A 24-hour population distribution dataset based on mobile phone data from Helsinki Metropolitan Area, Finland.

ABSTRACT: In this article, we present temporally dynamic population distribution data from the Helsinki Metropolitan Area, Finland, at the level of 250 m by 250 m statistical grid cells. An hourly population distribution dataset is provided for regular workdays (Mon - Thu), Saturdays and Sundays. The data are based on aggregated mobile phone data collected by the biggest mobile network operator in Finland. Mobile phone data are assigned to statistical grid cells using an advanced dasymetric interpolation method based on ancillary data about land cover, buildings and a time use survey. The dataset is validated by comparing population register data from Statistics Finland for night hours and a daytime workplace registry. The resulting 24-hour population data can be used to reveal the temporal dynamics of the city, and examine population variations relevant to spatial accessibility analyses, crisis management, planning and beyond.

Urbanity as a determinant of exposure to grass pollen in Helsinki Metropolitan area, Finland.

Project description:Little is known about the levels of exposure to grass pollen in urban environments. We assessed the spatio-temporal variation of grass pollen concentrations and the role of urbanity as a determinant of grass pollen exposure in the Helsinki Metropolitan area. We monitored grass pollen concentrations in 2013 at 16 sites during the peak pollen season by using rotorod-type samplers at the breathing height. The sites were in the cities of Helsinki and Espoo, Finland, and formed city-specific lines that represented urban-rural gradient. The monitoring sites were both visually and based on land use data ranked as high to low (graded 1 to 8) pollen area. The lowest grass pollen concentrations were observed in the most urban sites compared to the least urban sites (mean 3.6 vs. 6.8 grains/m3 in Helsinki; P<0.0001, and 5.2 vs. 87.5 grains/m3 in Espoo; P<0.0001). Significant differences were observed between concentrations measured in morning periods compared to afternoon periods (4.9 vs. 5.4 in Helsinki, P = 0.0186, and 21.8 vs. 67.1 in Espoo, P = 0.0004). The mean pollen concentration increased with decreasing urbanity both in Helsinki (0.59 grains/m3 per urbanity rank, 95% CI 0.25-0.93) and Espoo (8.42, 6.23-10.61). Pollen concentrations were highest in the afternoons and they were related to the ambient temperature. Urbanity was a strong and significant determinant of pollen exposure in two Finnish cities. Pollen exposure can periodically reach such high levels even in the most urban environments that can cause allergic reactions among individuals with allergies.

Dientamoeba fragilis - the most common intestinal protozoan in the Helsinki Metropolitan Area, Finland, 2007 to 2017.

Project description:BackgroundDespite the global distribution of the intestinal protozoan Dientamoeba fragilis, its clinical picture remains unclear. This results from underdiagnosis: microscopic screening methods either lack sensitivity (wet preparation) or fail to reveal Dientamoeba (formalin-fixed sample).AimIn a retrospective study setting, we characterised the clinical picture of dientamoebiasis and compared it with giardiasis. In addition, we evaluated an improved approach to formalin-fixed samples for suitability in Dientamoeba diagnostics.MethodsThis study comprised four parts: (i) a descriptive part scrutinising rates of Dientamoeba findings; (ii) a methodological part analysing an approach to detect Dientamoeba-like structures in formalin samples; (iii) a clinical part comparing demographics and symptoms between patients with dientamoebiasis (n = 352) and giardiasis (n = 272), and (iv) a therapeutic part (n = 89 patients) investigating correlation between faecal eradication and clinical improvement.ResultsThe rate of Dientamoeba findings increased 20-fold after introducing criteria for Dientamoeba-like structures in formalin-fixed samples (88.9% sensitivity and 83.3% specificity). A further increase was seen after implementing faecal PCR. Compared with patients with giardiasis, the symptoms in the Dientamoeba group lasted longer and more often included abdominal pain, cramping, faecal urgency and loose rather than watery stools. Resolved symptoms correlated with successful faecal eradication (p < 0.001).ConclusionsPreviously underdiagnosed, Dientamoeba has become the most frequently recorded pathogenic enteroparasite in Finland. This presumably results from improved diagnostics with either PCR or detection of Dientamoeba-like structures in formalin-fixed samples, an approach applicable also in resource-poor settings. Symptoms of dientamoebiasis differ slightly from those of giardiasis; patients with distressing symptoms require treatment.

