Project description:A catalytic role has been proposed in neoplastic angiogenesis and cancer progression for bone marrow-derived endothelial progenitor cells (EPCs). However, in preclinical and clinical studies the quantitative role of marrow-derived EPCs in cancer vascularization was found to be extremely variable. Adipose tissue represents an attractive source of autologous adult stem cells due to its abundance and surgical accessibility. CD34+cells from Lipotransfer aspirates (LAs) of patients undergoing breast reconstruction after breast cancer surgery were compared with CD34+ cells from Leucapheresis of normal subjects. Two samples of purified CD34+ cells from Lipotransfer Aspirates (LAs) were compared with two samples of CD34+ cells from Leucapheresis samples. Project description:ImportanceAs disease-modifying treatment options for multiple sclerosis increase, comparisons of the options based on real-world evidence may guide clinical decision-making.ObjectiveTo compare the relapse outcomes between 2 pairs of disease-modifying treatments: dimethyl fumarate vs fingolimod and natalizumab vs rituximab.Design, setting, and participantsThis comparative effectiveness study integrated data from a clinic-based multiple sclerosis research registry and its linked electronic health records (EHR) system between January 1, 2006, and December 31, 2016, and built treatment groups for each pairwise disease-modifying treatment comparison according to both registry records and electronic prescriptions. Parallel analyses were conducted from October 11, 2019, to July 7, 2021.Main outcomes and measuresThe main outcomes were the 1-year and 2-year relapse rates as well as the time to relapse. To compare relapse outcomes, the study adjusted for covariates from 2 sources (registry and EHR) and corrected for confounding biases among the covariates by the doubly robust estimation.ResultsThe study included 4 treatment groups: dimethyl fumarate (n = 260; 198 women [76.2%]; 227 non-Hispanic White individuals [87.3%]; mean [SD] age at diagnosis, 41.7 [10.4] years), fingolimod (n = 267; 190 women [71.2%]; 222 non-Hispanic White individuals [83.1%]; mean [SD] age at diagnosis, 37.9 [9.9] years), natalizumab (n = 204; 160 women [78.4%]; 172 non-Hispanic White individuals [84.3%]; mean [SD] age at diagnosis, 37.2 [10.6] years), and rituximab (n = 115; 83 women [72.2%]; 99 non-Hispanic White individuals [86.1%]; mean [SD] age at diagnosis, 44.1 [11.1] years). No significant differences were found in the relapse outcomes between dimethyl fumarate and fingolimod after correcting for confounding biases and multiple testing (difference in 1-year relapse rate, 0.028 [95% CI, -0.031 to 0.084]; difference in 2-year relapse rate, 0.071 [95% CI, 0.008-0.128]; relative risk of 2-year non-relapse, 0.957 [95% CI, 0.884-1.035] with dimethyl fumarate as reference). When compared with rituximab, natalizumab was associated with a higher relapse rate for all 3 outcomes after bias correction and multiple testing (difference in 1-year relapse rate, 0.080 [95% CI, 0.013-0.137]; difference in 2-year relapse rate, 0.132 [95% CI, 0.043-0.189]; relative risk of 2-year non-relapse, 0.903 [95% CI, 0.822-0.944]). Confounders were identified from EHR data not recorded in the registry data through data-driven feature selection.Conclusions and relevanceThis study reports real-world evidence of equivalent relapse outcomes between dimethyl fumarate and fingolimod and relapse reduction in favor of rituximab relative to natalizumab. This approach illustrates the value of incorporating EHR data as high-dimensional covariates in real-world treatment comparison.

