Self-Disclosure and Perceived Responsiveness Among Youth With Asthma: Links to Affect and Anti-Inflammatory Gene Expression.

ABSTRACT: Self-disclosure and perceived responsiveness are important building blocks of social relationships that have long-lasting consequences for health and well-being. However, the conditions under which self-disclosure and responsiveness are likely to benefit health, and how early in life these benefits arise, remain unclear. Among 141 youth (aged 10-17) with asthma, we investigated how average daily levels of self-disclosure and responsiveness are linked to positive and negative affect and the expression of the glucocorticoid receptor gene NR3C1, a marker of improved regulation of stress physiology and immune functioning. Higher levels of self-disclosure were associated with higher NR3C1 expression and positive affect only when perceptions of responsiveness were high. Furthermore, perceived responsiveness was linked to NR3C1 expression for females but not males. These results suggest that the potential benefits of self-disclosure depend on the extent to which interaction partners are perceived as responsive and that these benefits emerge prior to adulthood.

Project description:Recent research has investigated how the sense of relational entitlement (SRE, the extent to which a person expects that his/her needs and wishes will be fulfilled by the romantic partner) diminishes couple satisfaction, but little is known about how SRE affects the daily quality of close, romantic relationships. Moreover, the evidence on how SRE interacts with other features of a satisfying relationship (such as the variables of the interpersonal process model of relationships-self-disclosure, perceived partner disclosure, and perceived partner responsiveness) is scarce. Using an electronic daily diary, we examined 99 couples (198 participants) for 7 days, with two daily measurements for each partner. We used a dyadic double intercept multilevel model, which simultaneously computes effects for men and women. We tested a model where one partner's daily couple satisfaction was predicted by their overall levels of SRE (excessive, restricted, and assertive) and by their daily and overall levels of self-disclosure, perceived partner self-disclosure, and perceived partner responsiveness. The model also included person-level interactions and cross-level interactions between the SRE types and variables of the interpersonal process model of relationships for each gender. The analysis indicated that person-level excessive SRE lowers couple satisfaction. Also, day and person-level perceived partner responsiveness and person-level self-disclosure are related to couple satisfaction, but the latter association is significant only for men. Finally, we found some significant person-level interactions that account for changes in couple satisfaction. For men, the links between couple satisfaction, excessive and restricted SRE were moderated by self-disclosure and perceived partner responsiveness, respectively, perceived partner self-disclosure and perceived partner responsiveness. For women, the associations between couple satisfaction, restricted and assertive SRE were moderated by self-disclosure, respectively, perceived partner self-disclosure. This study advances our understanding of the general implications of SRE in the dynamics of couple relationships. More specifically, it shows how SRE interacts with other couple-specific variables in shaping day-to-day couple satisfaction. The theoretical and clinical implications for couple therapy are discussed.

Project description:BackgroundAsthma is a heterogeneous chronic airway disease, which may be classified into different phenotypes. YKL-40 is a chitin-binding glycoprotein with unclear functions, but its expression is associated with inflammation and tissue remodeling. However, few studies have explored whether YKL-40 is associated with inflammatory phenotypes of asthma.MethodsThe study had two parts. Study I (n = 115) was a one-year prospective cohort designed to explore the relationship of serum YKL-40 levels with inflammatory phenotypes of asthma at baseline, and during exacerbations. Study II (n = 62) was a four-week prospective cohort designed to define whether serum YKL-40 levels could predict responses to a fixed anti-asthma regimen. YKL-40, IL-6 and CCL22 levels were detected using ELISA, and a sputum inflammatory panel (including IL-1β, IL-5, IL-8 and TNF-α) was assessed using Luminex-based MILLIPLEX assay.ResultsStudy I: Serum YKL-40 levels in non-eosinophilic asthma (NEA) i.e. neutrophilic (47.77 [29.59, 74.97] ng/mL, n = 40) and paucigranulocytic (47.36 [28.81, 61.68] ng/mL, n = 31) were significantly elevated compared with eosinophilic asthma (31.05 [22.41, 51.10] ng/mL, n = 44) (P = 0.015). Serum YKL-40levels positively correlated with blood neutrophils, sputum IL-1β and plasma IL-6 but negatively correlated with serum IgE and blood eosinophils (all P ≤ 0.05). Baseline YKL-40 levels predicted moderate to severe exacerbations within a one-year period (aOR = 4.13, 95% CI = [1.08, 15.83]). Study II: Serum YKL-40 was an independent biomarker of negative responses to anti-asthma regimens (adjusted Odds Ratio [aOR] = 0.82, 95% CI = [0.71, 0.96].ConclusionsThese studies show that YKL-40 is a non-type 2 inflammatory signature for NEA, which could predict responsiveness or insensitivity to anti-asthma medications and more exacerbations. Further studies are needed to assess whether monitoring YKL-40 levels could provide potential implications for clinical relevance.
