Factors associated with depression and determining dimensions of job satisfaction among physicians in Bangladesh.

ABSTRACT:

Background

Depression in physicians emerges early in their academic and professional careers. Lengthy and irregular duty time, high levels of obligation, job dissatisfaction, workstation culture, organizational rules, and so on significantly increased the psychological pressure on physicians.

Objectives

The study's aim was to measure the level of depression, association, and influence of socio-demographic characteristics and job satisfaction on depression among physicians in Bangladesh, as well as to explore the factor structure of job satisfaction measure and examine its internal reliability.

Methods

Data were collected using a self-administered questionnaire in a cross-sectional survey of 301 physicians. The factors related to depression were investigated using a multivariable logistic regression model, and factor analysis was done to identify the important factors associated with job satisfaction.

Results

Male respondents made up 49.5 percent of the sample overall, while female respondents made up 50.5 percent. 24.58% of the physicians had mild depression, whereas 13.29%, 7.31%, and 0.66% of the participants had moderate, moderately severe, and severe depression, respectively. In multivariable analysis, sex (male vs. female, AOR: 2.16, 95% CI:1.28-3.62), monthly income <15000 BDT vs. >40000 BDT, (AOR: 0.35, 95% CI: 0.14-0.89), and income <15000 BDT vs. 15,000-24,999 BDT (AOR: 0.36, 95% CI: 0.15-0.89) were the essential factors associated with depression. Furthermore, with each unit increase in the job satisfaction score was related to a 71% decrease in the odds of physicians having depression.

Conclusion

The findings of this study indicate that providing appropriate organizational support, proper work assignments, and an adequate opportunity to develop their professional skills and career irrespective of sex may increase overall job satisfaction. Ultimately, this will serve to improve patient care as well as the whole health system's output.

Project description:Physician satisfaction is a multidimensional concept related to many factors. Despite the wide range of research regarding factors affecting physician satisfaction in different European countries, there is a lack of literature reviews analyzing and summarizing current evidence. The aim of the article is to synthetize the literature studying the factors associated with physician satisfaction. We searched: MEDLINE, Embase, PsycINFO, CINAHL and the Cochrane Library from January 2000 to January 2017. The eligibility criteria included: (1) target population: physicians working in European hospitals; (2) quantitative research aimed at assessing physician satisfaction and associated factors; (3) use of validated tools. We performed a narrative synthesis. After screening 8585 records, 368 full text articles were independently checked and finally 24 studies were included for qualitative analysis. The included studies surveyed 20,000 doctors from 12 European countries. The tools and scales used in the analyzed research to measure physician satisfaction varied to a large extent. We extracted all pre-specified factors, reported as statistically significant/non-significant. Analyzed factors were divided into three groups: personal, intrinsic and contextual factors. The majority of factors are modifiable and positively associated with characteristics of contextual factors, such as work-place setting/work environment. In the group of work-place related factors, quality of management/leadership, opportunity for professional development and colleague support have been deemed statistically significant in numerous studies. We identified more studies appraising the effect of contextual factors (such as work environment, work-place characteristics), highlighting a positive association between those factors and physician satisfaction, compared with personal and intrinsic factors. Numerous studies confirmed statistically significant associations between physician satisfaction and quality of management, professional development and colleague support/team climate. Due to the health workforce crisis, knowledge regarding physician satisfaction and associated factors is essential to healthcare managers and policy makers for more stable human resources management.

Project description:Background:Poor job satisfaction has been associated with less productivity and high staff turnover. Various factors are thought to contribute for job dissatisfaction among pharmacy professionals and very limited studies have been conducted in eastern part of Ethiopia. Therefore, the current study was aimed to assess the level of job satisfaction among pharmacy professionals and its predictors. Methods:A cross-sectional study was conducted among 232 pharmacy professionals to assess level of job satisfaction in public hospitals located in the eastern Ethiopia. The data were collected using self- administered semi-structured questionnaires. Data were entered into Epi-Data version 3.1 and exported to STATA version 14.2 for analysis. Associations between the dependent and independent variables were assessed by multivariate analysis using an Adjusted Odds Ratio (AOR) at a 95% confidence interval (CI) and p-value less than 0.05 was considered as significant. Results:A total 220 questionnaires were found complete and included in the analysis. The mean age of participants was 27.6(SD + 4.1). More than half of the respondents (55.4%) had a bachelor degree and the majority (86.4%) were working less than 40?h per week, mostly in dispensing units (75.4%). About one third of the participants (32.7, 95% CI; 26.8-39.2) were found to be satisfied with their job. Age category of 20 to 25?years in reference to age greater than 30?years (AOR?=?3.5, 95% CI; 1.1-9.7), holding a bachelor degree in reference to having diploma (AOR?=?4.2, 95% CI; 1.8-10.00), working for more than 40?h per week (AOR?=?6.2, 95% CI, 2.4-16), and working in dispensing units (AOR?=?2.4, 95% CI; 1.1-5.5) were found to have strong association with job dissatisfaction. Conclusion:In this study, the job satisfaction levels of pharmacy professionals were found to be very low. The age category of 20 to 25, holding a bachelor degree, working for more than 40?h per week, and working in dispensing unit were found to be strong predictors of job dissatisfaction. Hence, pharmacy directors and hospital administrators should work to reduce unnecessary workload on the staffs and create good working climate.
