Project description:The growing number of portable consumer-grade electroencephalography (EEG) wearables offers potential to track brain activity and neurological disease in real-world environments. However, accompanying open software tools to standardize custom recordings and help guide independent operation by users is lacking. To address this gap, we developed HEROIC, an open-source software that allows participants to remotely collect advanced EEG data without the aid of an expert technician. The aim of HEROIC is to provide an open software platform that can be coupled with consumer grade wearables to record EEG data during customized neurocognitive tasks outside of traditional research environments. This article contains a description of HEROIC's implementation, how it can be used by researchers and a proof-of-concept demonstration highlighting the potential for HEROIC to be used as a scalable and low-cost EEG data collection tool. Specifically, we used HEROIC to guide healthy participants through standardized neurocognitive tasks and captured complex brain data including event-related potentials (ERPs) and powerband changes in participants' homes. Our results demonstrate HEROIC's capability to generate data precisely synchronized to presented stimuli, using a low-cost, remote protocol without reliance on an expert operator to administer sessions. Together, our software and its capabilities provide the first democratized and scalable platform for large-scale remote and longitudinal analysis of brain health and disease.

Project description:A considerable number of patients with COVID-19 suffer from respiratory problems in the post-acute phase of the disease (the second-third month after disease onset). Individual telerehabilitation and telecoaching are viable, effective options for treating these patients. To treat patients individually, medical staff must have detailed knowledge of their physical activity and condition. A sensor network that utilizes medical-grade devices can be created to collect these data, but the price and availability of these devices might limit such a network's scalability to larger groups of patients. Hence, the use of low-cost commercial fitness wearables is an option worth exploring. This article presents the concept and technical infrastructure of such a telerehabilitation program that started in April 2021 in the Czech Republic. A pilot controlled study with 14 patients with COVID-19 indicated the program's potential to improve patients' physical activity, (85.7% of patients in telerehabilitation versus 41.9% educational group) and exercise tolerance (71.4% of patients in telerehabilitation versus 42.8% of the educational group). Regarding the accuracy of collected data, the used commercial wristband was compared with the medical-grade device in a separate test. Evaluating [Formula: see text]-scores of the intensity of participants' physical activity in this test, the difference in data is not statistically significant at level [Formula: see text]. Hence, the used infrastructure can be considered sufficiently accurate for the telerehabilitation program examined in this study. The technical and medical aspects of the problem are discussed, as well as the technical details of the solution and the lessons learned, regarding using this approach to treat COVID-19 patients in the post-acute phase.

Project description:BackgroundExtreme weather, including heat and extreme rainfall, is projected to increase owing to climate change, which can have adverse impacts on human health. In particular, rural populations in sub-Saharan Africa are at risk because of a high burden of climate-sensitive diseases and low adaptive capacities. However, there is a lack of data on the regions that are anticipated to be most exposed to climate change. Improved public health surveillance is essential for better decision-making and health prioritization and to identify risk groups and suitable adaptation measures. Digital technologies such as consumer-grade wearable devices (wearables) may generate objective measurements to guide data-driven decision-making.ObjectiveThe main objective of this observational study was to examine the impact of weather exposure on population health in rural Burkina Faso using wearables. Specifically, this study aimed to assess the relationship between individual daily activity (steps), sleep duration, and heart rate (HR), as estimated by wearables, and exposure to heat and heavy rainfall.MethodsOverall, 143 participants from the Nouna health and demographic surveillance system in Burkina Faso wore the Withings Pulse HR wearable 24/7 for 11 months. We collected continuous weather data using 5 weather stations throughout the study region. The heat index and wet-bulb globe temperature (WBGT) were calculated as measures of heat. We used linear mixed-effects models to quantify the relationship between exposure to heat and rainfall and the wearable parameters. Participants kept activity journals and completed a questionnaire on their perception of and adaptation to heat and other weather exposure.ResultsSleep duration decreased significantly (P<.001) with higher heat exposure, with approximately 15 minutes shorter sleep duration during heat stress nights with a heat index value of ≥25 °C. Many participants (55/137, 40.1%) reported that heat affected them the most at night. During the day, most participants (133/137, 97.1%) engaged in outdoor physical work such as farming, housework, or fetching water. During the rainy season, when WBGT was highest, daily activity was highest and increased when the daily maximum WBGT surpassed 30 °C during the rainiest month. In the hottest month, daily activity decreased per degree increase in WBGT for values >30 °C. Nighttime HR showed no significant correlation with heat exposure. Daytime HR data were insufficient for analysis. We found no negative health impact associated with heavy rainfall. With increasing rainfall, sleep duration increased, average nightly HR decreased, and activity decreased.ConclusionsDuring the study period, participants were frequently exposed to heat and heavy rainfall. Heat was particularly associated with impaired sleep and daily activity. Essential tasks such as harvesting, fetching water, and caring for livestock expose this population to weather that likely has an adverse impact on their health. Further research is essential to guide interventions safeguarding vulnerable communities.

