Project description:Although increasing evidence suggests that abnormal femur geometry in developmental dysplasia of the hip (DDH) may contribute to intra-articular damage and the development of hip osteoarthritis, a comprehensive 3D description of femoral abnormalities in DDH remains incomplete. Statistical shape modeling (SSM) was used to quantify three-dimensional (3D) geometric variation among femurs in female patients with DDH and control subjects. SSM correspondence points (n = 8,192) were placed on each femur using a gradient descent energy function to derive mean DDH and control femoral shapes and principal component analysis (PCA) was then used to describe shape variation. PCA results were associated with common 2D radiographic measures of femur shape using general linear models. For patients with DDH, the first eight principal components (modes) captured 90.9% of the cumulative variance accounted for (VAF). Notably, mode 2 captured 23.6% VAF and described variation in femoral version, the neck-shaft angle, and femoral neck length, while mode 3 captured 16.4% VAF and described variation in femoral version, femoral head size, and femoral offset. SSM captured complex geometric deformities in DDH, which may not be fully described by 2D measures of the acetabulum and proximal femur alone. By determining the primary shape variations among femurs in cases of DDH, SSM may further understanding of pathologies on the femoral side of dysplastic hips, in context with more commonly recognized acetabular deformities. © 2019 Orthopaedic Research Society. Project description:Knowledge about anatomical shape variations in the pelvis is mandatory for selection, fitting, positioning, and fixation in pelvic surgery. The current knowledge on pelvic shape variation mostly relies on point-to-point measurements on 2D X-ray images and computed tomography (CT) slices. Three-dimensional region-specific assessments of pelvic morphology are scarce. Our aim was to develop a statistical shape model of the hemipelvis to assess anatomical shape variations in the hemipelvis. CT scans of 200 patients (100 male and 100 female) were used to obtain segmentations. An iterative closest point algorithm was performed to register these 3D segmentations, so a principal component analysis (PCA) could be performed, and a statistical shape model (SSM) of the hemipelvis was developed. The first 15 principal components (PCs) described 90% of the total shape variation, and the reconstruction ability of this SSM resulted in a root mean square error of 1.58 (95% CI: 1.53-1.63) mm. In summary, an SSM of the hemipelvis was developed, which describes the shape variations in a Caucasian population and is able to reconstruct an aberrant hemipelvis. Principal component analyses demonstrated that, in a general population, anatomical shape variations were mostly related to differences in the size of the pelvis (e.g., PC1 describes 68% of the total shape variation, which is attributed to size). Differences between the male and female pelvis were most pronounced in the iliac wing and pubic rami regions. These regions are often subject to injuries. Future clinical applications of our newly developed SSM may be relevant for SSM-based semi-automatic virtual reconstruction of a fractured hemipelvis as part of preoperative planning. Lastly, for companies, using our SSM might be interesting in order to assess which sizes of pelvic implants should be produced to provide proper-fitting implants for most of the population.

Project description:IntroductionIntra- and inter-population variations of bone morphology have made the process of designing an anatomically well-fitting fracture fixation plate challenging. Although statistical bone models have recently been used for analysing morphological variabilities, it is not known to what extent they would also provide the basis for the design of a new plate shape. This would be particularly valuable in the case where no existing plate shape is available to start the process of fit optimisation. Therefore, this study investigated the anatomical fitting of a plate shape (statistical plate) derived from the mean shape of a statistical 3D tibia bone model in comparison to results available from two other plate shapes.MethodsForty-five 3D bone models of tibiae from Japanese cadaver specimens, as well as 3D models of the plate undersurface of both a commercial and shape optimised Medial Distal Tibia Plate, were utilised from earlier studies. The mean shape of the 3D statistical bone model was generated from the tibia models utilising the Statismo framework. With reverse engineering software, the plate undersurface of the statistical plate shape was derived directly from the mean surface of the statistical 3D bone model. Through an iterative process, the statistical plate model was placed at the correct surgical position on each bone model for fit assessment.ResultsThe statistical plate was fitting for 20% of the tibiae compared to 13% for the commercial and 67% for the optimised plate, respectively.ConclusionsThe plate shape derived directly from a statistical bone model was fitting better than the commercial plate, but considerably inferior to that of an optimised plate. However, the results do clearly indicate that this approach provides an appropriate and solid basis for commencing shape optimisation of the statistical plate. Studies of other anatomical regions are required to confirm whether these findings can be generalised.

