ABSTRACT: This dataset includes data from febrile patients recruited for a large hospital-based study in Sri Lanka from 2016 to 2019. The variables include primary socio-demographic data, exposure data, clinical data, biochemical and investigation data. Some of these data are available as serial data from admission to discharge daily. Microscopic agglutination test, quantitative PCR of whole blood, urine and serum and culture isolation was performed to diagnose the patients with leptospirosis.

Project description:Leptospirosis is considered a neglected tropical disease despite its considerable mortality and morbidity. Lack of prediction remains a major reason for underestimating the disease. Although many models have been developed, most of them focused on the districts situated in the wet zone due to higher case numbers in that region. However, leptospirosis remains a major disease even in the dry zone of Sri Lanka. The objective of this study is to develop a time series model to predict leptospirosis in the Anuradhapura district situated in the dry zone of Sri Lanka. Time series data on monthly leptospirosis incidences from January 2008 to December 2018 and monthly rainfall, rainy days, temperature, and relative humidity were considered in model fitting. The first 72 months (55%) were used to fit the model, and the subsequent 60 months(45%) were used to validate the model. The log-transformed dependent variable was employed for fitting the Univariate seasonal ARIMA model. Based on the stationarity of the mean of the five variables, the ARDL model was selected as the multivariate time series technique. Residuals analysis was performed on normality, heteroskedasticity, and serial correlation to validate the model. The lowest AIC and MAPE were used to select the best model. Univariate models could not be fitted without adjusting the outliers. Adjusting seasonal outliers yielded better results than the models without adjustments. Best fitted Univariate model was ARIMA(1,0,0)(0,1,1)12,(AIC-1.08, MAPE-19.8). Best fitted ARDL model was ARDL(1, 3, 2, 1, 0),(AIC-2.04,MAPE-30.4). The number of patients reported in the previous month, rainfall, rainy days, and temperature showed a positive association, while relative humidity was negatively associated with leptospirosis. Multivariate models fitted better than univariate models for the original data. Best-fitted models indicate the necessity of including other explanatory variables such as patient, host, and epidemiological factors to yield better results.

Project description:BackgroundTrichomonas vaginalis infection is underreported due to nonspecific clinical presentation and the nonavailability of sensitive laboratory diagnostic tests at the clinical setup. Hence, this study was designed to compare the sensitivity and specificity of microscopy and culture methods with polymerase chain reaction (PCR). The socio-demographic factors associated with the infection were explored.MethodsThe study was carried out at the National Sexually Transmitted Diseases and Acquired Immuno Deficiency Syndrome Control Programme in Colombo and Sexually Transmitted Diseases and Acquired Immuno Deficiency Syndrome Control Programme in Kandy. Samples were collected from a total of 385 patients including, 272 females (70.7%) and 113 males (29.3%), and tested using microscopy (wet mount and Giemsa staining), culture, and PCR. Genus-specific primer set (TFR1/TFR2) that amplifies 5.8S rRNA and species-specific primer sets (TV16Sf-2/TV16Sr-2 and TVK3/7) that amplifies 18S rRNA and repetitive DNA, respectively, were used. Patient's socio-demographic and sexual behaviour data were obtained using a standard interviewer-administered questionnaire. Data were analyzed with R statistical software Version 3.6.3.ResultsThe overall prevalence of trichomoniasis was 4.4% (17/385). Of these, six (1.6%) were positive for microscopic examination, 7 (1.8%) were positive for culture, and 13 (3.4%) for TVK3/7, 15 (3.9%) for TV16Sf/r, and TFR1/2 17 (4.4%) were positive for PCR. Sensitivities of PCR using TFR1/2, TV16Sf/r, and TVK3/7 primer sets were 100%, 88.20%, and 76.50%, respectively, against the expanded gold standard. Trichomoniasis was associated with age above 36 (p = 0.033), not using condoms in last three months (p = 0.016), multiple sex partners (p = 0.001), reason for attendance (p = 0.027), symptomatic nature (p = 0.015), and the presence of other sexually transmitted diseases (p = 0.001).ConclusionsThe study highlighted that age over 36 years, multiple sex partners, not using condoms, reason for attendance, symptomatic nature, and having other sexually transmitted diseases can increase the risk of acquiring trichomoniasis. Furthermore, this study confirmed PCR as highly sensitive and specific diagnostic test for the diagnosis of trichomoniasis in comparison to microscopy and culture methods.

Project description:IntroductionAcute febrile illness (AFI) is a common cause of hospital admissions in tropical settings. Identifying AFI aetiology is essential for guiding clinicians' diagnoses and developing diagnostic and management guidelines. We used rigorous, gold-standard testing for diverse viral and bacterial pathogens to confirm the aetiology of AFI in southern Sri Lanka.MethodsWe prospectively enrolled children and adults with AFI admitted to Teaching Hospital Karapitiya, Galle, the largest tertiary care hospital in Southern Province, Sri Lanka from June 2012 to May 2013. We obtained sociodemographic and clinical data, an acute blood sample, a nasopharyngeal sample, and a urine sample at enrolment and a convalescent blood sample 2-4 weeks later. Laboratory testing was conducted for dengue, respiratory viruses, leptospirosis, scrub typhus, spotted fever group (SFG) and typhus group (TG) rickettsioses and Q fever.ResultsA total of 976 patients were enrolled and a convalescent visit was completed in 878 (90.0%). Median age was 26.9 (14.2-41.4) years and the majority were male (628, 64.3%). A viral or bacterial aetiology was identified in 660 (67.6%). A viral aetiology was identified in 534 (54.7%), including 388 (39.8%) with dengue and 171 (17.5%) with respiratory viruses. Bacterial infection was found in 138 (14.1%) and included leptospirosis (79, 8.1%), SFG (17, 1.7%), TG (7, 0.7%), scrub typhus (53, 5.4%) and Q fever (5, 0.5%). Antibiotics were prescribed at enrolment for 45.5% with viral infections and 62.3% with bacterial infection. Overall, sensitivity of clinical diagnosis was low at approximately 50%.ConclusionWe identified an aetiology of AFI in two-thirds of patients in a setting where malaria is non-endemic. Sensitivity of clinical diagnosis was low, with overuse of antibiotics for viral infections and underuse of antibiotics for bacterial infections. Diagnostic algorithms for AFI may help improve clinical management in this and comparable settings with diverse AFI aetiologies.
