This article deepens in the differences in educational development between the Ecuadorian provinces and in their evolution over time by estimating the Provincial-level Education Index. This index is built using the micro-databases of the two latest rounds of the Ecuador`s Living Standards Measurement Survey (2005-2006 and 2013-2014). Project description:Based on the Office for National Statistics' delineation of the scope of the digital economy industry, this paper selects indicators from five industrial dimensions: digital product manufacturing, digital product service, digital technology application, digital factor drive and digital efficiency improvement, and constructs an evaluation system to measure the development level of China's digital economy at the provincial level. It is found that there is a wide gap in the development of China's provincial digital economy, with the eastern coastal provinces and cities having a high level of digital economy development. The coupling and coordination model was then applied to examine the interrelationships between the five industrial dimensions of the digital economy, and it was found that most of the coupling and coordination relationships of the five industrial dimensions are at the stage of medium-high coupling and low coupling and coordination, and each province and city has different coupling and coordination characteristics. The numerical evaluation results provide an intuitive understanding of the differences and deficiencies in the development of the digital economy in different regions, and serve as a reference for the medium and long-term digital economy development planning of provinces and municipalities as well as the whole country. In the future, the state should invest more in the digital economy in the central and western regions, and each province should cultivate and develop the digital economy in accordance with its own local conditions.

Project description:IntroductionThe continuum of care is a recommended framework for comprehensive health service delivery for maternal health, and it integrates health system and social determinants of health. There is a current lack of knowledge on a measurement approach to monitor performance on the framework. In this study we aim to develop and test a composite index for assessing the maternal health continuum in a province in South Africa with the possibility of nationwide use.Materials and methodsThe composite index was computed as a geometric mean of four dimensions of adequacy of the continuum of care. Data was sourced from the district health information system, household surveys and the census. The index formula was tested for robustness when alternative inputs for indicators and standardization methods were used. The index was used to assess performance in service delivery in the North West province of South Africa, as well as its four districts over a five-year period (2013-2017). The index was validated by assessing associations with maternal health and other outcomes. And factor analysis was used to assess the statistical dimensions of the index.ResultsThe provincial level index score increased from 62.3 in 2013 to 74 in 2017, showing general improvement in service delivery over time. The district level scores also improved over time, and our analysis identified areas for performance improvement. These include social determinants of health in some districts, and access and linkages to care in others. The provincial index was correlated with institutional maternal mortality rates (rs = -0.90, 90% CI = (-1.00, -0.25)) and the Human Development Index (r = 0.97, 95% CI = (0.63, 0.99). It was robust to alternative approaches including z-score standardization of indicators. Factor analysis showed three groupings of indicators for the health system and social determinants of health.ConclusionsThis study demonstrated the development and testing of a composite index to monitor and assess service delivery on the continuum of care for maternal health. The index was shown to be robust and valid, and identified potential areas for service improvement. A contextualised version can be tested in other settings within and outside of South Africa.

Project description:Despite being one of the most common measures of development, the Human Development Index [HDI] has been much criticized for its consistency, data requirements, difficulty of interpretation and trade-offs between indicators. The 'Human Life Indicator' [HLI] has been proposed as a 'simple effective means' of measuring development and, more specifically, as a viable alternative to the HDI. Reducing inequalities within countries is a core component of the Sustainable Development Goals; yet sub-national HDIs are subject to the same criticisms as national level indices (potentially more so). Our goal in this paper is to demonstrate 'proof of concept' in terms of the systematic application of the HLI to measure development at the subnational level. Using life tables for the United States of America, we calculate, for the first time, HLIs for each state for the period 1959-2016. This country was chosen for the comparatively long run of available sub-national life tables. We also calculate the extent to which mortality is distributed across the life course-a further measure of inequality and the role of the social determinants of health. The HLI clearly shows how striking regional inequalities exist across the United States. We find that HLI and HDI for the most recent time period are strongly correlated. The analysis demonstrates that HLI represents an effective means of measuring development at the sub-national level. Compared to HDI, HLIs are characterized by simpler calculation and interpretation; fewer data requirements; less measurement error; more consistency over time; and no trade-offs between components. A current challenge of producing sub-national HLIs is the lack of comprehensive civil registration and vital statistics systems in many parts of the Global South from which sub-national life tables can be generated. However, as more and more countries develop these systems the potential to produce HLIs will inevitably increase.

