Colonoscopic image synthesis with generative adversarial network for enhanced detection of sessile serrated lesions using convolutional neural network.

ABSTRACT: Computer-aided detection (CADe) systems have been actively researched for polyp detection in colonoscopy. To be an effective system, it is important to detect additional polyps that may be easily missed by endoscopists. Sessile serrated lesions (SSLs) are a precursor to colorectal cancer with a relatively higher miss rate, owing to their flat and subtle morphology. Colonoscopy CADe systems could help endoscopists; however, the current systems exhibit a very low performance for detecting SSLs. We propose a polyp detection system that reflects the morphological characteristics of SSLs to detect unrecognized or easily missed polyps. To develop a well-trained system with imbalanced polyp data, a generative adversarial network (GAN) was used to synthesize high-resolution whole endoscopic images, including SSL. Quantitative and qualitative evaluations on GAN-synthesized images ensure that synthetic images are realistic and include SSL endoscopic features. Moreover, traditional augmentation methods were used to compare the efficacy of the GAN augmentation method. The CADe system augmented with GAN synthesized images showed a 17.5% improvement in sensitivity on SSLs. Consequently, we verified the potential of the GAN to synthesize high-resolution images with endoscopic features and the proposed system was found to be effective in detecting easily missed polyps during a colonoscopy.

Project description:BackgroundThe efficient and accurate diagnosis of pulmonary adenocarcinoma before surgery is of considerable significance to clinicians. Although computed tomography (CT) examinations are widely used in practice, it is still challenging and time-consuming for radiologists to distinguish between different types of subcentimeter pulmonary nodules. Although there have been many deep learning algorithms proposed, their performance largely depends on vast amounts of data, which is difficult to collect in the medical imaging area. Therefore, we propose an automatic classification system for subcentimeter pulmonary adenocarcinoma, combining a convolutional neural network (CNN) and a generative adversarial network (GAN) to optimize clinical decision-making and to provide small dataset algorithm design ideas.MethodsA total of 206 nodules with postoperative pathological labels were analyzed. Among them were 30 adenocarcinomas in situ (AISs), 119 minimally invasive adenocarcinomas (MIAs), and 57 invasive adenocarcinomas (IACs). Our system consisted of two parts, a GAN-based image synthesis, and a CNN classification. First, several popular existing GAN techniques were employed to augment the datasets, and comprehensive experiments were conducted to evaluate the quality of the GAN synthesis. Additionally, our classification system processes were based on two-dimensional (2D) nodule-centered CT patches without the need of manual labeling information.ResultsFor GAN-based image synthesis, the visual Turing test showed that even radiologists could not tell the GAN-synthesized from the raw images (accuracy: primary radiologist 56%, senior radiologist 65%). For CNN classification, our progressive growing wGAN improved the performance of CNN most effectively (area under the curve =0.83). The experiments indicated that the proposed GAN augmentation method improved the classification accuracy by 23.5% (from 37.0% to 60.5%) and 7.3% (from 53.2% to 60.5%) in comparison with training methods using raw and common augmented images respectively. The performance of this combined GAN and CNN method (accuracy: 60.5%±2.6%) was comparable to the state-of-the-art methods, and our CNN was also more lightweight.ConclusionsThe experiments revealed that GAN synthesis techniques could effectively alleviate the problem of insufficient data in medical imaging. The proposed GAN plus CNN framework can be generalized for use in building other computer-aided detection (CADx) algorithms and thus assist in diagnosis.

Project description:PurposeThis study proposed an end-to-end unsupervised medical fusion generative adversarial network, MedFusionGAN, to fuse computed tomography (CT) and high-resolution isotropic 3D T1-Gd Magnetic resonance imaging (MRI) image sequences to generate an image with CT bone structure and MRI soft tissue contrast to improve target delineation and to reduce the radiotherapy planning time.MethodsWe used a publicly available multicenter medical dataset (GLIS-RT, 230 patients) from the Cancer Imaging Archive. To improve the models generalization, we consider different imaging protocols and patients with various brain tumor types, including metastases. The proposed MedFusionGAN consisted of one generator network and one discriminator network trained in an adversarial scenario. Content, style, and L1 losses were used for training the generator to preserve the texture and structure information of the MRI and CT images.ResultsThe MedFusionGAN successfully generates fused images with MRI soft-tissue and CT bone contrast. The results of the MedFusionGAN were quantitatively and qualitatively compared with seven traditional and eight deep learning (DL) state-of-the-art methods. Qualitatively, our method fused the source images with the highest spatial resolution without adding the image artifacts. We reported nine quantitative metrics to quantify the preservation of structural similarity, contrast, distortion level, and image edges in fused images. Our method outperformed both traditional and DL methods on six out of nine metrics. And it got the second performance rank for three and two quantitative metrics when compared with traditional and DL methods, respectively. To compare soft-tissue contrast, intensity profile along tumor and tumor contours of the fusion methods were evaluated. MedFusionGAN provides a more consistent, better intensity profile, and a better segmentation performance.ConclusionsThe proposed end-to-end unsupervised method successfully fused MRI and CT images. The fused image could improve targets and OARs delineation, which is an important aspect of radiotherapy treatment planning.

Project description:Deep neural networks (DNNs) can accurately decode task-related information from brain activations. However, because of the non-linearity of DNNs, it is generally difficult to explain how and why they assign certain behavioral tasks to given brain activations, either correctly or incorrectly. One of the promising approaches for explaining such a black-box system is counterfactual explanation. In this framework, the behavior of a black-box system is explained by comparing real data and realistic synthetic data that are specifically generated such that the black-box system outputs an unreal outcome. The explanation of the system's decision can be explained by directly comparing the real and synthetic data. Recently, by taking advantage of advances in DNN-based image-to-image translation, several studies successfully applied counterfactual explanation to image domains. In principle, the same approach could be used in functional magnetic resonance imaging (fMRI) data. Because fMRI datasets often contain multiple classes (e.g., multiple behavioral tasks), the image-to-image transformation applicable to counterfactual explanation needs to learn mapping among multiple classes simultaneously. Recently, a new generative neural network (StarGAN) that enables image-to-image transformation among multiple classes has been developed. By adapting StarGAN with some modifications, here, we introduce a novel generative DNN (counterfactual activation generator, CAG) that can provide counterfactual explanations for DNN-based classifiers of brain activations. Importantly, CAG can simultaneously handle image transformation among all the seven classes in a publicly available fMRI dataset. Thus, CAG could provide a counterfactual explanation of DNN-based multiclass classifiers of brain activations. Furthermore, iterative applications of CAG were able to enhance and extract subtle spatial brain activity patterns that affected the classifier's decisions. Together, these results demonstrate that the counterfactual explanation based on image-to-image transformation would be a promising approach to understand and extend the current application of DNNs in fMRI analyses.
