Occurrence, transformation, bioaccumulation, risk and analysis of pharmaceutical and personal care products from wastewater: a review.

ABSTRACT: Almost all aspects of society from food security to disease control and prevention have benefited from pharmaceutical and personal care products, yet these products are a major source of contamination that ends up in wastewater and ecosystems. This issue has been sharply accentuated during the coronavirus disease pandemic 2019 (COVID-19) due to the higher use of disinfectants and other products. Here we review pharmaceutical and personal care products with focus on their occurrence in the environment, detection, risk, and removal.

Pharmaceuticals and personal care products in Tunisian hospital wastewater: occurrence and environmental risk.

Project description:Despite concerns about the potential risk associated with the environmental occurrence of pharmaceuticals and personal care products (PPCPs), few studies address the emissions of hospitals to aquatic compartments. We examined within a 3-month sampling period the occurrence and environmental risk of PPCPs in seven Tunisian hospital wastewaters. From personal care products, UV filters, main metabolites, and benzotriazoles were quantified, with benzophenone 3 (oxybenzone, BP3) and benzotriazole (BZT) the most frequently found (71%) at median concentrations in the range 2.43 ± 0.87 ngL-1-64.05 ± 6.82 ngL-1 for BP3 and 51.67 ± 1.67 ngL-1-254 ± 9.9 ngL-1 for BZT. High concentrations were also found for 4-hydroxybenzophenone (4HB) (221 ± 6.22 ngL-1), one of the main metabolites of BP3. The antibiotics ofloxacin and trimethoprim, the anti-inflammatory acetaminophen, the antiepileptic carbamazepine, and the stimulant caffeine were present in all the wastewaters. The highest median concentration corresponded to acetaminophen, with 1240 ± 94 mgL-1 in Tunis Hospital, followed by ofloxacin with 78850 ± 39 μgL-1 in Sousse Hospital. For ecotoxicity assessment, acute toxicity was observed for Daphnia magna and Vibrio fischeri. The toxicity data were used in a hazard quotient (HQ) approach to evaluate the risk posed by the target PPCPs to aquatic organisms. The calculated HQs revealed that marbofloxacin (234 for V. fischeri), enrofloxacin (121 for D. magna), and BZT (82.2 for D. magna and 83.7 for V. fischeri) posed the highest risk, concluding that potential risk exists toward aquatic microorganisms. This study constitutes the first monitoring of UV filters in Tunisian hospital effluents and provides occurrence and toxicity data of PPCPs for reference in further surveys in the country.

Pharmaceutical products, drugs and personal care products in European waters: A protocol for systematic review and meta-analysis.

Project description:This study aims to describe a protocol for a systematic review and meta-analysis that assesses the detection and concentration of pharmaceutical products, drugs, and personal care products in European waters. This study protocol was developed following the recommendations of the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA-P) statement and the Cochrane Handbook of Systematic Reviews of Interventions. We will include studies conducted on European waters of various origins (watersheds, aquifers, rivers, seas, springs, wastewaters, and drinking water). A comprehensive search strategy will be implemented in the following databases: PubMed (Medline), EMBASE, Scopus, and Web of Science. Two independent reviewers will conduct all study selection procedures, data extraction, and methodological evaluation. Any disagreements will be referred to a third reviewer. If the studies are sufficiently homogeneous, we will conduct a meta-analysis to summarize the data. We will use the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) to assess the certainty of the evidence. The systematic review and meta-analysis will provide valuable information about the presence and concentration of these types of contaminants in water, aiding in the development of public policies regarding prevention and decontamination measures to enhancing water quality in Europe.

Pharmaceuticals and personal care products in Canadian municipal wastewater and biosolids: occurrence, fate, and time trends 2010-2013 to 2022.

Project description:The concentrations of 135 pharmaceuticals and personal care products (PPCPs) were determined in raw influent, final effluent, and treated biosolids at Canadian wastewater treatment plants (WWTPs) to evaluate the fate of PPCPs through liquid and solids trains of typical treatment types used in Canada and to assess changes in PPCP concentrations in wastewater matrices between 2010-2013 and 2022. PPCPs dominant in influent and effluent included the antidiabetic metformin, analgesics/anti-inflammatories (acetaminophen, ibuprofen, 2-hydroxy-ibuprofen), caffeine and its metabolite (1,7 - dimethylxanthine), theophylline (a bronchodilator and metabolite of caffeine), an insect repellent (N,N-diethyl-m-toluamide, DEET), and iopamidol (a contrast media for X-rays). PPCPs dominant in biosolids differed from those in influent/effluent and included antibiotics (fluoroquinolones and doxycycline), antidepressants (sertraline, citalopram, and amitriptyline), a preservative and antimicrobial agent (triclosan), an antihistamine (diphenhydramine), and an antifungal (clotrimazole). These elevated concentrations in influent/effluent and biosolids reflected their use in Canadian communities. PPCPs dominant in influent/effluent had relatively low hydrophobicity whereas those in biosolids tended to be more hydrophobic, or electrostatic forces governed their sorption. Higher removal of PPCPs was generally observed at WWTPs that used biological treatment compared to primary physical/chemical treatment. PPCP concentration changes in wastewater matrices between 2010-2013 and 2022 were influenced by risk management measures, warnings, the development of new pharmaceuticals, the COVID-19 pandemic, and other factors. These time trends reflected the limited information available on PPCP use in Canada. Continued periodic monitoring of PPCPs is recommended to fill data gaps on community use and release to the environment.

