Factors associated with successful passage of the American College of Veterinary Internal Medicine general examination.

ABSTRACT:

Background

Board certification relies on passing the American College of Veterinary Internal Medicine (ACVIM) general examination. Pass rates might depend on properties of residency training programs (RTP).

Hypothesis

We hypothesized that <4 weeks of dedicated study time, lack of board preparation lectures, status as a re-taker, and private practice RTP would result in lower pass rates of the ACVIM general examination.

Subjects

Two hundred forty-eight ACVIM general examinees.

Methods

Cross-sectional study. Examinees were surveyed using a Qualtrics survey over a 3-year period. Factors included: study weeks, on-call duty, board preparation lectures, academic or private practice program, and status as a re-taker.

Results

First-attempt examinees were more likely to pass (P < .0001, OR 5.12, 95% CI [2.53, 10.52]). For first-attempt examinees, on-call duty during study weeks resulted in a lower pass rate (P = .002, OR 0.31, 95% CI [0.16, 0.67]). General didactic and specific board-preparation lectures resulted in higher pass rates (P = .003, OR 3.08, 95% CI [1.44, 6.61]; P = .02, OR 3.04, 95% CI [1.20, 7.68]). Diplomate-led board-preparation lectures resulted in higher pass rates than resident-led (P = .007, OR 10.67, 95% CI [1.75, 64.91]). Using a mixed effect logistic model, predicted pass rates were highest with both lack of on-call duty and presence of didactic lectures (predicted pass rate 95%, 95% CI [0.87, 0.98]).

Conclusions and clinical importance

To optimize pass rates, RTP should provide study time without on-call duty. Provision of didactic lectures and specific board-preparation lectures by diplomates assist in candidate preparation.

Project description:BackgroundEvidence is lacking concerning re-introduction of feed and water following colic surgery.ObjectivesTo describe current approaches of European and American specialists to re-introduction of feed and water in adult horses following surgical treatment of common intestinal lesions, assuming an uncomplicated recovery.Study designCross-sectional survey.MethodsElectronic invitations, with a link to the online survey, were sent to 1,430 large animal specialists, including Diplomates of the ECVS, ACVS, ECEIM and ACVIM colleges.ResultsThe response rate was 12.6% including partial respondent data. Responses for each multiple-choice question were between 123 and 178. Results are expressed as the percentage of the total number of responses and as a range where specific lesions are grouped together. Respondents reported that horses with large intestinal displacements were offered free choice water (63%-65%) within 3 hours (55%-63%), whereas horses with a small intestinal strangulating lesion were offered < 2 L water (64%-74%) 12-24 hours (28%-34%) post-operatively. Horses with a large colon displacement were offered feed within 3 hours of surgery (16%) with the majority offered feed 6-12 hours (35%-36%) post-operatively. Horses with small intestinal strangulating lesions and small colon lesions were offered feed 24-48 hours (34%-42%) after surgery. Following small intestinal, small colon or caecal lesions, horses were re-introduced feed in handfuls (79%-93%) and initially with grass (41%-54%). Horses with large colon displacements were mostly fed handfuls (49%-50%) of forage initially, but a number of respondents would offer larger quantities such as a small bucket (35%-37%) and predominantly of hay (50%-51%).Main limitationsLow response rate. This study did not take into account common post-operative complications that may alter the clinical approach.ConclusionsThis post-operative colic nutrition survey is the first to describe current clinical practice. Further research is required to investigate nutritional strategies in post-operative colic cases.

Project description:ImportanceSuccess on the internal medicine (IM) examination is a central requirement of the American Board of Internal Medicine's (ABIM's) Maintenance of Certification program (MOC). Therefore, it is important to understand the degree to which this examination reflects conditions seen in practice, one dimension of content validity, which focuses on the match between content in the discipline and the topics on the examination questions.ObjectiveTo assess whether the frequency of questions on IM-MOC examinations were concordant with the frequency of conditions seen in practice.Design, setting, and participantsThe 2010-2013 IM-MOC examinations were used to calculate the percentage of questions for 186 medical condition categories from the examination blueprint, which balances examination content by considering importance and frequency of conditions seen in practice. Nationally representative estimates of conditions seen in practice by general internists were estimated from the primary diagnosis for 13 832 office visits (2010-2013 National Ambulatory Medical Care Surveys) and 108 472 hospital stays (2010 National Hospital Discharge Survey).ExposuresPrevalence of conditions included on the IM-MOC examination questions.Main outcomes and measuresThe outcome measure was the concordance between the percentages of IM-MOC examination questions and the percentages of conditions seen in practice during either office visits or hospital stays for each of 186 condition categories (eg, diabetes mellitus, ischemic heart disease, liver disease). The concordance thresholds were 0.5 SD of the weighted mean percentages of the applicable 186 conditions seen in practice (0.74% for office visits; 0.51% for hospital stays). If the absolute differences between the percentages of examination questions and the percentages of office visit conditions or hospital stay conditions seen were less than the applicable concordance threshold, then the condition category was judged to be concordant.ResultsDuring the 2010-2013 IM-MOC examination periods, 3600 questions (180 questions per examination form) were administered and 3461 questions (96.1%) were mapped into the 186 study conditions (mean, 18.6 questions per condition). Comparison of the percentages of 186 categories of medical conditions seen in 13 832 office visits and 108 472 hospital stays with the percentages of 3461 questions on IM-MOC examinations revealed that 2389 examination questions (69.0%; 95% CI, 67.5%-70.6% involving 158 conditions) were categorized as concordant. For concordance between questions and office visits only, 2010 questions (58.08%; 95% CI, 56.43%-59.72% of all examination questions) involving 145 conditions were categorized as concordant. For concordance between questions and hospital stays only, 1456 questions (42.07%; 95% CI, 40.42%-43.71% of all examination questions) involving 122 conditions were categorized as concordant.Conclusions and relevanceAmong questions on IM-MOC examinations from 2010-2013, 69% were concordant with conditions seen in general internal medicine practices, although some areas of discordance were identified.

Project description:BackgroundPeople who are homeless have a higher burden of illness and higher rates of hospital admission and readmission compared to the general population. Identifying the factors associated with hospital readmission could help healthcare providers and policymakers improve post-discharge care for homeless patients.ObjectiveTo identify factors associated with hospital readmission within 90 days of discharge from a general internal medicine unit among patients experiencing homelessness.DesignThis prospective observational study was conducted at an urban academic teaching hospital in Toronto, Canada. Interviewer-administered questionnaires and chart reviews were completed to assess medical, social, processes of care, and hospitalization data. Multivariable logistic regression with backward selection was used to identify factors associated with a subsequent readmission and estimate odds ratios and 95% confidence intervals.ParticipantsAdults (N = 129) who were admitted to the general internal medicine service between November 2017 and November 2018 and who were homeless at the time of admission.Main measuresUnplanned all-cause readmission to the study hospital within 90 days of discharge.Key resultsThirty-five of 129 participants (27.1%) were readmitted within 90 days of discharge. Factors associated with lower odds of readmission included having an active case manager (adjusted odds ratios [aOR]: 0.31, 95% CI, 0.13-0.76), having informal support such as friends and family (aOR: 0.25, 95% CI, 0.08-0.78), and sending a copy of the patient's discharge plan to a primary care physician who had cared for the patient within the last year (aOR: 0.44, 95% CI, 0.17-1.16). A higher number of medications prescribed at discharge was associated with higher odds of readmission (aOR: 1.12, 95% CI, 1.02-1.23).ConclusionInterventions to reduce hospital readmission for people who are homeless should evaluate tailored discharge planning and dedicated resources to support implementation of these plans in the community.