Project description:The increasing usage of smart wearable devices has made an impact not only on the lifestyle of the users, but also on biological research and personalized healthcare services. These devices, which carry different types of sensors, have emerged as personalized digital diagnostic tools. Data from such devices have enabled the prediction and detection of various physiological as well as psychological conditions and diseases. In this review, we have focused on the diagnostic applications of wrist-worn wearables to detect multiple diseases such as cardiovascular diseases, neurological disorders, fatty liver diseases, and metabolic disorders, including diabetes, sleep quality, and psychological illnesses. The fruitful usage of wearables requires fast and insightful data analysis, which is feasible through machine learning. In this review, we have also discussed various machine-learning applications and outcomes for wearable data analyses. Finally, we have discussed the current challenges with wearable usage and data, and the future perspectives of wearable devices as diagnostic tools for research and personalized healthcare domains.

Project description:Various GNSS applications require low-cost, small-scale, lightweight and power-saving GNSS devices and require high precision in terms of low noise for carrier phase and code observations. Applications vary from navigation approaches to positioning in geo-monitoring units up to integration in multi-sensor-systems. For highest precision, only GNSS receivers are suitable that provide access to raw data such as carrier phase, code ranges, Doppler and signal strength. A system integration is only possible if the overall noise level is known and quantified at the level of the original observations. A benchmark analysis based on a zero baseline is proposed to quantify the stochastic properties. The performance of the consumer grade GNSS receiver is determined and evaluated against geodetic GNSS receivers to better understand the utilization of consumer grade receivers. Results indicate high similarity to the geodetic receiver, even though technical limitations are present. Various stochastic techniques report normally distributed carrier-phase noise of 2 mm and code-range noise of 0.5-0.8 m. This is confirmed by studying the modified Allan standard deviation and code-minus-carrier combinations. Derived parameters serve as important indicators for the integration of GNSS receivers into multi-sensor-systems.

Project description:IntroductionA major challenge in the monitoring of rehabilitation is the lack of long-term individual baseline data which would enable accurate and objective assessment of functional recovery. Consumer-grade wearable devices enable the tracking of individual everyday functioning prior to illness or other medical events which necessitate the monitoring of recovery trajectories.MethodsFor 1,324 individuals who underwent surgery on a lower limb, we collected their Fitbit device data of steps, heart rate, and sleep from 26 weeks before to 26 weeks after the self-reported surgery date. We identified subgroups of individuals who self-reported surgeries for bone fracture repair (n = 355), tendon or ligament repair/reconstruction (n = 773), and knee or hip joint replacement (n = 196). We used linear mixed models to estimate the average effect of time relative to surgery on daily activity measurements while adjusting for gender, age, and the participant-specific activity baseline. We used a sub-cohort of 127 individuals with dense wearable data who underwent tendon/ligament surgery and employed XGBoost to predict the self-reported recovery time.ResultsThe 1,324 study individuals were all US residents, predominantly female (84%), white or Caucasian (85%), and young to middle-aged (mean age 36.2 years). We showed that 12 weeks pre- and 26 weeks post-surgery trajectories of daily behavioral measurements (steps sum, heart rate, sleep efficiency score) can capture activity changes relative to an individual's baseline. We demonstrated that the trajectories differ across surgery types, recapitulate the documented effect of age on functional recovery, and highlight differences in relative activity change across self-reported recovery time groups. Finally, using a sub-cohort of 127 individuals, we showed that long-term recovery can be accurately predicted, on an individual level, only 1 month after surgery (AUROC 0.734, AUPRC 0.8). Furthermore, we showed that predictions are most accurate when long-term, individual baseline data are available.DiscussionLeveraging long-term, passively collected wearable data promises to enable relative assessment of individual recovery and is a first step towards data-driven intervention for individuals.

Project description:BackgroundConsumer-grade wearables are becoming increasingly popular in research and in clinical contexts. These technologies hold significant promise for advancing digital medicine, particularly in remote and rural areas in low-income settings like sub-Saharan Africa, where climate change is exacerbating health risks. This study evaluates the data agreement between consumer-grade and research-established devices under standardized conditions.MethodsTwenty-two participants (11 women, 11 men) performed a structured protocol, consisting of six different activity phases (sitting, standing, and the first four stages of the classic Bruce treadmill test). We collected heart rate, (core) body temperature, step count, and energy expenditure. Each variable was simultaneously tracked by consumer-grade and established research-grade devices to evaluate the validity of the consumer-grade devices. We statistically compared the data agreement using Pearson's correlation r, Lin's concordance correlation coefficient (LCCC), Bland-Altman method, and mean absolute percentage error.ResultsA good agreement was found between the wrist-worn Withings Pulse HR (consumer-grade) and the chest-worn Faros Bittium 180 in measuring heart rate while sitting, standing, and slow walking on a treadmill at a speed of 2.7 km/h (r ≥ 0.82, |bias| ≤ 3.1 bpm), but this decreased with increasing speed (r ≤ 0.33, |bias| ≤ 11.7 bpm). The agreement between the Withing device and the research-established device worn on the wrist (GENEActiv) for measuring the number of steps also decreased during the treadmill phases (first stage: r = 0.48, bias = 0.6 steps/min; fourth stage: r = 0.48, bias = 17.3 steps/min). Energy expenditure agreement between the Withings device and the indirect calorimetry method was poor during the treadmill test (|r| ≤ 0.29, |bias | ≥ 1.7 MET). The Tucky thermometer under the armpit (consumer-grade) and the Tcore sensor on the forehead were found to be in poor agreement in measuring (core) body temperature during resting phases (r ≤ 0.53, |bias| ≥ 0.8°C) and deteriorated during the treadmill test.ConclusionThe Withings device showed adequate performance for heart rate at low activity levels and step count at higher activity levels, but had limited overall accuracy. The Tucky device showed poor agreement with the Tcore in all six different activity phases. The limited accuracy of consumer-grade devices suggests caution in their use for rigorous research, but points to their potential utility in capture general physiological trends in long-term field monitoring or population-health surveillance.

