Sex matters: The impact of oxytocin on healthy conditions and psychiatric disorders.

ABSTRACT: Oxytocin (OT) is involved in the regulation of physiological processes and emotional states, with increasing evidence for its beneficial actions being mediated by the autonomic and immune systems. Growing evidence suggests that OT plays a role in the pathophysiology of different psychiatric disorders. Given the limited information in humans the aim of this study was to retrospectively explore plasma OT levels in psychiatric patients, particularly focusing on sex-related differences, as compared with healthy controls. The patients studied here were divided into three groups diagnosed with obsessive-compulsive disorder (OCD), post-traumatic stress disorder (PTSD) or major depressive disorder (MDD). Plasma OT levels were significantly different between healthy men and women, with the latter showing higher values, while none of the three psychiatric groups showed sex-related differences in the parameters measured here. The intergroup analyses showed that the OT levels were significantly higher in OCD, lower in PTSD and even more reduced in MDD patients than in healthy subjects. These differences were also confirmed when gender was considered, with the exception of PTSD men, in whom OT levels were similar to those of healthy men. The present results indicated that OT levels were higher amongst healthy women than men, while a sex difference was less apparent or reversed in psychiatric patients. Reductions in sex differences in psychopathologies may be related to differential vulnerabilities in processes associated with basic adaptive and social functions.

Project description:In a recent review, it was suggested that much larger cohorts are needed to prove the diagnostic value of neuroimaging biomarkers in psychiatry. While within a sample, an increase of diagnostic accuracy of schizophrenia (SZ) with number of subjects (N) has been shown, the relationship between N and accuracy is completely different between studies. Using data from a recent meta-analysis of machine learning (ML) in imaging SZ, we found that while low-N studies can reach 90% and higher accuracy, above N/2 = 50 the maximum accuracy achieved steadily drops to below 70% for N/2 > 150. We investigate the role N plays in the wide variability in accuracy results in SZ studies (63-97%). We hypothesize that the underlying cause of the decrease in accuracy with increasing N is sample heterogeneity. While smaller studies more easily include a homogeneous group of subjects (strict inclusion criteria are easily met; subjects live close to study site), larger studies inevitably need to relax the criteria/recruit from large geographic areas. A SZ prediction model based on a heterogeneous group of patients with presumably a heterogeneous pattern of structural or functional brain changes will not be able to capture the whole variety of changes, thus being limited to patterns shared by most patients. In addition to heterogeneity (sample size), we investigate other factors influencing accuracy and introduce a ML effect size. We derive a simple model of how the different factors, such as sample heterogeneity and study setup determine this ML effect size, and explain the variation in prediction accuracies found from the literature, both in cross-validation and independent sample testing. From this, we argue that smaller-N studies may reach high prediction accuracy at the cost of lower generalizability to other samples. Higher-N studies, on the other hand, will have more generalization power, but at the cost of lower accuracy. In conclusion, when comparing results from different ML studies, the sample sizes should be taken into account. To assess the generalizability of the models, validation (by direct application) of the prediction models should be tested in independent samples. The prediction of more complex measures such as outcome, which are expected to have an underlying pattern of more subtle brain abnormalities (lower effect size), will require large samples.

Project description:BackgroundPsychodermatological disorders (PDs) and their associations with mental health problems are one of the most frequent research themes in dermatology outpatient settings. Surprisingly, very few studies have been conducted to evaluate PDs among patients with primary psychiatric conditions. As such, the relationship between preexisting psychiatric conditions and comorbid PDs is underrepresented in the literature.ObjectiveThis study examined the prevalence and distribution of PDs among adults with primary psychiatric conditions and determined their association with underlying psychiatric diagnoses.MethodsWe conducted a cross-sectional analysis at a tertiary health care facility in southwestern Nigeria. Comorbid PDs were identified and classified using preexisting classification systems. A bivariate analysis was conducted to determine the association between PDs and underlying psychiatric conditions. The level of statistical significance was set at P<.05.ResultsThe study included 107 patients with mental health disorders, of whom 64 (59.8%) were female. The mean age of the patients was 40.73 (SD 13.08) years. A total of 75 (75/107, 70%) patients had at least one comorbid PD. The prevalence of PDs was highest in patients with affective disorders (15/20, 75%) and least in those with schizophrenia (45/66, 68%). PDs associated with delusions or hallucinations and somatoform symptoms were 9 and 13 times more frequent in patients with anxiety disorders compared to those with other psychiatric conditions (P=.01; odds ratio [OR] 9.88, 95% CI 1.67-58.34 and P=.003; OR 13.13, 95% CI 2.34-73.65), respectively. In contrast, patients with schizophrenia were significantly less likely to be diagnosed with dermatoses resulting from delusions or hallucinations (P=.002; OR 0.04, 95% CI 0.00-0.75). A weak but significant negative association was also found between psychophysiological PDs and anxiety disorders (ϕ=-0.236; P=.02).ConclusionsThis study provides important insights into the overwhelming burden of psychodermatological conditions in patients with mental health disorders and specific associations with underlying psychiatric diagnosis.