Project description:We develop a multivariate analysis of brain anatomy to identify the relevant shape deformation patterns and quantify the shape changes that explain corresponding variations in clinical neuropsychological measures. We use kernel Partial Least Squares (PLS) and formulate a regression model in the tangent space of the manifold of diffeomorphisms characterized by deformation momenta. The scalar deformation momenta completely encode the diffeomorphic changes in anatomical shape. In this model, the clinical measures are the response variables, while the anatomical variability is treated as the independent variable. To better understand the "shape-clinical response" relationship, we also control for demographic confounders, such as age, gender, and years of education in our regression model. We evaluate the proposed methodology on the Alzheimer's Disease Neuroimaging Initiative (ADNI) database using baseline structural MR imaging data and neuropsychological evaluation test scores. We demonstrate the ability of our model to quantify the anatomical deformations in units of clinical response. Our results also demonstrate that the proposed method is generic and generates reliable shape deformations both in terms of the extracted patterns and the amount of shape changes. We found that while the hippocampus and amygdala emerge as mainly responsible for changes in test scores for global measures of dementia and memory function, they are not a determinant factor for executive function. Another critical finding was the appearance of thalamus and putamen as most important regions that relate to executive function. These resulting anatomical regions were consistent with very high confidence irrespective of the size of the population used in the study. This data-driven global analysis of brain anatomy was able to reach similar conclusions as other studies in Alzheimer's disease based on predefined ROIs, together with the identification of other new patterns of deformation. The proposed methodology thus holds promise for discovering new patterns of shape changes in the human brain that could add to our understanding of disease progression in neurological disorders.

Project description:The aim was to develop a morphometric and anatomically accurate atlas (statistical shape model) of calcaneus. The model is based on 18 left foot and 18 right foot computed tomography studies of 28 male individuals aged from 17 to 62 years, with no known foot pathology. A procedure for automatic atlas included extraction and identification of common features, averaging feature position, obtaining mean geometry, mathematical shape description and variability analysis. Expert manual assistance was included for the model to fulfil the accuracy sought by medical professionals. The proposed for the first time statistical shape model of the calcaneus could be of value in many orthopaedic applications including providing support in diagnosing pathological lesions, pre-operative planning, classification and treatment of calcaneus fractures as well as for the development of future implant procedures.

Project description:BackgroundBone shapes strongly influence force and moment predictions of kinematic and musculoskeletal models used in motion analysis. The precise determination of joint reference frames is essential for accurate predictions. Since clinical motion analysis typically does not include medical imaging, from which bone shapes may be obtained, scaling methods using reference subjects to create subject-specific bone geometries are widely used.Research questionThis study investigated if lower limb bone shape predictions from skin-based measurements, utilising an underlying statistical shape model (SSM) that corrects for soft tissue artefacts in digitisation, can be used to improve conventional linear scaling methods of bone geometries.MethodsSSMs created from 35 healthy adult femurs and tibiae/fibulae were used to reconstruct bone shapes by minimising the distance between anatomical landmarks on the models and those digitised in the motion laboratory or on medical images. Soft tissue artefacts were quantified from magnetic resonance images and then used to predict distances between landmarks digitised on the skin surface and bone. Reconstruction results were compared to linearly scaled models by measuring root mean squared distances to segmented surfaces, calculating differences of commonly used anatomical measures and the errors in the prediction of the hip joint centre.ResultsSSM reconstructed surface predictions from varying landmark sets from skin and bone landmarks were more accurate compared to linear scaling methods (2.60-2.95 mm vs. 3.66-3.87 mm median error; p < 0.05). No significant differences were found between SSM reconstructions from bony landmarks and SSM reconstructions from digitised landmarks obtained in the motion lab and therefore reconstructions using skin landmarks are as accurate as reconstructions from landmarks obtained from medical images.SignificanceThese results indicate that SSM reconstructions can be used to increase the accuracy in obtaining bone shapes from surface digitised experimental data acquired in motion lab environments.

Project description:BackgroundCraniosynostosis is a condition caused by the premature fusion of skull sutures, leading to irregular growth patterns of the head. Three-dimensional photogrammetry is a radiation-free alternative to the diagnosis using computed tomography. While statistical shape models have been proposed to quantify head shape, no shape-model-based classification approach has been presented yet.MethodsWe present a classification pipeline that enables an automated diagnosis of three types of craniosynostosis. The pipeline is based on a statistical shape model built from photogrammetric surface scans. We made the model and pathology-specific submodels publicly available, making it the first publicly available craniosynostosis-related head model, as well as the first focusing on infants younger than 1.5 years. To the best of our knowledge, we performed the largest classification study for craniosynostosis to date.ResultsOur classification approach yields an accuracy of 97.8 %, comparable to other state-of-the-art methods using both computed tomography scans and stereophotogrammetry. Regarding the statistical shape model, we demonstrate that our model performs similar to other statistical shape models of the human head.ConclusionWe present a state-of-the-art shape-model-based classification approach for a radiation-free diagnosis of craniosynostosis. Our publicly available shape model enables the assessment of craniosynostosis on realistic and synthetic data.

