Project description:AI is a broad concept, grouping initiatives that use a computer to perform tasks that would usually require a human to complete. AI methods are well suited to predict clinical outcomes. In practice, AI methods can be thought of as functions that learn the outcomes accompanying standardized input data to produce accurate outcome predictions when trialed with new data. Current methods for cleaning, creating, accessing, extracting, augmenting, and representing data for training AI clinical prediction models are well defined. The use of AI to predict clinical outcomes is a dynamic and rapidly evolving arena, with new methods and applications emerging. Extraction or accession of electronic health care records and combining these with patient genetic data is an area of present attention, with tremendous potential for future growth. Machine learning approaches, including decision tree methods of Random Forest and XGBoost, and deep learning techniques including deep multi-layer and recurrent neural networks, afford unique capabilities to accurately create predictions from high dimensional, multimodal data. Furthermore, AI methods are increasing our ability to accurately predict clinical outcomes that previously were difficult to model, including time-dependent and multi-class outcomes. Barriers to robust AI-based clinical outcome model deployment include changing AI product development interfaces, the specificity of regulation requirements, and limitations in ensuring model interpretability, generalizability, and adaptability over time.

Project description:Failure to predict stroke promptly may lead to delayed treatment, causing severe consequences like permanent neurological damage or death. Early detection using deep learning (DL) and machine learning (ML) models can enhance patient outcomes and mitigate the long-term effects of strokes. The aim of this study is to compare these models, exploring their efficacy in predicting stroke. This study analyzed a dataset comprising 663 records from patients hospitalized at Hazrat Rasool Akram Hospital in Tehran, Iran, including 401 healthy individuals and 262 stroke patients. A total of eight established ML (SVM, XGB, KNN, RF) and DL (DNN, FNN, LSTM, CNN) models were utilized to predict stroke. Techniques such as 10-fold cross-validation and hyperparameter tuning were implemented to prevent overfitting. The study also focused on interpretability through Shapley Additive Explanations (SHAP). The evaluation of model's performance was based on accuracy, specificity, sensitivity, F1-score, and ROC curve metrics. Among DL models, LSTM showed superior sensitivity at 96.15%, while FNN exhibited better specificity (96.0%), accuracy (96.0%), F1-score (95.0%), and ROC (98.0%) among DL models. For ML models, RF displayed higher sensitivity (99.9%), accuracy (99.0%), specificity (100%), F1-score (99.0%), and ROC (99.9%). Overall, RF outperformed all models, while DL models surpassed ML models in most metrics except for RF. DL models (CNN, LSTM, DNN, FNN) achieved sensitivities from 93.0 to 96.15%, specificities from 80.0 to 96.0%, accuracies from 92.0 to 96.0%, F1-scores from 87.34 to 95.0%, and ROC scores from 95.0 to 98.0%. In contrast, ML models (KNN, XGB, SVM) showed sensitivities between 29.0% and 94.0%, specificities between 89.47% and 96.0%, accuracies between 71.0% and 95.0%, F1-scores between 44.0% and 95.0%, and ROC scores between 64.0% and 95.0%. This study demonstrates the efficacy of DL and ML models in predicting stroke, with the RF models outperforming all others in key metrics. While DL models generally surpassed ML models, RF's exceptional performance highlights the potential of combining these technologies for early stroke detection, significantly improving patient outcomes by preventing severe consequences like permanent neurological damage or death.

Project description:The etiology of cerebral palsy (CP) is complex and remains inadequately understood. Early detection of CP is an important clinical objective as this improves long term outcomes. We performed genome-wide DNA methylation analysis to identify epigenomic predictors of CP in newborns and to investigate disease pathogenesis. Methylation analysis of newborn blood DNA using an Illumina HumanMethylation450K array was performed in 23 CP cases and 21 unaffected controls. There were 230 significantly differentially-methylated CpG loci in 258 genes. Each locus had at least 2.0-fold change in methylation in CP versus controls with a FDR p-value ≤ 0.05. Methylation level for each CpG locus had an area under the receiver operating curve (AUC) ≥ 0.75 for CP detection. Using Artificial Intelligence (AI) platforms/Machine Learning (ML) analysis, CpG methylation levels in a combination of 230 significantly differentially-methylated CpG loci in 258 genes had a 95% sensitivity and 94.4% specificity for newborn prediction of CP. Using pathway analysis, multiple canonical pathways plausibly linked to neuronal function were over-represented. Altered biological processes and functions included: neuromotor damage, malformation of major brain structures, brain growth, neuroprotection, neuronal development and de-differentiation, and cranial sensory neuron development. In conclusion, blood leucocyte epigenetic changes analyzed using AI/ML techniques appeared to accurately predict CP and provided plausible mechanistic information on CP pathogenesis.