Treatment Trends and Combined Methods in Removing Pharmaceuticals and Personal Care Products from Wastewater-A Review.

Project description:When discharged into wastewater, pharmaceuticals and personal care products (PPCPs) become microorganic contaminants and are among the largest groups of emerging pollutants. Human, animal, and aquatic organisms' exposures to PPCPs have linked them to an array of carcinogenic, mutagenic, and reproductive toxicity risks. For this reason, various methods are being implemented to remove them from water bodies. This report critically reviews these methods and suggests improvements to removal strategies. Biological, physical, and chemical methods such as biological degradation, adsorption, membrane filtration, and advanced electrical and chemical oxidation are the common methods used. However, these processes were not integrated into most studies to take advantage of the different mechanisms specific to each process and are synergistic in the removal of the PPCPs that differ in their physical and chemical characteristics (charge, molecular weight, hydrophobicity, hydrogen bonding, structure). In the review articles published to date, very little information is available on the use of such integrated methods for removing PPCPs. This report attempts to fill this gap with our knowledge.

Bioaccumulation of pharmaceuticals and personal care product chemicals in fish exposed to wastewater effluent in an urban wetland.

Project description:The bioaccumulation of a broad range of pharmaceuticals and personal care product chemicals (PPCPs) was studied in Cootes Paradise Marsh (CPM), an urban wetland that receives tertiary treated municipal waste waters as well as urban storm runoff. We measured PPCPs in caged and wild goldfish, as well as wild carp, and compared observed bioaccumulation factors (BAFP) using concentrations in surface waters and fish blood plasma, with modeled BAFs. Thirty-two PPCPs were detected in water from the central CPM site (CPM3) while 64 PPCPs were found at higher concentrations at a site immediately downstream of the effluent outflow (CPM1). Following a 3-week deployment, 15 PPCPs were detected in the plasma of caged goldfish at CPM1, and 14 at CPM3, compared to only 3 in goldfish caged at a reference site. The highest BAFP in goldfish were for the antidepressant Σfluoxetine averaging 386 L/kg in caged and 906 L/kg in wild goldfish, respectively. In carp, ΣDiazepam (diazepam and oxazepam) had the highest BAFP (927 L/kg). This study identified a broader range of PPCPs in fish and surface waters than previously reported. However, modeled BAFs did not show good agreement with observed whole body or plasma BAFs, demonstrating that more work is needed to better explain bioaccumulation of PPCPs.

Nanoplastics and sunscreens contained in personal care products induce neurotoxicity in early zebrafish embryos

Project description:Nanoplastics with small particle size and high surface area/volume ratio make them easy to absorb environmental pollutants and affect their bioavailability. In this study, polystyrene nanoplastic beads (PS-NPBs) with a particle size of 100nm and sunscreen butyl methoxydibenzoylmethane (BMDBM) contained in personal care products were chosen as target pollutants to study their developmental toxicity and interactive effects on zebrafish embryo. The exposure period was 2-12 hours post fertilization (hpf). The BMDBM and PS-NPBs significantly upregulated the genes related to antioxidant enzymes and downregulated the gene expression of aromatase and DNA methyltransferases, but the influenced genes were not exactly the same, and the combined exposure reduced the adverse effects on the expression of all genes. With the help of single-cell RNA sequencing technology, the neural mid cells were identified as the target cells of both pollutants, and brain development, head development and notch signaling pathway were the functions they commonly altered. The key genes and functions that are specifically affected by BMDBM and/or PS-NPBs were identified. The BMDBM mainly affect the differentiation and fate of neurons in central nervous system through the regulation of her5, her6, her11, lfng, pax2a and fgfr4. The PS-NPBs regulated the expression of olig2, foxg1a, fzd8b, six3a, rx1, lhx2b, nkx2.1a and sfrp5 to alter nervous system development, retinal development, and stem cell differentiation. The phenotypic responses of zebrafish larvae at 120 hpf were tested, and significant inhibition of locomotor activity was found, indicating that early disrupting effect on central nervous system would have a sustained impact on the behavior of zebrafish.

