Parental intentions to vaccinate children against COVID-19: Findings from a U.S. National Survey.

ABSTRACT: We examined parents' COVID-19 vaccination intentions for their children, reasons for not vaccinating, and the potential impact of a school/daycare vaccination requirement or pediatrician's recommendation on vaccination intentions. Two online surveys were conducted in June-July and September-October 2021, before pediatric COVID-19 vaccines were authorized for emergency use in children age < 12 years, with an internet-based, non-probability sample of U.S. adults. Respondents with children (age < 18 years) in the household were asked about their intention (likelihood) of vaccinating these children against COVID-19. Weighted Chi-square tests using a Rao-Scott correction were performed. Vaccinated (45.7 %) versus unvaccinated (6.9 %) parents were almost seven times more likely to have vaccinated their 12-17-year-old children against COVID-19. Approximately 58.4 % of respondents with unvaccinated children ages 2-11 years and 42.4 % of those with children < 2 years said they are "very" or "extremely likely" to vaccinate these children against COVID-19. Female parents were significantly more likely (p < .01 to p < .001) to express lower levels of COVID-19 vaccine intentions. Across all age groups of children unvaccinated against COVID-19, parental vaccine intentions increased with increased household income and education levels. COVID-19 vaccine side effects and safety concerns were primary reasons for not vaccinating children. Strategies including school vaccination requirements and recommendations from pediatricians were shown to increase parental COVID-19 vaccination intentions for some. More research is needed on factors that increase/hinder COVID-19 pediatric vaccine uptake.

Acceptance decreased with unconfirmed past COVID-19, greater financial hardship and non-white British ethnicity; black/black British participants had lowest acceptance. Overall, healthcare and scientific sources of information were most trusted. Compared with white British participants, other ethnicities had lower trust in healthcare and scientific sources. Those with lower educational attainment or financial hardship had lower trust in healthcare and scientific sources. Those with no qualifications had higher trust in media and family/friends. While trust was low overall in community or faith leaders, it was higher among those with Asian/Asian British and black/black British ethnicity compared with white British participants. Views of vaccine prioritisation were mostly consistent with UK official policy but there was support for prioritising additional groups. There was high support for having the second vaccine dose.ConclusionsTargeted engagement is needed to address COVID-19 vaccine hesitancy in non-white British ethnic groups, in younger adults, and among those with lower education, greater financial hardship and unconfirmed past infection. Healthcare professionals and scientific advisors should play a central role in communications and tailored messaging is needed for hesitant groups. Careful communication around vaccination prioritisation continues to be required.

Project description:BackgroundThe objective of this study was to determine the factors associated with low or no parental intention to vaccinate children of 0-4-years in Canada with a COVID-19 vaccine through the 2022 Childhood COVID-19 Immunization Coverage Survey (CCICS). The CCICS was conducted prior to the introduction of a COVID-19 vaccine and a vaccine recommendation for this age group.MethodsSimple and multiple logistic regression models were used to determine associations between sociodemographic factors as well as knowledge, attitudes and beliefs and low/no intentions to vaccinate against COVID-19 among parents of children 0-4 years.ResultsFactors associated with low intentions to vaccinate children against COVID-19 included being male (aOR: 2.0; 95% CI: 2.0‒2.1) compared to female; being 30-39 (aOR 1.1; 95% CI: 1.1‒1.2) compared to 40+; being Black (aOR: 2.3, 95% CI: 2.2‒2.5), East/Southeast Asian (aOR: 3.6, 95% CI: 3.3‒3.8), or having multiple ethnicities (aOR: 1.3, 95% CI: 1.1‒1.6) compared to White European ethnicity; living in a rural (aOR: 2.0, 95% CI: 1.9‒2.1) compared to urban community; having a total 2021 household income of $60,000‒$79,999 CAD (aOR: 1.4, 95% CI: 1.3‒1.5) compared to $150,000 CAD and above; and trusting government bodies (aOR: 2.4; 95% CI: 1.1‒1.2), international bodies (aOR: 2.4; 95% CI: 2.2‒2.5), or media (aOR: 2.0, 95% CI: 1.9‒2.2) for information about COVID-19 vaccines compared to health care providers.ConclusionsThe findings of this study demonstrate that several sociodemographic factors and parental beliefs impact the decision to vaccinate children 0-4 years of age against COVID-19. Future research should focus on sociodemographic barriers to vaccination and how to most appropriately tailor the delivery of vaccination programs to specific groups, in an effort to narrow the gap between intentions and uptake of COVID-19 vaccination in younger children. As well, messaging should specifically be targeted to parents who have lower confidence in the COVID-19 vaccine and the government to provide correct information and build trust.

