Paroxysmal Atrial Fibrillation Originating From the Inferior Vena Cava: A Case Report and Literature Review.

ABSTRACT: Atrial fibrillation is a common arrhythmia, but atrial fibrillation originating in the inferior vena cava is extremely rare. Here, we present a case of a 51-year-old woman with symptomatic paroxysmal atrial fibrillation, who was admitted to the Second Affiliated Hospital of Dalian Medical University and underwent radiofrequency ablation. The atrial fibrillation persisted despite pulmonary vein isolation. The inferior vena cava was then identified not only as a trigger but also as the driver to maintain atrial fibrillation, and tachycardia terminated successfully by discharging at the inferior vena cava. Furthermore, we performed a literature review of five previous case reports on this subject.

Project description:BackgroundTransseptal puncture and pulmonary vein isolation (PVI) in patients with atrial fibrillation (AF) are generally performed via the inferior vena cava (IVC). However, in cases where the IVC is inaccessible, a specific strategy may be needed.Case summaryAn 86-year-old woman with paroxysmal AF and an IVC filter in situ was referred to our hospital for ablation therapy. An IVC filter for pulmonary embolism and deep venous thrombosis had been implanted 15 years prior, therefore we selected a transoesophageal echocardiography (TOE)-guided transseptal puncture using a superior vena cava (SVC) approach. After the single transseptal puncture, we performed fast anatomical mapping, voltage mapping by multipolar mapping catheter, and then PVI by contact force-guided radiofrequency catheter using a steerable sheath. Following the ablation, bidirectional conduction block between the four pulmonary veins and the left atrium was confirmed by both radiofrequency and mapping catheter. No complications occurred and no recurrence of AF was documented in the 12 months after the procedure.DiscussionWhen performing a transseptal puncture during AF ablation, an SVC approach, via access through the right internal jugular vein, enables the sheath to directly approach the left atrium without angulation and improves operability of the ablation catheter. Combining the use of general anaesthesia, TOE, a steerable sheath, and contact force-guided ablation may contribute to achieving minimally invasive PVI with a single transseptal puncture via an SVC approach.

Project description:Common atrium (CA), also called three-chambered heart, is one of the rare congenital anomalies, defined by a complete absence of the atrial septum, eventually associated with malformation of the atrioventricular (AV) valves. We report the case of a 57-year-old woman with CA complicated with Eisenmenger syndrome and inferior vena cava interruption, who suffered from symptomatic persistent atrial fibrillation (AF). She underwent an initial successful pulmonary vein isolation procedure. A repeat procedure for perivalvular atrial flutter was complicated with inadvertent complete AV block, due to unusual AV node location in this challenging anatomy.

Endometrial stromal sarcoma metastatic to the inferior vena cava: a case report and literature review.

Project description:BackgroundEndometrial stromal sarcoma (ESS) is a uterine stromal tumor with a very low incidence, accounting for 10-15% of all uterine stromal malignancies and 0.2% of all uterine malignancies. The most common extrauterine location of ESS is the ovary, and it is relatively rare outside the uterus. Although most recurrences occur within the pelvis, distant metastases can occur.Case descriptionWe report a rare case of low-grade ESS (LG-ESS) metastatic to the inferior vena cava (IVC) which is difficult to distinguish from leiomyoma clinically. A 56-year-old woman attended outpatient complaining right thigh pain. She underwent a surgery of hysterectomy and bilateral adnexectomy 12 years ago. Abdominal contrast-enhanced computed tomography (CT) demonstrated that the vaginal stump was thick, with peripheral multiple nodular shadow. Soft tissue shadow in the right pelvic cavity. Thickening and enhancement of soft tissue shadow were observed in the peripheral blood vessels of the vaginal stump, the right internal iliac vein and the external iliac vein to the IVC of the liver segment. Malignancy (recurrence or metastasis) were considered. After multidisciplinary consultant, a preoperative diagnosis of leiomyomatosis of the IVC was made and surgical treatment was performed. Surgeons performed laparotomy, resection of tumor in IVC, right common iliac vein, right external iliac vein, right internal iliac vein and left common iliac vein. Post-operative pathology of dissected tumor demonstrated LG-ESS. The source may be the ovarian venous stump left after surgery 12 years ago. After a gynecological consultant, chemotherapy is recommended and is currently under follow-up.ConclusionsWe report a rare case of LG-ESS metastatic to the IVC, which was probably a lesion derived from the ovarian venous stump remaining after surgery 12 years ago.

