Postdischarge coping difficulties and influencing factors of pregnant women with gestational diabetes mellitus: A cross-sectional study.

ABSTRACT:

Aims and objectives

To identify coping difficulties after discharge and influencing factors of pregnant women with gestational diabetes mellitus.

Design

A cross-sectional descriptive survey was conducted with gestational diabetes mellitus women from four tertiary hospitals in the Shandong Peninsula in China.

Methods

The study consists of 252 gestational diabetes mellitus women. Survey measures included demographic information, quality of discharge teaching scale, readiness for hospital discharge scale and postdischarge coping difficulty scale. Multiple linear regression analysis was used to explore the factors of coping difficulties after discharge.

Results

The situation of coping difficulties in pregnant women with gestational diabetes mellitus after discharge is not optimistic. Age, education level, type of birth, parity, quality of discharge teaching and discharge readiness are six factors that significantly influence postdischarge coping difficulties.

Project description:BackgroundStudies reveal that promoting the breastfeeding knowledge level help to improve breastfeeding behaviors. Promoting breastfeeding knowledge is a simple and economical way to increase breastfeeding rates. However, there are no studies focus on the level of breastfeeding knowledge and factors influencing the knowledge in women with gestational diabetes mellitus (GDM), which is defined as any degree of glucose tolerance impairment first diagnosed during pregnancy. Thus, the objectives of this study were to investigate the breastfeeding knowledge level of GDM pregnant women and explore factors influencing the knowledge level.MethodsCross-sectional survey and convenience sampling were conducted in this study. The sociodemographic characteristics, caregivers in pregnancy, knowledge source, breastfeeding status and breast status information of participants were collected. Breastfeeding Knowledge Scale was used to assess the breastfeeding knowledge level of pregnant women with GDM. Multiple linear regression was used to analyze the influence factors of breastfeeding knowledge level in this study.ResultsA total of 226 questionnaires were issued and finally 212 valid questionnaires were collected. Some misconceptions still existed (e.g. 'breastfeeding cannot prevent your baby from being overweight' and 'it is advisable to breastfeed 3-4 times per day within 2-3 days after delivery'), although women with GDM had a good score of breastfeeding knowledge (mean score: 103.5 ± 10.4). Multiple linear regression analysis found that gestational age, family per capita monthly income, educational level, knowledge source were the independent protective factors for breastfeeding knowledge and minority nationality was the independent risk factor. The educational level had the greatest influence on the breastfeeding knowledge level of GDM pregnant women (β = 0.210, t = 2.978, P = 0.003).ConclusionGDM pregnant women with insufficient gestational age, low educational level, low family per capita monthly income and single access to knowledge should be included in the focus of health education on breastfeeding. In-depth and systematic health education should be conducted for pregnant women with GDM to improve their breastfeeding rate.

Project description:BackgroundCurrently, most studies only focus on the glucose management level and self- management behavior of pregnant women with gestational diabetes mellitus, but lack analysis and discussion on their decision-making behavior and influencing factors during glucose management.AimThis study aimed to investigate the decision-making behavior of blood glucose management and its influencing factors among pregnant women with gestational diabetes mellitus in China.MethodsThis was a prospective study. A convenience sampling method was used to select pregnant women with gestational diabetes mellitus. A total of 1,405 pregnant women with gestational diabetes mellitus from eight hospitals in China were included between June 1 and December 31, 2023. Three types of questionnaires on knowledge, protection motivation, and decision-making behavior regarding blood glucose management were administered to participants. Multivariable logistic regression was conducted to determine the decision-making behavior of blood glucose management and its influencing factors (P<0.05).ResultsThe mean score of the glucose management knowledge was 39.56 ± 8.45 points (standard score 65.9%). The score for decision-making behavior was 70.05 ± 9.85 points (standard score 93.4%). Most dimensions of protection motivations were higher than 20 points (standard score 80.0%).ConclusionsThe mean score for glucose management knowledge was at a medium score level. Most dimensions of protection motivations were at a high or medium score level. The decision-making behavior was at a high score level. The pregnant women with rich glucose management knowledge and protection motivation are more likely to take active decision-making behaviors and actively manage their blood glucose levels. The influencing factors of decision-making behavior included length of time after diagnosed with gestational diabetes mellitus, gestational diabetes mellitus history, self-efficacy, severity, susceptibility, response efficacy, and response costs (P<0.05, for all).
