Social determinants of adherence to COVID-19 preventive guidelines: a comprehensive review.

ABSTRACT: Adherence to coronavirus disease 2019 (COVID-19) preventive guidelines (ACPG) is an important strategy to control the COVID-19 pandemic effectively. The present study aimed to identify and summarize the social determinants of ACPG among the general population. A comprehensive review was performed from December 2019 to February 2021 through searching electronic databases. Two independent reviewers assessed and selected relevant studies. Next, the characteristics and main findings of the included studies were summarized. Finally, the World Health Organization's conceptual framework of social determinants of health was used to synthesize the identified social determinants of ACPG. Forty-one of 453 retrieved articles met the inclusion criteria. The study results showed different patterns of ACPG among various communities. Furthermore, 84 social determinants were identified and categorized into structural and intermediary determinants. ACPG is a set of complex behaviors associated with different individual sociodemographic and behavioral characteristics; living and working conditions; COVID-19 knowledge, attitudes, and risk perceptions; exposure to sources and information level; leisure activities; social support; trust; social norms; psychosocial well-being; socio-economic position; and the socio-economic and political context. Interventions to promote ACPG among the general population should consider the identified social determinants of ACPG.

Project description:BackgroundSocial determinants of health (SDOH) may limit the practice of coronavirus disease 2019 (COVID-19) risk mitigation guidelines with health implications for individuals with underlying cardiovascular disease (CVD). Population-based evidence of the association between SDOH and practicing such mitigation strategies in adults with CVD is lacking. We used the National Opinion Research Center's COVID-19 Household Impact Survey conducted between April and June 2020 to evaluate sociodemographic disparities in adherence to COVID-19 risk mitigation measures in a sample of respondents with underlying CVD representing 18 geographic areas of the United States.MethodsCVD status was ascertained by self-reported history of receiving heart disease, heart attack, or stroke diagnosis. We built de novo, a cumulative index of SDOH burden using education, insurance, economic stability, 30-day food security, urbanicity, neighborhood quality, and integration. We described the practice of measures under the broad strategies of personal protection (mask, hand hygiene, and physical distancing), social distancing (avoiding crowds, restaurants, social activities, and high-risk contact), and work flexibility (work from home, canceling/postponing work). We reported prevalence ratios and 95% CIs for the association between SDOH burden (quartiles of cumulative indices) and practicing these measures adjusting for age, sex, race/ethnicity, comorbidity, and interview wave.ResultsTwo thousand thirty-six of 25 269 (7.0%) adults, representing 8.69 million in 18 geographic areas of the United States, reported underlying CVD. Compared with the least SDOH burden, fewer individuals with the greatest SDOH burden practiced all personal protection (75.6% versus 89.0%) and social distancing measures (41.9% versus 58.9%) and had any flexible work schedule (26.2% versus 41.4%). These associations remained statistically significant after full adjustment: personal protection (prevalence ratio, 0.83 [95% CI, 0.73-0.96]; P=0.009), social distancing (prevalence ratio, 0.69 [95% CI, 0.51-0.94]; P=0.018), and work flexibility (prevalence ratio, 0.53 [95% CI, 0.36-0.79]; P=0.002).ConclusionsSDOH burden is associated with lower COVID-19 risk mitigation practices in the CVD population. Identifying and prioritizing individuals whose medical vulnerability is compounded by social adversity may optimize emerging preventive efforts, including vaccination guidelines.

Project description:BackgroundThe likelihood of COVID-19 spreading from one individual to another is impacted by personal factors, preventive actions taken, and the quantity and length of exposure. Social media instantly shares health information with the public so medical professionals can interact with them.MethodsThe review used Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) checklist. We retrieve articles by using keywords on Pub Med, Cochrane Library, and grey literatures. The Joanna Briggs Institute (JBI) Meta-Analysis of Statistics Assessment and Review Instrument was used to check the quality of articles. The main qualitative synthesis was including comments related to the characteristics of the studied populations, the study period, and the main results obtained from the social media platforms.ResultThis study includes 32 studies conducted in 20 countries globally. Most of the studies discussed that during the pandemics different types of social medias were utilized to provide knowledge to change the attitude of the people and helps to practices COVID-19 preventive behaviors. By enabling people to seek and share knowledge, socialize, and find pleasure, social media use encourages people to adopt preventive behaviors. This review demonstrated a substantial relationship between higher levels of precautionary behaviors and factors such as educational status, knowledge, fear, and reading medical pages on social media. According to the studies, watching videos is a reliable means to get accurate information, and watching animated films posted on social media can help people learn more about COVID-19 preventive strategies.ConclusionIn these urgent times, social media could even help with quick information availability; misinformation or inadequate understanding can cause misunderstandings within the community. This analysis revealed that following medical pages on social media, having knowledge, feeling afraid, and having an educational background were all strongly correlated with greater levels of aware preventive behavior. Therefore, it is advised for policy makers to develop social behavioral change health communication strategies, guidelines on COVID-19 prevention behaviors. Health care providers should prepare health learning materials, and provide accurate, updated and timely information using all forms of social media to correct mis- information, misperceptions, depression, and anxiety for better control of the pandemics. Future research shall focus on evaluating effectiveness of each social media platform interventions during such emergency crisis.

Project description:BackgroundSince the mass vaccination against SARS-CoV-2 was launched in Israel, the Arab ethnicity minority had lower vaccine uptake. The syndemics theory suggests a closely interrelated complex of health and social crises among vulnerable societies results in an increased disease burden or in more adverse health conditions. Syndemics may explain the health disparities between different people or communities. Likewise, acculturation was found to be associated with different health outcomes among minority populations. The purpose of the study is to explore the association between syndemic construct, acculturation style, and adherence to recommended COVID-19 vaccination among the Arab ethnicity in Israel.MethodsA cross-sectional study among 305 participants who completed a self-report questionnaire. Syndemic construct (syndemics score and syndemics severity) was calculated from the participants' health behavior index, self-rated health status, and adherence to flu vaccination. Four acculturation strategies were defined according to Barry's acculturation model: assimilation, integration, separation, and marginalization style. Linear regression (stepwise method) was conducted to determine the explanatory factors for COVID-19 vaccine adherence.ResultsAssimilation and separation acculturation styles and syndemics severity were significantly associated with higher adherence to the recommended COVID-19 vaccination (B = 1.12, 95%CI = 0.34-1.98; B = 0.45, 95%CI = 0.10-0.80; B = 0.18, 95%CI = 0.09-0.28; respectively). The explained variance of the model (R 2) was 19.9%.ConclusionSyndemics severity, assimilation and separation acculturation styles were associated with higher adherence to recommended COVID-19 vaccination in the Israeli Arab minority population. Syndemics score was not associated with recommended COVID-19 vaccination. To encourage COVID-19 vaccination among minority communities, campaigns should be tailored to the social determinants in a sensitive and individualized manner.
