Treatment of pseudoaneurysm of internal maxillary artery: a case report.

ABSTRACT: Pseudoaneurysms are rare lesions secondary to blunt or penetrating trauma, temporomandibular joint surgery, or orthognathic surgery. Nonsurgical interventions are the treatment of choice for pseudoaneurysms. In the case reported here, endovascular injection of acrylic glue was successful in the treatment of a pseudoaneurysm of the internal maxillary artery secondary to fracture of the zygomatic bone.

The role of interventional radiology is immaculate in the management of these aneurysms.

Pulmonary Artery Pseudoaneurysm due to Mucormycosis: Case Report and Literature Review

Project description:Pulmonary artery pseudoaneurysm (PAP) is a rare vascular phenomenon with a high mortality rate, as these entities can enlarge, rupture, and lead to asphyxiation. Pulmonary mucormycosis (PM), an underdiagnosed but an increasingly seen entity in the era of chemotherapy and immunosuppression, is a known cause of PAP, and should be suspected in immunosuppressed patients with hemoptysis. We present a case of PAP due to PM in a patient with recently diagnosed diffuse large B-cell lymphoma of the liver who underwent chemotherapy and developed acute cavitary lung disease and hemoptysis. His diagnosis was delayed due to the withholding of iodinated contrast with computer tomography (CT) imaging in the setting of renal failure. He then underwent embolization of his PAP with resolution of his hemoptysis. PAP is an uncommon cause of hemoptysis that can be diagnosed with CT pulmonary angiography, and mucormycosis is a known but rare cause of PAP in patients with malignancy receiving immunosuppression.

Left maxillary artery pseudoaneurysm: a rare and late postoperative complication after head and neck cancer treatment.

Project description:We report a case of a 50-year-old man presented with pulsatile swelling in the left side of the face since 2 months and a continuous sinus discharge since 3 days. Three years earlier, he was operated for squamous cell carcinoma (SCC) of the left lower alveolus and underwent surgical excision with myocutaneous flap reconstruction, followed by chemoradiotherapy. CT angiogram of the expansile swelling revealed pseudoaneurysm of third part of the left maxillary artery and treated by surgical intervention.

Endovascular treatment for traumatic thoracic aortic pseudoaneurysm: a case report.

Project description:Cases of an endovascular treatment for traumatic aortic injury are extremely rare. A prompt diagnosis of traumatic thoracic aortic pseudoaneurysm through a 3-dimensional computed tomographic angiography of aorta and emergency repair are mandatory to rescue the life-threatening condition. An endovascular treatment is a trend for traumatic aortic injury because of lower invasivity, morbidity and mortality. We reported a rare case of traumatic aortic injury with thoracic aortic pseudoaneurysm definitively diagnosed by the reconstructional computed tomographic angiography of aorta and successfully treated with endovascular stent-graft.

Coil embolization of a thoracic aorta hematoma with branch artery pseudoaneurysm - case report.

Project description:BACKGROUND:A thoracic aorta hematoma with branch artery pseudonaneurysm is a very rare complication of thoraric blunt trauma. The standard treatment of this type of injury is aortic endograft placement. CASE PRESENTATION:We present a case in which a thoracic aorta hematoma with branch artery pseudoaneurysm was treated with coil embolization instead of endografting. CONCLUSIONS:Coil embolization of aortic injuries may be a safe and definitive treatment alternative in selected cases. This technique has the potential to reduce the risk of procedure-related complications.

Endovascular treatment of a post-traumatic pseudoaneurysm arising from a ruptured posterior tibial artery with Menox. A case report.

Project description:Posterior tibial artery pseudoaneurysms are rare and can occur in the setting of trauma. Endovascular treatment of post-traumatic pseudoaneurysms has become a viable less-invasive method particularly in young patients in whom the presence of collaterals guarantees distal vascularization. We report an unusual case of a 16-year- old patient who sustained blunt trauma to his right lower extremity due to a bicycle injury and developed a delayed symptomatic pseudoaneurysm of the posterior tibial artery in the absence of tibia fracture. Superselective endovascular treatment with Ethylene Vinyl Alcohol Copolymer based liquid embolic system (Menox) was successfully performed, achieving exclusion of the pseudoaneurysm with preservation of the remaining vascularization of the lower limb.

