Impact of an Interoception-Based Program on Emotion Regulation in Autistic Children.

ABSTRACT:

Purpose

The aim of this study was to determine the feasibility and effectiveness of a 25-week school-based intervention and its ability to improve interoception and emotion regulation in an autistic pediatric population.

Method

One-group pre- and posttest design implementing The Interoception Curriculum: A Guide to Developing Mindful Self-Regulation in a self-contained school. Participants were 14 (11 male, 3 female) students between 9 and 19 years old. The Behavior Rating Inventory of Executive Function 2 (BRIEF-2) and the Caregiver Questionnaire for Interoceptive Awareness-2nd Edition (CQIA-2) were used to determine changes in interoceptive awareness and emotion regulation.

Results

Statistically significant improvements were found between the preintervention and postintervention scores for both interoceptive awareness and emotion regulation.

Conclusion

This was the first study to examine the Interoception Curriculum in its entirety, providing evidence that the use of the Interoception Curriculum is feasible in a school setting and suggests that this intervention is effective for improvement of interoception. Findings also suggest that this improvement in interoception is related to improvement in emotional regulation for an autistic pediatric population.

Project description:Background: Autistic children often face challenges with attention and emotion regulation, which can impact their socio-communication skills and overall well-being. Vibroacoustic therapy (VAT), a sensory-based intervention using low-frequency sound vibrations, may offer a novel approach to address these challenges. The objective of this pilot study is to explore the feasibility and potential efficacy of VAT in improving attention and emotion regulation in autistic children aged 9-12 years. Methods: Eighteen children were recruited with assistance from the school psychologist, using purposive sampling to identify participants with autism spectrum disorder and attentional challenges. The study was conducted at a primary school in Pretoria, South Africa, in March 2023. Participants were divided into treatment (n = 9) and control (n = 9) groups. The treatment group underwent 10 VAT sessions over six weeks. Attention was assessed using the NEPSY-II and Joint-Attention Test (JTAT), focusing on sustained, selective, alternating, and joint attention. Qualitative data were collected through observations and creative semi-structured interviews to understand the children's experiences of VAT. Tests were conducted at baseline, midway, post-intervention, and one week post-intervention. Statistical analysis was conducted using a Generalised Linear Mixed Model (GLMM) in R version 4.3.1, with significance assessed using a Likelihood Ratio Test (p < 0.05) to assess attention improvements. Results: Quantitative analysis revealed significant improvements in joint attention in the treatment group (X2 = 11.64, df = 3, p = 0.008). Qualitative findings highlighted positive experiences related to emotion regulation, with children reporting a sense of calm and enjoyment during VAT sessions. Teachers also noted improvements in attention and engagement. Conclusions: VAT appears to be a feasible and acceptable intervention for autistic children, with potential benefits for attention and emotion regulation. These findings support further research to validate its efficacy and explore its adaptability for diverse sensory profiles. VAT may hold promise as a holistic therapeutic tool in autism intervention programmes.

Project description:BackgroundMost mental health disorders are first experienced in childhood. The rising rates of mental health difficulties in children highlight the need for innovative approaches to supporting children and preventing these difficulties. School-based digital interventions that address shared risk factors and symptoms, such as emotion dysregulation, present exciting opportunities to enhance mental health support for children on a larger scale.ObjectiveThis study investigates the use of a new app-based intervention designed to support children's emotion regulation in schools. The aim is to optimize the usability, acceptability, and utility of the app and explore its scope for implementation with the target user in the school context.MethodsAs part of an interdisciplinary development framework, the app is being evaluated in a 3-month trial across 4 primary schools. In total, 144 children (aged 10-12 years) took part and accessed the intervention app in the classroom or at home. Outcomes regarding usability, acceptability, and implementation opportunities were assessed through digital user data, self-report questionnaires (132/144, 91.6%), and semistructured interviews with children (19/144, 13.2%) and teachers (6/8, 75%).ResultsThe app usage data showed that 30% (128/426) of the users were returning users. Self-report data indicated that 40.1% (53/132) of the children had not used the app, whereas 57.5% (76/132) had used it once or more. Of the children who had used the app, 67% (51/76) reported that the app was helpful. Interviews with children and teachers suggested positive experiences with the app and that it helped them to calm down and relax. Children reported that they perceived the app as acceptable, usable, and helpful. In terms of the intervention's usability, most features functioned well; however, certain technical issues were reported, which may have led to reduced engagement levels. Teachers not only reported overall positive experiences but also discussed access difficulties and reported a lack of content as one of the main barriers to implementing the app. Having a web-based app significantly enhanced accessibility across devices and settings and provided teachers with more opportunities to use it. We identified the need for new, activating app features in addition to the existing, primarily relaxing ones. The findings indicated that it is possible to use and evaluate an app intervention in the school context and that the app could help enhance children's emotion regulation. We discuss areas for improvement regarding the app, study design, and future implementation strategies.ConclusionsWe share important insights with regard to the development, implementation, and evaluation of a new app for supporting children's emotion regulation in schools. Our results demonstrate that mental health apps represent a promising means to facilitate effective mental health service provision in and outside of the school context. Important lessons learned are shared to support other researchers and clinicians on similar journeys.

Project description:According to the shared signal hypothesis (SSH) the impact of facial expressions on emotion processing partially depends on whether the gaze is directed toward or away from the observer. In autism spectrum disorder (ASD) several aspects of face processing have been found to be atypical, including attention to eye gaze and the identification of emotional expressions. However, there is little research on how gaze direction affects emotional expression processing in typically developing (TD) individuals and in those with ASD. This question is investigated here in two multimodal experiments. Experiment 1 required processing eye gaze direction while faces differed in emotional expression. Forty-seven children (aged 9-12 years) participated. Their Autism Diagnostic Observation Schedule (ADOS) scores ranged from 0 to 6 in the experiment. Event-related potentials (ERPs) were sensitive to gaze direction and emotion, but emotion processing did not depend on gaze direction. However, for angry faces the gaze direction effect on the N170 amplitude, as typically observed in TD individuals, diminished with increasing ADOS score. For neutral expressions this correlation was not significant. Experiment 2 required explicit emotion classifications in a facial emotion composite task while eye gaze was manipulated incidentally. A group of 22 children with ASD was compared to a propensity score-matched group of TD children (mean age = 13 years). The same comparison was carried out for a subgroup of nine children with ASD who were less trained in social cognition, according to clinician's report. The ASD group performed overall worse in emotion recognition than the TD group, independently of emotion or gaze direction. However, for disgust expressions, eye tracking data revealed that TD children fixated relatively longer on the eyes of the stimulus face with a direct gaze as compared with averted gaze. In children with ASD we observed no such modulation of fixation behavior as a function of gaze direction. Overall, the present findings from ERPs and eye tracking confirm the hypothesis of an impaired sensitivity to gaze direction in children with ASD or elevated autistic traits, at least for specific emotions. Therefore, we conclude that multimodal investigations of the interaction between emotional processing and stimulus gaze direction are promising to understand the characteristics of individuals differing along the autism trait dimension.