The effect of sampling height on grass pollen concentrations in different urban environments in the Helsinki Metropolitan Area, Finland.

Project description:IntroductionIt is important to study potential differences in pollen concentrations between sampling heights because of diverse outdoor and indoor activity of humans (exposure) at different height levels in urban environments. Previous studies have investigated the effect of height on pollen concentrations based on just one or a few sampling points. We studied the effect of sampling height on grass pollen concentrations in several urban environments with different levels of urbanity.MethodsThis study was conducted in the Helsinki Metropolitan Area, Finland, in 2013 during the pollen season of grasses. Pollen grains were monitored in eight different points in the morning and afternoon. Rotorod-type samplers were attached on sampling poles at the heights of 1.5 meters and 4 meters.ResultsGrass pollen concentrations were on average higher at the height of 1.5 meters (Helsinki mean 5.24 grains / m3; Espoo mean 75.71 grains / m3) compared to the height of 4 meters (Helsinki mean 3.84 grains / m3; Espoo mean 37.42 grains / m3) with a difference of 1.40 grains / m3 (95% CI -0.21 to 3.01) in Helsinki, and 38.29 grains / m3 (7.52 to 69.07) in Espoo, although not always statistically significant. This was detected both in the morning and in the afternoon. However, in the most urban sites the levels were lower at 1.5 meters compared to 4 meters, whereas in the least urban sites the concentrations were higher at 1.5 meters. In linear regression models with interaction terms, the modifying effect of urbanity on concentration-height relation was statistically significant in both cities. The effect of urbanity on pollen concentrations at both heights was stronger in less urban Espoo.ConclusionsThe present study provides evidence that height affects the abundance and distribution of grass pollen in urban environments, but this effect depends on the level of urbanity.

The green view dataset for the capital of Finland, Helsinki.

Project description:Recent studies have incorporated human perspective methods like making use of street view images and measuring green view in addition to more traditional ways of mapping city greenery [1]. Green view describes the relative amount of green vegetation visible at street level and is often measured with the green view index (GVI), which describes the percentage of green vegetation in a street view image or images of a certain location [2]. The green view dataset of Helsinki was created as part of the master's thesis of Akseli Toikka at the University of Helsinki [3]. We calculated the GVI values for a set of locations on the streets of Helsinki using Google Street View (GSV) 360° panorama images from summer months (May through September) between 2009 and 2017. From the available images, a total of 94 454 matched the selection criteria. These were downloaded using the Google application programming interface (API). We calculated the GVI values from the panoramas based on the spectral characteristics of green vegetation in RGB images. The result was a set of points along the street network with GVI values. By combining the point data with the street network data of the area, we generated a dataset for GVI values along the street centre lines. Streets with GVI points within a threshold distance of 30 meters were given the average of the GVI values of the points. For the streets with no points in the vicinity (∼67%), the land cover data from the area was used to estimate the GVI, as suggested in the thesis [3]. The point and street-wise data are stored in georeferenced tables that can be utilized for further analyses with geographical information systems.

Clonal dissemination of successful emerging clone <i>mecA</i>-MRSA t304/ST6 among humans and hedgehogs in the Helsinki metropolitan area in Finland.

