions of the proximal femur allows adequate bony consolidation of the cavity after extended curettage and can be effectively used as a permanent solution to manage such lesions in most cases. The intermediate period of lucency seen on radiographs must not be confused for recurrence, and patients must be followed up continuously.Study designRetrospective Case Series.Level of evidenceLevel IV.

Arthroscopic Treatment of Capitellum Osteochondritis Dissecans With Micronized Allogeneic Cartilage Scaffold.

Project description:Osteochondritis dissecans (OCD) of the capitellum is a common disorder that causes pain and functional limitation in the adolescent elbow. Although conservative management is the first line of treatment, surgical intervention is usually recommended for patients with persistent mechanical symptoms, loose bodies, or unstable lesions. Elbow arthroscopy has become the gold standard to evaluate and treat symptomatic OCD lesions. Arthroscopic debridement and bone marrow stimulation have been shown to yield good short-term results. Unfortunately, long-term follow-up has shown less favorable outcomes, with degenerative changes frequently documented. Current marrow stimulation techniques promote the formation of fibrocartilage, which is known to be less durable than hyaline cartilage. We describe an arthroscopic technique of debridement and drilling supplemented with a micronized allogeneic cartilage scaffold to address OCD lesions of the capitellum in an effort to promote hyaline cartilage formation.

Fresh Osteochondral Allograft Transplantation for Uncontained, Elongated Osteochondritis Dissecans Lesions of the Medial Femoral Condyle.

Project description:Osteochondritis dissecans (OCD) lesions of the knee are a significant source of pain and disability. Although the pathologic process for this condition remains poorly understood, histologic studies suggest vascular insufficiency of the subchondral bone may be the underlying cause for focal necrosis and subsequent compromise of the overlying articular cartilage. These lesions most commonly affect the medial femoral condyle and can be found along the margins of the intercondylar notch. Because of significant bone involvement, osteochondral allograft (OCA) transplantation has emerged as a dominant treatment option for OCD lesions because it can accurately restore the entire osteochondral unit. Given the characteristic location and large, irregular shapes of these lesions, surgical management can be challenging. These lesions are often uncontained along the periphery of the condyle, which can compromise OCA graft fixation and healing. We describe our preferred technique for the treatment of large, uncontained OCD lesions of the medial femoral condyle using a unicompartmental OCA augmented with screw fixation.
