Quadriceps Muscle Morphology Is an Important Determinant of Maximal Isometric and Crank Torques of Cyclists.

ABSTRACT: The aim of this study was to determine if quadriceps morphology [muscle volume (MV); cross-sectional area (CSA)], vastus lateralis (VL) muscle architecture, and muscle quality [echo intensity (ECHO)] can explain differences in knee extensor maximal voluntary isometric contraction (MVIC), crank torque (CT) and time-to-exhaustion (TTE) in trained cyclists. Twenty male competitive cyclists performed a maximal incremental ramp to determine their maximal power output (PO_MAX). Muscle morphology (MV; CSA), muscle architecture of VL and muscle quality (ECHO) of both quadriceps muscles were assessed. Subsequently, cyclists performed three MVICs of both knee extensor muscles and finally performed a TTE test at PO_MAX with CT measurement during TTE. Stepwise multiple regression results revealed right quadriceps MV determined right MVIC (31%) and CT (33%). Left MV determined CT (24%); and left VL fascicle length (VL-FL) determined MVIC (64%). However, quadriceps morphological variables do not explain differences in TTE. No significant differences were observed between left and right quadriceps muscle morphology (p > 0.05). The findings emphasize that quadriceps MV is an important determinant of knee extensor MVIC and CT but does not explain differences in TTE at PO_MAX. Furthermore, quadriceps morphological variables were similar between the left and right quadriceps in competitive cyclists.

Project description:IntroductionKnee osteoarthrosis (KOA) is commonly associated with a dysfunction of the quadriceps muscle which contributes to alterations in motor performance. The underlying neuromuscular mechanisms of muscle dysfunction are not fully understood. The main objective of this study was to analyze how KOA affects neuromuscular function of the quadriceps muscle during different contraction intensities.Materials and methodsThe following parameters were assessed in 20 patients and 20 healthy controls: (i) joint position sense, i.e. position control (mean absolute error, MAE) at 30° and 50° of knee flexion, (ii) simple reaction time task performance, (iii) isometric maximal voluntary torque (IMVT) and root mean square of the EMG signal (RMS-EMG), (iv) torque control, i.e. accuracy (MAE), absolute fluctuation (standard deviation, SD), relative fluctuation (coefficient of variation, CV) and periodicity (mean frequency, MNF) of the torque signal at 20%, 40% and 60% IMVT, (v) EMG-torque relationship at 20%, 40% and 60% IMVT and (vi) performance fatigability, i.e. time to task failure (TTF) at 40% IMVT.ResultsCompared to the control group, the KOA group displayed: (i) significantly higher MAE of the angle signal at 30° (99.3%; P = 0.027) and 50° (147.9%; P < 0.001), (ii) no significant differences in reaction time, (iii) significantly lower IMVT (-41.6%; P = 0.001) and tendentially lower RMS-EMG of the rectus femoris (-33.7%; P = 0.054), (iv) tendentially higher MAE of the torque signal at 20% IMVT (65.9%; P = 0.068), significantly lower SD of the torque signal at all three torque levels and greater MNF at 60% IMVT (44.8%; P = 0.018), (v) significantly increased RMS-EMG of the vastus lateralis at 20% (70.8%; P = 0.003) and 40% IMVT (33.3%; P = 0.034), significantly lower RMS-EMG of the biceps femoris at 20% (-63.6%; P = 0.044) and 40% IMVT (-41.3%; P = 0.028) and tendentially lower at 60% IMVT (-24.3%; P = 0.075) and (vi) significantly shorter TTF (-51.1%; P = 0.049).ConclusionKOA is not only associated with a deterioration of IMVT and neuromuscular activation, but also with an impaired position and torque control at submaximal torque levels, an altered EMG-torque relationship and a higher performance fatigability of the quadriceps muscle. It is recommended that the rehabilitation includes strengthening and fatiguing exercises at maximal and submaximal force levels.

Project description:BackgroundDifferent field tests are used to evaluate muscle capacity, in particular maximal voluntary isometric endurance. However, although there are some normative values for a few muscle endurance tests, these do not consider the weight, height, gender, or age of individuals, which are well-known factors that influence muscle performance.Hypothesis/purposeThe purpose of this study was to investigate the test-retest reproducibility of eight field tests and establish muscle endurance norms, in a healthy population, based on their anthropometric characteristics, which could allow the optimal evaluation of the entire muscle function in a quick manner.DesignCase series.MethodsThis study was conducted in two phases. The first phase was to check the reproducibility inter- and intra-assessor for eight isometric muscle field tests on 20 volunteer subjects aged 40.9 ± 11.6 years old (age range, 21-58 years). The second part was to establish muscle maximal voluntary isometric endurance norms according to these tests on a total of 400 healthy participants grouped by age (50 males and females in each of the age brackets, 20-29; 30-39; 40-49; 50-59 years old, for a total of 200 males and 200 females).ResultsThe intra- and inter-assessor reproducibility tests are good for all muscle measurements (the intraclass correlation coefficients varied between 0.915 and 0.996 and the coefficient of variation between 3.6 and 11.8%). The area under the receiver operating characteristic curves demonstrates a good sensibility with values greater than 0.7 for each test. Each muscle belt presents same ratio regardless of the age and gender group. The simultaneous multiple regression analyses highlight that the anthropometric characteristics of subjects influence significantly the performance of isometric tests.ConclusionThis study has permitted establishing prediction equations in a healthy population according to their anthropometric characteristics as well as agonist/antagonist ratios for eight muscle isometric field tests after demonstrating a good reproducibility of all tests.