Project description:BackgroundVariation in tibia geometry is a risk factor for tibial stress fractures. Geometric variability in bones is often quantified using statistical shape modelling. Statistical shape models (SSM) offer a method to assess three-dimensional variation of structures and identify the source of variation. Although SSM have been used widely to assess long bones, there is limited open-source datasets of this kind. Overall, the creation of SSM can be an expensive process, that requires advanced skills. A publicly available tibia shape model would be beneficial as it enables researchers to improve skills. Further, it could benefit health, sport and medicine with the potential to assess geometries suitable for medical equipment, and aid in clinical diagnosis. This study aimed to: (i) quantify tibial geometry using a SSM; and (ii) provide the SSM and associated code as an open-source dataset.MethodsLower limb computed tomography (CT) scans from the right tibia-fibula of 30 cadavers (male n = 20, female n = 10) were obtained from the New Mexico Decedent Image Database. Tibias were segmented and reconstructed into both cortical and trabecular sections. Fibulas were segmented as a singular surface. The segmented bones were used to develop three SSM of the: (i) tibia; (ii) tibia-fibula; and (iii) cortical-trabecular. Principal component analysis was applied to obtain the three SSM, with the principal components that explained 95% of geometric variation retained.ResultsOverall size was the main source of variation in all three models accounting for 90.31%, 84.24% and 85.06%. Other sources of geometric variation in the tibia surface models included overall and midshaft thickness; prominence and size of the condyle plateau, tibial tuberosity, and anterior crest; and axial torsion of the tibial shaft. Further variations in the tibia-fibula model included midshaft thickness of the fibula; fibula head position relative to the tibia; tibia and fibula anterior-posterior curvature; fibula posterior curvature; tibia plateau rotation; and interosseous width. The main sources of variation in the cortical-trabecular model other than general size included variation in the medulla cavity diameter; cortical thickness; anterior-posterior shaft curvature; and the volume of trabecular bone in the proximal and distal ends of the bone.ConclusionVariations that could increase the risk of tibial stress injury were observed, these included general tibial thickness, midshaft thickness, tibial length and medulla cavity diameter (indicative of cortical thickness). Further research is needed to better understand the effect of these tibial-fibula shape characteristics on tibial stress and injury risk. This SSM, the associated code, and three use examples for the SSM have been provided in an open-source dataset. Project description:Injury is an important risk factor for osteoarthritis (OA), a highly prevalent and disabling joint disease. Joint shape is linked to OA, but the interplay of injury and joint shape and their combined role in OA, particularly at the ankle, is not well known. Therefore, we explored cross-sectional associations between ankle shape and injury in a large community-based cohort.Ankles without radiographic OA were selected from the current data collection of the Johnston County OA Project. Ankles with self-reported prior injury were included as injury cases (n = 108) along with 1:1 randomly selected non-injured ankles. To define ankle shape, a 68 point model on weight-bearing lateral ankle radiographs was entered into a statistical shape model, producing a mean shape and a set of continuous variables (modes) representing variation in that shape. Nineteen modes, explaining 80% of shape variance, were simultaneously included in a logistic regression model with injury status as the dependent variable, adjusted for intra-person correlation, sex, race, body mass index (BMI), baseline OA radiographic grade, and baseline symptoms.A total of 194 participants (213 ankles) were included; mean age 71 years, BMI 30 kg/m2, 67% white and 71% women. Injured ankles were more often symptomatic and from whites. In a model adjusted only for intra-person correlation, associations were seen between injury status and modes 1, 6, 13, and 19. In a fully adjusted model, race strongly affected the estimate for mode 1 (which was no longer statistically significant).This study showed variations in ankle shape and history of injury as well as with race. These novel findings may indicate a change in ankle morphology following injury, or that ankle morphology predisposes to injury, and suggest that ankle shape is a potentially important factor in the development of ankle OA.

Project description:BackgroundTibialis anterior (TA) muscle is the largest dorsiflexor of the ankle joint and plays an important role during gait movement. However, descriptions of the TA attachment site are inconsistent even among major anatomy textbooks, and its origin, especially the attachment site for the tibia, has not been reported in detail. This study is the first experimental attempt to investigate the origin of the TA in detail, paying particular attention to the relationship with the shape of the tibia, including sex differences.MethodsForty legs (20 males, 20 females) from twenty Japanese cadavers were examined. Gross anatomical examination of the TA's attachment site to the tibia and the tibia's shape was performed.ResultsThe location of the distal end of the TA's attachment on tibia was significantly more distal in males than in females (p < 0.01). The anterior border of the tibia had a gentle S-like curve, with a medially convex curve in the proximal region and a laterally convex curve in the distal region in frontal plane. The most protruding point of the distal curve of the anterior border located significantly more proximal in females than in males (p = 0.02).ConclusionsThere were sex differences in the distal end of the attachment site on tibia of the TA and the shape of the tibia. Consequently, the variations in the attachment site of TA were considered to provide for differences in function of TA. In males, the TA may enable advantageous power exertion, whereas in females it may work efficiently for dorsiflexion of ankle, respectively. Sex differences in TA's attachment site and the shape of the tibia may be involved in gait movement as well as frequency of lower leg disorders such as chronic exertional compartment syndrome.