Project description:ImportanceConflicting evidence exists on the association between azithromycin use and cardiac events.ObjectiveTo compare the odds of cardiac events among new users of azithromycin relative to new users of amoxicillin using real-world data.Design, setting, and participantsThis retrospective cohort study used data from Truven Health Analytics MarketScan database from January 1, 2009, to June 30, 2015. Patients receiving either amoxicillin or azithromycin and enrolled in a health care plan 365 days before (baseline period) the dispensing date (index date) were included in the study. Patients were matched 1:1 on high-dimensional propensity scores. Data were analyzed from October 1, 2018, to December 31, 2019.ExposuresNew use of azithromycin compared with new use of amoxicillin.Main outcomes and measuresThe primary outcome consisted of cardiac events, including syncope, palpitations, ventricular arrhythmias, cardiac arrest, or death as a primary diagnosis for hospitalization at 5, 10, and 30 days from the index date. Logistic regression models were used to estimate odds ratios (ORs) with 95% CIs.ResultsAfter matching, the final cohort included 2 141 285 episodes of each index therapy (N = 4 282 570) (mean [SD] age of patients, 35.7 [22.3] years; 52.6% female). Within 5 days after therapy initiation, 1474 cardiac events (0.03%) occurred (708 in the amoxicillin cohort and 766 in the azithromycin cohort). The 2 most frequent events were syncope (1032 [70.0%]) and palpitations (331 [22.5%]). The odds of cardiac events with azithromycin compared with amoxicillin were not significantly higher at 5 days (OR, 1.08; 95% CI, 0.98-1.20), 10 days (OR, 1.05; 95% CI, 0.97-1.15), and 30 days (OR, 0.98; 95% CI, 0.92-1.04). Among patients receiving any concurrent QT-prolonging drug, the odds of cardiac events with azithromycin were 1.40 (95% CI, 1.04-1.87) greater compared with amoxicillin. Among patients 65 years or older and those with a history of cardiovascular disease and other risk factors, no increased risk of cardiac events with azithromycin was noted.Conclusions and relevanceThis study found no association of cardiac events with azithromycin compared with amoxicillin except among patients using other QT-prolonging drugs concurrently. Although azithromycin is a safe therapy, clinicians should carefully consider its use among patients concurrently using other QT-prolonging drugs.

Project description:"Diminishing returns" in leaf economics occurs when increases in lamina mass (M), which can either be represented by lamina dry mass (DM) or fresh mass (FM), fail to produce proportional increases in leaf surface area (A), such that the scaling exponent (α) for the M vs. A scaling relationship exceeds unity (i.e., α > 1.0). Prior studies have shown that FM vs. A is better than DM vs A in assessing diminishing returns in evergreen species. However, the superiority of FM vs. A over DM vs. A has been less well examined for deciduous species. Here, we applied reduced major axis protocols to test whether FM vs. A is better than DM vs. A to describe the M vs. A scaling relationship, using a total of 4271 leaves from ten deciduous and two evergreen tree species in the Fagaceae and Ulmaceae for comparison. The significance of the difference between the scaling exponents of FM vs. A and DM vs. A was tested using the bootstrap percentile method. Further, we tested the non-linearity of the FM (DM) vs. A data on a log-log scale using ordinary least squares. We found that (i) the majority of scaling exponents of FM vs. A and DM vs. A were >1 thereby confirming diminishing returns for all 12 species, (ii) FM vs. A was more robust than DM vs. A to identify the M vs. A scaling relationship, (iii) the non-linearity of the allometric model was significant for both DM vs. A and FM vs. A., and (iv) the evergreen species of Fagaceae had significantly higher DM and FM per unit area than other deciduous species. In summary, FM vs. A is a more reliable measure than DM vs. A when dealing with diminishing returns, and deciduous species tend to invest less biomass in unit leaf light harvesting area than evergreen species.