Project description:Deep learning has emerged as the leading method in machine learning, spawning a rapidly growing field of academic research and commercial applications across medicine. Deep learning could have particular relevance to rheumatology if correctly utilized. The greatest benefits of deep learning methods are seen with unstructured data frequently found in rheumatology, such as images and text, where traditional machine learning methods have struggled to unlock the trove of information held within these data formats. The basis for this success comes from the ability of deep learning to learn the structure of the underlying data. It is no surprise that the first areas of medicine that have started to experience impact from deep learning heavily rely on interpreting visual data, such as triaging radiology workflows and computer-assisted colonoscopy. Applications in rheumatology are beginning to emerge, with recent successes in areas as diverse as detecting joint erosions on plain radiography, predicting future rheumatoid arthritis disease activity, and identifying halo sign on temporal artery ultrasound. Given the important role deep learning methods are likely to play in the future of rheumatology, it is imperative that rheumatologists understand the methods and assumptions that underlie the deep learning algorithms in widespread use today, their limitations and the landscape of deep learning research that will inform algorithm development, and clinical decision support tools of the future. The best applications of deep learning in rheumatology must be informed by the clinical experience of rheumatologists, so that algorithms can be developed to tackle the most relevant clinical problems.

In these focused research areas, the use of artificial intelligence (AI), machine learning (ML), and deep learning (DL) algorithms and associated models have been found very popular. With the help of available articles and information, this review provide an introduction to this novel research trend. Ion channel understanding is usually made considering the structural and functional perspectives, gating mechanisms, transport properties, channel protein mutations, etc. Focused research on ion channels and related findings over many decades accumulated huge data which may be utilized in a specialized scientific manner to fast conclude pinpointed aspects of channels. AI, ML, and DL techniques and models may appear as helping tools. This review aims at explaining the ways we may use the bioinformatics techniques and thus draw a few lines across the avenue to let the ion channel features appear clearer.

Project description:BackgroundArtificial intelligence approaches can integrate complex features and can be used to predict a patient's risk of developing lung cancer, thereby decreasing the need for unnecessary and expensive diagnostic interventions.ObjectiveThe aim of this study was to use electronic medical records to prescreen patients who are at risk of developing lung cancer.MethodsWe randomly selected 2 million participants from the Taiwan National Health Insurance Research Database who received care between 1999 and 2013. We built a predictive lung cancer screening model with neural networks that were trained and validated using pre-2012 data, and we tested the model prospectively on post-2012 data. An age- and gender-matched subgroup that was 10 times larger than the original lung cancer group was used to assess the predictive power of the electronic medical record. Discrimination (area under the receiver operating characteristic curve [AUC]) and calibration analyses were performed.ResultsThe analysis included 11,617 patients with lung cancer and 1,423,154 control patients. The model achieved AUCs of 0.90 for the overall population and 0.87 in patients ≥55 years of age. The AUC in the matched subgroup was 0.82. The positive predictive value was highest (14.3%) among people aged ≥55 years with a pre-existing history of lung disease.ConclusionsOur model achieved excellent performance in predicting lung cancer within 1 year and has potential to be deployed for digital patient screening. Convolution neural networks facilitate the effective use of EMRs to identify individuals at high risk for developing lung cancer.

Project description:Deep learning involves an artificial intelligence (AI) approach and has been shown to provide superior performance for automating image recognition tasks, as well as exceeding human capabilities in both time and accuracy. Histopathology diagnostics is one of the more popular challenges at the intersection of artificial intelligence, computer vision, and medicine. Developing methods to automatically detect and identify pathologies in digitized histology slides imposes unique challenges due to the large size of these images and the complexity of the features present in biological tissue. Most methods that are capable of human-level recognition in histopathology are tuned to a specific problem since the computational complexity exceeds that of traditional image classification problems. In the current study, a deep learning approach is developed and presented that can be trained to locate and accurately classify different types of pathologies in gigapixel digitized histology slides along with completing the binary disease classification for the entire image. The approach uses a novel pyramid tiling approach to take advantage of spatial awareness around the area to be classified, while maintaining efficiency and scalability for gigapixel images. The approach is trained and validated on a wide variety of tissue types (i.e., testis, ovary, prostate, kidney) and pathologies taken from an epigenetically altered histology study at Washington State University. The newly developed procedure was optimized and validated along with comparison and validation on public histology datasets. The current developed procedure was found to be optimal and more reproducible when compared to manual procedures, and optimal to previous protocols that used fragmented tissue or slide analysis. Observations demonstrate that the deep learning histopathology analysis is significantly more efficient and accurate than standard manual histopathology analysis.