Project description:BackgroundMuscle force testing is one of the more common categories of diagnostic tests used in clinical practice. Clinicians have little evidence to guide interpretations of muscle force tests when pain is elicited during testing.ObjectiveThe purpose of this study was to examine the construct validity of isometric quadriceps muscle strength tests by determining whether the relationship between maximal isometric quadriceps muscle strength and functional status was influenced by pain during isometric testing.DesignA cross-sectional design was used.MethodsData from the Osteoarthritis Initiative were used to identify 1,344 people with unilateral knee pain and Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) pain subscale scores of 1 or higher on the involved side. Measurements of maximal isometric quadriceps strength and ratings of pain during isometric testing were collected. Outcome variables were WOMAC physical function subscale, 20-m walk test, 400-m walk test, and a repeated chair stand test. Multiple regression models were used to determine whether pain during testing modified or confounded the relationship between strength and functional status.ResultsPearson r correlations among the isometric quadriceps strength measures and the 4 outcome measures ranged from -.36 (95% confidence interval=-.41, -.31) for repeated chair stands to .36 (95% confidence interval=.31, .41) for the 20-m walk test. In the final analyses, neither effect modification nor confounding was found for the repeated chair stand test, the 20-m walk test, the 400-m walk test, or the WOMAC physical function subscale. Moderate or severe pain during testing was weakly associated with reduced strength, but mild pain was not.LimitationsThe disease spectrum was skewed toward mild or moderate symptoms, and the pain measurement scale used during muscle force testing was not ideal.ConclusionsGiven that the spectrum of the sample was skewed toward mild or moderate symptoms and disease, the data suggest that isometric quadriceps muscle strength tests maintain their relationship with self-report or performance-based disability measures even when pain is elicited during testing.

Project description:BackgroundMaximal isometric finger dead-hangs are used in rock climbing to strengthen finger flexors. Although various grip positions are often used when performing finger dead-hangs, little is known regarding how these grip positions can affect forearm muscle activity. Understanding how forearm muscles are recruited during dead-hangs could help foreseeing the potential for training of different grip positions. The aim of the present study was to explore the training applications of the various grip positions by comparing the activity of forearm muscles during maximal dead-hangs in rock climbers.Materials & methodsTwenty-five climbers performed maximal dead-hangs in three climbing-specific grip positions: CRIMP, SLOPE, and SLOPER. We recorded the maximal loads used and the sEMG of the flexor digitorum profundus (FDP), the flexor digitorum superficialis (FDS), the flexor carpi radialis (FCR), and the extensor digitorum communis (EDC). Individual and global (sum of all muscles) root mean square (RMS) and neuromuscular efficiency (NME) values were computed. Repeated measures analysis were performed to assess grip differences (p < 0.05).ResultsSLOPER showed the largest maximal load values among the three grip positions (p < 0.001, d ≥ 2.772). Greater global (p ≤ 0.044, d ≥ 0.268), FDS (p ≤ 0.005, d ≥ 0.277), and FCR (p < 0.001, d ≥ 1.049) activity was observed for the SLOPER compared to CRIMP and SLOPE, while EDC (p ≤ 0.005, d ≥ 0.505) showed lower activity in the SLOPER compared to the other two grip positions. SLOPER presented the highest global (p < 0.001, d ≥ 0.629), FDP (p < 0.001, d ≥ 0.777), FDS (only CRIMP vs SLOPER: p < 0.001, d = 0.140), and EDC NME (p < 0.001, d ≥ 1.194). The CRIMP showed greater FDS activity (p = 0.001, d = 0.386) and lower NME (p = 0.003, d = 0.125) compared to SLOPE.ConclusionsThese results revealed that, under maximum intensity conditions, SLOPER could stimulate the FDS and FCR better than the other grip positions at the expense of using greater loads. Similarly, maximum CRIMP dead-hang could better stimulate the FDS than the SLOPE, even when using similar loads.