Project description:The Sustainable Development Goals (SDGs) represents the first globally agreed framework to address human development and environmental stewardship in an integrated way. One approach to summarising national SDG status is our "barometer for inclusive sustainable development in South Africa". The barometer downscales global social and planetary boundaries to provide status and trends for 20 critical indicators of environmental stress and social deprivation. In this paper, we explore the sub-national heterogeneity in sustainable development indicators by creating barometers defining the 'safe and just operating space' for South Africa's nine provinces. Our results show that environmental stress varies significantly and provinces need to focus on quite different issues. Although generally environmental stress is increasing, there are areas where it is decreasing, most notably, marine harvesting. Social deprivation results show more of a pattern with high levels of deprivation in employment, income and safety across the provinces, and historically disadvantaged provinces showing the most deprivation overall. Although deprivation is generally decreasing, there are notable exceptions such as food security in six provinces. Our provincial barometers and trend plots are novel in that they present comparable environmental and social data on key indicators over time for all South Africa's provinces. They are visual tools that communicate the range of key challenges and risks that provincial governments face, and are non-specialist and accessible to a range of audiences. In addition, the paper provides a critical case study of spatial disaggregation of national data that is required for the SDGs implementation.

Project description:High-income countries often outsource material demands to poorer countries along with the associated environmental damage. This phenomenon can also occur within (large) countries, such as China, which was responsible for 24 to 30% of the global material footprint (MF) between 2007 and 2010. Understanding the distribution and development of China's MF is hence critical for resource efficiency and circular economy ambitions globally. Here we present a comprehensive analysis of China's MF at the provincial and sectoral levels. We combine provincial-level input-output data with sector- and province-specific trade data, detailed material extraction data, and the global input-output database EXIOBASE. We find that some provinces have MFs equivalent to medium-sized, high-income countries and limited evidence of material decoupling. Lower-income regions with high levels of material extraction can have an MF per capita as large as developed provinces due to much higher material intensities. The higher-income south-coastal provinces have lower MF per capita than equally developed provinces. This finding relates partly to differences in economic structure but indicates the potential for improvement across provinces. Investment via capital formation is up to 4 times more resource-intensive than consumption and drives 49 to 86% of provincial-level MFs (the Organisation for Economic Co-operation and Development average is 37%). Resource-efficient production, efficient use of capital goods/infrastructure, and circular design are essential for reductions in China's MF. Policy efforts to shift to a high-quality development model may reduce material intensities, preferably while avoiding the further outsourcing of high-intensity activities to other provinces or lower-income countries.

Project description:BackgroundA universal set of disability weights(DWs) is mainly based on the survey of North America, Australia and Europe, whereas the participants in Asia was limited. The debate hasn't yet settled whether a universal DW is desirable or useful.The focus of the debate is its representativenes-s.After all, the DWs come from people's subjective evaluation of pain, and they may vary according to cultural background.The differences of the DWs could have implications for the magnitude or ranking of disease burdens.The DWs of Anhui Province has not been completely presented.This paper aims to obtain the DWs suitable for the general population of Anhui Province of China, and attempts to explore the differences between different DWs by comparing the DWs with the similar-cultural background and the DWs with cross-cultural background.MethodsA web-based survey was conducted to estimate the DWs for 206 health states of Anhui province in 2020. Paired comparison (PC) data were analyzed and anchored by probit regression and fitting loess model. We compared the DWs in Anhui with other provinces in China and those in Global burden of disease (GBD) and Japan.ResultsCompared with Anhui province, the proportion of health states which showed 2 times or more differences ranged from 1.94% (Henan) to 11.17% (Sichuan) in China and domestic provinces. It was 19.88% in Japan and 21.51% in GBD 2013 respectively. In Asian countries or regions, most of the health states with top 15 DWs belonged to the category of mental, behavioral, and substance use disorders. But in GBD, most were infectious diseases and cancer. The differences of DWs in neighboring provinces were smaller than other geographically distant provinces or countries.ConclusionPC responses were largely consistent across very distinct settings,but the exceptions do need to be faced squarely.The differences of DWs among similar-cultural regions were smaller than cross-cultural regions. There is an urgent need for relevant gold standards.

