A case report of cardiac cysticercosis in a returning traveller: a rare cause of myocarditis.

ABSTRACT:

Background

A 38-year-old male of Indian origin presented via ambulance directly to the cardiac catheter lab with chest pain and electrocardiogram changes suggestive of an ST-elevation myocardial infarction. Serum troponin was recorded at >10 000 ng/L.

Case summary

Angiogram revealed normal coronary arteries. Echocardiography showed myocardial lesions and a subsequent cardiac magnetic resonance imaging showed myocardial cysticercosis.

Discussion

This is a case of myocardial cysticercosis causing myocarditis. He was treated successfully with albendazole for Taenia solium infection and non-steroidal anti-inflammatory drugs and colchicine for myocarditis.

Rare case of disseminated cysticercosis and taeniasis in a Japanese traveler after returning from India.

Project description:We report disseminated cysticercosis concurrent with taeniasis in a 31-year-old male Japanese, who had visited India three times and stayed for 1 month each time during the previous 1 year. The patient presented increasing numbers of subcutaneous nodules and expelled proglottids, although numerous cysts were also found in the brain in imaging findings, though no neurological symptoms were observed. Histopathological and serological findings strongly indicated cysticercosis. We found taeniid eggs in his stool by microscopic examination and revealed them as the Indian haplotype of Taenia solium by mitochondrial DNA analysis. We concluded that disseminated cysticercosis was caused by the secondary autoinfection with eggs released from the tapeworm carrier himself. After confirming the absence of adult worms in the intestine by copro-polymerase chain reaction, the patient was successfully treated with albendazole at a dose of 15 mg/kg/day for 28 days. Subcutaneous and intracranial lesions had completely disappeared by the end of the treatment period.

Case report: Primary pericardial angiosarcoma, a rare cause of cardiac tamponade.

Project description:Primary pericardial angiosarcoma is a rare malignancy of the pericardium with variable clinical features and imaging characteristics. Herein, we report a case of histopathologically confirmed pericardial angiosarcoma in a 66-year-old man. The patient developed cardiac tamponade in a short time period. The transthoracic echocardiography showed the presence of multiple irregular echodensities, heterogeneous in echogenicity, encasing the apex of both ventricles in the pericardial space, initially misinterpreted as pericardial effusion. The patient died of cardiogenic shock despite undergoing a surgical pericardiectomy. Pericardial angiosarcoma can manifest as a mass obliterating the pericardial sac, rather than the typical pericardial effusion observed on echocardiography. Multimodality imaging studies aid in diagnosing primary pericardial angiosarcoma, but the final diagnosis relies on tissue histopathology.

Beta 2-microglobulin: case report of a rare cause of cardiac amyloidosis.

Project description:BackgroundCardiac amyloidosis is caused by the deposition of misfolded proteins in the myocardium. The majority of cases of cardiac amyloidosis is caused by misfolded transthyretin or light chain proteins. In this case report, we discuss a case of a rare form of cardiac amyloidosis related to beta 2-microglobulin (B2M) in a patient not on dialysis.Case summaryA 63-year-old man was referred for workup of possible cardiac amyloidosis. Serum and urine immunofixation electrophoresis demonstrated no monoclonal bands, and the serum kappa/lambda light chain ratio was normal, excluding light chain amyloidosis. Bone scintigraphy imaging showed diffuse radiotracer uptake in the myocardium, and genetic testing of the Transthyretin gene was negative for variants. This workup was consistent with wild-type transthyretin cardiac amyloidosis. The patient, however, later underwent endomyocardial biopsy due to factors inconsistent with this diagnosis, including a young age of presentation and a strong family history of cardiac amyloidosis despite no variants in the Transthyretin gene. This showed B2M-type amyloidosis, and genetic testing of the B2M gene showed a heterozygous Pro32Leu (p. P52L) mutation. The patient underwent heart transplantation with normal graft function 2 years post transplant.DiscussionWhile contemporary advancements allow for the non-invasive diagnosis of transthyretin cardiac amyloidosis with positive bone scintigraphy and negative monoclonal protein screen, clinicians should be aware of rarer forms of amyloidosis where endomyocardial biopsy is required to make the diagnosis.

Behcet's disease: a case report about a rare cause of intra-cardiac mass.

