Key factors influencing paediatric COVID-19 vaccine hesitancy: a brief overview and Decision-making Trial and Evaluation Laboratory analysis.

ABSTRACT:

Objectives

The purpose of this study was to examine the current literature on paediatric COVID-19 vaccine hesitancy among parents and identify key influencing factors, thus enabling targeted policy development and implementation.

Study design

This was a systematic literature review and Decision-making Trial and Evaluation Laboratory (DEMATEL) analysis.

Methods

A review of the quantitative and qualitative literature focusing on factors influencing paediatric COVID-19 vaccine hesitancy was conducted. Searches were performed in PubMed, ScienceDirect, SpringerLink and Embase. Because of the immediacy of the topic, commentaries were included in addition to research and review articles. Influencing factors were categorised according to the Health Ecology Theory and screened using the DEMATEL method.

Results

A total of 44 articles were included in the study, and 44 factors influencing paediatric COVID-19 vaccine hesitancy were identified. Of these, 18 were categorised as key factors using the DEMATEL method, including a history of COVID-19 infection in parents and perceived safety of the paediatric COVID-19 vaccine.

Conclusions

Policymakers and public health personnel should pay more attention to the key factors influencing paediatric COVID-19 vaccine hesitancy. The outcome of this research will benefit and motivate decision-makers to consider strategies to overcome various challenges of COVID-19 vaccine hesitancy.

Project description:BackgroundThe organ transplantation sector in China is facing a severe shortage of donors, and the organ donation rate needs to be increased. Since 2015, voluntary donation by citizens has become the only source of organs for transplantation in China. In recent years, there has been a relatively positive change in young people's attitudes toward organ donation after death. The aim of the study was to understand young people's perceptions and attitudes toward organ donation and the factors that influence them and can positively impact the promotion of organ donation.MethodsBy analyzing relevant literature and legal texts, we developed a questionnaire. Information was obtained through questionnaires and interviews, and 501 valid questionnaires were returned from the target group. A chi-square test was used to examine whether there were significant differences in the willingness to organ donation among young people with different characteristics. A factor analysis was used to investigate the main factors influencing the different attitudes of young people toward organ donation, and a one-way ANOVA was used to examine whether young people with different characteristics were affected differently by different factors.ResultsIn our survey of young people aged 18-30 years, 99.2% of respondents knew about organ donation, 47.1% were willing to donate organs, and 15.2% understood that there were corresponding laws and regulations for organ donation. The study's findings showed that urban residents are more willing to be organ donators than rural residents; people with higher education levels have better awareness and are more willing to donate an organ; and people with religious beliefs are more likely to donate organs. The main factors that support the willingness of young people to donate are the social environment that provides support, their optimism in dealing with death, and their desire to realize their final value after death. The main factors for those unwilling to donate were low awareness or misconceptions about organ donation among individuals and their families and their attitudes toward death. As the people who took the questionnaire are probably interested in organ donation, the sample results will show a higher percentage of people who know about organ donation. We hope to discuss further with a larger and broader sample coverage to improve the estimates' validity and reflect the overall picture more accurately in a future study.ConclusionYoung people knew about organ donation but had a low depth of awareness. Household registration type, education level, and religious affiliation significantly correlate with people's willingness to donate. The supportive environment for organ donation in society and the correct understanding of the organ donation process and laws and regulations can influence people's willingness to donate.

Project description:ObjectiveThis study describes the prevalence of vaccine hesitancy associated with the Catalan systematic childhood vaccination calendar and some related psychosocial determinants among paediatric primary care nurses in Barcelona (Spain).MethodsCross-sectional descriptive study. In 2017 we invited the paediatric nurses (N = 165) working in Barcelona public primary health centres with paediatric departments (N = 41) to participate. They answered a questionnaire with sociodemographic and behavioural variables: severity and perceived probability of contracting the diseases of the vaccines in the vaccination schedule; safety and protection offered by each vaccine; and beliefs, social norms, and knowledge about vaccines. Outcome variable was vaccine hesitancy, dichotomized into not hesitant (nurses who would vaccinate their own offspring), and hesitant (including those who would not vaccinate them, those who had doubts and those who would delay the administration of one or more vaccines). We performed bivariate analysis and adjusted logistic regression models.Results83% of paediatric nurses (N = 137) agreed to participate. 67.9% had the intention to vaccinate their children of all the vaccines in the systematic schedule. 32.1% of nurses experienced vaccine hesitancy, especially about the HPV (21.9%) and varicella (17.5%) vaccines. The multivariate analysis suggests associations between hesitancy and low perception of the severity of whooping cough (aOR: 3.88; 95%CI:1.32-11.4), low perception of safety of the HPV vaccine (aOR:8.5;95%CI:1.24-57.8), the belief that vaccines are administered too early (aOR:6.09;95%CI:1.98-18.8), and not having children (aOR:4.05;95%CI:1.22-13.3).ConclusionsAlthough most paediatric nurses had the intention to vaccinate their own children, almost one-third reported some kind of vaccine hesitancy, mainly related to doubts about HPV and varicella vaccines, as well as some misconceptions. These factors should be addressed to enhance nurses' fundamental role in promoting vaccination to families.

