Functional Movement Screen Task Scores and Joint Range-of-motion: A Construct Validity Study.

ABSTRACT: Little is known about the construct validity of the Functional Movement Screen (FMS). We aimed to assess associations between FMS task scores and measures of maximum joint range-of-motion (ROM) among university varsity student-athletes from 4 sports (volleyball, basketball, ice hockey, and soccer). Athletes performed FMS tasks and had their maximum ankle, hip and shoulder ROM measured. Multivariable linear regression was used to estimate associations between FMS task scores and ROM measurements. 101 university student-athletes were recruited (52 W/49 M; mean age 20.4±1.9 years). In general, athletes with higher FMS task scores had greater ROM compared to those with lower task scores. For example, athletes who scored 2 on the FMS squat task had 4° (95% CI, 1° to 7°) more uni-articular ankle dorsiflexion ROM compared with those who scored 1, while those who scored 3 on the FMS squat task had 10° (4° to 17°) more uni-articular ankle dorsiflexion ROM compared with those who scored 1. Large variation in ROM measurements was observed. In sum, substantial overlap in joint ROM between groups of athletes with different FMS task scores weakens the construct validity of the FMS as an indicator of specific joint ROM.

Project description:BackgroundEarly detection of cognitive impairment is among the top research priorities aimed at reducing the global burden of dementia. Currently used screening tools have high sensitivity but lack specificity at their original cut-off, while decreasing the cut-off was repeatedly shown to improve specificity, but at the cost of lower sensitivity. In 2012, a new screening tool was introduced that aims to overcome these limitations - the Quick mild cognitive impairment screen (Qmci). The original English Qmci has been rigorously validated and demonstrated high diagnostic accuracy with both good sensitivity and specificity. We aimed to determine the optimal cut-off value for the German Qmci, and evaluate its diagnostic accuracy, reliability (internal consistency) and construct validity.MethodsWe retrospectively analyzed data from healthy older adults (HOA; n = 43) and individuals who have a clinical diagnosis of 'mild neurocognitive disorder' (mNCD; n = 37) with a biomarker supported characterization of the etiology of mNCD of three studies of the 'Brain-IT' project. Using Youden's Index, we calculated the optimal cut-off score to distinguish between HOA and mNCD. Receiver operating characteristic (ROC) curve analysis was performed to evaluate diagnostic accuracy based on the area under the curve (AUC). Sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) were calculated. Reliability (internal consistency) was analyzed by calculating Cronbach's α. Construct validity was assessed by analyzing convergent validity between Qmci-G subdomain scores and reference assessments measuring the same neurocognitive domain.ResultsThe optimal cut-off score for the Qmci-G was ≤ 67 (AUC = 0.96). This provided a sensitivity of 91.9% and a specificity of 90.7%. The PPV and NPV were 89.5% and 92.9%, respectively. Cronbach's α of the Qmci-G was 0.71 (CI95% [0.65 to 0.78]). The Qmci-G demonstrated good construct validity for subtests measuring learning and memory. Subtests that measure executive functioning and/or visuo-spatial skills showed mixed findings and/or did not correlate as strongly as expected with reference assessments.ConclusionOur findings corroborate the existing evidence of the Qmci's good diagnostic accuracy, reliability, and construct validity. Additionally, the Qmci shows potential in resolving the limitations of commonly used screening tools, such as the Montreal Cognitive Assessment. To verify these findings for the Qmci-G, testing in clinical environments and/or primary health care and direct comparisons with standard screening tools utilized in these settings are warranted.

Project description:BackgroundSoccer is the world's most popular sport for both men and women. Tests of athletic and functional performance are commonly used to assess physical ability and set performance goals. The Functional Movement Screen (FMS™) is a widely used seven-test battery developed by practitioners to provide interpretable measure of movement quality. The main objective of the present study was twofold, to analyze the relationship between FMS™ results from male and female soccer players and to compare their physical performance in different tests.MethodsA total of twenty-eight semi-professional soccer players: fourteen male (age: 21.29 ± 1.64 years; weight: 70.66 ± 5.29 kg; height: 171.86 ± 5.35 cm; BMI: 20.90 ± 2.22 kg/m2) and fourteen females (age: 20.64 ± 1.98 years; weight: 63.44 ± 5.83 kg; height: 166.21 ± 12.18 cm; BMI: 23.02 ± 2.50 kg/m2) were recruited for this study. A paired sample t-test was used for determining differences as a repeated measures analysis. All the participants conducted the following tests: The Functional Movement Test (FMS™), 10-m linear sprint, 5-0-5 COD Test and Yo-Yo Intermittent Recovery Test-Level 1 (YYIRT Level 1).ResultsA t-test with data from 505 COD (change of direction) test showed significant differences between groups, p = 0.001, d = 1.11, revealing faster times in male soccer players (2.50 ± 0.19) in respect with female soccer players (2.70 ± 0.17). Crucially, a t-test with data from FMS did not reveal significant differences between groups. Multiple regression for V02max revealed significant effects (r = 0.55, r2 = 0.30, adjusted r2 = 0.24, F = 5.21, p = 0.04 and standard error = 2.20). On the other hand, multiple regression for 10-m sprint showed significant effects (r = 0.58, r2 = 0.33, adjusted r2 = 0.28, F = 5.98, p = 0.03). The impact of these factors on the correlation between FMS™ scores and physical performance measures can vary among individuals.Discussion/conclusionThis study demonstrates the necessity of utilizing and applying multiple field-based tests to evaluate the movement and capabilities of physical performance in sports. Crucially, consider individual variations and factors such as training background, fitness level, and sport-specific demands when interpreting the relationship between the FMS™ and physical performance in both sexes.

Project description:BackgroundDry needling (DN) has been established as an effective treatment for myofascial pain, however, there are no studies thus far investigating the benefit to movement and motor control.PurposeThe primary purpose of this study was to compare differences in a series of outcomes between dry needling, dry needling and stretching, and stretching only in a sample of healthy males. A secondary purpose was to compare change over time.DesignBlinded, randomized controlled trial.MethodsThirty healthy male subjects were randomly assigned to one of three intervention groups: DN, stretching, or combination DN +stretching. Subjects in the DN group and DN+stretch group received DN to a palpated trigger point (TrP) in the triceps surae to elicit local twitch response. Subjects in the stretch group and DN+stretch group were instructed in a home stretching program for gastrocnemius and soleus muscles. All groups were tested for dorsiflexion range of motion and performed functional tasks (overhead deep squat, and Y-Balance test, Lower Quarter) prior to intervention, directly after intervention, and four days post intervention. Group comparisons were performed using a repeated measure Analysis of Variance and a partial eta squared calculation for effect size. For all measures a p-value of < 0.05 was used to determine significance. Cohen's criteria were used to categorize strength of effect size.ResultsThere were no statistically significant differences among groups for range of motion nor functional measures, with the exception of the deep squat. Proportionally, the DN group improved significantly in deep squat performance (p<0.01) compared to the other groups. Time oriented improvements were seen for the YBT posterior-lateral reach (p = 0.02) only. Between groups effect sizes ranged from 0.02 (small) to 0.17 (large).ConclusionsIncluding DN did not markedly influence range of motion nor functional assessment measures, excluding those seen during the overhead deep squat. Effect measures suggest the lack of significant findings may be an issue of statistical power.Level of evidence1b.
