Solitary median maxillary central incisor in association with hemifacial microsomia: A rare case report and review of literature.

ABSTRACT: Solitary median maxillary central incisor (SMMCI) is a rare dental anomaly. It is estimated to occur in 1:50,000 live births. The SMMCI tooth differs from the normal central incisor in that the crown form is symmetric and it develops and erupts precisely in the midline of the maxillary dental arch in both primary and permanent dentitions. Presence of SMMCI with hemifacial microsomia (HFM) is a very rare clinical condition. We report a case of HFM in a male of Indian origin who presented with SMMCI in both primary and permanent dentitions. The association of HFM with SMMCI may be due to defective development of neural crest cells and/or lack of space in maxilla.

Project description:Solitary median maxillary central incisor (SMMCI) is a unique developmental anomaly in primary dentition. It involves central incisor tooth germs and may or may not be associated with other anomalies. Its presence, concomitant with fusion of right mandibular incisors has not previously been reported. A 5-year-old girl was presented with a single symmetrical primary maxillary incisor at the midline, with the absence of labial frenulum, an indistinct philtrum and a prominent midpalatal ridge. There was an associated fused tooth in the right incisor region and radiographic examination confirmed only one maxillary central incisor in both the dentitions. Family history revealed that the father of the girl also had a similar anomaly providing probable evidence of etiological role for heredity in SMMCI.

Multiple talon cusps on maxillary central incisor: A case report.

Project description:Dental anomalies affecting the teeth are relatively common. Simultaneous occurrence of multiple dental abnormalities in a single tooth is uncommon and relatively rare. One such abnormality routinely encountered in dental clinics is the talon cusp. It is also referred to as dens evaginatus, characterized by the presence of an accessory cusp-like structure projecting from the cingulum of anterior teeth. It has an increased predilection for maxillary teeth and permanent dentition. Although numerous cases of talon cusp have been reported in the literature, occurrence of multiple talon cusps in maxillary central incisors has not been found in the literature. This case report highlights the presence of talon cusps in maxillary anterior teeth with multiple impacted supernumerary teeth.

Solitary splenic metastasis of pulmonary carcinoid: A rare case report and literature review.

Project description:We herein report a rare case of solitary splenic metastasis of a pulmonary carcinoid, for which hand-assisted laparoscopic splenectomy was performed. A 73-year-old man was admitted to the Inoue Hospital (Fukuyama, Japan) in March 2015 with an abnormal shadow of the spleen. The patient had a history of right upper lobectomy with lymph node dissection for a pulmonary typical carcinoid (pT1N0M0 stage IA) 7 years earlier. Plain abdominal computed tomography (CT) revealed a low-density area in the spleen. Enhanced abdominal CT revealed a mass in the spleen measuring 3×3 cm. Blood tests and tumor marker analysis revealed no abnormalities. Abdominal magnetic resonance imaging revealed a mass that was low-intensity on T1- and high-intensity on T2-weighted images, and positron emission tomography revealed abnormal fluorodeoxyglucose accumulation in the mass. Hand-assisted laparoscopic splenectomy was performed for diagnosis and treatment. The resected specimen included a reddish-brown mass. On immunohistochemistry, the mass was positive for chromogranin A, synaptophysin and CD56, which was similar to the resected pulmonary carcinoid tumor; thus, splenic metastasis of pulmonary carcinoid was diagnosed. The postoperative course was uneventful and the patient is currently under observation and remains recurrence-free, with no adjuvant chemotherapy.

However, there are few reports on retroperitoneal SFT. To better understand its biological behavior, clinical diagnosis and follow-up strategy, this case studied the manifestations, diagnosis and treatment of retroperitoneal SFT in a 57-year-old woman, and reviewed the relevant literature.

Non-Surgical Retreatment of Maxillary Lateral Incisor with Unusual Anatomy: A Case Report and Mini Review.

Project description:Knowledge about internal anatomy plays a crucial role in the success of the root canal treatment. Many studies on internal anatomy have repeatedly reported that maxillary lateral incisors have only one canal. The primary aim of this article was to describe successful non-surgical retreatment of a permanent maxillary lateral incisor with two root canals and open apices. The treatment was carried out using dental operating microscope and the canals were obturated with mineral trioxide aggregate (MTA) as an apical plug. A review of literature was also conducted to evaluate the anatomical variations of maxillary lateral incisors.

Solitary fibrous tumor behind the pubic bone: A rare case report.