Project description:Methicillin-resistant Staphylococcus aureus (MRSA) carrying mecC gene (mecC-MRSA) is frequently reported among European hedgehogs (Europeaus erineaus) due to co-evolutionary adaptation to dermatophyte infection in European hedgehogs. The occurrence of MRSA in European hedgehogs in Finland is unknown. Consequently, we investigated the occurrence of MRSA in wild hedgehogs from urban Helsinki metropolitan area in 2020-2021 and applied whole genome sequencing (WGS) to further characterize the studied isolates and compared them with human clinical MRSA isolates. Altogether 115 dead hedgehogs were screened for MRSA using selective cultivation methods. Presumptive MRSA isolates were tested for antimicrobial susceptibility and confirmed MRSA isolates were further characterized by spa-typing and WGS. Hedgehog derived MRSA isolates were compared with clinical human MRSA isolates using core genome multilocus sequence typing (cgMLST). In total MRSA was recovered from 11 out of 115 (10%) hedgehogs. Among these four different spa types (t304; n = 4, t8835; n = 4, t5133; n = 2 and t622; n = 1) and three different sequence types (STs) (ST6; n = 6, ST7663; n = 4 and ST2840; n = 1) were identified. From the studied MRSA isolates seven harboured the mecA gene (mecA-MRSA) and four were identified as mecC-MRSA. All mecA-MRSA isolates carried immune evasion cluster genes, and one isolate was positive for Panton-Valentine leukocidin. cgMLST comparison revealed close genetic relatedness among three hedgehog and two human mecA-MRSA isolates all belonging to t304/ST6. Our results suggest a clonal dissemination of a successful MRSA clone among humans and hedgehogs. Further studies are warranted to investigate the sources and dissemination of such clone in urban environments. We observed a relatively low occurrence of mecC-MRSA in Finnish hedgehogs.

Feasibility, Acceptability and Usage Patterns of a 24-Hour Mobile Phone Helpline Service for Women Discharged from a Mother-Baby Psychiatry Unit (MBU) in India.

Project description:BackgroundFor women with perinatal mental illness, phone helplines may be a useful way of accessing help. This study assessed the feasibility, acceptability, limitations, and usage patterns of a helpline service for mothers discharged from a mother-baby psychiatry unit.MethodsMothers discharged from a mother baby unit during an 18-month period were provided with a helpline number. A social worker answered the calls. Details of the calls, including the reasons and the interventions provided, were recorded. Feasibility and acceptability were assessed by calling all users and nonusers. Satisfaction with the helpline was recorded among users, and reasons for not calling were assessed among nonusers.ResultsAmong 113 mothers, 51 (45%) made 248 calls. Calls were regarding medication, sleep problems, planning pregnancies, symptom exacerbation, appointments, and suicidal ideation. Some calls were related to domestic violence (n = 13, 5.24%), and infant health and breastfeeding (n = 11, 4.44%). Seventy-six (67%, 44 callers and 32 noncallers) were contacted. The majority (41/44) of the callers found it useful: 91% said they got help, and 95% said they would recommend it to others. However, language difficulties (9%) and technical problems (5%) were reported. Among the noncallers, the majority reported having experienced no problem related to mental health or had contacted a doctor. However, of the noncallers, one woman died of suicide, did not have access to a phone, and the family did not choose to call.ConclusionsHelpline phone service appears to be feasible and acceptable and can be adapted in other mother-baby psychiatry units in low and middle-income countries. However, in some women, the nonavailability of a phone may be a limiting factor.

Occupational exposure to vapors, gasses, dusts, and fumes in relation to causes of death during 24 years in Helsinki, Finland.

Project description:PurposeEnvironmental particulate matter (PM) exposure has been shown to cause excess all-cause and disease-specific mortality. Our aim was to compare disease-specific mortality by estimated occupational exposure to vapors, gasses, dusts, and fumes (VGDF).MethodsThe data source is the Helsinki part of the population-based FinEsS study on chronic obstructive pulmonary diseases including information on age, education level, main occupation, sex, and tobacco smoking combined with death registry information. We compared estimated VGDF exposure to mortality using adjusted competing-risks regression for disease-specific survival analysis for a 24-year follow-up.ResultsCompared to the no-exposure group, the high occupational VGDF exposure group had sub-hazard ratios (sHR) of 1.7 (95% CI 1.3-2.2) for all cardiovascular-related and sHR 2.1 (1.5-3.9) for just coronary artery-related mortality. It also had sHR 1.7 (1.0-2.8) for Alzheimer's or vascular dementia-related mortality and sHR 1.7(1.2-2.4) for all respiratory disease-related mortality.ConclusionLong-term occupational exposure to VGDF increased the hazard of mortality- to cardiovascular-, respiratory-, and dementia-related causes. This emphasizes the need for minimizing occupational long-term respiratory exposure to dust, gasses, and fumes.

Heat, Heatwaves and Cardiorespiratory Hospital Admissions in Helsinki, Finland.