Project description:An increasing number of aging researchers believes that multi-system physiological dysregulation may be a key biological mechanism of aging, but evidence of this has been sparse. Here, we used biomarker data on nearly 33, 000 individuals from four large datasets to test for the presence of multi-system dysregulation. We grouped 37 biomarkers into six a priori groupings representing physiological systems (lipids, immune, oxygen transport, liver function, vitamins, and electrolytes), then calculated dysregulation scores for each system in each individual using statistical distance. Correlations among dysregulation levels across systems were generally weak but significant. Comparison of these results to dysregulation in arbitrary 'systems' generated by random grouping of biomarkers showed that a priori knowledge effectively distinguished the true systems in which dysregulation proceeds most independently. In other words, correlations among dysregulation levels were higher using arbitrary systems, indicating that only a priori systems identified distinct dysregulation processes. Additionally, dysregulation of most systems increased with age and significantly predicted multiple health outcomes including mortality, frailty, diabetes, heart disease, and number of chronic diseases. The six systems differed in how well their dysregulation scores predicted health outcomes and age. These findings present the first unequivocal demonstration of integrated multi-system physiological dysregulation during aging, demonstrating that physiological dysregulation proceeds neither as a single global process nor as a completely independent process in different systems, but rather as a set of system-specific processes likely linked through weak feedback effects. These processes--probably many more than the six measured here--are implicated in aging.

Project description:BackgroundHypoactive lifestyle contributes to the development of secondary complications and lower quality of life in wheelchair users. There is a need for objective and user-friendly physical activity monitors for wheelchair-dependent individuals in order to increase physical activity through self-monitoring, goal setting, and feedback provision.ObjectiveTo determine the validity of Activ8 Activity Monitors to 1) distinguish two classes of activities: independent wheelchair propulsion from other non-propulsive wheelchair-related activities 2) distinguish five wheelchair-related classes of activities differing by the movement intensity level: sitting in a wheelchair (hands may be moving but wheelchair remains stationary), maneuvering, and normal, high speed or assisted wheelchair propulsion.MethodsSixteen able-bodied individuals performed sixteen various standardized 60s-activities of daily living. Each participant was equipped with a set of two Activ8 Professional Activity Monitors, one at the right forearm and one at the right wheel. Task classification by the Active8 Monitors was validated using video recordings. For the overall agreement, sensitivity and positive predictive value, outcomes above 90% are considered excellent, between 70 and 90% good, and below 70% unsatisfactory.ResultsDivision in two classes resulted in overall agreement of 82.1%, sensitivity of 77.7% and positive predictive value of 78.2%. 84.5% of total duration of all tasks was classified identically by Activ8 and based on the video material. Division in five classes resulted in overall agreement of 56.6%, sensitivity of 52.8% and positive predictive value of 51.9%. 59.8% of total duration of all tasks was classified identically by Activ8 and based on the video material.ConclusionsActiv8 system proved to be suitable for distinguishing between active wheelchair propulsion and other non-propulsive wheelchair-related activities. The ability of the current system and algorithms to distinguish five various wheelchair-related activities is unsatisfactory.

Project description:ObjectiveConsumer wearables, such as the Apple Watch or Fitbit devices, have become increasingly commonplace over the past decade. The application of these devices to health care remains an area of significant yet ill-defined promise. This review aims to identify the potential role of consumer wearables for the monitoring of otolaryngology patients.Data sourcesPubMed.Review methodsA PubMed search was conducted to identify the use of consumer wearables for the assessment of clinical outcomes relevant to otolaryngology. Articles were included if they described the use of wearables that were designed for continuous wear and were available for consumer purchase in the United States. Articles meeting inclusion criteria were synthesized into a final narrative review.ConclusionsIn the perioperative setting, consumer wearables could facilitate prehabilitation before major surgery and prediction of clinical outcomes. The use of consumer wearables in the inpatient setting could allow for early recognition of parameters suggestive of poor or declining health. The real-time feedback provided by these devices in the remote setting could be incorporated into behavioral interventions to promote patients' engagement with healthy behaviors. Various concerns surrounding the privacy, ownership, and validity of wearable-derived data must be addressed before their widespread adoption in health care.Implications for practiceUnderstanding how to leverage the wealth of biometric data collected by consumer wearables to improve health outcomes will become a high-impact area of research and clinical care. Well-designed comparative studies that elucidate the value and clinical applicability of these data are needed.