Project description:Solitary fibrous tumors (SFT) is a rare type of soft tissue tumors and there have been only a few reported cases of SFTs at the perineum region. We report a SFT below the pubic bone in a 35-year-old male causing compression on his corpus spongiosum. The patient underwent en bloc tumor resection with preservation of spermatic vessels and the bladder neck. Pathological evaluation and immunohistochemistry (IHC) staining with CD34(+), CD99(+), STAT6(+), S100(-) and Desmin(-) confirmed the diagnosis of SFT. Surgery plays a key role in treatment strategies and pathological examination with IHC is important in the diagnosis of SFTs.

Solitary vaginal paraganglioma with mature sacrococcygeal teratoma: a rare case report.

Project description:BackgroundVaginal paraganglioma are rare, atypical, solitary tumors which originate from the female genital tract. Sacrococcygeal teratoma are also rare neoplasms which derive from one (or more) primordial germ cell layers. Here we report a unique case of vaginal paraganglioma with sacrococcygeal teratoma.Case presentationA 44-year-old female experienced paroxysmal hypertension, palpitations and dizziness for almost six years. Enhanced CT and MRI highlighted two abnormal soft tissue lesions located in the left vaginal wall and coccyx anteriorly, and Iodine-131 metaiodobenzylguanidine (131I-MIBG) demonstrated abnormal radioactive uptake in perineum area. Endocrine tests showed elevated plasma normetanephrine (NMN) and 24 h urine norepinephrine. There was a well-circumscribed soft tissue lesion of approximately 3.5 cm in the left lateral vaginal wall which could be palpated during bimanual examination, together with a 1.5 cm tumor in the posterior wall of the rectum. We completely resected the two lesions in stages with the support of a senior gynecologist and general surgeons. Postoperative histopathological examinations suggested the vaginal paraganglioma and mature sacrococcygeal teratoma. Targeted sanger sequencing for the 36 mostly common paraganglioma-related genes, with a depth of 1000x, revealed no mutations. Post-operatively, plasma NMN and 24 h urine norepinephrine returned to the normal range and her symptoms completely disappeared.ConclusionsWe reported an extremely rare case and the successful treatment of functional vaginal paraganglioma coexisting with adult sacrococcygeal teratoma.

The inflection point: a torque reference for lingual bracket positioning on the palatal surface curvature of the maxillary central incisor.

Project description:BACKGROUND:Contrary to buccal orthodontics, lingual orthodontics has no reference for vertical bracket positioning on the maxillary central incisor. The aim of this study was to provide a reference point in relation to torque for lingual bracket positioning on the palatal surface curvature (PSC) of the maxillary central incisor. METHODS:Cone beam computed tomography (CBCT) radiographs of 50 right maxillary central incisors from archives of a dental radiographic center were transferred to Photoshop, where their PSC was traced using pen-tool. The PSC torque angle values of the incisors were calculated in Excel using cubic poly-Bezier curves at 0.5-mm increments and at the inflection point of PSC. Descriptive statistics for the torque angle values of the increments and for the inflection point for the 50 incisors were then calculated. One-way ANOVA test was used to detect systematic differences between the increments, and Tukey test was used post-hoc. RESULTS:For all incisors, increments incisal to inflection point exhibited progressive decrease in torque angle values from the first-calculated increment to inflection point while increments cervical to inflection point exhibited progressive increase from inflection point to last-calculated increment. Mean torque angle values of all the increments and inflection point showed high standard deviations and vast range of values. One-way ANOVA test was highly statistically significant (p?<?0.0001) and most pairwise comparisons of the increments using Tukey test were significant. CONCLUSIONS:Inflection point can be used as a reference for bracket positioning on PSC. Cervically oriented shifts in vertical bracket position cause crown labial/root palatal movement cervical to inflection point and crown palatal/root labial movement incisal to it. A scientific mathematical justification for customized bracket torque prescriptions on PSC of maxillary central incisor was also provided.

Synovial sarcoma of the maxillary sinus and chest wall: An exceptional association of two synchronous rare locations, Case report.

Project description:IntroductionSynovial sarcoma is a malignant tumour of mesenchymal origin with an aggressive character and a rare cervicofacial location. Its management is multidisciplinary in order to improve prognosis and survival.Case reportWe report the case of two rare and synchronous localizations of monophasic synovial sarcoma of the maxilla and chest wall in a 58-year-old woman, whose management was based on exclusive chemotherapy.DiscussionThe cervicofacial location of synovial sarcoma represents 3% of all locations in the body. Its clinical and radiological manifestations are aspecific. Its diagnosis is based on a combination of histological and genetic arguments. Its therapeutic management depends on the tumour stage and prognostic factors.ConclusionEarly diagnosis of cervicofacial synovial sarcoma allows to avoid a mutilating procedure for the patient and to improve the long-term prognosis by a good control of the tumour.