Ozonation of Selected Pharmaceutical and Personal Care Products in Secondary Effluent-Degradation Kinetics and Environmental Assessment.

Project description:The efficiency of ozonation depends on the water matrix and the reaction time. Herein, these factors were addressed by assessing the removal of five pharmaceutical and personal care products (PPCPs) by ozonation. The main aims were: (i) to assess the effects of the water matrix on the degradation kinetics of PPCPs, individually and in mixture, following ozonation; and (ii) to assess the ecotoxicological impact of the ozone reaction time on the treatment of a spiked municipal wastewater (MW) added the five PPCPs over several species. The degradation of the PPCPs was faster in ultrapure water, with all PPCPs being removed in 20 min, whereas in the MW, a 30 min ozonation period was required to achieve a removal close to 100%. Increasing the number of PPCPs in the water matrix did not affect the time required for their removal in the MW. Regarding the ecotoxicity assessment, Raphidocelis&nbsp;subcapitata and Daphnia magna were the least sensitive species, whereas Lemna minor was the most sensitive. The temporal variation of the observed effects corroborates the degradation of the added PPCPs and the formation of toxic degradation by-products. The removal of the parent compounds did not guarantee decreased hazardous potential to biological species.

Assessment of the Presence of Transformation Products of Certain Pharmaceutical Products (Psychotropic Family) by Suspect and Non-Targeted HRMS Screening in Wastewater Treatment Plants.

Project description:Aquatic environments are the final receptors of human emissions and are therefore contaminated by molecules, such as pharmaceuticals. After use, these compounds and their metabolites are discharged to wastewater treatment plants (WWTPs). During wastewater treatment, compounds may be eliminated or degraded into transformation products (TPs) or may be persistent. The aim of this study was to develop an analytical method based on high resolution mass spectrometry (HRMS) for the identification of six psychotropic drugs that are widely consumed in France and present in WWTPs, as well as their potential associated metabolites and TPs. Four out of six psychotropic drugs and between twenty-five and thirty-seven potential TPs were detected in wastewater, although this was based on full scan data. TPs not reported in the literature and specific to the study sites and therefore to the wastewater treatment processes were tentatively identified. For the selected drugs, most known and present TPs were identified, such as desmethylvenlafaxine or norcitalopram. Moreover, the short fragmentation study led rather to the identification of several TPs of carbamazepine as ubiquitous persistent TPs.

Reduction of nutrients, microbes, and personal care products in domestic wastewater by a benchtop electrocoagulation unit.

Project description:To preserve environmental and human health, improved treatment processes are needed to reduce nutrients, microbes, and emerging chemical contaminants from domestic wastewater prior to discharge into the environment. Electrocoagulation (EC) treatment is increasingly used to treat industrial wastewater; however, this technology has not yet been thoroughly assessed for its potential to reduce concentrations of nutrients, a variety of microbial surrogates, and personal care products found in domestic wastewater. This investigation's objective was to determine the efficiency of a benchtop EC unit with aluminum sacrificial electrodes to reduce concentrations of the aforementioned biological and chemical pollutants from raw and tertiary-treated domestic wastewater. EC treatment resulted in significant reductions (p < 0.05, α = 0.05) in phosphate, all microbial surrogates, and several personal care products from raw and tertiary-treated domestic wastewater. When wastewater was augmented with microbial surrogates representing bacterial, viral, and protozoan pathogens to measure the extent of reduction, EC treatment resulted in up to 7-log10 reduction of microbial surrogates. Future pilot and full-scale investigations are needed to optimize EC treatment for the following: reducing nitrogen species, personal care products, and energy consumption; elucidating the mechanisms behind microbial reductions; and performing life cycle analyses to determine the appropriateness of implementation.

Pharmaceutical Transformation Products Formed by Ozonation-Does Degradation Occur?

Project description:The efficiency of an advanced oxidation process (AOP) using direct and indirect ozonation for the removal of pharmaceutical residues from deliberately spiked deionized water was examined. Both direct and indirect ozonation demonstrated 34% to 100% removal of the parent compounds. However, based on the products' chemical structure and toxicity, we suggest that despite using accepted and affordable ozone and radical concentrations, the six parent compounds were not fully degraded, but merely transformed into 25 new intermediate products. The transformation products (TPs) differed slightly in structure but were mostly similar to their parent compounds in their persistence, stability and toxicity; a few of the TPs were found to be even more toxic than their parent compounds. Therefore, an additional treatment is required to improve and upgrade the traditional AOP toward degradation and removal of both parent compounds and their TPs for safer release into the environment.
