The medical profession transformed by artificial intelligence: Qualitative study.

ABSTRACT:

Background

Healthcaare delivery will change through the increasing use of artificial intelligence (AI). Physicians are likely to be among the professions most affected, though to what extent is not yet clear.

Objective

We analyzed physicians' and AI experts' stances towards AI-induced changes. This concerned (1) physicians' tasks, (2) job replacement risk, and (3) implications for the ways of working, including human-AI interaction, changes in job profiles, and hierarchical and cross-professional collaboration patterns.

Methods

We adopted an exploratory, qualitative research approach, using semi-structured interviews with 24 experts in the fields of AI and medicine, medical informatics, digital medicine, and medical education and training. Thematic analysis of the interview transcripts was performed.

Results

Specialized tasks currently performed by physicians in all areas of medicine would likely be taken over by AI, including bureaucratic tasks, clinical decision support, and research. However, the concern that physicians will be replaced by an AI system is unfounded, according to experts; AI systems today would be designed only for a specific use case and could not replace the human factor in the patient-physician relationship. Nevertheless, the job profile and professional role of physicians would be transformed as a result of new forms of human-AI collaboration and shifts to higher-value activities. AI could spur novel, more interprofessional teams in medical practice and research and, eventually, democratization and de-hierarchization.

Conclusions

The study highlights changes in job profiles of physicians and outlines demands for new categories of medical professionals considering AI-induced changes of work. Physicians should redefine their self-image and assume more responsibility in the age of AI-supported medicine. There is a need for the development of scenarios and concepts for future job profiles in the health professions as well as their education and training.

Project description:IntroductionArtificial intelligence (AI) has the potential to transform clinical decision-making as we know it. Powered by sophisticated machine learning algorithms, clinical decision support systems (CDSS) can generate unprecedented amounts of predictive information about individuals' health. Yet, despite the potential of these systems to promote proactive decision-making and improve health outcomes, their utility and impact remain poorly understood due to their still rare application in clinical practice. Taking the example of AI-powered CDSS in stroke medicine as a case in point, this paper provides a nuanced account of stroke survivors', family members', and healthcare professionals' expectations and attitudes towards medical AI.MethodsWe followed a qualitative research design informed by the sociology of expectations, which recognizes the generative role of individuals' expectations in shaping scientific and technological change. Semi-structured interviews were conducted with stroke survivors, family members, and healthcare professionals specialized in stroke based in Germany and Switzerland. Data was analyzed using a combination of inductive and deductive thematic analysis.ResultsBased on the participants' deliberations, we identified four presumed roles that medical AI could play in stroke medicine, including an administrative, assistive, advisory, and autonomous role AI. While most participants held positive attitudes towards medical AI and its potential to increase accuracy, speed, and efficiency in medical decision making, they also cautioned that it is not a stand-alone solution and may even lead to new problems. Participants particularly emphasized the importance of relational aspects and raised questions regarding the impact of AI on roles and responsibilities and patients' rights to information and decision-making. These findings shed light on the potential impact of medical AI on professional identities, role perceptions, and the doctor-patient relationship.ConclusionOur findings highlight the need for a more differentiated approach to identifying and tackling pertinent ethical and legal issues in the context of medical AI. We advocate for stakeholder and public involvement in the development of AI and AI governance to ensure that medical AI offers solutions to the most pressing challenges patients and clinicians face in clinical care.

