Fully automated platelet differential interference contrast image analysis via deep learning.

ABSTRACT: Platelets mediate arterial thrombosis, a leading cause of myocardial infarction and stroke. During injury, platelets adhere and spread over exposed subendothelial matrix substrates of the damaged blood vessel wall. The mechanisms which govern platelet activation and their interaction with a range of substrates are therefore regularly investigated using platelet spreading assays. These assays often use differential interference contrast (DIC) microscopy to assess platelet morphology and analysis performed using manual annotation. Here, a convolutional neural network (CNN) allowed fully automated analysis of platelet spreading assays captured by DIC microscopy. The CNN was trained using 120 generalised training images. Increasing the number of training images increases the mean average precision of the CNN. The CNN performance was compared to six manual annotators. Significant variation was observed between annotators, highlighting bias when manual analysis is performed. The CNN effectively analysed platelet morphology when platelets spread over a range of substrates (CRP-XL, vWF and fibrinogen), in the presence and absence of inhibitors (dasatinib, ibrutinib and PRT-060318) and agonist (thrombin), with results consistent in quantifying spread platelet area which is comparable to published literature. The application of a CNN enables, for the first time, automated analysis of platelet spreading assays captured by DIC microscopy.

Project description:ObjectivesTime-resolved, 2D-phase-contrast MRI (2D-CINE-PC-MRI) enables in vivo blood flow analysis. However, accurate vessel contour delineation (VCD) is required to achieve reliable results. We sought to evaluate manual analysis (MA) compared to the performance of a deep learning (DL) application for fully-automated VCD and flow quantification and corrected semi-automated analysis (corSAA).MethodsWe included 97 consecutive patients (age = 52.9 ± 16 years, 41 female) with 2D-CINE-PC-MRI imaging on 1.5T MRI systems at sinotubular junction (STJ), and 28/97 also received 2D-CINE-PC at main pulmonary artery (PA). A cardiovascular radiologist performed MA (reference) and corSAA (built-in tool) in commercial software for all cardiac time frames (median: 20, total contours per analysis: 2358 STJ, 680 PA). DL-analysis automatically performed VCD, followed by net flow (NF) and peak velocity (PV) quantification. Contours were compared using Dice similarity coefficients (DSC). Discrepant cases (> ± 10 mL or > ± 10 cm/s) were reviewed in detail.ResultsDL was successfully applied to 97% (121/125) of the 2D-CINE-PC-MRI series (STJ: 95/97, 98%, PA: 26/28, 93%). Compared to MA, mean DSC were 0.91 ± 0.02 (DL), 0.94 ± 0.02 (corSAA) at STJ, and 0.85 ± 0.08 (DL), 0.93 ± 0.02 (corSAA) at PA; this indicated good to excellent DL-performance. Flow quantification revealed similar NF at STJ (p = 0.48) and PA (p > 0.05) between methods while PV assessment was significantly different (STJ: p < 0.001, PA: p = 0.04). A detailed review showed noisy voxels in MA and corSAA impacted PV results. Overall, DL analysis compared to human assessments was accurate in 113/121 (93.4%) cases.ConclusionsFully-automated DL-analysis of 2D-CINE-PC-MRI provided flow quantification at STJ and PA at expert level in > 93% of cases with results being available instantaneously.Key points• Deep learning performed flow quantification on clinical 2D-CINE-PC series at the sinotubular junction and pulmonary artery at the expert level in > 93% of cases. • Location detection and contouring of the vessel boundaries were performed fully-automatic with results being available instantaneously compared to human assessments which approximately takes three minutes per location. • The evaluated tool indicates usability in daily practice.

Project description:BackgroundUltrasound imaging is pivotal for point of care non-invasive diagnosis of musculoskeletal (MSK) injuries. Notably, MSK ultrasound demands a higher level of operator expertise compared to general ultrasound procedures, necessitating thorough checks on image quality and precise categorization of each image. This need for skilled assessment highlights the importance of developing supportive tools for quality control and categorization in clinical settings. To address these challenges, we aim to develop and evaluate an automated system designed to assess whether an ultrasound image meets established standards and to identify its specific category, thereby assisting clinicians in improving diagnostic efficiency and accuracy in MSK ultrasound.MethodsWe proposed a two-stage system built upon a Swin Transformer-based Unet model. Initially, an ultrasound image of the elbow is categorized, following which its quality is evaluated to determine if it meets the established standard criteria set by experienced radiologists.ResultsThe proposed method surpassed other approaches in performance, with the Swin Transformer, serving as the backbone, outperforming conventional convolutional neural network (CNN) models. Additionally, when utilizing the same backbone, the two-stage model exceeded the capabilities of the single-stage model. Notably, the accuracy of our model exceeded 97%, with each category achieving over 95%. Additionally, processing time was significantly reduced from over 16 minutes to less than 15 seconds. Furthermore, this model reduces the workload for experienced radiologists, demonstrating its efficiency and effectiveness. This allows human experts to focus more on rare and challenging cases that machine learning struggles to solve, and to create high-quality labels for the models to learn from.ConclusionsWe have introduced an elbow ultrasound image recognition system leveraging the Swin Transformer, achieving competitive outcomes in both categorization and standard verification. This approach significantly reduces diagnostic time, allowing radiologists to focus more on scientific validation rather than image analysis.
