Improvement of survival in Korean breast cancer patients over a 14-year period: A large-scale single-center study.

ABSTRACT:

Purpose

The aim of this study was to evaluate the chronological changes over 14 years in the survival of Korean patients with breast cancer. We also sought to investigate the factors that may have influenced the changes in survival rate.

Materials and methods

We retrospectively analyzed 17,776 breast cancer patients who were treated at Asan Medical Center between January 2000 and December 2013. Patient information was collected from the Asan database, including age at diagnosis, clinical manifestation, pathology report, types of treatment and modality, types of recurrence, and follow-up period. We classified the patients into two cohorts according to the year of their surgery (P1: 2000-2007 and P2: 2008-2013) and compared survival and recurrence between both cohorts.

Results

We observed that patients treated more recently had better survival outcomes. The 5-year breast cancer-specific survival increased from 94.0% in P1 to 96.6% in P2 (p<0.001), and the 5-year disease-free survival increased from 87.9% in P1 to 91.2% in P2 (p<0.001). When analyzed by type of recurrence, distant metastasis-free survival increased to a significant degree. In subgroup analysis by the subtypes of breast cancer, the survival rates improved in all of the subtypes except triple negative breast cancer, and the improvement was more prominent in subtypes with overexpressed human epidermal growth factor receptor 2.

Conclusion

This study showed improvement in breast cancer survival over the succeeding years, which is consistent with the advancement in systemic therapy.

Project description:ObjectivesThe prevalence of allergic sensitization and diseases varies over time due to changes in climate, environment, and lifestyle. This study analyzed changes in sensitization to house dust mites (HDM) and dogs, as well as the prevalence of allergic diseases in the Korean general population over a 9-year period and identified contributing factors.MethodsThis cross-sectional study used data from the Korea National Health and Nutrition Examination Survey conducted in 2010 and 2019. Participants were aged ≥10 years, with the analysis stratified by adolescents (10-18 years) and adults (≥19 years). We evaluated the prevalence of allergic sensitization and diseases and examined links to clinicodemographic factors across the two survey years. Multivariate logistic regression was performed to evaluate the association between sensitization and clinicopathological factors.ResultsIn total, 4,084 participants (2010: n=2,019, 2019: n=2,065) were enrolled. In 2010, HDM sensitization was 36.9%, declining to 36.6% in 2019. Dog sensitization decreased from 6.1% in 2010 to 5.8% in 2019. There were no significant differences between the two years among total participants; however, dog sensitization increased significantly among adolescents (2010: 8.7% vs. 2019: 15.3%; P=0.027). In 2010, HDM sensitization was negatively associated with urban residence (odds ratio [OR], 0.76; 95% CI, 0.60-0.96) and apartment dwelling (OR, 0.78; 95% CI, 0.64-0.95) but was without significance in 2019. In adolescents, dog sensitization was strongly correlated with urban residence (OR, 8.06; 95% CI, 1.53-42.49) in 2010, whereas HDM sensitization was associated with obesity (OR, 2.02; 95% CI, 1.04-3.91) in 2019. In adults, heavy drinking was linked to dog sensitization (OR, 2.35; 95% CI, 1.20-4.62) in 2010 and HDM sensitization in 2019 (OR, 1.44; 95% CI, 1.03-2.02).ConclusionThe prevalence of HDM sensitization remained stable over 9 years, while dog sensitization increased in adolescents. The factors associated with sensitization differed between age groups and years. Understanding these trends and related factors in the general population can help healthcare providers develop new strategies to reduce allergic sensitization and diseases.

Project description:Background and aimThe Helping Babies Breathe program gave major reductions in perinatal mortality in Tanzania from 2009 to 2012. We aimed to study whether this effect was sustained, and whether resuscitation skills changed with continued frequent training.MethodsWe analysed prospective data covering all births (n = 19,571) at Haydom Lutheran Hospital in Tanzania from July 2013 -June 2018. Resuscitation training was continued during this period. All deliveries were monitored by an observer recording the timing of events and resuscitation interventions. Heart rate was recorded by dry-electrode ECG and bag-mask-ventilation by sensors attached to the resuscitator device. We analyzed changes over time in outcomes, use of resuscitation interventions and performance of resuscitation using binary regression models with the log-link function to obtain adjusted relative risks.ResultsWith introduction of user fees for deliveries since 2014, the number of deliveries decreased by 30% from start to the end of the five-year period. An increase in low heart rate at birth and need for bag-mask-ventilation indicate a gradual selection of more vulnerable newborns delivered in the hospital over time. Despite this selection, newborn deaths <24 hours did not change significantly and was maintained at an average of 8.8/1000 live births. The annual reductions in relative risk for perinatal death adjusted for vulnerability factors was 0.84 (95%CI 0.76-0.94). During the five-year period, longer duration of bag-mask ventilation sequences without interruption was observed. Delivered tidal volumes were increased and mask leak was decreased during ventilation. The time to initiation or total duration of ventilation did not change significantly.ConclusionThe reduction in 24-hour newborn mortality after introduction of Helping Babies Breathe was maintained, and a further decrease over the five-year period was evident when analyses were adjusted for vulnerability of the newborns. Perinatal survival and performance of ventilation were significantly improved.