Project description:BackgroundIntra-cardiac masses are always a challenging diagnosis, especially when it involves the right side of the heart. There are multiples aetiologies that can be responsible for these masses, namely thrombosis, neoplasm, or vegetations. Occasionally, these may be related to an autoimmune process not yet diagnosed. We present a case of a 17-year-old patient with an exuberant right ventricular mass due to a not yet diagnosed Behçet's disease. The best approach and treatment for these patients remains uncertain.Case summaryThe authors present a case of a 17-year-old patient with a right ventricular mass who presented as an initial manifestation of Behçet's disease. It was firstly assumed as a thrombotic mass and medicated with anticoagulation, with no resolution. After performing a cardiac magnetic resonance, the case was discussed in a multidisciplinary team, including cardiology, paediatrics, and rheumatology, and the diagnosis of Behçet's disease with cardiac complication was established. The patient started immunosuppressive therapy with clinical and echocardiographic response.DiscussionBehçet's disease is a multi-systemic autoimmune vasculitis that usually manifests by recurrent oral and genital ulcers as well as ocular symptoms. Cardiac manifestations are rare but important aspects of the course of the disease, especially in what concerns morbidity burden. The treatment of these cardiovascular complications is generally empirical and involves the treatment of the underlying disease.

Multiple chromoanasynthesis in a rare case of sporadic renal leiomyosarcoma: case report

Project description:Renal leiomyosarcomas (LMS) are extremely rare neoplasms with aggressive behaviour and poor survival prognosis. The most frequent somatic events in leiomyosarcomas are mutations in TP53, RB1, ATRX and PTEN genes, chromosomal instability and chromoanagenesis. By using chromosomal microarray analysis we identified monosomy of chromosomes 3 and 11, gain of Xp (ATRX) arm and three chromoanasynthesis regions (6q21-q27, 7p22.3-p12.1 and 12q13.11-q21.2), with MDM2 and CDK4 oncogenes copy number gains, whereas no CNVs or tumor specific SNVs in TP53, RB1 and PTEN genes were observed.

First case of Plasmodium knowlesi infection in a Japanese traveller returning from Malaysia.

Project description:This is the first case of Plasmodium knowlesi infection in a Japanese traveller returning from Malaysia. In September 2012, a previously healthy 35-year-old Japanese man presented to National Center for Global Health and Medicine in Tokyo with a two-day history of daily fever, mild headaches and mild arthralgia. Malaria parasites were found in the Giemsa-stained thin blood smear, which showed band forms similar to Plasmodium malariae. Although a nested PCR showed the amplification of the primer of Plasmodium vivax and Plasmodium knowlesi, he was finally diagnosed with P. knowlesi mono-infection by DNA sequencing. He was treated with mefloquine, and recovered without any complications. DNA sequencing of the PCR products is indispensable to confirm P. knowlesi infection, however there is limited access to DNA sequencing procedures in endemic areas. The extent of P. knowlesi transmission in Asia has not been clearly defined. There is limited availability of diagnostic tests and routine surveillance system for reporting an accurate diagnosis in the Asian endemic regions. Thus, reporting accurately diagnosed cases of P. knowlesi infection in travellers would be important for assessing the true nature of this emerging human infection.

Fulminant cardiac sarcoidosis resembling giant cell myocarditis: a case report.

Project description:BackgroundSevere cardiac sarcoidosis (CS) can share clinical and histopathologic features with giant cell myocarditis (GCM).Case summaryA 56-year-old female presented with 1 week of exertional chest pressure and dyspnoea. Echocardiogram demonstrated extensive regional dysfunction with left ventricular ejection fraction (LVEF) 38%. Cardiac catheterization revealed no obstructive coronary artery disease and cardiac index 1.5 L/min/m2. Cardiac magnetic resonance imaging (MRI) demonstrated diffuse late gadolinium enhancement. Positron emission tomography with fluorodeoxyglucose (FDG) (FDG-PET) computed tomography showed FDG uptake in the anteroseptal and anterior wall and no extracardiac activity. Endomyocardial biopsy (EMB) demonstrated fragments of endocardial fibrosis with mixed inflammatory infiltrate including histiocytic giant cells, which could be due to CS or GCM. She was initially treated for GCM with high dose steroids, tacrolimus, and mycophenolate mofetil. Repeat EMB was pursued and demonstrated multiple granulomas with sharp demarcation from adjacent uninvolved myocardium consistent with CS. A dual-chamber implantable cardioverter-defibrillator was placed, and immunosuppression was changed to prednisone alone with plan for infliximab.DiscussionThis case illustrates a rare presentation of fulminant isolated CS. Endomyocardial biopsy with sufficient tissue was critical to establish a diagnosis and initiate appropriate immunosuppression.

Its rarity makes the diagnosis challenging, especially when no other organ is involved, and the endoscopic findings may resemble malignancy.Case presentationWe report a unique case of a 37-year-old woman who presented with dysphagia secondary to esophageal TB with an endoscopic appearance of a submucosal mass resembling malignancy.ConclusionEsophageal TB is a rare cause of dysphagia, especially in a western setting. It should always be considered as a potential etiology in patients with dysphagia.
