Enhancing Data Integration, Interoperability, and Reuse to Address Complex and Emerging Environmental Health Problems.

ABSTRACT: Environmental health sciences (EHS) span many diverse disciplines. Within the EHS community, the National Institute of Environmental Health Sciences Superfund Research Program (SRP) funds multidisciplinary research aimed to address pressing and complex issues on how people are exposed to hazardous substances and their related health consequences with the goal of identifying strategies to reduce exposures and protect human health. While disentangling the interrelationships that contribute to environmental exposures and their effects on human health over the course of life remains difficult, advances in data science and data sharing offer a path forward to explore data across disciplines to reveal new insights. Multidisciplinary SRP-funded teams are well-positioned to examine how to best integrate EHS data across diverse research domains to address multifaceted environmental health problems. As such, SRP supported collaborative research projects designed to foster and enhance the interoperability and reuse of diverse and complex data streams. This perspective synthesizes those experiences as a landscape view of the challenges identified while working to increase the FAIR-ness (Findable, Accessible, Interoperable, and Reusable) of EHS data and opportunities to address them.

Project description:Climate change is a global challenge, and the Brazilian Amazon Forest is a particular concern due to the possibility of reaching a tipping point that could amplify environmental crises. Despite many studies on the Amazon Forest, this research was conducted in Manaus, the capital of Amazonas state, to address five gaps, including the lack of local citizen consultation on urban environmental issues, Smart Cities, decarbonization, and disruptive technologies. This study holds significance for the academy community, government bodies, policymakers, and investors, as it offers novel insights into the Amazon region and proposes a model to engage citizens in Smart Cities. This model could also guide other municipalities aspiring for participatory sustainable development with a decarbonization focus, mitigating future risks, and protecting future generations. Basically, it is an explanatory and applied study that employs mixed methods, including literature, bibliometric and documentary reviews, two questionnaires, and descriptive statistical approaches, organized in four phases to reach the following goals: (a) provide information on the main challenges facing humanity, the Brazilian Amazon state, and the city of Manaus; (b) identify the best Smart City approaches for engaging citizens in solving urban problems; (c) contextualize and consult Manaus City Hall about the effectiveness of the Smart City project; (d) investigate the perceptions of citizens living in Manaus on the main city's environmental problems, as well as their level of knowledge and interest on issues related to Smart Cities, decarbonization, and disruptive technologies; (e) propose a participatory Smart City model with recommendations. Among the result, the study found that the term "Smart City" dominates scholarly publications among nineteen urban-related terms, and the five main environmental problems in Manaus are an increase in stream pollution, garbage accumulation, insufficient urban afforestation, air pollution, and traffic congestion. Although citizens are willing to help, the majority lack knowledge on Smart City and Decarbonized City issues, but there is a considerable interest in training related to these issues, as well as disruptive technologies. It was found that Amsterdam, Melbourne, Montreal, San Francisco, Seoul, and Taipei all have a formal model to engage citizens in solving their urban problems. The main conclusion is that, after 6 years, the Smart City Project in Manaus is a political fallacy, as no model, especially with a citizen participatory approach, has been effectively adopted. In addition, after conducting a literature and documentary review and analyzing 25 benchmark Smart Cities, the P5 model and the Citizen Engagement Kit model are proposed with 120 approaches and guidelines for addressing the main environmental problems by including Manaus' citizens in the Smart City and/or decarbonization journey.

Project description:BackgroundDespite the many opportunities data reuse offers, its implementation presents many difficulties, and raw data cannot be reused directly. Information is not always directly available in the source database and needs to be computed afterwards with raw data for defining an algorithm.ObjectiveThe main purpose of this article is to present a standardized description of the steps and transformations required during the feature extraction process when conducting retrospective observational studies. A secondary objective is to identify how the features could be stored in the schema of a data warehouse.MethodsThis study involved the following 3 main steps: (1) the collection of relevant study cases related to feature extraction and based on the automatic and secondary use of data; (2) the standardized description of raw data, steps, and transformations, which were common to the study cases; and (3) the identification of an appropriate table to store the features in the Observation Medical Outcomes Partnership (OMOP) common data model (CDM).ResultsWe interviewed 10 researchers from 3 French university hospitals and a national institution, who were involved in 8 retrospective and observational studies. Based on these studies, 2 states (track and feature) and 2 transformations (track definition and track aggregation) emerged. "Track" is a time-dependent signal or period of interest, defined by a statistical unit, a value, and 2 milestones (a start event and an end event). "Feature" is time-independent high-level information with dimensionality identical to the statistical unit of the study, defined by a label and a value. The time dimension has become implicit in the value or name of the variable. We propose the 2 tables "TRACK" and "FEATURE" to store variables obtained in feature extraction and extend the OMOP CDM.ConclusionsWe propose a standardized description of the feature extraction process. The process combined the 2 steps of track definition and track aggregation. By dividing the feature extraction into these 2 steps, difficulty was managed during track definition. The standardization of tracks requires great expertise with regard to the data, but allows the application of an infinite number of complex transformations. On the contrary, track aggregation is a very simple operation with a finite number of possibilities. A complete description of these steps could enhance the reproducibility of retrospective studies.

Project description:BackgroundSince the emergence of electronic health records, nursing information is increasingly being recorded and stored digitally. Several studies have shown that a wide range of nursing information is not interoperable and cannot be re-used in different health contexts. Difficulties arise when nurses share information with others involved in the delivery of nursing care. The aim of this study is to develop a nursing subset of patient problems that are prevalent in nursing practice, based on the SNOMED CT terminology to assist in the exchange and comparability of nursing information.MethodsExplorative qualitative focus groups were used to collect data. Mixed focus groups were defined. Additionally, a nursing researcher and a nursing expert with knowledge of terminologies and a terminologist participated in each focus group. The participants, who work in a range of practical contexts, discussed and reviewed patient problems from various perspectives.ResultsSixty-seven participants divided over seven focus groups selected and defined 119 patient problems. Each patient problem could be documented and coded with a current status or an at-risk status. Sixty-six percent of the patient problems included are covered by the definitions established by the International Classification of Nursing Practice, the reference terminology for nursing practice. For the remainder, definitions from either an official national guideline or a classification were used. Each of the 119 patient problems has a unique SNOMED CT identifier.ConclusionsTo support the interoperability of nursing information, a national nursing subset of patient problems based on a terminology (SNOMED CT) has been developed. Using unambiguously defined patient problems is beneficial for clinical nursing practice, because nurses can then compare and exchange information from different settings. A key strength of this study is that nurses were extensively involved in the development process. Further research is required to link or associate nursing patient problems to concepts from a nursing classification with the same meaning.