Project description:Improving electrification feasibility is essential for reducing emissions from non-electric energy sources, thereby enhancing air quality and public health. Concurrently, climate mitigation actions, such as carbon pricing policies, have significant potential to alleviate increasing carbon dioxide (CO2) and other co-emitted air pollutants. However, the interactions between climate policy and the improvement of electrification feasibility at the provincial level remain unclear, collectively impacting the net-zero transition of energy-intensive sectors. Here we combine a technologically rich economic-energy-environment model with air quality modeling across China to examine the health, climate, and economic implications of large-scale upgrades in electrification feasibility and climate policies from 2017 to 2030. The results indicate that advancing electrification feasibility, coupled with adopting carbon pricing policies, is likely to facilitate a transition towards electricity-dominant energy systems. Improved electrification feasibility is projected to yield a 7-25% increase in nationwide climate benefits and a 5-14% increase in health benefits by 2030. These incremental benefits, coupled with reduced economic costs, result in a 22-68% increase in net benefits. However, regionally, improvements in electrification feasibility will lead to heightened power demand and unintended emissions from electric energy production in certain provinces (e.g., Nei Mongol) due to the coal-dominated power system. Additionally, in major coal-producing provinces like Shanxi and Shaanxi, enhanced electrification feasibility exacerbates the negative economic impacts of climate policies. This study provides quantitative insights into how improving electrification feasibility reshapes energy evolution and the benefit-cost profile of climate policy at the provincial level. The findings underscore the necessity of a well-designed compensation scheme between affected and unaffected provinces and coordinated emission mitigation across the power and other end-use sectors.

Project description:The development of the eco-economy has become an important way to promote sustainable development and address climate change worldwide. Implementing eco-economic developmental policy globally or locally requires establishing precise indicators. Currently, there are many studies on eco-economy indicators at the academic level, but the eco-economy indicators researched at the academic level are difficult to be implemented and applied by local governments in China, and there is a knowledge gap between the political sector and the academic sector in the process of cooperation. This mainly stems from the lack of whole-process research and analysis that combines government practice and academic research. We attempt to analyze the differences in the understanding of eco-economic indicators between academics and government decision-makers through the study of the establishment process of China's local eco-economic indicator system. We try to find out the reasons for the knowledge gap between academics and government decision-makers, and to build a knowledge bridge between government practice and academic research. At the same time, China, as the largest developing country and an emerging country in the construction of ecological civilization, is worth studying and learning from its experience in the construction of eco-economic indicators. Therefore, we systematically study the connotation of China's eco-economy and the development process of the indicators. And we combine with the practical experience, describe the method and specific process of constructing eco-economy indicators at the provincial scale of the Chinese government. Meanwhile, we put forward the limitations of the construction of the eco-economy indicator system in Liaoning Province. In addition, we analyze in detail the characteristics and attributes of the ecological economy indicators in Liaoning Province, as well as the relationship of these indicators to the implementation of national strategies and to the SDGs. The discipline contributions and scientific and technological concerns of the indicator system's creation are reviewed, and additional improvement ideas are presented. It is expected that the practice of eco-economic indicators in China will further promote eco-economy development and provide methodological reference for countries to measure the level of eco-economic development.

Project description:ObjectivesLow-value care is associated with increased healthcare costs and direct harm to patients. We sought to develop and validate a simple diagnostic intensity index (DII) to quantify hospital-level diagnostic intensity, defined by the prevalence of advanced imaging among patients with selected clinical diagnoses that may not require imaging, and to describe hospital characteristics associated with high diagnostic intensity.MethodsWe utilized State Inpatient Database data for inpatient hospitalizations with one or more pre-defined discharge diagnoses at acute care hospitals. We measured receipt of advanced imaging for an associated diagnosis. Candidate metrics were defined by the proportion of inpatients at a hospital with a given diagnosis who underwent associated imaging. Candidate metrics exhibiting temporal stability and internal consistency were included in the final DII. Hospitals were stratified according to the DII, and the relationship between hospital characteristics and DII score was described. Multilevel regression was used to externally validate the index using pre-specified Medicare county-level cost measures, a Dartmouth Atlas measure, and a previously developed hospital-level utilization index.ResultsThis novel DII, comprised of eight metrics, correlated in a dose-dependent fashion with four of these five measures. The strongest relationship was with imaging costs (odds ratio of 3.41 of being in a higher DII tertile when comparing tertiles three and one of imaging costs (95 % CI 2.02-5.75)).ConclusionsA small set of medical conditions and related imaging can be used to draw meaningful inferences more broadly on hospital diagnostic intensity. This could be used to better understand hospital characteristics associated with low-value care.