Project description:Ventricular premature contractions (VPC) originating from right ventricular outflow tract is the most common type of ventricular arrhythmias in clinic settings, which can be effectively cured by catheter ablation. Interruption of the inferior vena cava (IVC) is a rare vascular anomaly resulting from aberrant development during embryogenesis. Herein, we reported a very rare case of VPC originating from right ventricular outflow tract (RVOT) with interruption of IVC, which was successfully ablated by transfemoral vein approach.

Wilms tumor with inferior vena cava duplication: a rare case report.

Project description:BackgroundWilms tumor is the most common renal tumor of childhood. Duplication of the inferior vena cava is an uncommon anomaly. In the present study, we present a case of Wilms tumor with the inferior vena cava duplication, which has not been reported previously.Case presentationA 14-month-old female presented with an enlarging abdominal mass. Computed tomography imaging demonstrated a large mass in the right kidney, duplication of the inferior vena cava below the renal veins and compression of the right inferior vena cava caused by the enormous mass. A right radical nephrectomy was performed. Final pathology was consistent with Wilms tumor. Postoperative adjuvant chemotherapy was executed. Computed tomography imaging at 3 months postoperatively showed the right inferior vena cava played a dominant role and the left inferior vena cava was not detected clearly. During the follow-up of 18 months, no local recurrence or metastasis has been observed.ConclusionIt is important to recognize the case of Wilms tumor with the inferior vena cava duplication to avoid injury of retroperitoneal venous anomalies and life-threatening hemorrhage during surgery through preoperative computed tomography.

Subsequent contrast enhanced, ECG-gated cardiac CTA was completed and images were reconstructed to yield cine images on syngo.via (Siemens, Erlangen, Germany) and vitrea workstation (version: 6.6.3; Vital, Toshiba Medical Systems) to guide heart team discussion and clinical management.DiscussionOur case illustrates the benefit of CTA with cineography in characterizing the location, size, and extent of fragment involvement within the myocardium. Moreover, this case serves as a reminder to medical professionals to carefully consider IVC filter placement in the appropriate patient, remain vigilant regarding potential complications, and to aspire follow-up of removable filters.

Robot-assisted radical nephrectomy with inferior vena cava thrombectomy: a case report.

Project description:Recently, robot-assisted surgery has been widely used to treat several urological cancers. Robot-assisted radical nephrectomy (RARN) was approved by the health insurance system in April 2022; however, RARN with inferior vena cava tumor thrombectomy (IVCTT) is still challenging. Also, its safety and feasibility have not yet been established owing to lack of literature, especially in Japan. We performed RARN with IVCTT in four patients between April 2022 and March 2023 at Fujita Health University Hospital. To reduce the risk of tumor embolism and major hemorrhage, an "IVC-first, kidney-last" robotic technique was developed. The safety and feasibility of RARN with IVCTT were evaluated by assessing the perioperative outcomes. Three women and one man were enrolled in this study. The median age was 72 years, and the tumor was on the right side in all cases. According to the Mayo Clinic thrombus classification, two patients were classified as level I, and the others were classified as level II. The two patients at level I did not undergo presurgical treatments, whereas the others at level II underwent presurgical treatments, which were combinations of tyrosine kinase inhibitors and immune-checkpoint inhibitors. The median operation and console times were 341 and 247 min, respectively. The median bleeding volume was 577 mL, and no complications beyond grade III of the Clavien-Dindo classification were observed. The median length of postoperative hospital stay was 10 days. Although the sample size was relatively small, we demonstrated the safety and feasibility of RARN with IVCTT in the Japanese population.
