Atherogenic index of plasma and coronary artery disease: A systematic review.

ABSTRACT: Various studies suggest that the atherogenic index of plasma (AIP) is associated with the risk of coronary artery disease (CAD) in different clinical scenarios. This review aimed to synthesize evidence of the association between AIP values and CAD. A literature search was carried out on four databases, namely, PubMed, Scopus, Web of Science, and Ovid-Medline. A handsearch was performed on preprint repositories (MedRxiv and Research Square). The effect measurements were expressed as odds ratios (OR) with their corresponding 95% confidence intervals (CI). For the quantitative synthesis, we employed a random-effects model. We analyzed 14 articles (with 40,902 participants) from seven different countries. The quantitative analysis revealed that an increase in one unit of AIP was associated with higher odds of developing CAD (OR 2.11; 95% CI 1.65-2.69; P < 0.001; I2 = 98%). We conducted subgroup analyses of Chinese (OR 1.89; 95% CI 1.40-2.56; P < 0.001) and non-Chinese studies (OR 2.51; 95% CI 1.42-4.42; P < 0.001). The sensitivity analysis by risk of bias continued to demonstrate an association, and the heterogeneity remained unchanged (OR 1.75; 95% CI 1.33-2.31; P < 0.001; I2 = 98%). Higher AIP values were associated with higher odds of developing CAD.

Project description:BackgroundAtherogenic index of plasma (AIP), a novel logarithmic index that combines fasting triglyceride and high-density lipoprotein cholesterol concentrations, is associated with the burden of atherosclerosis. This study aimed to evaluate the relationship between AIP and coronary artery disease (CAD) risk, severity, and prognosis in populations with and without established CAD.MethodsPubMed, Embase, and Web of Science were systematically searched from the inception of each database to August 13, 2024. Cross-sectional studies, case-control studies, and prospective or retrospective cohort studies using multivariate analysis were included. Given that the true effect size may differ across studies, a random-effects model for all analyses was applied.ResultsFifty-one observational studies were included in this study. Patients with higher AIP were more likely to have CAD (odds ratio (OR): 2.79, 95% CI 1.75-4.45, P < 0.00001). Furthermore, these patients were more likely to have coronary artery calcification (OR: 2.28, 95% CI 1.74-3.00, P < 0.00001), multivessel CAD (OR: 2.04, 95% CI 1.50-2.77, P < 0.00001), and an increased risk of plaque progression (OR: 1.49, 95% CI 1.17-1.91, P = 0.001). In populations without established CAD, higher AIP levels were associated with an increased risk of Major adverse cardiovascular events (MACE) (hazard ratio (HR): 1.28, 95% CI 1.22-1.35, P < 0.00001). Interestingly, this finding was consistent in patients presenting with acute coronary syndrome (HR: 1.59, 95% CI 1.33-1.89, P < 0.00001) and patients with chronic coronary syndrome or stable CAD (HR: 1.65, 95% CI 1.15-2.37, P = 0.007).ConclusionsThis meta-analysis demonstrates that elevated AIP is strongly associated with increased CAD risk, greater severity, and poorer prognosis in populations with and without established CAD. However, more studies are needed to evaluate the predictive performance and determine the optimal cut-off for AIP in different populations.

Project description:BackgroundDyslipidemia is a well-known risk factor for cardiovascular disease (CVD). Recently, atherogenic index of plasma (AIP) has been proposed as a novel predictive marker for CVD, and few cross sectional studies have demonstrated a relationship between AIP and coronary artery disease. The present study investigated the association between AIP and the progression of coronary artery calcification (CAC) in Korean adults without CVD.MethodsA total of 1124 participants who had undergone CAC measurement at least twice by multi-detector computed tomography (CT) at a health check-up center were enrolled. Their anthropometric measurements and various cardiovascular risk factors were assessed. AIP was defined as the base 10 logarithm of the ratio of the concentration of triglyceride (TG) to high-density lipoprotein-cholesterol (HDL-C). CAC progression was defined as either incident CAC in a CAC-free population at baseline, or an increase of ≥2.5 units between the square roots of the baseline and follow-up coronary artery calcium scores (CACS) in subjects with detectable CAC at baseline.ResultsCAC progression was observed in 290 subjects (25.8%) during the mean follow-up of 4.2 years. All subjects were stratified into three groups according to AIP. There were significant differences in cardiovascular parameters among groups at baseline. The follow-up CAC and the incidence of CAC progression increased gradually with rising AIP tertiles. In logistic regression analysis, the odds ratio for CAC progression was 2.27 when comparing the highest to the lowest tertile of AIP (95% CI: 1.61-3.19; P for trend < 0.01). However, this association was attenuated after adjustment for multiple risk factors (P for trend = 0.67).ConclusionsThere is a significant correlation between AIP and the progression of CAC in subjects without CVD. Although AIP was not an independent predictor of CAC progression, AIP should be considered when estimating the current as well as future CVD risk, along with other traditional risk factors.

Project description:The atherogenic index of plasma (AIP) is a recent biomarker linked to atherosclerosis that has been validated as a novel indicator for myocardial infarction (MI). However, the relationship between AIP and the severity of coronary artery disease (CAD) in MI patients is still ambiguous, particularly among individuals with different glucose metabolic conditions. A total of 741 participants who were immediately assessed with coronary angiography upon admission and diagnosed with acute MI were recruited. The severity of CAD was assessed based on the number of narrowed vessels. AIP tertiles were used to divide the patients into three groups (T1: AIP < 0.030; T2: 0.030 ≤ AIP ≤ 0.316; T3: AIP > 0.316). The American Diabetes Association's guidelines define three types of glucose metabolic state: diabetes mellitus (DM), prediabetes (Pre-DM), and normal glucose regulation (NGR). Logistic regression analysis was utilized to confirm an association between AIP and CAD severity in MI patients. ROC curves were employed to evaluate the diagnostic utility of AIP for CAD severity in MI patients. In MI patients, a statistically significant correlation was found between AIP and the severity of CAD, with logistic regression analysis revealing a strong association (OR: 2.055; 95% CI: 1.189-3.550; P = 0.009). Following adjustments for risk factors in the logistic regression model, AIP remained an independent predictor of multi-vessel CAD (OR: 2.902;95% CI: 1.555-5.521 ; P < 0.001). Moreover, compared with the T1 group, the odds ratios for multi-vessel CAD in the T2 and T3 groups were 2.039 (95% CI: 1.321-3.175; P = 0.001) and 2.087 (95% CI: 1.317-3.340; P = 0.001), respectively. The area under the curve for predicting CAD severity with AIP was 0.568 (95% CI: 0.520-0.616; p = 0.006). In addition, a significant association was observed between AIP and an increased risk of multi-vessel CAD in the Pre-DM group. In MI patients, AIP is closely associated with the risk of multi-vessel CAD and the prediction of the severity of CAD. In Pre-DM patients, AIP is clearly associated with the severity of CAD, whereas this association is absent in the NGR and DM groups.