Case report of a maxillary antrolith.

Project description:INTRODUCTION:Antroliths, which are pathological calcifications within the maxillary sinus, are a rare occurrence. While some may present with symptoms, most maxillary antroliths are asymptomatic incidental findings. PRESENTATION OF CASE:We report a case of a 67-year-old lady with a round radiopacity within her right maxillary sinus as seen on the panoramic radiograph. Subsequent cone beam computed tomography (CBCT) scan detailed a well circumscribed radiopacity at the floor of the maxillary sinus. Surgical excision was done via Caldwell-Luc procedure. DISCUSSION:A review of literature was carried out with particular reference to the etiology, clinical and radiographic features and management of antroliths. Radiological examination of the sinus by standard Waters, panoramic radiograph and CT scan can aid in identification and diagnosis. While biopsy may be indicated to rule out differential diagnoses with similar presentations, in otherwise small and asymptomatic antroliths, they are generally left alone and periodic check-ups appear to be the primary choice of treatment. Surgical removal is considered the treatment of choice for antroliths with complications. CONCLUSION:With regular long term follow ups, prophylactic removal of small and asymptomatic antroliths may not be necessary. However, treatment options should be presented to the patient and the ultimate decision lies with the patient with informed consent taken.

Progressive cervical tumour in an HIV-patient: giant pseudoaneurysm of the carotid artery: a case report.

Project description:BackgroundAneurysms of the extracranial carotid artery are a rare entity and correspond to <1% of all arterial aneurysms.Case summaryA 58-year-old male with known human immunodeficiency virus (HIV) infection presented in the emergency department with a massive cervical tumour on the right side of the neck and a severe occipital pain. Contrast computed tomography demonstrated a pseudoaneurysm of the proximal right internal carotid artery (ICA). Open surgery of the ICA was performed with reconstruction of the posterior vessel wall, embolectomy of the ICA, and anterior pericardial patch reconstruction. After an uneventful postoperative course, the patient was readmitted 4 weeks later with a right retrobulbous haematoma caused by a carotid cavernous fistula. Therefore a coil-embolization of the fistula and finally of the distal right carotid artery was performed.DiscussionIn patients with a proven HIV infection, the occurrence of a vasculopathy in the extra- or intracranial carotid artery is significantly increased and the second most common site after the lower extremities. In patients with progredient neck swelling it should be considered in the differential. Surgical therapy is the preferred treatment strategy in the extracranial aneurysm type, especially in this patient collective.

Setting things “right”: right internal mammary artery on anomalous right coronary artery - a case report

Project description:Anomalous aortic origin of a coronary artery (AAOCA) is a rare pathology that may cause episodic ischemia owing to possible vessel compression during systolic expansion of the aortic root. This anomaly can lead to myocardial infarction, malignant arrhythmias and sudden cardiac death (SCD). Several surgical techniques have been described; however, there are no defined guidelines regarding the treatment of AAOCA. We report the case of a 47-year-old woman with ectopic origin of the right coronary artery (RCA) from the left sinus of Valsalva, with an interarterial course of the proximal segment of the artery, running between the aorta and the pulmonary trunk. Revascularization was accomplished by harvesting the right internal mammary artery (RIMA) and anastomosing it to the anomalous RCA, given the small portion of the RCA following an intramural course and our familiarity with the procedure. The RCA was ligated proximal to the anastomosis to avoid the string sign phenomenon. This procedure is safe and fast and can be considered an alternative to coronary reconstruction.

Management of radial artery pseudoaneurysm in radial forearm free flap: case report and review of the literature.

Project description:The formation of a pseudoaneurysm in a revascularized free flap is an extremely rare complication in microsurgery. The most effective treatment modality is still the subject of debate. We present the management of a case of pseudoaneurysm of the arterial pedicle of a radial free flap used after hemiglossopelvectomy in a patient with squamous cell carcinoma of the tongue. In our case, a 74-year-old man with the pseudoaneurism was successfully treated by endovascular stenting. Endovascular stenting can be considered an effective and safe procedure and a relevant alternative to open neck surgical treatment.