Project description:Background: There is a lack of knowledge concerning the effects of ambient heat exposure on morbidity in Northern Europe. Therefore, this study aimed to evaluate the relationships of daily summertime temperature and heatwaves with cardiorespiratory hospital admissions in the Helsinki metropolitan area, Finland. Methods: Time series models adjusted for potential confounders, such as air pollution, were used to investigate the associations of daily temperature and heatwaves with cause-specific cardiorespiratory hospital admissions during summer months of 2001-2017. Daily number of hospitalizations was obtained from the national hospital discharge register and weather information from the Finnish Meteorological Institute. Results: Increased daily temperature was associated with a decreased risk of total respiratory hospital admissions and asthma. Heatwave days were associated with 20.5% (95% CI: 6.9, 35.9) increased risk of pneumonia admissions and during long or intense heatwaves also with total respiratory admissions in the oldest age group (≥75 years). There were also suggestive positive associations between heatwave days and admissions due to myocardial infarction and cerebrovascular diseases. In contrast, risk of arrhythmia admissions decreased 20.8% (95% CI: 8.0, 31.8) during heatwaves. Conclusions: Heatwaves, rather than single hot days, are a health threat affecting morbidity even in a Northern climate.

Distribution and genetic diversity of Feline calicivirus in Moscow metropolitan area.

Project description:BackgroundFeline calicivirus (FCV) is widespread throughout the world. An FCV infection is associated with conjunctivitis, rhinitis, and mouth ulcers that can lead to the animal's death. Because vaccination is not always effective, it is necessary to monitor the infection regularly.ObjectivesThis study examined the FCV epizootic situation in the Moscow metropolitan area by conducting a molecular phylogenetic analysis of the virus isolates.MethodsSamples from 6213 animals were examined by a reverse transcription polymerase chain reaction. For phylogenetic analysis, 12 nucleotide sequences obtained from animal samples were selected. Sequencing was performed using the Sanger method. Phylogenetic analysis was conducted using the Maximum Likelihood method.ResultsThe FCV genome was detected in 1,596 (25.7%) samples out of 6,213. In 2018, calicivirus was detected in 18.9% of samples, 27.8% in 2019, 21.4% in 2020, and 32.6% in 2021. Phylogenetic analysis of the F ORF2 region and the ORF3 start region led to division into two FCV genogroups. Most of the isolates (8 out of 12) were close to the Chinese strains. On the other hand, there were isolates closely related to European and American strains. The isolates circulating in Moscow were not included in clusters with vaccine strains; their nucleotide similarity varied from 77% to 83%.ConclusionsThis study revealed a high prevalence and genetic diversity of the FCV in Moscow. The epizootic situation remains stably tense because 24 viruses were detected in 25% of animals annually.

Weekend effect on mortality by medical specialty in six secondary hospitals in the Helsinki metropolitan area over a 14-year period.

Project description:BACKGROUND:The weekend effect is the phenomenon of a patient's day of admission affecting their risk for mortality. Our study reviews the situation at six secondary hospitals in the greater Helsinki area over a 14-year period by specialty, in order to examine the effect of centralization of services on the weekend effect. METHODS:Of the 28,591,840 patient visits from the years 2000-2013 in our hospital district, we extracted in-patients treated only in secondary hospitals who died during their hospital stay or within 30?days of discharge. We categorized patients based on the type of each admission, namely elective versus emergency, and according to the specialty of their clinical service provider and main diagnosis. RESULTS:A total of 456,676 in-patients (292,399 emergency in-patients) were included in the study, with 17,231 deaths in-hospital or within 30?days of discharge. A statistically significant weekend effect was observed for in-hospital and 30-day post-discharge mortality among emergency patients for 1 of 7 specialties. For elective patients, a statistically significant weekend effect was visible in in-hospital mortality for 4 of 8 specialties and in 30-day post-discharge mortality for 3 of 8 specialties. Surgery, internal medicine, and gynecology and obstetrics were most susceptible to this phenomenon. CONCLUSIONS:A weekend effect was present for the majority of specialties for elective patients, indicating a need for guidelines for these admissions. More disease-specific research is necessary to find the diagnoses, which suffer most from the weekend effect and adjust staffing accordingly.