Project description:IntroductionVaccinating children against COVID-19 protects children's health and can mitigate the spread of the virus to other community members.ObjectiveThe primary objective of this study was to use a socio-ecological perspective to identify multi-level factors associated with US parents' intention to vaccinate their children.MethodsThis study used a longitudinal online cohort. Multinomial logistic regression models assessed socio-ecological predictors of negative and uncertain child COVID-19 vaccination intentions compared to positive intentions.ResultsIn June 2021, 297 parents were surveyed and 44% reported that they intended to vaccinate their children while 25% expressed uncertainty and 31% did not intend to vaccinate their children. The likelihood of reporting uncertain or negative intention, compared to positive intention to vaccinate their children was higher among parents who had not received a COVID-19 vaccination and those who did not have trusted information sources. Parents who talked to others at least weekly about the COVID-19 vaccine were less likely to endorse uncertain compared to positive vaccine intentions (aRRR: 0.44; 95% CI: 0.20-0.93). A sub-analysis identified that parents had significantly higher odds of intending to vaccinate older children compared to younger children (children ages 16-17 years v. 0-4 years OR: 2.01, 95% CI: 1.05-3.84). An additional sub-analysis assessed the stability of parents' intention to vaccinate their children between March 2021 and June 2021 (N=166). There was transition within each intention group between the study periods; however, symmetry and marginal homogeneity test results indicated that the shift was not statistically significant. Parents expressing uncertainty in March 2021 were the most likely to change their intention, with 24% transitioning to positive intention and 23% to negative intention in June 2021.ConclusionStudy findings suggest that programs to promote vaccination uptake should be dyadic and work to promote child and parent vaccination. Peer diffusion strategies may be particularly effective at promoting child vaccination uptake among parents expressing uncertainty.

Project description:Coronavirus disease 2019 (COVID-19) vaccination hesitancy has emerged as a substantial concern among the adult population globally. However, limited evidence is available about parental hesitancy to vaccinate their children against COVID-19 within the Pakistani context. Thus, the present investigation aimed to assess parental attitudes, perceptions, and willingness regarding vaccination hesitancy and associated predictors of getting their children vaccinated against COVID-19. We conducted a cross-sectional population-based, self-administered online questionnaire in Punjab, Pakistan, on randomly selected parents between October 2022 and February 2023. The data were collected based on socio-demographics, attitudes, perceptions, and willingness of parents regarding COVID-19 vaccine hesitancy for their children. Adjusted odds ratios with 95% confidence intervals were estimated to identify the predictors of vaccine hesitancy. The findings demonstrated that among 1,478 participants, a total of 40% believed that the COVID-19 vaccine may pose a greater risk to children than adults, while 38% exhibited no concerns. Around 13% of children were not vaccinated in our study sample. More than half expressed hesitancy toward vaccination, and only 35.25% were inclined to get their children vaccinated in our study sample. In addition, only 16% of the parents believed that the COVID-19 vaccination may cause an alteration in their children's DNA. A similar proportion of parents were aware of the significance of getting their children vaccinated and expressed their willingness to vaccinate their children to prevent the COVID-19 infection. However, a higher odds ratio was observed in females with a higher educational background and those in the healthcare profession. In conclusion, healthcare awareness-supporting programs for educating parents should be designed and implemented. These insights might aid in the development of strategies to eradicate barriers in existing coronavirus vaccination programs and may vaccinate a larger child population to reduce the adverse consequences of the pandemic.
