Adaptation and Validation of the COVID-19 Stigma Instrument in Nurses:A Cross-sectional Survey.

ABSTRACT: Background: Stigma is a prominent issue among nurses working with patients with infectious diseases, but the unavailability of validated measures of such stigma. The aim of our study was to adapt, modify, and validate the COVID-19 Stigma Instrument-Nurse -Version 3 (CSI-N-3) with both classical test theory and item response theory (IRT) analysis. Methods: We administered the scale to 249 eligible nurses who worked in a COVID-19 designed hospital in Shanghai, China. Results: The two-factor structure was confirmed by confirmatory factor analysis. The 15-item CSI-N-3 achieved Cronbach's α of 0.64 to 0.84. Convergent validity was also demonstrated. In IRT analysis, the CSI-N-3 has ordered response thresholds, with the appropriate item difficulty and infit and outfit mean squares. Self-reported social support was the only factor influencing nurses' COVID-19 stigma (standardized coefficients β =-0.21). Conclusions: The CSI-N-3 is an instrument with sound psychometric properties that can be used to measure COVID-19 stigma during the COVID-19 outbreak or afterward among nurses.

Project description:Aims and objectivesThis study aimed to portray the prevalence and associated factors of psychological distress among frontline nurses during COVID-19 outbreak.BackgroundThe COVID-19 outbreak has posed great threat to public health worldwide. Nurses fighting against the epidemic on the frontline might be under great physical and psychological distress. This psychological distress was predominantly described as sleep disturbance, symptoms of anxiety and depression, post-traumatic stress, inability to make decisions and even somatic symptoms.DesignCross-sectional study.MethodsFrontline nurses from designated hospitals for COVID-19 patients were invited to complete an online survey by convenience sampling, and the survey included six main sections: the General Health Questionnaire, the Perceived Social Support Scale, the Simplified Coping Style Scale, the Impact of Event Scale-Revised, socio-demographic, occupation and work history. Multiple logistic analysis was used to identify the potential risk factors of psychological distress. The study methods were compliant with the STROBE checklist.ResultsOf the 263 frontline nurses, 66 (25.1%) were identified as psychological distress. Multiple logistic analysis revealed that working in emergency department, concern for family, being treated differently, negative coping style and COVID-19-related stress symptom were positive related to psychological distress. Perceived more social support and effective precautionary measures were negatively associated with psychological distress.ConclusionsThe study demonstrated that COVID-19 had a significant psychological impact on frontline nurses. Early detection of psychological distress and supportive intervention should be taken according to the associated factors to prevent more serious psychological impact on frontline nurses.Relevance to clinical practiceThis study highlighted that the frontline nurses were suffering from varying degrees of psychological distress, which needed early screening and supportive intervention for preventing more serious psychological impact on frontline nurses. Beside, more specific measurement should be combined with the GHQ-12 to assess the varying degrees of psychological distress in frontline nurses.

Project description:BackgroundResearch predominantly suggests that nurses are at high risk of developing psychopathology. The empirical data show that the occurrence rate of problem-related sleep quality among clinical nurses is high. Therefore, this study was conducted to address the lack of information on the relationship between the coronavirus disease 2019 (COVID-19) pandemic and insomnia.MethodsA convenience sample of nurses (n = 680) completed an online survey that included the Insomnia severity index, the COVID-19-related psychological distress scale, the general health questionnaire, neuroticism, dysfunctional beliefs, attitudes about sleep scale, and difficulties in emotion regulation scale.ResultsThe results showed that 35.8% (n = 253) of nurses were classified as individuals with moderate to severe clinical insomnia. The results showed that the psychological distress generated by COVID-19 predicted insomnia (β = .47, SE = 0.02, P < .001, t = 13.27, 95% CI 0.31-0.46). Additionally, the association is mediated by psychopathology vulnerabilities, emotion dysregulation, dysfunctional beliefs about sleep, and neuroticism. Moreover, female nurses exhibited higher levels of insomnia (Cohen's d = .37), neuroticism (Cohen's d = 30), psychopathology vulnerability (Cohen's d = .26), and COVID-19-related psychological distress (Cohen's d = .23).ConclusionThe present study's findings help to explain how pandemic consequences can be associated with insomnia. Additionally, the findings make a significant contribution to better understanding the role of neuroticism, emotion dysregulation, beliefs, and psychopathology vulnerability in the development of insomnia among nurses. The findings suggest the potential influence of cognitive behavioral therapy for insomnia (CBT-I) and transdiagnostic integrated therapies that could be incorporated into therapeutic programs designed to develop as a way of inhibiting or preventing insomnia among clinical nurses.
