Machine Learning in Prediction of Bladder Cancer on Clinical Laboratory Data.

ABSTRACT: Bladder cancer has been increasing globally. Urinary cytology is considered a major screening method for bladder cancer, but it has poor sensitivity. This study aimed to utilize clinical laboratory data and machine learning methods to build predictive models of bladder cancer. A total of 1336 patients with cystitis, bladder cancer, kidney cancer, uterus cancer, and prostate cancer were enrolled in this study. Two-step feature selection combined with WEKA and forward selection was performed. Furthermore, five machine learning models, including decision tree, random forest, support vector machine, extreme gradient boosting (XGBoost), and light gradient boosting machine (GBM) were applied. Features, including calcium, alkaline phosphatase (ALP), albumin, urine ketone, urine occult blood, creatinine, alanine aminotransferase (ALT), and diabetes were selected. The lightGBM model obtained an accuracy of 84.8% to 86.9%, a sensitivity 84% to 87.8%, a specificity of 82.9% to 86.7%, and an area under the curve (AUC) of 0.88 to 0.92 in discriminating bladder cancer from cystitis and other cancers. Our study provides a demonstration of utilizing clinical laboratory data to predict bladder cancer.

Project description:BackgroundIntensive care unit (ICU) patients demand continuous monitoring of several clinical and laboratory parameters that directly influence their medical progress and the staff's decision-making. Those data are vital in the assistance of these patients, being already used by several scoring systems. In this context, machine learning approaches have been used for medical predictions based on clinical data, which includes patient outcomes.AimTo develop a binary classifier for the outcome of death in ICU patients based on clinical and laboratory parameters, a set formed by 1087 instances and 50 variables from ICU patients admitted to the emergency department was obtained in the "WiDS (Women in Data Science) Datathon 2020: ICU Mortality Prediction" dataset.MethodsFor categorical variables, frequencies and risk ratios were calculated. Numerical variables were computed as means and standard deviations and Mann-Whitney U tests were performed. We then divided the data into a training (80%) and test (20%) set. The training set was used to train a predictive model based on the Random Forest algorithm and the test set was used to evaluate the predictive effectiveness of the model.ResultsA statistically significant association was identified between need for intubation, as well predominant systemic cardiovascular involvement, and hospital death. A number of the numerical variables analyzed (for instance Glasgow Coma Score punctuations, mean arterial pressure, temperature, pH, and lactate, creatinine, albumin and bilirubin values) were also significantly associated with death outcome. The proposed binary Random Forest classifier obtained on the test set (n = 218) had an accuracy of 80.28%, sensitivity of 81.82%, specificity of 79.43%, positive predictive value of 73.26%, negative predictive value of 84.85%, F1 score of 0.74, and area under the curve score of 0.85. The predictive variables of the greatest importance were the maximum and minimum lactate values, adding up to a predictive importance of 15.54%.ConclusionWe demonstrated the efficacy of a Random Forest machine learning algorithm for handling clinical and laboratory data from patients under intensive monitoring. Therefore, we endorse the emerging notion that machine learning has great potential to provide us support to critically question existing methodologies, allowing improvements that reduce mortality.

Project description:BackgroundThe COVID-19 pandemic is probably the greatest health catastrophe of the modern era. Spain's health care system has been exposed to uncontrollable numbers of patients over a short period, causing the system to collapse. Given that diagnosis is not immediate, and there is no effective treatment for COVID-19, other tools have had to be developed to identify patients at the risk of severe disease complications and thus optimize material and human resources in health care. There are no tools to identify patients who have a worse prognosis than others.ObjectiveThis study aimed to process a sample of electronic health records of patients with COVID-19 in order to develop a machine learning model to predict the severity of infection and mortality from among clinical laboratory parameters. Early patient classification can help optimize material and human resources, and analysis of the most important features of the model could provide more detailed insights into the disease.MethodsAfter an initial performance evaluation based on a comparison with several other well-known methods, the extreme gradient boosting algorithm was selected as the predictive method for this study. In addition, Shapley Additive Explanations was used to analyze the importance of the features of the resulting model.ResultsAfter data preprocessing, 1823 confirmed patients with COVID-19 and 32 predictor features were selected. On bootstrap validation, the extreme gradient boosting classifier yielded a value of 0.97 (95% CI 0.96-0.98) for the area under the receiver operator characteristic curve, 0.86 (95% CI 0.80-0.91) for the area under the precision-recall curve, 0.94 (95% CI 0.92-0.95) for accuracy, 0.77 (95% CI 0.72-0.83) for the F-score, 0.93 (95% CI 0.89-0.98) for sensitivity, and 0.91 (95% CI 0.86-0.96) for specificity. The 4 most relevant features for model prediction were lactate dehydrogenase activity, C-reactive protein levels, neutrophil counts, and urea levels.ConclusionsOur predictive model yielded excellent results in the differentiating among patients who died of COVID-19, primarily from among laboratory parameter values. Analysis of the resulting model identified a set of features with the most significant impact on the prediction, thus relating them to a higher risk of mortality.