Project description:AbstractsIntroductionVaccine hesitancy among mothers in low-income communities in Kenya presents a serious obstacle to achieving successful childhood immunisation. The aim of this study was to explore the determinants of vaccine hesitancy among mothers from low-income rural communities, risk perceptions associated with vaccines and how these factors influence decision-making regarding their children's immunisation.MethodsA qualitative descriptive study was conducted in three counties in Kenya (Murang'a, Kiambu and Machakos) using in-depth interviews and focus group discussions with mothers of children under 5 years. Participants were purposively sampled from low-income rural communities and were mothers attending postnatal clinics at various health facilities. Thematic analysis was used to identify key themes and subthemes, coding of transcripts, identification of patterns and organisation of themes into categories.ResultsSeveral critical factors that contribute to vaccine hesitancy among mothers regarding their children's immunisation were identified through thematic analysis. Safety concerns emerged as a primary issue, with mothers expressing fears of adverse reactions such as fever, pain, swelling or other unexpected complications. Misinformation significantly influenced perceptions, including beliefs that vaccines might cause infertility or long-term health problems. Distrust in the health system further exacerbated hesitancy, with mothers questioning the quality, administration and motives of vaccinators. Limited vaccine knowledge also played a role, which led to misconceptions about the severity and risks of vaccine-preventable diseases. Moreover, sociocultural and religious beliefs influenced decision-making, with some mothers viewing vaccines as unsafe or ineffective, driven by cultural norms, reliance on traditional remedies or religious objections.ConclusionsThis study provides valuable insights into the complexities of vaccine hesitancy among mothers in low-income communities in Kenya. Furthermore, the results emphasise the complex nature of hesitancy, driven by an interplay of safety concerns, misinformation, distrust, limited knowledge, and sociocultural and religious influences. Addressing these determinants requires interventions that would prioritise clear and accurate communication about vaccine safety, engagement with community and religious leaders, and strengthening trust in healthcare providers and systems. Additionally, enhancing vaccine knowledge through targeted health education programmes would be crucial for empowering mothers to make informed decisions about their children's health.

Project description:BackgroundThe introduction of COVID-19 vaccines exposed volatility and hesitancy around vaccines. Some health care models, including ICU recovery clinics (ICU-RCs), are structured to provide vaccine counseling. However, information regarding provider and patient vaccine conversations is limited in this postacute setting.Research questionWhat factors influence the decision-making process of patients who have survived an ICU stay surrounding influenza and COVID-19 vaccination?Study design and methodsTo understand further vaccine perceptions after critical illness, a secondary qualitative thematic analysis was performed using transcripts from a randomized controlled trial designed to develop and refine a telemedicine approach to ICU recovery. Thirty-three ICU-RC visits with 19 adult patients and 13 caregivers were conducted within 12 weeks of hospital discharge. The analysis was guided by the theory of planned behavior (TPB).ResultsFive themes were elicited from the data. The first four themes arose from the TPB: (1) behavioral and attitudinal beliefs (not being susceptible to the flu, concerns about the COVID-19 vaccine causing fertility issues, and not being tested enough), (2) normative beliefs (everyone they know is getting the influenza vaccine so they are, too), (3) control vaccine beliefs (patients are more likely to get the COVID-19 vaccine if it is easy to obtain), and (4) intention to vaccinate. Another theme not related to the TPB arose and could contribute to vaccine intent and behavior: (5) health team engagement with patients and caregivers (allowing for ICU clinicians to correct vaccine misinformation in real time).InterpretationUsing the information learned in our study, the period after critical illness or other acute illness events may be an especially fruitful target for designing an action plan for improving public trust in vaccines and improving overall completion rates; however, further research is needed.Trial registryClinicalTrials.gov; No.: NCT03926533; URL: www.clinicaltrials.gov.