Project description:IntroductionArtificial intelligence (AI) offers several opportunities to enhance medical care, but practical application is limited. Consideration of patient needs is essential for the successful implementation of AI-based systems. Few studies have explored patients' perceptions, especially in Germany, resulting in insufficient exploration of perspectives of outpatients, older patients and patients with chronic diseases. We aimed to explore how patients perceive AI in medical care, focusing on relationships to physicians and ethical aspects.MethodsWe conducted a qualitative study with six semi-structured focus groups from June 2022 to March 2023. We analysed data using a content analysis approach by systemising the textual material via a coding system. Participants were mostly recruited from outpatient settings in the regions of Halle and Erlangen, Germany. They were enrolled primarily through convenience sampling supplemented by purposive sampling.ResultsPatients (N = 35; 13 females, 22 males) with a median age of 50 years participated. Participants were mixed in socioeconomic status and affinity for new technology. Most had chronic diseases. Perceived main advantages of AI were its efficient and flawless functioning, its ability to process and provide large data volume, and increased patient safety. Major perceived disadvantages were impersonality, potential data security issues, and fear of errors based on medical staff relying too much on AI. A dominant theme was that human interaction, personal conversation, and understanding of emotions cannot be replaced by AI. Participants emphasised the need to involve everyone in the informing process about AI. Most considered physicians as responsible for decisions resulting from AI applications. Transparency of data use and data protection were other important points.ConclusionsPatients could generally imagine AI as support in medical care if its usage is focused on patient well-being and the human relationship is maintained. Including patients' needs in the development of AI and adequate communication about AI systems are essential for successful implementation in practice.Patient or public contributionPatients' perceptions as participants in this study were crucial. Further, patients assessed the presentation and comprehensibility of the research material during a pretest, and recommended adaptations were implemented. After each FG, space was provided for requesting modifications and discussion.

Project description:BackgroundTo investigate the attitude and formal suggestions on talent cultivation in the field of medical artificial intelligence (AI).MethodsAn electronic questionnaire was sent to both medical-related field or non-medical field population using the WenJuanXing web-application via social media. The questionnaire was designed to collect: (I) demographic information; (II) perception of medical AI; (III) willingness to participate in the medical AI related teaching activities; (IV) teaching content of medical AI; (V) the role of medical AI teaching; (VI) future career planning. Respondents' anonymity was ensured.ResultsA total of 710 respondents provided valid answers to the questionnaire (57.75% medical related, 42.25% non-medical). About 73.8% of respondents acquired related information from network and social platform. More than half the respondents had basic perception of AI applicational scenarios and specialties in medicine, meanwhile were willing to participate in related general science activities (conference and lectures). Respondents from medical healthcare related fields, with high academic qualifications of male ones demonstrated showed significant better understanding and stronger willingness (P<0.05). The majority agreed medical AI courses should be set as major elective (42.82%) during undergraduate stages (89.58%) involving medical and computer science contents. An overwhelming majority of respondents (>80%) acknowledged the potential roles of medical AI teaching. Surgeon, ophthalmologist, physicians and researchers are the top tier considerations for ideal career regardless of AI influence. Radiology and clinical laboratory subjects are more preferred considering the development of medical AI (P>0.05).ConclusionsThe potential role of medical AI talent cultivation is widely acknowledged by public. Medical related professions demonstrated higher level of perception and stronger willingness for medical AI educational events. Merging subjects as radiology and clinical laboratory subjects are preferred with broad talents demands and bright prospects.

Project description:Objectives: The medical community is in agreement that artificial intelligence (AI) will have a radical impact on patient care in the near future. The purpose of this study is to assess the awareness of AI technologies among health professionals and to investigate their perceptions toward AI applications in medicine. Design: A web-based Google Forms survey was distributed via the Royal Free London NHS Foundation Trust e-newsletter. Setting: Only staff working at the NHS Foundation Trust received an invitation to complete the online questionnaire. Participants: 98 healthcare professionals out of 7,538 (response rate 1.3%; CI 95%; margin of error 9.64%) completed the survey, including medical doctors, nurses, therapists, managers, and others. Primary outcome: To investigate the prior knowledge of health professionals on the subject of AI as well as their attitudes and worries about its current and future applications. Results: 64% of respondents reported never coming across applications of AI in their work and 87% did not know the difference between machine learning and deep learning, although 50% knew at least one of the two terms. Furthermore, only 5% stated using speech recognition or transcription applications on a daily basis, while 63% never utilize them. 80% of participants believed there may be serious privacy issues associated with the use of AI and 40% considered AI to be potentially even more dangerous than nuclear weapons. However, 79% also believed AI could be useful or extremely useful in their field of work and only 10% were worried AI will replace them at their job. Conclusions: Despite agreeing on the usefulness of AI in the medical field, most health professionals lack a full understanding of the principles of AI and are worried about potential consequences of its widespread use in clinical practice. The cooperation of healthcare workers is crucial for the integration of AI into clinical practice and without it the NHS may miss out on an exceptionally rewarding opportunity. This highlights the need for better education and clear regulatory frameworks.