Project description:Different polymer architectures behave differently regarding their dynamics. We have used a combination of dielectric spectroscopy, and fast field cycling nuclear magnetic resonance (NMR) to compare the dynamical behavior of two different polymer architectures, with similar overall molecular weight. The systems of interest are a bottlebrush polymer and a linear one, both based on poly(dimethylsiloxane) (PDMS). To verify the structure of the PDMS-g-PDMS bottlebrush in the melt, small-angle neutron scattering was used, yielding a spherical shape. Information about the segmental dynamics was revealed by dielectric spectroscopy and extended to higher temperatures by fast field cycling NMR. One advantage of fast field cycling NMR is the detection of large-scale chain dynamics, which dielectric spectroscopy cannot probe for PDMS. While segmental relaxation seems to be independent of the architecture, the large-scale chain dynamics show substantial differences, as represented by the mean square displacement. Here, two regions are detected for each polymer. The linear polymer shows the Rouse regime, followed by reptation. In contrast, the bottlebrush polymer performs Rouse dynamics and diffusion in the available time window, and entanglement effects are completely missing.

Project description:Although several randomized clinical trials have reported the potential benefit of sodium-glucose cotransporter 2 inhibitors (SGLT2i) in reducing blood pressure (BP), whether SGLT2i can reduce incident hypertension is unknown. We analyzed individuals with diabetes who were newly prescribed SGLT2i or dipeptidyl peptidase 4 inhibitors (DPP4i) in a large-scale epidemiological database. The primary outcome was the incidence of hypertension. A propensity score matching algorithm was employed to compare the subsequent development of hypertension between the SGLT2i and DPP4i groups. After propensity score matching, 5708 well-balanced pairs of SGLT2i and DPP4i users were identified. SGLT2i administration was associated with a reduced risk of hypertension (HR 0.91, 95% CI: 0.84-0.97). The advantage of SGLT2i use over DPP4i use for incident hypertension was generally consistent in several sensitivity analyses, and subgroup analyses showed that SGLT2i use was significantly associated with a lower risk of hypertension in men, patients with baseline HbA1c of <7.5%, and baseline systolic blood pressure ≥127 mmHg. Our investigation using nationwide real-world data demonstrated the potential advantage of SGLT2i over DPP4i in reducing the development of hypertension in individuals with diabetes.

Project description:The effects of two types of auditory distracters (steady-state noise vs. four-talker babble) on visual-only speechreading accuracy were tested against a baseline (silence) in 23 participants with above-average speechreading ability. Their task was to speechread high frequency Swedish words. They were asked to rate their own performance and effort, and report how distracting each type of auditory distracter was. Only four-talker babble impeded speechreading accuracy. This suggests competition for phonological processing, since the four-talker babble demands phonological processing, which is also required for the speechreading task. Better accuracy was associated with lower self-rated effort in silence; no other correlations were found.

Project description:AimsThe Digitalis Investigation Group (DIG) trial, the only large randomized trial of digoxin in heart failure, reported a neutral effect on mortality and a significant reduction in heart failure hospitalizations. Recent observational studies reported increased mortality with digoxin treatment. We present further analyses of the DIG trial displaying the inability to control bias in observational treatment comparisons despite extensive statistical adjustments.Methods and resultsForty-four percent of the 6800 patients in the DIG trial had been treated with digoxin before randomization, and half of them were randomly withdrawn from digoxin treatment. We contrast the main randomization-based result of the DIG trial with the observational non-randomized comparison of patients pre-treated or not pre-treated with digoxin. Mortality [hazard ratio (HR) 1.22, 95% confidence interval (CI) 1.12-1.34; P < 0.001] and heart failure hospitalizations (HR 1.47, 95% CI 1.33-1.61; P < 0.001) were significantly higher in patients pre-treated with digoxin even after adjustment for baseline population differences. The higher risks for both outcomes in those who had previously received digoxin persisted even if they received placebo during the trial (HR 1.24, 95% CI 1.10-1.40; P < 0.001). This sharply contradicts the neutral effect on mortality and the significant reduction in heart failure hospitalizations observed in the randomized comparison.ConclusionPrescription of digoxin is an indicator of disease severity and worse prognosis, which cannot be fully accounted for by covariate adjustments in the DIG trial where patients were well-characterized. It is unlikely that weaker research approaches (observational studies of administrative data or registries) can provide more reliable estimates of the effects of cardiac glycosides.