Project description:The development of digital pathology and progression of state-of-the-art algorithms for computer vision have led to increasing interest in the use of artificial intelligence (AI), especially deep learning (DL)-based AI, in tumor pathology. The DL-based algorithms have been developed to conduct all kinds of work involved in tumor pathology, including tumor diagnosis, subtyping, grading, staging, and prognostic prediction, as well as the identification of pathological features, biomarkers and genetic changes. The applications of AI in pathology not only contribute to improve diagnostic accuracy and objectivity but also reduce the workload of pathologists and subsequently enable them to spend additional time on high-level decision-making tasks. In addition, AI is useful for pathologists to meet the requirements of precision oncology. However, there are still some challenges relating to the implementation of AI, including the issues of algorithm validation and interpretability, computing systems, the unbelieving attitude of pathologists, clinicians and patients, as well as regulators and reimbursements. Herein, we present an overview on how AI-based approaches could be integrated into the workflow of pathologists and discuss the challenges and perspectives of the implementation of AI in tumor pathology.

Project description:PurposeTo present fully automated DL-based prostate cancer detection system for prostate MRI.MethodsMRI scans from two institutions, were used for algorithm training, validation, testing. MRI-visible lesions were contoured by an experienced radiologist. All lesions were biopsied using MRI-TRUS-guidance. Lesions masks, histopathological results were used as ground truth labels to train UNet, AH-Net architectures for prostate cancer lesion detection, segmentation. Algorithm was trained to detect any prostate cancer ≥ ISUP1. Detection sensitivity, positive predictive values, mean number of false positive lesions per patient were used as performance metrics.Results525 patients were included for training, validation, testing of the algorithm. Dataset was split into training (n = 368, 70%), validation (n = 79, 15%), test (n = 78, 15%) cohorts. Dice coefficients in training, validation sets were 0.403, 0.307, respectively, for AHNet model compared to 0.372, 0.287, respectively, for UNet model. In validation set, detection sensitivity was 70.9%, PPV was 35.5%, mean number of false positive lesions/patient was 1.41 (range 0-6) for UNet model compared to 74.4% detection sensitivity, 47.8% PPV, mean number of false positive lesions/patient was 0.87 (range 0-5) for AHNet model. In test set, detection sensitivity for UNet was 72.8% compared to 63.0% for AHNet, mean number of false positive lesions/patient was 1.90 (range 0-7), 1.40 (range 0-6) in UNet, AHNet models, respectively.ConclusionWe developed a DL-based AI approach which predicts prostate cancer lesions at biparametric MRI with reasonable performance metrics. While false positive lesion calls remain as a challenge of AI-assisted detection algorithms, this system can be utilized as an adjunct tool by radiologists.

Project description:PurposeTo investigate the correlation between choroidal thickness and myopia progression using a deep learning method.MethodsTwo data sets, data set A and data set B, comprising of 123 optical coherence tomography (OCT) volumes, were collected to establish the model and verify its clinical utility. The proposed mask region-based convolutional neural network (R-CNN) model, trained with the pretrained weights from the Common Objects in Context database as well as the manually labeled OCT images from data set A, was used to automatically segment the choroid. To verify its clinical utility, the mask R-CNN model was tested with data set B, and the choroidal thickness estimated by the model was also used to explore its relationship with myopia.ResultsCompared with the result of manual segmentation in data set B, the error of the automatic choroidal inner and outer boundary segmentation was 6.72 ± 2.12 and 13.75 ± 7.57 µm, respectively. The mean dice coefficient between the region segmented by automatic and manual methods was 93.87% ± 2.89%. The mean difference in choroidal thickness over the Early Treatment Diabetic Retinopathy Study zone between the two methods was 10.52 µm. Additionally, the choroidal thickness estimated using the proposed model was thinner in high-myopic eyes, and axial length was the most significant predictor.ConclusionsThe mask R-CNN model has excellent performance in choroidal segmentation and quantification. In addition, the choroid of high myopia is significantly thinner than that of nonhigh myopia.Translational relevanceThis work lays the foundations for mask R-CNN models that could aid in the evaluation of more intricate changes occurring in chorioretinal diseases.
